Overview
Peace River Center (PRC) is a non-profit provider of behavioral health and victim services in Polk, Highlands and Hardee counties. We serve children, adolescents, adults and seniors who are insured, underinsured, uninsured or indigent. Founded in 1948, PRC offers a wide range of programs including crisis stabilization, inpatient and outpatient, residential, mental illness recovery programs, domestic violence shelters, rape recovery, substance use disorder services, and more.
We provide care to more than 27,900+ people each year, with services that are: person-centered, trauma-informed, evidencebased and culturally-responsive. Our commitment to the wellbeing of our patients extends beyond the services we provide directly. We work in collaboration with other healthcare providers in each community to ensure the total healthcare need of our patients are met.

Peace River Center’s Rape Recovery Program is dedicated to reaching out to communities in Polk, Hardee, and Highlands counties to identify risk factors that might make a person more vulnerable to sexual assault. Narcan Distribution Program
Narcan is a potentially lifesaving medication designed to help reverse the effects of an opioid overdose in minutes. Since most opioid overdoses occur in the home and are most often witnessed, having a Narcan rescue kit nearby can make all the difference. Peace River Center offers free Narcan to anyone who may need it. We do not document your name, only the number of kits given. Kits are available at the following locations:

Mental Health First Aid is a national program to teach the skills to respond to the signs of mental illness and substance use. Peace River Center offers free MHFA courses thanks to funding from Florida Blue Foundation. MHFA is conducted by a certified trainer to fit your organization or group’s needs. Treatment is available for all persons struggling with substance use challenges, whether you are early in your recovery or are in the process of maintaining a long-term recovery lifestyle. PRC offers substance use treatment and recovery-focused services for individuals, families and affected others. Each PRC outpatient site offers the following substance use services:

This program is for individuals who use opioids or alcohol and have become dependent on them. Opioid and alcohol use physically changes the brain and affects behavior and emotions. Medication Assisted Treatment combines medication and addictions treatment. An evaluation and assessment is completed first to see if you are a good candidate for this type of treatment. The Thrift Shoppe is open to the public and offers clothing, household items, toys, books, small furniture items, seasonal decor, and miscellaneous goods at affordable prices. Qualified Peace River Center clients can shop for necessities through a voucher program. Annually, the Thrift Shoppe provides more than $50,000 worth of donated goods to clients. Funds generated by the Thrift Shoppe help to cover underfunded and free programs such as Children’s Services and Victim Services.

Behavioral Health Navigation is designed to improve access to, and knowledge of, mental health and substance use disorder treatment services available in Polk County. The program is available for Polk County residents of all ages who have behavioral health needs and would benefit from help navigating the care system. Our navigators can provide information about what services are available, assist with referrals to care, and even help individuals apply for benefits. Providing both individual, family and group therapy approaches, Behavioral Health Services are offered to children or adolescents (ages 2 to 19) who have Title XIX (Medicaid) and a mental health diagnosis or a suspected diagnosis. The therapist will work with the child and family to assist in providing support to increase positive behavior and strengthen healthy family dynamics.
Community Action Treatment (CAT) Team

CAT is a self-contained, multi-disciplinary clinical team that provides comprehensive, intensive community-based treatment to families with youth or young adults who are at risk of out-of-home placement due to a mental health or cooccurring substance use disorder or other related complex issues. The goal is to support and sustain the youth or young adult in his/her family system with the least restrictive and most clinically appropriate environment. The family is treated as a unit and all family members’ needs are addressed. Family commitment and participation are essential and expected for success. Referral required.
Group Home Housing

PRC’s unlocked group home, Success House, has 15 beds and operates with 24-7 staff supervision and assistance. Individuals entering the program are assessed to determine their functional skill level. Clients receive direct skills supervisions and training, paraprofessional counseling, medication supervisions and training, crisis intervention, interaction therapy, and socialization skills training while in the program. The Housing and Urban Development (HUD) Subsidized Housing Program provides income-based, permanent housing opportunities to individuals with mental illness. Referral required.
Short-Term Adult Residential Treatment (SRT)

