PFIA’S

Charitable Fund PFIA is accepting charitable contributions to our Charitable Fund, which are tax deductible for the person or company donating.

In April of every year, the Board will distribute PFIA’s contributions and donations from our members to these predetermined charities from the prior year.

We will lean heavily towards children and military families. The four funds that we will contribute to will be, Make a Wish Foundation, Special Olympics, Special Operations Warrior Foundation, and St. Jude Children’s Research Hospital.

Everyone gives money to charities of their choice but we think giving our members a place to give, that they know 100% of funds are distributed to charities close to their heart will be a very benevolent endeavor.

We think that will be great extension of our fraternal mission and hope it is received well and becomes the place to go to for members to help others.

D

ealing with a basement fire is one of the most dangerous encounters on the job. This specific area of a home is critical and will challenge even the most experienced and well-trained firefighters. The treacherous conditions of having limited access, few ventilation options, and the possibility of collapse can be difficult to recognize, so a strong command presence by the incident commander is essential. Properly determining size-up conditions sets the groundwork for assessing this particular location and making the calculated decision whether or not you want to risk your firefighters in that structure.

the best way down is particularly done in pairs. You’ll also want to ensure that your firefighters have enough hoseline, migrating quickly and all the way. It’s either all or nothing at this point. The backup position is very crucial as well, feeding the hoseline down the steps and continuously monitoring conditions.

When we think of what's in a basement, what comes to mind? I would almost always say some type of storage, for example, clothing, furniture, bulk household items, non-perishable food, or outdoor accessories, and more importantly, hazardous materials that are flammable and combustible liquids.

Basements are also commonly used as a utility space, where items such as a water heater, breaker panel or fuse box, and airconditioning system are located, generally fueled by natural gas, propane, or fuel oil.

Sometimes basements are often used as living space that will, depending on how large the area is, have walls just as the other levels of the home. Unless you are speaking with the evacuated occupants, you never know what you are walking into. Even then, they may not mention everything.

You'll also have to consider the potential for collapse, many basements have exposed ceilings that can quickly compromise components such as floor joists and floorboards. The floor above can partially or completely fail and collapse into the basement, trapping the crew below.

This can result in disorientation or even being lost, not knowing where to send help, and in some cases, in death. The entering crew must keep a close look on if the floor is hot, weak, or shows signs of embers lifting. This will call for an immediate reverse of the engine group and change in tactics.

Entering the basement of any dwelling, there will be one way in, one way out, and the windows will almost always be small. With the heat and flames trying to rise, proper ventilation is imperative. With this in mind, the first crew will encounter high heat, lots of smoke, and generally low visibility.

After entering into the home and finding yourself at the top of the basement stairwell,

Ventilation will be tricky in this particular case because it can also feed an oxygenstarved fire, resulting in a backdraft. To avoid a backdraft, you’ll want to make sure the basement windows are vented. You may even need metal-cutting tools to assist in ventilation.

No matter what, this is the most challenging places to work with in a home or dwelling There is a great risk involved but can be controlled rather quickly with everyone doing their job and completing the entire operation efficiently.

Rhode Island — Lieutenant Sean Asher of the Pawtucket Fire Department was in the process of rescuing a victim off of a fire escape when, while repositioning the aerial, his foot slipped between the rungs.

The injury resulted in an amputation. The members of the Pawtucket FD hosted a fundraiser in Sean Asher's honor.

Louisiana — On March 10, 2019, Cpl. Marcus Thompson of the Baton Rouge Police Department was involved in a car accident while on duty and suffered major injuries. The Baton Rouge Police Department held a fundraiser dinner to help Cpl. Thompson with medical bills. There was plenty of chicken and ribs and a good time was had by all.

Connecticut — Another successful year for the Bike Helmet Safety Program. Firefighter Jay and The North Haven Professional Firefighters handed out bike helmets to 100 first graders and one lucky winner from each elementary school won a new bike! Congratulations to Joseph, Lia, Lucas, and Juliana.

FALLEN HEROES Mujahid Ramzziddin - February 21, 2018 Prince George’s County Police Department (MD) 19-year veteran Sgt. Mujahid Ramzziddin was shot and killed while attempting to protect a female subject who was being attacked in a domestic violence incident in a neighborhood.

