Managing Menopause Migraines You knew I’d say this at the start of an article about migraines, didn’t you, but it happens to be true: I got a huge throbbing migraine just thinking about the difficulty of writing an article about migraines, my assignment for this month, mainly because 1) I’m prone to migraines, alas, and anxiety can set one off and 2) I was really reticent to write this article because I wasn’t sure that there was anything new I could tell you.

I got over my reticence pretty quickly, though, when my wife reminded me, as she does whenever I complain that some assignment is more than I can handle, that, “If you don’t write this article, you don’t get paid” which, I must admit, is a very strong inducement to find something to say.

So what is a migraine headache?

Well, despite what most people believe, migraines are not just “bad” headaches.

In fact, lots of very bad headaches are not migraines, as for example, a very bad tension headache which can feel, as anyone who’s ever had one can attest (and hasn’t everyone?), as if the top of your head is about to explode or that an unrelenting vice is gripping your forehead, or one associated with trauma or post-concussion, or more ominously, a headache due to a brain aneurysm bursting or one due to another source of bleeding into brain tissue (a so-called hemorrhagic stroke), (The latter cause for a severe headache, by the way, is the reason that anyone who gets a sudden-onset very intense headache,

especially if they’re over age 40 or so, must treat it as an acute emergency – it could literally save your life to get the right treatment for that kind of emergency).

But the problem in defining the specific nature of a migraine headache is that we’re not really sure what happens during a migraine so all I can tell you is that the latest theories (subject to change at a moment’s notice) is that in a migraine, a combination of changes in the neurological and vascular systems sets off an inflammatory cascade that results in the severe pain associated with that type of headache.

Migraines may (or may not) be associated with other prominent symptoms such as nausea, an aversion to light and/or sound, and warning signs known as an aura which can take the form of flashes of light, tingling sensations in the limbs, and even speech difficulties (a while back, a young TV interviewer started talking gibberish while doing her interview and although many pundits instantly chimed in that she may have suffered a minor stroke on air – this is, after all, an age where any idiot “expert with an ego” is ready to offer an opinion on TV without knowing even 2 facts of the situation – it turns out that she was just suffering the onset of a migraine).

Migraines, are said to affect roughly 3 million Canadians but they affect three times as many women as men, very likely because of hormonal influences, especially changes associated with the menstrual cycle.

Eczema Eczema (atopic dermatitis) is a common skin condition that affects about 10-15% of the population. Its prevalence in our society appears to be on the rise; it affects 15-20% of school aged children today, compared with 5% of children in the 1950s . It is not entirely clear what causes this itchy skin condition, but it can be very uncomfortable to those affected. This month I will discuss the causes, treatments and ways to help prevent the recurrence of eczema.

The symptoms of eczema include an itchy, red and raised rash. There may also be tiny blisters, which can break and ooze a clear fluid. The skin may also appear to be dry and thickened. In children, the rash usually appears on the scalp, face, buttocks, and upper legs. Symptoms of eczema may resolve in up to 50% of children by the age of six years old. In adults, it usually appears on the neck and in the bend of the knees and elbows.

Symptoms in adults tend to flare up and subside, depending upon a number of factors.

It is not fully understood what causes or exacerbates eczema, but it is thought to be an allergy-like reaction.

People who have, or have had, eczema tend to develop asthma or hayfever. There also appears to be a genetic component to this disease; it tends to run in families.

There is some evidence that avoidance of certain allergenic foods may help to reduce the symptoms of eczema. Symptoms also tend to flare-up when the skin comes into contact with irritants.

So what can you do? The first thing to do is get a proper diagnosis from your Physician or Dermatologist.

Skin rashes can be a symptom of other diseases. At this time you can ask if home remedies can be tried before other treatment options.

There are many possible home treatment options.

They mostly involve keeping the skin hydrated and

avoiding irritants. Try taking shorter baths or showers; ten minutes or less, in lukewarm (not hot) water. Pat the skin dry after bathing and apply a plain moisturizing cream. Choosing a good cream is hard because everyone’s skin is different; here are a few guidelines. The cream should have almost no smell or colour; this demonstrates the lack of artificial, and possibly irritating, fragrances and dyes. Do not buy a large bottle of cream if you will not use it all in 3 months. Unused portions of creams have a tendency to ‘go off’ and may become irritating. This is common for sunscreen lotions. Try before you buy. Look for sample bottles, or ask your Peoples Pharmacist if they have opened bottles behind the counter. Ask your friends to try some of their creams.

