At Phoenix Children’s, Families Are Never Alone We know it’s difficult for families to be alone and far from home at a time when they most need their own caring community. Since 1983, this knowledge has inspired us to provide hope, healing and the best health care for Arizona’s children—in Arizona. Families no longer need to take their children to faraway cities for care. Families come from across the U.S.—and around the world—to Phoenix Children’s so their children with complex, life-threatening illnesses can be treated and cured by leading physicians. Once a “hospital within a hospital” with only 74 beds inside Good Samaritan Medical Center, Phoenix Children’s has grown to now provide inpatient, outpatient, trauma and emergency care across more than 75 subspecialties. We see more than 200,000 children each year and have cared for millions of children since our founding. These aren’t just statistics—every child has a story, a family and a future they are fighting for. And so does every child we have yet to meet. Because of the generosity of our supporters, we are able to provide the same exceptional standard of medical care to all children in need, regardless of their family’s ability to pay. No family or child is turned away. With the help of caring and generous people, our patients and their families never face serious illness alone.
Every gift in a will or by beneficiary designation, of every size, matters and can change the future for a child.
When a child is sick, their parents need answers and they can’t wait. Research will lead us to more innovation, earlier diagnoses, less invasive treatments and more cures than we ever thought possible. Every day, we work with a sense of urgency to make the impossible become accessible faster. Our commitment to providing our pediatric population with the best health care, when and where they need it, is unwavering.

Your Gift for the Future Many of our earliest patients are grown now, perhaps with children of their own. They are our teachers and firefighters, our pastors and police officers. They are your friends and your neighbors. They’ve faced extraordinary challenges and persevered.
A Gift in Your Will or Trust to Create a Lasting Legacy A growing community of Phoenix Children’s supporters have made the deeply meaningful decision to include a gift in their will or trust, or to name Phoenix Children’s as the beneficiary of their IRA, other retirement plan, life insurance or other financial account. They have come together to fund a future where no child suffers from cancer, diabetes or other serious illnesses. Will you join them?
With a gift in your will: You can leave most of your resources to your loved ones and reserve a small percentage for Phoenix Children’s. You can give by beneficiary designation, which requires no immediate donation. The gift comes to Phoenix Children’s only after your lifetime. You can change your mind at any time. You can give any amount. Because the value is in what these gifts represent— a healthy child, a healthy future.
“I included Phoenix Children’s in my will with a smile and a light heart because that’s what people should do. We need to take care of each other, especially when it comes to children. If not me, then who? I would like to be remembered as a good person, a kind person, who cared about others. Someone who walked her talk, who gave of her time and money to support people in need. I can’t think of anyone more deserving than those kids at Phoenix Children’s Hospital. It will be a wonderful legacy.”

A Gift by Beneficiary Designation You can name Phoenix Children’s as a beneficiary of your retirement plan, life insurance or financial accounts by simply updating your beneficiary designation forms.
Retirement plan assets Many of us have built our retirement savings in expectation of a long and comfortable retirement. Most retirement plans are subject to income and possibly estate taxes when transferred to loved ones other than your spouse, which is why many supporters choose this option to make their legacy gift.
Life insurance policies If you have a policy that has outlived its usefulness, you can name Phoenix Children’s as the beneficiary. You may also name us as a contingent beneficiary, so the policy passes to us only if your family members predecease you.
Other financial accounts You can name Phoenix Children’s as a beneficiary of your bank account, brokerage account, donor-advised fund or other financial account. You don’t need an attorney to update your forms (although we always recommend that you discuss your plans with your attorney or financial advisor), and you can change your mind at any time by updating your forms again.
Important information you will need to make a gift in your will, trust or through a beneficiary designation: You can use this language as an example to share with your attorney: I give, devise and bequeath to Phoenix Children’s Foundation, located in Phoenix, Arizona, with tax identification number 74-2421549, [the residue of my estate] [__ percent of my estate] [specific dollar amount] [description of asset] for its general purposes (or for the support of a specific fund or program). For beneficiary designations, please use tax identification number 74-2421549. To discuss ways to direct your gift to a particular fund, program or area of research, please contact us for assistance at 602-933-3870 or giftplanning@phoenixchildrens.com.

Time Is of the Essence “No gift will ever truly be able to repay the gift of our daughter’s life, but we feel good knowing our legacy will help the children of the future.”
As a parent, there’s no worse feeling than worrying for the health and safety of your child. When Olivia Rose needed treatment for diabetes that was not available in her small town, her parents made the four-hour drive with a very ill little girl to Phoenix Children’s. “I can truly say they helped save our daughter’s life,” shares Becky Dodd. “After a week where Rose seemed to just get sicker and sicker, it was a relief to walk through the doors of Phoenix Children’s and know we were in the best possible place to help her.” Rose was 4, and it was Halloween. In addition to insulin, her care team gave her a costume, so she didn’t have to miss out. Her mom knew then that they would never be alone in managing her diabetes. She even kept the same doctor until she went to college. “We’re incredibly fortunate that our daughter remains in wonderful health today, and that’s because of the excellent care she received at Phoenix Children’s.”
Norris Dodd Legacy Society member
The Dodds have included a gift in their will to recognize the profound role Phoenix Children’s has played in their daughter’s life— and to ensure that other children will be able to access the same compassionate care in the future. “With our will, we can provide for our daughters, Rose and Elizabeth, first, and then give some of what’s left to Phoenix Children’s,” says Norris Dodd. “All of us understand how important this hospital is and will be for so many families ... including ours, of course.”

Ultimately, I believe strongly that each of us has a responsibility to leave our world—and our community—better than we found it. Including Phoenix Children’s in my estate plans is one of the ways I intend to do that.”