Sgt. Ramzziddin, who was off duty and lived nearby, attempted to protect the female and confronted the man who was armed with a shotgun. The subject, who had a protective order issued against him, fatally shot Sergeant Ramzziddin before stealing his service weapon and fleeing in a vehicle.

Responding officers pursued the man approximately 10 miles before he stopped and attempted to flee into a nearby wooded area. The man was shot and killed by pursuing officers after shooting at them.

nce again, Police and Firemen’s Insurance Association has awarded 100 children, step-children, grandchildren, and great-grandchildren of PFIA members with scholarships this year.

The scholarships amount to $1,500 per year, and are renewable for three more years, provided eligibility requirements are continually met.

To avoid bias, winning students are selected by Scholarship America®; employees of PFIA do not participate.

Scholarship America bases their decision on the students’ academic performance, demonstrated leadership abilities, and participation in school and community activities.

Considerations are also based on work experience, unusual personal or family circumstances, letters of recommendation, and a statement of the applicant’s goals and aspirations. All of this information is confidential. Conditions and procedures under which awards are granted are reviewed occasionally by PFIA.

The Dangers of Vaping Include Brain Damage, Lung Damage, and More

Vaping or Juuling is all the rage among teens and young adults. It is also popular among older adults who hope to kick the smoking habit, and then turn to e-cigarettes while they go through withdrawal.

However, could these devices prove more harmful than smoking tobacco itself? The dangers of vaping, unlike tobacco, are not fully known—but early research is not promising.

Vaping device works by using a battery to heat liquid tobacco suspended in a solvent. While this sounds like a harmless distillation of a plant, in reality, the solutions used in making vapes contain harmful chemicals that can lead to a host of negative health outcomes.

The Health Dangers of Vaping Here’s what you need to know about the dangers of vaping before you take your first drag.

Chemicals Used in Making Vape Cartridges Vape cartridges contain a host of dangerous chemicals. One found in many e-cigarettes is benzene, which is used in pesticides and gasoline, it disrupts the endocrine system. Acute exposure results in drowsiness and dizziness, whereas leukemia can develop from long-term exposure.

Additionally, vapes contain formaldehyde, the same substance used to preserve dead bodies. Far from giving you a rosy glow of health, formaldehyde ingestion leads to the development of several forms of cancer. Prolonged exposure may result in ending up on a slab with a tag on your toe.

Polypropylene glycol, an ingredient used to make polyester clothing, can irritate the eyes, nose and throat when exposure lasts less than one minute.

Long-term exposure results in asthma in children, and adults get more upper respiratory infections than nonusers.

Toulene, which converts to benzene during the burning process, disrupts the central nervous system (CNS). Exposure results in developmental delays, CNS dysfunction and limb malformation in developing children. The EPA says there’s not enough evidence yet to prove whether the substance is carcinogenic.

One study found 91 percent of e-cigarette aerosols killed human cells, even when low voltages were used to heat them and they contained only glycerin. Juul users inhale unknown levels of these chemicals daily.

Heavy Metals in E-Cigarettes Solvents aren’t the only problematic material found in e-cigarettes. Vapes also contain high levels of various heavy metals toxic to human health.

Lead can build up in the human body and cause health risks, especially among young children. Infants and children exposed to high lead levels often have developmental delays, irritability, loss of appetite, fatigue, and seizures. Adults who experience lead poisoning often experience abdominal distress, mood disorders, difficulty with sperm production, and trouble concentrating.

Cadmium is found in municipal waste and released when you burn fossil fuels like gas or coal. It can build up in the kidneys, where it causes disease. One recent study of 17,000 individuals indicated rates of chronic kidney disease increased significantly in individuals exposed to even low levels of cadmium.

The study linked cigarette smoking as a likely contributor to exposure. The EPA considers cadmium a probable carcinogen.

Finally, e-cigarettes contain nickel, a known carcinogen. Nickel exposure can cause dermatitis and itching, usually in the extremities. According to the EPA, certain nickel formulations can cause lung cancer.

Flavors Add Additional Woes E-cigarette manufacturers are no strangers to controversy, and the fun flavors they use to entice younger users come with additional health risks.

Other than the obvious problem of getting young children hooked on the devices, flavors like cinnamon and citrus change chemical composition when exposed to solvents such as polypropylene glycol and glycerol. In doing so, they became more irritating to the lungs and soft tissues of the throat when inhaled.

