AM/PM Program

AM/PM Program offers a well-rounded program in a safe and secure environment located in the elementary schools within the City of Peoria. All sites are licensed by the Department of Health Services (DHS).

Department of Economic Security (DES) funding available for those that qualify, call (602) 771-0014.

BREAK CARE CAMP

Break Care Camp is scheduled during regular school breaks. We offer field trips such as Sky Zone, Harkins movie, Out of Africa and Wildlife Zoo along with on-site vendors like Star Gazing for Everyone Planetarium and BMX Masters. We will have STEM projects, group games and arts and crafts to round out the day in a fun, safe and secure environment. Parents must sign their child in and out each camp day.

Fee $25 per day, includes scheduled field trips and activities.

Children participate in a variety of activities and expand education, social and developmental skills in a supervised and secure environment. The goal is the total well-being of each child. The program is licensed by the Department of Health Services (DHS).

How to Apply

1. Obtain required certification by taking a StarGuard class.

2. Fill out application online, www.peoriaaz.gov/aquatics and click on Employment and Training for more information. Applications must be completed before water testing. No late applications will be accepted.

3. All applicants must attend one of the City of Peoria Water Tests.

Water Test #1: March 21 at 4pm – Online applications due March 17 Water Test #2: May 4 at 8am – Online applications due April 28 4. For the water tests, all applicants must bring:: Current Lifeguard certification, government- issued photo ID (high school ID is ok), and suit and towel.

StarGuard Lifeguard Training (15+) This course includes certification in StarGuard Lifeguard Training and ASHI CPR/AED for the Professional Lifeguard. Participants must be at least 15 years old by the last class, be able to swim 50 yards (with head up), retrieve a 10 pound brick from 12 feet of water, tread water for 1 minute (without hands), and attend ALL classes.

To apply to become a coach, please submit an application at www.peoriaaz.gov/youthsports. Incentives are available.

Teams Teams will be based on the number of registrations we receive for each sport / age division. We will create teams based on location and requests. Participants will be provided the opportunity to request a specific coach along with providing their closest PUSD school; however participants will not be provided the opportunity to request a friend to be placed on the same team with.

There is absolutely no guarantee of placement onto a specific team if provided at the time of registration. Once teams are formed, we are unable to switch children to a different team due to the many moving parts and large amount of special requests already received.

Practices/Games Teams will practice one weeknight per week (Mon.Thur.) beginning the week of Feb. 11 that is chosen by the volunteer coach. All games will be on Sat. beginning Feb. 23. Times and locations will be determined based on availability and registration numbers.

Age Groups Please make sure you are registering for the proper age/gender for your child. Children may not play in a younger age group; however, they may play up if they are less than 12 months from the age cut off date and meet the minimum age requirement for the sports (ex.

An 8 year old may play 9-10 soccer, but a 3 year old cannot play 4-5 soccer). The age cut off date for this season is Feb. 23 (first day of games).

DES/DDD FUNDED PROGRAMS

The City of Peoria Adaptive Recreation Program contracts with the Department of Economic Security, Division of Developmental Disabilities, to offer a variety of Day Treatment and Training Services. All programs are staffed on a 1:4 staff-to-participant ratio, so a moderate-to-high level of independence is required.

After School Recreation Program provides age appropriate activities for students 12-17 and 18-21. Program days follow the Peoria Unified School District calendar.

Adult Day Recreation Program is designed to provide unique and challenging activities in a small group setting. To inquire about openings at our two sites, contact Jodi at (623)773-7436.

Elementary Summer Recreation Program is an 8 week program for individuals 6-11. This program runs 8 a.m. - 2:30 p.m. five days a week as an integrated program in conjunction with the City of Peoria’s Summer Recreation Program. This program uses DTS hours.

High School Summer Recreation Program is an 8 week program for individuals 12-17 and 18-21. This program runs 8 a.m. - 2 p.m. five days a week as a self-contained program and uses a combination of DTS and RSP hours.

These programs are first come first serve so call (623) 773-7436 and ask for Jodi or Paula to request an application. Make sure you ask your support coordinator for DTS paperwork for the summer!

Volunteer counselors, specially trained by the Area Agency on Aging, Region One’s Benefits Assistance Program, provide objective, free personalized assistance to seniors and adults with disabilities.

Millennium Martial Arts Chuck Norris System

This is an introductory class geared to acquaint students with the Chuck Norris system of martial arts.

Child abductions are a scary and increasing occurrence in our world today. As of 2011, more than 700,000 people nationally with intellectual disabilities were living with caregivers over the age of 60. Join us at the Mosaic seminar to learn about how services have moved beyond the group home, and how you can be paid to provide a person with intellectual disabilities a meaningful life in a caring environment. The NAMI Family-to-Family Education Program is a free 12-session course for family members, caregivers, and loved ones of adults (18+) living with mental illness. The Community Services Department guarantees 100% satisfaction for all recreation programs. If you are not completely satisfied with the activities and programs offered, we will assist you in the following ways:

Transfer you to another activity or class immediately.

Credit your account in the computer immediately, which then can

be used by any family member for any recreation program in the future.

Refund your money. If you paid by credit or debit card, the amount

will be credited to your account. There will be no cash refunds. If you paid by cash or check, refunds take approximately three to four weeks to process and will be mailed to the address on the account. Refund checks will be made payable to the head of the household unless otherwise directed.

Class General Policies

• Assume you are registered and attend the first class.

• NO confirmations will be sent.

• Other than Parent Tot classes, parents remain outside the classroom.

• Non-registered individuals are not allowed to attend classes.

Active Adults General Policies

Registration for trips is on a first-come, first-served basis. PAYMENT IS DUE AT TIME OF REGISTRATION. Times and, occasionally prices, are subject to change.

Cancellations: For events where a ticket has been purchased or space booked from an outside agency (theater, travel company, etc.), we cannot issue a full refund, we can only try to resell your ticket, or you may sell it yourself. In some cases, a partial refund will be mailed to you in approximately 3 - 4 weeks.

Transportation is by school bus, city van or charter bus. All trips require a minimum number of registrations. Arrive at least 15 minutes before departure time.

NO refunds, credits or transfers will be issued after the program or session has concluded. Refunds will be given for registration fees only.

Ramada and Field Rentals

Park ramadas, softball fields and multi-purpose fields are available to rent.

Resident Rate

If where you permanently reside or live is located within the corporate limits of the City of Peoria and you pay property taxes to the City, you are eligible for the resident rate. Proof of residency is required at the time of registration. Acceptable proof is a copy of your property tax statement showing the physical location of your residence or we can look this up at www.maricopa.gov.

Notice to Persons with Disabilities

The City of Peoria Community Services Department strives to make its programs, services and facilities accessible to individuals with disabilities. If you would like to request modification that would allow you or a family member with a disability to participate successfully in a program, contact the Community Services Department at (623) 773-7137 and request that an application for modification be mailed to you. The city requires two weeks to implement a reasonable modification. Participation in a desired program may not begin until the request for modification has been thoroughly reviewed.

Therefore, complete an application two or more weeks prior to a desired start date.
