Do masks really matter? At the beginning of the coronavirus pandemic, some of the early messages were that masks really did not make a difference. In fact, the public was advised to save masks for use by the health care professionals who were treating coronavirus patients.

Later, we were told that masks were important, primarily to help people who had coronavirus from spreading it to others. The high number of people who had coronavirus without symptoms became one of the strongest points in support of masking.

A recent study entitled “Universal Masking is Urgent in the COVID-19 Pandemic� by the Population Research Institute found when 80 percent of the population wears masks in public, meaningful progress is made in stopping the spread of the virus.

The study looked at the effects of mass face mask wearing over time

compared to the effects of social distancing and lockdown. The researchers modeled four scenarios: n Lockdown for two months followed by social distancing n Lockdown for an indefinite period of time n Lockdown followed by 50 percent of a population wearing masks in public n Lockdown followed by 80 percent of a population wearing masks in public The results of the scientific simulation found that a lockdown followed by universal masking with 80 percent of

the population flattens the curve significantly more than maintaining a strict lockdown indefinitely. Having only 50% of the population wearing a mask is not sufficient to prevent continued spread. Replacing the lockdown with social distancing results in unchecked spread.

The findings have significant impacts for each approach to slowing the spread of COVID-19. Utilizing only social distancing after the lockdown is lifted would result in an increase in the infection rate in the United States to nearly half of the population. Utilizing an indefinite lockdown brings the virus spread under control after around six months. However, the economic and social costs of a lengthy lockdown would be enormous.

The approach where 50 percent of the population uses a mask and social distancing significantly slows the spread of the virus, though the number of deaths does continue to climb in the future. The approach where 80 percent of the population uses a mask and social distancing results in a sub-

stantial reduction in the spread of the virus, eventually eliminating it.

To validate the study, the researchers looked at the experience of 38 countries, analyzing the growth rate of the virus and the prevalence of mask wearing within the respective countries.

What they found is that the lowest growth rates for COVID-19 infections are found in countries with a prior culture of mass face mask wearing, most of whom have also made mask wearing in public mandatory during the epidemic. The researchers noted that the results showed a “near perfect” correlation between early universal masking and successful suppression of the growth rates of infection.

The lesson from this study is that wearing masks in public is important for our own safety, the safety of others and to reduce the spread of the virus.

There’s nothing better we can do on our own than to wear masks.

IN A POST-COVID WORLD TE L E ME DICIN E IS ON T H E R IS E , AND I T’S HERE TO STAY Telemedicine, the practice of physicians tending to patients remotely, has been with us for more than 60 years, originally with the advent of the landline telephone. But it has taken a once-in-a-century global pandemic for this system of care to reach nearubiquity among health care providers around the world. Adhering to CDCsanctioned limitations on face-to-face contact, private practices of all sizes have adapted to this “new normal” in patient care, with doctors seeing patients not in the same room, but on the screens of smartphones, iPads and desktop computers.

Dr. Mignon Metcalf, an internist at Good Samaritan Medical Center in West Palm Beach, had practiced telemedicine herself long before the coronavirus made it standard operating procedure. Its benefits, she believes, are manifold.

“On an outpatient basis, it is convenient not only for the physician but for the patient, and it decreases travel time,” she said. “It also decreases cancelation of appointments, because patients are able to conduct their appointments from the comfort of their own home. For the physician, it protects their office staff, it keeps them safer, and it keeps the other patients safer from any potential exposures to

various infections—not just COVID but any infection.

“In an inpatient setting, the same.

It decreases travel time between various floors to evaluate patients, and it also decreases exposure after evaluating a potentially contagious patient and then seeing another patient thereafter. So overall, it is more convenient, and it is safe.”

Telemedicine is also cost-effective, requiring no specialized equipment for the physician or patient—just an internet-connected device and access to free apps such as FaceTime or Zoom.

But, Metcalf says, “there are generational differences with using telemedicine. Based on how technologically savvy you are, and also based on how comfortable you are with using electronics, it can be either beneficial or challenging. So there has been a learning curve for some physicians, but I will say that the majority of physicians have enjoyed using telemedicine during this time for several reasons.”