When mixed with solvents, 40 percent of the flavors transform into acetal compounds, which are carcinogenic. Children and teenagers get more lung irritation from inhaling these substances than adults.

In addition to disease, vaping poses quality-oflife issues. Those who vape regularly may find themselves growing short of breath from everyday activities like walking up a flight of stairs. They may lack energy for the activities they once enjoyed.

Tobacco use of any kind is associated with mental health disorders. Although 14 percent of all American adults use tobacco in some form, including vaping, 23 percent of individuals with a mental health disorder adopt the habit. It is unclear if people with mental health issues use nicotine in an attempt to selfmedicate or whether such individuals are more prone to substance abuse due to structural differences in the brain.

How You Can Quit the Habit

Some research on the dangers of vaping focuses on nicotine toxicity, a known carcinogen released by any type of tobacco product. According to the American Cancer Society, half of all smokers will eventually succumb to diseases related to the habit.

Nicotine use increases the risk of several diseases: • Lung cancer: Smoking accounts for 87 percent of all lung cancer deaths.

• Other forms of cancer: Smoking increases the risk of developing other types of cancer, including rectal and prostate.

• Heart disease: Both smoking and vaping cause your arteries to harden. When this happens, the risk of heart attack or stroke increases significantly. Smoking also lowers HDL, or good cholesterol levels, making blockages more likely to occur.

• Diabetes: Those who smoke at least 20 cigarettes per day double their risk of developing diabetes.

Smoking increases insulin resistance, which explains why some people find weight loss difficult even though they may fear additional gain from quitting.

• Dental disease. Smokers run double the risk of losing their teeth than nonsmokers. According to a 2003 Canadian Community Health Survey, those who smoke are more likely to report oral health problems.

The good news is, even if you vape, quitting now can better your health outlook greatly. As little as a month after quitting, you can expect your lung function to improve significantly. After 15 years, your risk of certain diseases rivals that of nonsmokers.

If you’re ready to quit, the Centers for Disease Control offers a host of free resources to help you. Many individuals find success with the aid of cessation medications such as Chantix. Others find a gradual approach works better than cold turkey, and once they begin seeing the positive health benefits of cutting back, they feel empowered to quit altogether.

Above and Beyond Baseball Game Save Savannah, GA — Savannah Firefighter Joseph Alamillo was honored for his heroic rescue on June 20, 2019. He saved a life at a Savannah Bananas baseball game.

It started out as an average day for Advanced Firefighter Alamillo.“My fiance and I decided to have a date night.

It was one of my only days off for that week so she got some Banana’s tickets, we were going to enjoy the game,” said Alamillo. But his fiance noticed some commotion.

When Alamillo realized a man in the crowd was unconscious, he knew he had to help.

Unresponsive Woman New Orleans, LA — On December 16, 2017, while assigned to a traffic checkpoint, Third District Task Force Detectives Timothy Krennerich and Herbert Franklin heard a call come out via the NOPD radio of an unresponsive woman. Detectives Franklin and Krennerich, who were on their lunch break, decided to cut their lunch short and proceeded to the location to assist the responding Eighth District and EMS Units. Detectives Franklin and Krennerich arrived on the scene where they observed a female

lying in the supine position on the bed unresponsive. Detectives Franklin felt for a pulse and the female did not have one. Now realizing that they were the only first responders on the scene, they sprang into action to save this woman’s life. They resorted to their police academy training and began CPR on the woman.

Observing that they could not get proper stability of the chest compressions on the bed, they moved her to the floor, where they performed CPR on the woman and were able to get her pulse back. She regained consciousness minutes before EMS arrived.

This act of selfless service demonstrated by Detectives Franklin and Krennerich not only saved the woman’s life but it also reinforced the core foundations of what makes New Orleans Police Officers some of the finest law enforcement agencies in our nation.

Stabbing His Mother Albany, NY — On June 22, 2018, Police Officer Elston Mackey arrived on scene and could see the body of a woman bleeding from her head, face, and neck, collapsed on the floor.

As he and another officer tried to get into the home to help the woman, a man emerged at the top of the staircase holding a knife.

“Put the knife down!

Drop it!” Officer Mackey yelled at the

suspect, recorded in body camera footage released by the Albany County District Attorney’s office.

“Drop the knife! Drop the knife!

Drop the knife!”

The suspect ignored the command, came down the stairs to the woman, which was his mother, and began to stab her again.