The SRT is a secure (locked) 30-bed specialized residential unit that serves patients who have been determined by the court to require a longer-term stay than the crisis stabilization unit (CSU) offers or are diverting from the state psychiatric hospital. The SRT provides high-risk patients with a safe, therapeutic environment for an extended length of time (179-day court order) to be assessed more thoroughly, receive more intensive treatment, and develop a more comprehensive discharge plan to enhance the likelihood of success in a community-based setting. This program provides a wide range of services to individuals involved in the criminal justice system, including community monitoring and resource linkage, for adult individuals who have been deemed incompetent to proceed or not guilty by reason of insanity. Forensic Multidisciplinary Team (FMT)

This team includes a team lead, case managers, a therapist, and a medication provider. The team helps clients found incompetent due to mental illness become competent in order to proceed with trial. The FMT also provides community monitoring for clients found not guilty by reason of insanity who are on a conditional release order by the court. Referral required.
Psychosocial Rehabilitation Services (PSR)

PSR offers structured psychoeducation and support groups for adults with a primary diagnosis of Schizophrenia, Bipolar, Anxiety or Depression. Services assist participants with learning skills related to symptom management, socialization, independent living, life management and educational/ vocational endeavors. Participants choose the groups in line with their personal recovery goals. A different topic is taught every day in small group settings, which allows for more oneon-one time. Referral required.

Home to Stay Program

Home to Stay is an intensive family support program designed to strengthen healthy parenting strategies and stabilize the family system using an evidence-based parenting curriculum. Families may be at risk of potential placement breakdown, in need of specialized support after an adoption or recent change in guardianship, or involved in the Child Welfare system preparing for reunification with their child. Home to Stay care managers work one-on-one with caregivers to address need areas associated with the initial out-of-home placement by providing case management support as well as educational skills such as problem solving, behavior modification, parenting support, conceptual change, and healthy communication with the hopes that once the family system is strengthened, the child will be “home to stay.” Referral required.
Parent-Child Interaction Therapy (PCIT)

PCIT is a short-term, specialized behavior management program designed for families with young children experiencing behavioral and/or emotional difficulties. Caregivers learn special skills for interacting with their child, and then the therapist coaches the family during real-time interactions using a microphone-in-the-ear. This allows families to receive support during challenging moments, such as tantrums, as well as learn helpful tips on the timing and use of specialized skills. This team is comprised of targeted case managers, in-home/ on-site therapeutic therapists, and parenting support staff who work together with the child and family to strengthen overall emotional functioning for children diagnosed with a serious emotional disturbance and who have not experienced measurable progress through less intensive therapeutic treatment. The purpose of the Resiliency Team is to foster and strengthen the overall resilience of the family system in order to reduce the need for out of home placement or residential care for the child. Program staff provide regular assessments and develop individualized service and treatment plans to target both therapeutic and case management need areas. Referral required.

ACM assesses people with mental illnesses for service needs and links them to the needed community resources such as social security, Medicaid/Medicare and food assistance. Club Success is a recovery program for people with mental illness and attendees are called members rather than clients. Certified by Clubhouse International, Club Success is designed to mirror a workplace day. Members learn responsibility, develop self-esteem, and learn to interact professionally. They use their abilities to help others and build relationships. The clubhouse provides transitional employment opportunities, guidance to secure housing, and/or continue their education. It also promotes members’ on-going psychiatric care.