When applying a cream for the first time, apply it on a small area. I recommend applying an amount no larger than a loonie on your neck, just below your ear.

When bathing or showering use mild soaps and shampoos. Ask your Peoples Pharmacist to recommend a mild shampoo or soap. I recommend baby shampoo for use everywhere. I also feel that we use soaps and detergents too much and too often. Most of us do not get that dirty with basic day-to-day living; if you work in construction or on a farm that is another story. Use a small amout of soap and only wash your private areas and underarms.

When you use soap, it tends to remove your bodies natural protective film that helps it to retain moisture. What happens if you wash your hands too often? They tend to get dry. The same principle applies to the rest of your body.

Clothes washing detergents are also thought to be a major source of skin irritants. They are soaps, and if any remains on your clothes it can act just like soap in the shower and remove your skin’s protective barrier. I personally use a fragrance free, hypoallergenic, biodegradContinued On Page 8

able soap. For most basic washing, I use half the recommended amount of soap. Again, how dirty do our clothes get with day-to-day use; usually not very dirty.

For certain items, like bed sheets and baby clothes, I set the machine for an extra rinse to remove any left over soap.

I suppose you have guessed by now that this part of the article will answer the question, “Are there any supplements that will help?” One of the studies I read used Oolong tea to help treat eczema. In this study, subjects drank a brew made with a teabag, containing 10 grams of dried oolong tea leaves, that was steeped in 1000 mL of boiling water for five minutes. The tea was divided into three equal parts and was drunk after the three daily meals. After one month of drinking the tea, 17% of pa-

tients had a marked improvement and 46% had a moderate improvement in their symptoms of eczema. There were no side effects reported. Oolong is a form of tea that is semi-fermented, and can be found in most grocery or specialty tea retailers. I find its taste to be a little stronger than green tea, but still milder than the black teas like Earl Grey.

Various forms of acidophilus have been shown to help with eczema in children. Supplements that contain the good bacteria that line ones digestive tract (collectively called probiotics) can have a positive effect on the immune system. While it is not clear how these probiotics can help eczema, I do have a few ideas. Probiotics have the ability to stabilize the mucosal barrier and decrease intestinal permeability. How To Effectively Apply Sunscreens Health & Wellness Information From Your Peoples Pharmacist

A majority of people use sunscreen improperly by not applying enough. They apply only 25% to 50% of the recommended amount.

Sunscreen should be applied liberally enough to all sun-exposed areas that it forms a film when initially applied. It takes 20-30 minutes for sunscreen to be absorbed by the skin, so it should be applied at least 30 minutes before going out in the sun.

The amount of time it takes for unprotected skin to burn depends on skin type. In general, fair skin burns more quickly than darker skin. If you have the type of skin that would burn after 20 minutes in the sun without protection, then the proper application of sunscreen with SPF 15 would allow you to

spend up to 300 minutes (15 times longer) in the sun without getting a sunburn. However, this does not mean that your skin is protected from all UV effects. Damage to your skin, other than sunburning, may have already started.

Most instructions on sunscreen labels recommend reapplying sunscreen "frequently," but the definition of "frequently" is vague. A common instruction is to reapply sunscreen after 2-4 hours in the sun. However, one study has shown that reapplying sunscreen 20 to 30 minutes after being in the sun is more effective than waiting 2 hours. It is possible that this time period is more effective because most people do not apply enough sunscreen initially, and this second application approximates the actual amount needed. Sunscreen should also be reapplied after swimming, excessive sweating, or toweling.

Obese Teenagers Lack Vitamin D Low levels of vitamin D are common in obese adolescents, reports a recent study published in the Journal of Adolescent Health.Vitamin D is needed by the body to stimulate calcium absorption. This is important for growing bones as well as for healthy teeth and immune support.

Researchers screened 68 obese adolescents and found low vitamin D levels in all of the girls (72 percent were deemed deficient and 28 percent insufficient) and in 91 percent of the boys (69 percent deficient and 22 percent insufficient).

After treatment, 43 of the youths had their vitamin D levels measured again and, although levels generally increased, normal levels were achieved in just 28 percent of the participants. In the others, repeated bouts of vitamin D treatment did not bring the teens' vitamin D levels to normal, which the researchers described as "concerning."