Officer Mackey pulled out his baton, broke the glass pane of the door, stuck his hand through and fired several shots. The suspect, who was hit by the bullets, died at the scene. The quick thinking of Officer Mackey ended the attack that left the mother and an uncle with stab wounds.

They were both transported to the hospital and survived.

Had Officer Mackey not fired the shots, “You’d have two people

Special Needs A

REPLACING FEAR Fire departments are good about community outreach to the public in general and kids more specifically.

Most departments have dedicated programs that teach fire safety, first aid, escape plans, and emergency situations.

Every October, during Fire Safety Week, fire departments put on programs at schools, give fire station tours, and offer Survive Alive Houses to their communities.

With this in mind, Lt. Jacob Seaton of

the Owensboro Fire Department in Owensboro, Kentucky got to thinking, "What about those with special needs living at home? Are they being reached?"

The OFD has reached out to the public for years with programs dedicated to information about what to do in emergency situations. Every October, during fire safety week, they have programs dedicated to local second graders.

Recently Lt. Seaton realized the need to

WITH KNOWLEDGE reach out to those with special needs. Are they being exposed to the kind of life-saving information that is so readily accessible to the general public and the area children?

In the fall of 2017, the Owensboro Autism Network, along with the University of Louisville, taught the Owensboro firefighters how to deal with those they come in contact with who have special needs. Lt. Seaton said it was a very much needed and eye-opening experience.

The Owensboro area has recently had an influx of Burmese refugees. The department recognized language and cultural barriers and reached out to that population to improve how they interact in emergency situations. There were also trust issues in

that community, and the outreach has gone a long way in improving interaction.

Jacob decided it was time to flip the table.

He reached out to Whitney Priar with the Owensboro Autism Network to ask if those served by her organization would benefit from a program geared towards helping them understand what to do in an emergency situation. She was immediately on board.

Several other area organizations joined in as well.

On July 25, 2019, the Owensboro Fire Department put on a program at Century Christian Church entitled “Replacing Fear with Knowledge.” They connected with families from the Owensboro Autism Network, Puzzle Pieces, Green River Area Fall 2019 • The PFIA Protector

Downs Syndrome Association, and Wendell Foster. About 40 kids and adults with autism, down syndrome, cerebral palsy, among others, with their families were present. They learned basic information about what to do and expect in various emergency situations.

They were split into two groups and went through five different stations.

Battalion Chief Steve Leonard taught exit drills, focusing on how to get out of your home in the event of a fire. Captain Richard Peach demonstrated fire extinguisher basics.

Lt. Jacob Seaton and Capt. Richard Peach went through basic first aid. Paul Nave, the Central Dispatch Director in Owensboro explained how the communication system worked and how to make calls to 911. Local Meteorologist Jeff Lyons discussed how to react and what to do in inclement weather emergencies.

Owensboro Fire Chief Steve Mitchell said they first contacted the area organizations asking if they served individuals living on their own. They all did and were excited to help put on such an event. Chief Mitchell said the venue is also important. The

church they used is already handicapped accessible and they have even worked with some of the organizations in the past.

It is also important, according to Mitchell, to ask the groups for input as to what needs to be taught and addressed. Since they work with the families directly, they will have a good understanding of their issues and what may present a problem both in an emergency situation and for this event itself. For example, some in attendance had issues with loud noises, the sight of blood, and bright lights that can cause seizures.

Knowing this ahead of time helps to tailor the program and keep from causing a problem and creating a bad situation rather than fostering an enjoyable one.

Priar, of the autism network, said that doctors’ offices can be a good resource to find those in the community that may benefit from a program like this.

The event was a huge success. The participants and their families learned some good information and especially liked the on-on-one interaction with the firefighters.

Mitchell Schell, who is 14 and on the autism

spectrum, said he got the most out of the fire prevention and fire extinguisher demonstration. His brother William, 10, said his favorite station was the exit drills. Their mother Loretta Schell said she couldn’t be happier with this event. She was so impressed that Lt. Seaton let Mitchell sit in the Captain’s seat of the fire engine and wear the fire helmet.

Mitchell said that the best thing a first responder can do to help him in an emergency situation is to “keep a calm voice.” “That would really help me if I’m in the middle of a meltdown,” Mitchell said.

William agreed that anything to help calm a chaotic situation would help his brother deal with it a lot better.