Some in-person doctors’ visits, of course, remain essential. There are conditions and specializations that will continue to necessitate in-person care—injuries requiring orthopedic care, X-rays, CT scans, EKGs and lab work, for instance. But Metcalf sees specialties such as dermatology and

psychiatry moving toward a majority telemedicine approach. Common issues such as colds and flus, diarrhea, insect bites and pink eye can be diagnosed and treated remotely as well.

The technology to perfect telemedicine, and to create hybrid models of in-person and tele-care, will only improve. “There are some resources available now,” Metcalf says.

“For example, the Apple Watch can generate an EKG monitor of your heart rate. I think those will continue to expand to items like a virtual stethoscope, where the patient can put the

stethoscope on their chest, and those sounds will be transmitted to the physician via telemedicine. I think there will be more kits at home that patients can use to check their blood sugar or check various lab results, and all of those can be transmitted directly to the physician to conduct a telemedicine follow-up visit thereafter.

“I don’t think that in-person visits will become obsolete, because there are times where a patient really needs to be examined physically, hands-on, by the provider,” she added. “But I do think that telemedicine will evolve.”

KO R I T Z Registered dietitian Julianne Koritz urges her clients to take a “LEAP.” The founder of Delray Beach-based Nutrition Coach, Inc., Koritz has helped more than 300 food-sensitive patients improve their health through her simple acronym: Lifestyle Eating Affiliated Program. In other words, food is medicine, and by changing our diets we can reduce or eliminate health issues that drugbased therapies have been unable to solve.

As health experts have noted since the beginning of the coronavirus pandemic, building a healthy immune system is perhaps the body’s best defense against the virus. Koritz, who specializes in patients with chronic conditions in the Palm Beaches and throughout the world through in-person and online consultations, discusses her career and how best to build immunity in this vital time.

What led you to become a dietitian?

I grew up in a medical family. My dad was a doctor, my mom was a nurse, and we’re all very into being healthy. My dad always told us, the most important thing you can have is to be healthy. Because without your health, you don’t have anything. So nutrition was interesting to me. And I was really good at science in high school. It was fun learning about the body. But I am 180 degrees out from what I learned in college,

which is interesting. Now I can help the human body by using food as medicine.

I’ll take arthritic people and get them off their medications. I’ll take migraine people, and within four to six months, they don’t have any more migraines. Nobody had been able to help them, and medicine isn’t available for that. Now that I change their diet around, all of a sudden they’re functioning on a different level. They’ve become a whole different person.

Why is it important to have a strong immune system, especially in the era of COVID? With viruses, if we’re stressed, or we’ve got a poor immune system, we’re going to be the first ones to get the flu, or a cold, or UTIs, or whatever. And people have chronic conditions that are related to our immune system—hence autoimmune conditions, where our immune system starts to attack us. So if our immune system is already causing lots of inflammation, and a virus comes

along, we’re going to go downhill real fast, because we don’t have a way to protect ourselves.

What are the best foods we should be consuming to boost our immune system?

Fresh fruits and vegetables. It’s as simple as that. People eat frozen vegetables, or cook them up and use them as a base for stews. People tend to eat grilled meats, and they’ll have a baked potato, or a sweet potato, because sweet potatoes are better than white potatoes, so we think—which is not always true. It depends on which foods your body thinks are enemies, and which foods your body thinks are good for you.

What’s the difference between consuming, say, a frozen head of broccoli versus a fresh one?

You’re going to get different enzymes, and the enzymes are what’s going to help process those nutrients through our immune system, and through our small intestine. Remember that 80 percent of your immune system is in your small intestine. If your immune system is impaired, or hypersensitive, or inflamed, you don’t absorb as many nutrients. That’s where people have metabolic syndrome, which is pre-diabetes or high cholesterol.

What foods should we be avoiding or limiting for optimal immune health?

It’s different for everybody. I can’t give you one food. I can say dairy isn’t the best, because they feed the cows corn and put them on antibiotics to beef them up and make more milk, instead of feeding them a grass diet. Milk is one of

the first foods that if anybody has a child with skin issues, take them off of dairy. Or if they have behavioral issues, take them off of dairy. But everybody’s different.