The adolescents' lack of response to treatment may be due

to the fact that vitamin D is sequestered in body fat, the researchers said.

Vitamin D is produced by the skin in response to exposure to sunlight. It is also found in certain foods, including eggs, fish and fortified foods such as dairy products and breakfast cereals.

It is possible that the association between obesity and low vitamin D status is indirect, arising from obese individuals having fewer outdoor activities than lean individuals and, therefore, less exposure to sun. It is also possible that obese individuals do not consume enough foods that contain vitamin D. Obesity -- now estimated to affect 16.4 percent of kids and teens between 10 and 17 -- can increase the risk of high blood pressure, heart disease, and type 2 diabetes, as well as some types of cancer.

meability of the intestine may allow allergic food proteins to enter the body and cause allergic reactions.

Probiotics also have been shown to modulate the immune system, meaning they can reduce an over-reactive immune system and strengthen a weakened one.

There are few health concerns with the use of probiotics, even for infants. Studies have shown efficacy and safety in newborns and one year olds. My little daughter has been taking it since she was twelve months old. One study showed improvements of eczema in children who had allergies to cow’s milk after they took probiotics supplements.

Doses range from about 6 -10 billion live cells per day.

Other supplements that show promise are evening primrose oil, vitamin A and zinc. While all these supplements, in theory, can help the skin, studies have yet to conclusively prove that they can improve eczema symptoms. Some people I talk to say that evening primrose oil, 6 grams daily, has been helpful. Early studies have confirmed these reports. Taking too much vitamin A and zinc can be detrimental to one’s health, so ask

your Peoples Pharmacist if these supplements are right for you.

Another helpful treatment that has been studied, involved soaking in a broth made from rice bran. The bran, or outer layer, of many grains is very nutritious. Rice bran is an excellent source of vitamins B1, B3, B5 and B6, zinc and magnesium. Patients who soaked in a broth made from rice bran daily for 2-5 months noticed an improvement in their symptoms.

One important, and often overlooked, part of eczema in children is food allergies.

The most common allergens are cow’s milk, wheat and soy products. Elimination diets tend to produce some benefits. I noticed that when both my wife and little girl stopped eating dairy, our little ones’ eczema improved. For more severe cases, I recommend people get tested for food allergies. I admit the idea of trying to get a blood sample from a one year old does not sound pleasant, but neither does a bad case of eczema.

Migraines are more common in people with a family history of migraines and those on certain medications such as birth control pills.

Migraines can also be triggered by stress (but then what can’t?), alcohol, and many foods, especially some cheeses and chocolate (so I sometimes wonder why everyone in France doesn’t walk around with a constant migraine), changes in meal patterns or atmospheric pressure or sleep, even sex (but that’s a headache that hits during sex, not the common variety headache that so many people claim to get before having sex).

Migraines are often one-sided (although often not), and throbbing, and they can last for only a few hours or several days.

Migraines are diagnosed entirely from history, that is, no test can help make the diagnosis, although some tests are often done on people with migraines just to reassure the patient and the doctor that there is no more sinister explanation for the headaches.

As always, treatment starts with prevention and again as always, much of this is plain old shortsupply common sense.

That means, for a start, avoiding known triggers so if you know, for example, that a combo of cabernet, camembert and chocolate always gives you a migraine, you really shouldn’t spend a night drinking Chateau Rotgut and gobbling gouda and Godivas.

Also, for many people, especially those who get an aura, immediate attention to the problem – going into a dark room to lie down, ceasing the activity they’re involved in – can mitigate the pain.

This is also a health problem for which a diary can offer good insight so if you’re not sure what your triggers might be, start keeping a record of what you did before the onset of each headache and you might come up with a strategy to minimize your headaches.

As always, also, a healthy lifestyle – eating right, regular exercise, etc – is a good idea because even if it doesn’t reduce the frequency of the headaches too much, it offers countless other benefits.

That said, most migraineurs end up with some sort of medication, and there is literally a cornucopia of drug choices, some of which are meant to reduce the frequency of headaches, some of which are used only when the pain starts to hit.

So depending on many factors such as the severity, the frequency, the disruption to your life, the cost, and so on, that grab-bag of choices includes painkillers, anti-inflammatories, vascular mediators, anti-nauseants, opiates, sedatives, antiseizure drugs, cardiovascular drugs like beta blockers and calcium channel blockers, anti-depressants, even Botox, all of which have their own unique benefits and uses.