Priar said that the meet-and-greet aspect of the program is a very important part of the whole day because it helps both sides. The individuals get to meet the firefighters and see that they are just real people there to help. Just as important, she said, is that the firefighters get to see how wide the autism spectrum is. An individual can be very highfunctioning, all the way to nonverbal and low

functioning. Priar said that the Owensboro Autism Network serves about 100 families in three or four counties, so the need is definitely there. She would very much like to see this become an annual event.

Owensboro Fire Chief Mitchell agreed with Priar that the one-on-one nature of this event is important. He said there is a special needs dance being planned for some time in August, and the fire department has been requested to be a part of it. They would like the firefighters to show up in uniform to interact while they are having a good time.

This is a good first step to stay in contact and keep it from being just a one-time event.

Everyone involved would like for this to become an annual event.

Lt. Seaton got a lot of handshakes, highfives, hugs, and praise from the participants.

He responded to them by saying, “I am so much more impressed by all of you than you will ever be with me.”

When asked what he thought of the Owensboro Fire Department putting on this event, Mitchell Schell said, “This is awesome and I absolutely want to do it again!" Mitchell has a dream of one day becoming a firefighter.

A Little Humor Loud fart gives away suspect’s hiding spot, leads to arrest Missouri — When tracking down a suspect, law enforcement might use a K-9 to track down a scent.

This was not necessary for a recent arrest.

According to the Clay County Sheriff’s Office, Liberty police was searching for a suspect wanted on a felony warrant. The suspect was wanted for possession of a controlled substance.

The suspect hid to avoid police, but apparently let out a fart so loudly, it gave up his hiding spot.

Man calls 911 for air conditioning to be turned up Ohio — An Ohio man being detained in the back of a police cruiser was hot and wanted the air conditioning turned up, so he called 911, according to the Perkins Township Police Department.

Police were investigating a domestic case when they detained a man in the back of a cruiser while they got things sorted out, police told WEWS. While in the cruiser, the man started to get uncomfortably hot

and tried to get the attention of officers, according to authorities.

Unable to get the attention of officers on scene, the man decided to call 911 to ask police to turn the air conditioning up in the cruiser so he could cool down, police said.

Officers came to the cruiser and cranked the air for the man, making sure he was cool enough before heading back to their investigation, according to police.

Man spent 3 hours licking doorbell California — Police tracked down a man who spent three hours licking a doorbell at a California home.

Surveillance cameras caught a man in the act.

The occupants said they were not home at the time, but their children were inside the house sleeping.

Police said a newly installed surveillance system helped them investigate the case.

“We were pleasantly surprised the image was so clear it didn’t take us long to identify the individual,” Salinas Police Department spokesperson Miguel Cabrera said.

The security cameras also caught the man moving an extension cord

to the front yard and relieving himself.

“You kind of laugh about it afterwards because, technically, he didn’t harm anybody, he didn’t break anything,” the homeowner said.

Police said the man could face misdemeanor charges of petty theft and prowling.

2 tbs. olive oil 1 onion, diced 7 cloves garlic, chopped 1 tsp. sugar 1 tbs. chili powder 1 tbs. ground cumin 1 tsp. pumpkin pie spice 2 tsp. oregano 2 tsp. ground coriander 1 lb. ground turkey 3 tbs. tomato paste 2 green + 1 orange/ yellow bell pepper, seeded and chopped 2 cans (14.5 oz. each) fire-roasted tomatoes, with juices 3 cups chicken or turkey broth 2 (14 oz. each) cans black beans, rinsed well and drained ½ cup pure pumpkin puree kosher salt and freshly ground black pepper shredded cheddar cheese for topping, optional

INSTRUCTIONS

In a large heavy pot or Dutch oven, heat olive oil until hot. Add onion and garlic, stirring until fragrant, about 30 seconds. Add sugar, chili powder, cumin, pumpkin pie spice, oregano, and coriander. Stir to combine, 10 seconds.

Add ground turkey to pot and sprinkle 1 tsp salt over it; cook/stir to break up into small pieces. When turkey is cooked through, add tomato paste and stir 30 seconds.

Add bell peppers, fire-roasted tomatoes with juices, and broth. Scrape bottom of pan while stirring. Bring chili to a simmer; simmer covered for 20 minutes.

Add beans and pumpkin puree. Bring chili back to a simmer for 15-20 minutes or until heated through.