Some may assume that being on a diet means eating tasteless, bland food. Can participants in your programs still enjoy delicious food?

Oh gosh, yes. I’ve given recipes where people tell me, “this will become a staple in my household. I never knew that just grilling this food with a little salt would taste so good.” Or “Oh my God, I’ve never used paprika, or I didn’t realize rosemary was this good; I never would have used it on sweet potatoes.” So it really expands their knowledge of food and how the body functions, and how it actually speaks to you.

You also offer IV therapy for first responders?

We wanted to offer that so that first responders would have a means of staying healthier and boosting their immune systems, because they’re working very long hours, which is stressful. And it’s stressful working next to COVID people—even though they’re gowned and masked and taped, they could still contract it. In order to help them and improve their immune system, we offered in one hospital IV therapy, where they took a vial of nutrients. They could have a fellow nurse administer it to them. It was convenient for them, and it’s amazing when you bypass the gut and do nutrients through the vein, how much it will boost the immune system.

TO THE RESCUE PET ADOPTIONS, AT RESCUE FACILITIES LIKE PEGGY ADAMS, SOAR DURING THE PANDEMIC

There has been at least one sign of unadulterated good news to emerge from the coronavirus pandemic: Interest in pet adoption has skyrocketed to almost unprecedented levels.

West Palm Beach’s Peggy Adams Animal Rescue League, an institution for nearly 100 years, witnessed this change over the tumultuous spring and summer of 2020.

“To give you an idea of how the demand for pets spiked, our communications center, which takes in hundreds of phone calls every day, has 32

experienced a nearly 300-percent increase in calls inquiring about adoption, compared to the same time last year,” said Rich Anderson, Peggy Adams’ executive director and CEO. “I think it’s become clear that people are stuck at home, and a lot of people that, in the past, didn’t feel like they could give the time and attention to a new pet suddenly found themselves working from home, and had the kids out of school. It has turned out to be an incredibly beneficial time to introduce a new pet into a home.”

A beloved community shelter, Peggy Adams does it all, from pet medical procedures and humane education to behavior and training classes, in addition to its adoption service. It has been operating since 1925, surviving wars, a depression, hurricanes and 9-11.

But in the early days of the COVID19 pandemic, apprehension surfaced even at this most resilient of nonprofits—and even as it remained open as an “essential” business. As Anderson noted, “the common assumption was that animals were going to be surrendered and abandoned at very high levels and, secondly, that people would not be in the position to adopt.”

Yet, Anderson said, “the opposite has proven true on both counts. Fewer animals were abandoned than normal.

Pets were not being surrendered. And there was a huge demand to adopt a pet. Like most shelters in Florida and throughout the country, the challenge has been keeping up with that demand.”

The increase in adoptions not only provides fullness and comfort to the animal—it allows us humans to reap the health benefits of pet ownership.

“There have been a multitude of studies done on the health impacts of having a pet,” Anderson said. “Studies have shown that pets clearly relieve stress levels in people. There have been studies that show that they lower blood pressure. And they’ve been proven to prevent depression. So particularly during this pandemic, and particularly for people who live alone,

when you can’t socially interact with other people like you typically would, having a cat or a dog or another companion animal naturally helps you get through this challenge.”

Studies have also shown that pet owners tend to be in better physical health, too; those three to four walks each day don’t hurt. “We talk to people all the time who have adopted a dog from our shelter, and they report back to us that the dog has forced them to be more active, and get out of the house, and get some exercise,” Anderson said. “There are endless cases of people who adopt dogs and see an improvement in their health and their waistlines.”

Anderson said, “it’s humbling to be chosen among the hundreds of nonprofits in Palm Beach County. I think most people really don’t have a good sense of the tremendous impact nonprofits have each and every day in Palm Beach County.

And in many cases that impact is saving lives, whether it be animals or people. What the Chamber is doing is really commendable and important to raise awareness for the work that is happening, and being done by nonprofits of all shapes and sizes throughout our community.”