My only advice about using these drugs is this: you are the sole judge of how seriously your headaches affect your life, so you are also the sole judge (with your primary health care provider as an assistant, of course) as to what to do about them, namely which drugs you can tolerate, how frequently to take them, etc.

And if you do opt for medical treatment, another piece of wise advice: make nice friends with your pharmacist because all of these drugs have risks and potential drug interactions (some of which can be serious) so you really want to know all you can know about any medication you will take, especially since you are likely to be on that drug for a long time.

And now, I think I’ll go lie down and let this headache run its course, which it has already begun to do, thank God, in large part, I think because I’ve started to focus on the cheque I’ll get instead of the editing I’ll still have to do.

Canada’s Youth – A Growing Weight Concern I grew up in the 1970’s and was that chubby girl in gym class who wished she were invisible when it came to getting picked for a team. Not only was I self-conscious of my body image, but it was also painfully obvious that I was the minority. Overweight kids, were an atypical breed back when the majority of kids were well within the healthy weight range, however, that demographic has shifted dramatically over the past 4 decades. By today’s estimates, over 25% of Canadian children are overweight or obese and these numbers are climbing.

One major contributor to this trend is the reliance on sugary drinks and processed foods that are high in calories and low in nutrition. When the primary diet is built mainly around fast foods or convenience foods the scales of balance are tipped heavily in favour of sugar and fat and come up short when it comes to whole foods and fresh produce. The unfortunate truth here is that our perception of a healthy diet has become quite distorted and portion sizes have been supersized to unhealthy measures. It does not help either that nutritionally inferior foods are less expensive than their wholesome competition. Soda pop is a fraction of the cost of unsweetened fruit juice. When a family is on a strict budget, shopping criteria often falls to how much bulk they can get for their dollar; nutritional value is secondary and often overlooked.

Another major contributor is the fact that today’s youth are spending more of their leisure time in sedentary activities and less time engaged in physical activity. Today, screen time, the time a child spends in front of a computer or watching television, can sometimes equal the hours spent in a classroom. The problem here is two-fold. These are hours where the body is inactive and these are also hours where mindless snacking occurs. Television commercials make it hard to resist the urge to snack as the brain is constantly bombarded by images of junk food. Getting up to raid the fridge is hardly enough activity to warrant a slice of pizza. I was happy to see a commercial the other night urging parents to make sure their children get a minimum 1 hour of physical activity daily. This is a positive step in the right direction. I also like the present trend toward interactive video games that get the whole body involved.

Children who are obese suffer both psychological and physiological consequences. Self-esteem may drop, as they are more likely to be targets for bullying and teasing; this may also lead to depression, anxiety, isolation and/or a lack of social skills. Physically, obesity stresses the body and increases the risk of developing serious medical conditions including type 2 diabetes, hypertension, respiratory problems and joint issues. Typically many of these conditions used to only show up in the older population. Sadly, this is no longer true. As these children enter into adult years, health issues increase the risk of premature aging and shorter life spans.

Dieting is not a healthy answer to solving the obesity epidemic our country is facing and it is especially harmful to a growing child. The only sure fire way to conquer this problem is by making permanent changes in the diet and lifestyle habits of families. Parents must lead by example; they are the role models by which children learn even the unspoken rules. Here are some tips for families wishing to make some changes: 1. Stop buying sugary drinks and weed out the junk food.

Bring in healthier alternatives like whole grain cereals, beans, legumes and fresh produce. Children should be eating a minimum of 5 servings of fruit and vegetables daily. Have cut up veggies in the fridge so they are handy when the snack attack hits.

2. Be active together. Find activities that everyone enjoys; walking, hiking, biking and swimming are all family friendly. Canada’s Physical Activity Guide recommends children get 30 minutes of vigorous activity and 60 minutes of moderate activity daily.

3. Limit screen time to less than 2 hours daily. One of the most frightening trends that I see is a TV in children’s bedrooms. Setting up a schedule or using a timer can help to monitor screen hours spent. Encourage kids to use leisure time in a more active manner.

It is by the very choices we make that we shape our children’s future; choose wisely, shop with awareness and teach them the value of optimal nutrition. They deserve the best that we can offer and are well worth the investment.