Cider-Braised Chicken, Brussels Sprouts, and Apples INGREDIENTS

4 slices of chopped bacon 8 bone-in chicken thighs (about 3½ pounds total) 2 medium tart red apples cored and cut into wedges 1 12 - ounce bottle hard cider 2 tbs. chopped fresh thyme 2 tbs. whole grain mustard 1 ts. kosher salt 12 ounces fresh brussels sprouts, trimmed and halved if large (2 cups)

INSTRUCTIONS

Cook bacon in a skillet over medium heat until crisp; remove from pan, reserving drippings in skillet. Add chicken, skin side down, then cook 10 minutes or until it is browned, turning just once. Then remove from skillet. Add apples to the skillet and cook 4 minutes or until browned on both sides; remove from skillet. Drain and discard drippings from skillet.

Next, include cider, thyme, mustard, and salt to skillet, scraping up any browned bits. Bring it to a boil and reduce heat. Return chicken to the skillet.

Make sure to cover and simmer 10 minutes. Add Brussels sprouts. Cover and cook 5 minutes. Add apples and cook while it is uncovered for 5 minutes more or until chicken is cooked (at least 175° F).

CANCER IN THE FIRE SERVICE AND POSSIBLE REDUCTION METHODS Keith Tyson Vice President,

C

ancer in the fire service is our new reality and we are beginning to understand some of the reasons and causes, which include the following:

1. Cancer can be caused by a number of different issues, including some of the things that we eat, drink, smoke or breathe, and our working environment, however, 2. According to a recent NIOSH study of approximately 30,000 firefighters, 9% have a higher incidence for cancer and a 14% higher mortality from cancer than the general public. (a)

3. According to the IAFF, 61% of the lineof-duty deaths since 2002 have been attributed to cancer, and that number was 70% of the line of duty deaths in 2016 alone.

4. Within Miami-Dade Fire Rescue, of the total 313 deaths since 1952, 110 (35%) of the members died from cancer. In breaking that number down further and looking at the 140 members that died

between July 2006 and July 2016 alone, 56 (40%) died from cancer and 10 (7%) died from cardiac issues.

5. Boston Fire has stated that their LODDs between 1990 and 2015 due to cancer alone, was 67% of their total LODDs!

Many departments across the country are also seeing high rates of cancer within their members and are attempting to reduce those rates through different methods. continued on next page Fall 2019 • The PFIA Protector

A study done in January 2015, (b), funded by the IAFF and performed by Jeffrey Stull illustrated an issue of the potential for chemicals and carcinogens to get through our PPE, particularly in the area of the hood as well as the interfaces between jacket and pants, and between pants and boots. Subsequently to this study there have been many design changes in PPE, our hood design, and discussions on how better to clean our PPE, both on the fire scene, as well as afterwards.

So let’s talk about some of these methods: 1. On scene gross Decon of PPE: A recent study (c) showed that using soap, water and a brush was approximately 85% effective in getting rid of the soot and PAHs on the PPE. Off-gassing VOC levels increased postfire and then decreased 17-36 minutes later regardless of whether field decontamination was performed. Because of previously released preliminary data, departments across the country began a program of gross decon on the scene in order to reduce the firefighters' exposures, while others were resistant due to the potential for steam burns.

Recently Palm Beach County Fire Rescue did a series of training burns along with on-scene gross decon procedures which showed their crews that even after multiple fires and multiple decons in the same gear, no one

suffered burns (d). It is fully understood that this is not a perfect method, particularly in the winter months of northern states, but it is a viable workable solution for the remainder of the year.

2. Other departments have begun a one-for-one hood swap after every fire, along with skin wipes to immediately begin the decon process. Again the study above showed that cleansing wipes were able to reduce PAH contamination on neck skin by a median of 54%. (c) 3. This same study (c) showed much higher contamination on the hands than previously thought and some departments have even begun to consider glove swap outs on a regular basis 4. Still other departments, such as Jacksonville, FL, have begun programs where they will rotate in fresh crews to do overhaul and the policy

specifically states that all PPE, including SCBA, shall be worn throughout the overhaul process. At the same time, they send their first-in crews back to the stations for showers.

5. Many stations across the country either have two sets of PPE already, (I have recently heard of one that has three sets per person) or are in the process of acquiring a second set, for swapping out into clean gear and cleaning after fires. Some departments have even gone to different color systems to have a visual reference as to which gear is being used. In any case, all gear must be barcoded and tracked thoroughly to show proof that gear was cleaned and inspected as per NFPA standards.