Care Coordination
Care Coordination offers temporary support to connect those living with severe mental illness, severe emotional disturbance, and co-occurring disorders to the appropriate level of services and support needed to promote stability in community-based care. Care Coordination assists with linking clients to services that support their overall well-being, such as mental health treatment, primary physical health care, employment services, and social connectedness. Telehealth
Peace River Center offers telehealth, the delivery of health services through video chat to qualified patients. Clients can meet with one of our skilled staff members from the convenience of their home or other approved location. Telehealth enables real-time communication remotely between patients and the therapist, psychiatrist, or primary care provider. Telehealth also helps break down barriers that can prevent treatment such as ongoing transportation challenges.
COMMUNITY-BASED INTENSIVE TREATMENT

The FIT Team program integrates substance use and mental health treatment for substance using parents who are involved in the child welfare system. In-home interventions and Family Mentors are used to achieve parental engagement in services and improve safety and well-being for children. The model is unique in that it uses mentors as team members of the treatment team to help support parents. These mentors themselves have experienced success with substance use treatment and have been substance free for at least three years. The mentors and clinicians play a vital role, providing guidance, support, and a timely response to parents who are recovering from their addictions and healing from trauma they have experienced. Florida Assertive Community Treatment (FACT)

FACT is a multi-disciplinary team that provides continuous comprehensive treatment, rehabilitation and support services to individuals with a severe and persistent mental illness (SPMI) with recurring acute episodes of illness. FACT is designed to promote psychiatric and physical health, independent living, supportive relationships, and full community participation for individuals it serves. Ultimately the goals are to keep clients stable in the community of their choice without cycling through hospitals, crisis units, jails, or shelters and be as engaged with their community, family and friends as desired. OnTrack is an innovative treatment program for adolescents and young adults who recently had unusual thoughts and behaviors or who have started hearing or seeing things that others don’t. OnTrack helps people achieve their goals for school, work and relationships. OnTrack uses a “shared decision-making” model and consists of:
• Comprehensive care using evidence-based practices offered by a team of professionals
• Services can include mental health treatment, education and employment support, substance use treatment, family education and support, and other services as needed. The program is for teens and young adults between the ages of 16 and 30 years old who have recently started experiencing unusual thoughts or beliefs, or disorganized thinking for more than a week, but less than two (2) years. PRC offers therapy and psychiatric services in a collaborative setting so that care is connected for patients who may participate in more than one line of service.
Outpatient Services
Our therapists work with individuals to develop individualized treatment plans to help clients reach their mental health, wellness and recovery goals. We treat adults and children. Services include the following:

Peace River Center’s certified inpatient receiving facilities are located in Bartow and Lakeland. Though the CSUs take individuals of all ages, Peace River Center is the ONLY CSU that takes children aged 10 and under in Polk, Hardee, and Highlands Counties. These secure units provide individuals under Florida’s Baker Act (mental health commitment law) psychiatric crisis assessment and stabilization services including group counseling, medication management, recovery education, discharge planning, referrals, and family education. Individuals are assessed and may be admitted voluntarily or involuntarily until the mental health emergency is stabilized and continuing care referrals are made. The average length of stay from time of admission to discharge is typically less than four days; however, there are exceptions.

Community Mobile Support Team (CMST)
The CMST provides crisis outreach counselors at Polk County Sheriff’s Office (PCSO) District Substations throughout Polk County to assist with mental health related calls/cases. These Counselors can provide a crisis assessment, which may include a suicide severity screening, and provides crisis intervention to deescalate the current situation so an action plan can be developed for services at the appropriate level of care. The goal of the response is to bring the situation to a level where a Baker Act is not necessary, reduce the acute stress, build coping and resiliency, and connect the person to needed services. The MCRT is available 24/7 in Polk, Hardee, and Highlands County for immediate, free crisis assessment and intervention by phone call, text message, computer Internet browser chat, mobile response (to your location), or walk-in at our Bartow or Lakeland Crisis Stabilization Units. The MCRT is available for emotional support, mental health help, and for those struggling with urges to use alcohol and drugs. The MCRT provides linkage and referral, and post-crisis follow-up contact. The purpose of this service is to decrease involuntary psychiatric inpatient admissions of individuals who can more appropriately be served by other community resources. The MCRT can provide mental health support following critical incidents such as hostage situations, school violence, sudden loss of life or other trauma-related incidents.