6. By the way, the 2018 version of NFPA 1500 has had some major changes with regards to PPE and gross decon, which must be reviewed by all.

Let’s next look around the fire stations/houses and determine what we can do here: Do you have direct source control exhaust systems, i.e., hoses connected to the truck exhaust, and are they working and being used properly? Having a system

and not enforcing its use does nothing! Diesel exhaust was classified as a known human carcinogen in June 12, 2012 (e)

7. Where do you store your PPE when not being used? Is it a separate room, with closing doors and exhausted to the outside, or is

it sitting right next to your apparatus and their exhaust systems absorbing all the carcinogens into your gear?

8. Where are your ice machines located, are they being bathed with the same exhaust systems and subsequently being ingested into your body? a. Just because you filter your water does not mean that you filtered out your carcinogens from the truck bays. b. Also, just because you use it only for our water coolers does not make it any better to drink or chill your water!

9. Some new concepts being discussed in fire station design is the red/yellow/green zone

concept similar to a haz-mat incident. a. All contaminated or potentially contaminated gear is left in the red zone. This includes PPE, tools, trucks etc. b. The green zone is for living and sleeping quarters and no contaminated gear is allowed within this area. c. The yellow zone is the transition zones between, usually a hallway or airlock system where airflow controls the direction of air to the red zone.

10. Even more recently is the concept of the Clean Cab apparatus being developed and used by departments such as Coral Springs, FL and others. In this design contaminated PPE is

never allowed into the cab area. No SCBA or tools are placed in the cab.

Instead they are located typically in a curbside compartment where all the crew members are quickly briefed by the OIC upon arrival on the scene!

After they are released from the incident, they go through gross decon as previously discussed, all PPE is bagged and sealed and the crews are sent back to the stations to shower and swap out into their secondary set of PPE. Additionally, they have two sets of flashlights, yellow for EMS and orange for fires, which also never go in the cab!

While we fully recognize that not every one of these methods is possible in every single department, we must begin to make changes to reduce the numbers of members being diagnosed with cancer.

There is no question that we have a cancer issue in the fire service and that we must do something to change. There is also no question that while science may prove or disprove some of the methods that many are currently using, unless we begin to think differently postfire and change our standards, that we will continue to see growing numbers of members developing and dying of cancer at alarmingly young ages. It still may come to some, let’s do the most we can to reduce those issues!!

It’s one thing to die of a Line of Duty Death on the job, but cancer is certainly not a hero's death!

PTSD and ME 22-year-veteran Carmel Fire Department Engineer Kenneth Brant

I

I was first introduced to PTSD on the night of May 8, 2015. My station crew had a run on a 5-month-old baby girl in respiratory arrest. I just knew it was going to be a bad one when we pulled up to the apartments and the father was yelling at us from an open stairwell, “Please come save our baby!” I started choking up at the thought of what we were going to see.

At the time, I was 18 years into my career and was 53 years old and had seen just about all I wanted to see in our line of work. The thing was, none of the bad runs I’d seen for all those years affected me; they’d been stored away safely in my brain. When I came home in the morning after a shift, my wife Theresa would ask, “Did you have a quiet night?” and I would tell her about the runs we had and then go lay on the couch and watch Sportcenter, wanting to be by myself.

I had already dealt with anxiety issues when my dad died after battling cancer when I was 7 years old. I was the youngest of four kids; 2 years later I started acting out in anger by getting in fights with kids at school, and was pulling my hair out watching ball games on tv when a game was close. There weren’t many counseling facilities back in the late 60s early 70s.

So I think I had a little head start on my brain being worn down by tragedy by the time I became a firefighter, but I always enjoyed helping people and getting paid for it. It seemed like I’d hit the jackpot and loved the job.

When our crew got to the apartment, a police officer was doing chest compressions, and I gently grabbed his shoulder and told him I’d take over for him. With our CPR, our two medics got her heart rate back enroute to the hospital. As we were in the ER room with her, the

doctor told the young mother that her baby was brain dead from lack of oxygen. I knew my life and career were never going to be the same again after that night.

I went to bed numb when we got back but had another regular sick person run at 3 a.m. and cried myself to sleep when we got back from it.

When I came home the next morning, my wife Theresa was leaving for work and cheerfully greeted me at the door, and I cried uncontrollably on her shoulder telling her what had happened last night.

The next two months I was having crying episodes at random places and times: cutting grass, in the middle of a set working out at my gym, riding my road bike. Songs I was listening to set me off. I was lucky enough to notice something was wrong and told Theresa that.

Me: Baby, something’s wrong with me Her: Do you want to go talk to someone?

Me: Let’s give it two weeks and see if it goes away …

Yeah, it didn’t. On runs, I always did chest compressions during cardiac arrests, and every time after cleaning up at the scene I was having crying and anxiety spells out by the engine, washing up like big wave then it would go away.

Finally, my Captain at the time handed me a slip of paper with

our EAP number on it.

3 weeks later...

Me: Baby, I’m not right, I’m going to see someone.

I compared all these bad runs I’d been on that didn’t seem to bother me or kind of shook me up to blocks stacking up, and this baby run knocked that stack over.

I called our EAP who specialize in treating police and fire. The first couple visits, my counselor and I just talked. On the third visit, she put me through a process called EMDR, a left/ right eye movement therapy that I can only describe as short-circuiting the bad memories. Afterward, it felt like a piano was off my back.

Things have been up and down emotionally over the last 4 years since this run. I was having a hard time going on senior living runs, feeling awful for these patients that were suffering. This, after my father-in-law passed away shortly after the baby run, and then my mom passing a year later after 5 years of dementia.

The anxiety attacks out by the engine were coming back again after cardiac arrest runs.

I felt like I had a tornado in my head constantly spinning.

So after talking to Theresa many times about retiring and making sure she was OK with it and reassuring me we were good financially, I decided to retire in February 2019 at age 57 and at the end of 22 years of service.

The reason I’m writing this article is that there are a lot of brothers and sisters in the police and fire service that won’t get help because they’re embarrassed. Why?

Doing drugs to make it go away?

Marriage in trouble because you won’t get help and treating your spouse and kids like crap?

GETTING HELP SHOWS YOU’RE STRONG! My story came out when my Chief was introducing me to our City Council for my retirement, and going over my career and brought up my issues and about trying to be an advocate. I got a standing ovation for just admitting to having a problem I’m trying to fix!

saw the story in the paper and interviewed me, and then my story was on television in central Indiana. Very humbling but the same time I thought, why not? I came forward with my issues because why not share something that helped me?

After the story came out on the local news channel, I can’t tell you how many veteran firefighters have thanked me for coming forward and said to me: “I think I have this.”

Just like our jobs, you’re identifying a problem and fixing it.

The Carmel Fire Department has been incredible and very supportive. They started a peer support team so men and women can just open up to somebody; sometimes that’s all it takes.

Lastly, Theresa has been incredible through this. If I’m having a bad day and things are bubbling inside me, I’ll text her and tell her I’m having a bad time and she’ll call, we’ll talk, and that settles me down.

I’m very lucky to have a great partner like her in my life.

New Jersey — In July 2019, a couple PFIA members from the Hanover Fire District worked with their local teen group for a N.J. Teen Community Emergency Response Team (C.E.R.T.). “This is 9-1-1. What is your emergency?” A history of raising the alarm At 2 p.m. on February 16, 1968, a special red telephone rang at the police station in Haleyville, Alabama. Rather than a police officer, U.S. Congressman Tom Bevill answered the call. On the other end of the line was Alabama Speaker of the House Rankin Fite, calling from the mayor’s office (actually located in another part of the same building). Bevill’s simple answer of “hello” may not rank alongside Samuel Morse’s “What hath God wrought,” but it ushered in an important part of daily life, one that has saved countless American lives over the past 50 years. The call marked the first use of the emergency number 9-1-1, a technological answer to a life-and-death question—how do you get help quickly in the event of an emergency? Americans wrestling with the problem have experimented with many innovative solutions over the years.

In the late 18th and early 19th centuries, getting to the scene of a fire as quickly as possible was the best defense against a damaging conflagration. Just as today, time was of the essence. Watchmen would alert the populace with wooden rattles and raise the alarm by shouting through the streets (sometimes known as “hallooing fire”). Citizens and volunteer firefighters alike would grab

A simple fire pumper might be drawn by hand to the scene as well. But finding a fire fast, especially in a warren of urban streets, could be difficult.
