A VALUABLE RESOURCE Welcome to the eighth edition of the free Fauquier County Community Resource Guide which provides a convenient listing of a wide range of the health and human services that provide a “safety net” for the citizens of our county. The Table of Contents reflects the many categories of health and human services provided by this network of service organizations including government agencies, nonprofits, religious groups, and other human service programs and companies in the county. This year, for the first time, the Guide includes a Spanish-language section. The printed version of this Guide is available throughout the county at the many sites listed on the next page. The online version is available at www.fauquierresources.com. We want to thank the Piedmont Publishing Group whose commitment to community service and expert professional production make it possible to provide this valuable Guide at no cost to the community. The free listings in this guide are for organizations that are providing health and human services. We appreciate the support provided by the many companies and organizations who purchase ads in the Guide. The Partnership for Community Resources (PCR) creates the Resource Guide each year

as a service to the community. If you have suggestions for future editions, please contact PCR via Facebook. PCR is an informal group of staff and volunteers from numerous nonprofit, government, and for-profit human services agencies in and near Fauquier County. “Our mission is to strengthen, support, raise public awareness, and enhance collaboration and communication between…agencies concerning the health and human service needs of our community.” PCR meets at 10:00 a.m. on the first Friday of each month (except for major holidays) in the Sycamore Room of Fauquier Hospital. Typically, at these meetings participants briefly share information about their own agencies (services offered, coming events, needs, etc.), followed by a presentation from one of these organizations highlighting specific community needs and services. Anyone providing services such as those listed in the Guide is invited to attend these PCR meetings. Please inform your friends and neighbors regarding the availability of this new edition of the free Fauquier County Community Resource Guide.

APS investigates reports of abuse, neglect and exploitation of adults 60 years of age or older and incapacitated adults age 18 or older to stop the mistreatment or prevent further mistreatment. Services offered may include home-based care, transportation, adult day services, and adult foster care in order to protect the adult. Services may also be arranged for individuals in emergency situations who lack the capacity to consent to services. To report suspected adult abuse, neglect or exploitation, call your local department of social services or the Virginia Department of Social Services’ 24-hour, tollfree Adult Protective Services hotline at: (888) 832-3858.

If you care about Fauquier County seniors, you should look into Aging Together, which forms partnerships and finds funds to expand and enhance services for people over 60 in Fauquier, Rappahannock, Culpeper, Madison and Orange Counties. We identify gaps in service and facilitate educational programs, caregiver supports, volunteer transportation and much more. Please join our Fauquier Team that meets the third Tuesday of each month at 9:00 am at Fauquier Hospital in the Sycamore Room. The “VIP Senior Gold Card,” available to any Fauquier County senior citizen age 60 and older, is an admission ticket to almost any Fauquier public school event that charges admission. To obtain a VIP Senior Gold Card, apply in person at the front desk of any Fauquier County public school or download the brochure from the county schools website. Fill it out and take it to any FCPS school. You must present a driver’s license or other proof of date of birth at the time of application. You will receive your VIP Senior Gold Card in the mail. The card is non-transferable. The VIP Senior Gold Card applies only to school events held in Fauquier County.

Registration forms are available from May 15 to July 15 at Dept. of Social Services, Fauquier Community Food Bank and Fauquier FISH Food Pantry. Weekend Power Pack Program: Eligible children receive a backpack of food to take home from school each Friday during the school year. Bags typically contain enough food to feed a family of four 3-meals per weekend day. Request information at school from the principal or guidance counselors on your child’s eligibility. Baby Supplies: Fauquier FISH Food Pantry stocks diapers and wipes. A limited supply of formula is also available. Bring in your formula container or label. Over a period of 10 weeks, girls in the 3rd through 5th grades participate in an after-school program like no other. The Girls on the Run lessons encourage positive emotional, social, mental and physical development. Heart & Sole is Girls on the Run’s new program for middle school girls (grades 6th-8th)! Heart & Sole creates a positive, structured space for middle school girls to learn about themselves, explore new ideas, cultivate empathy, strengthen connections and develop life skills that will help them as they move through adolescence and beyond.

Decoding Dyslexia Virginia is a grassroots movement focused on linking families to resources, support, and educational interventions for dyslexia. We aim to raise dyslexia awareness, empower families to support their children and inform policy-makers on best practices to identify, remediate and support students with dyslexia in Virginia. DDVA Haymarket meets monthly at The Piedmont Club in Haymarket and offers a variety of educational seminars and resources to local parents.

Demand Service is curb-to-curb transportation service open to the general public by reservation. All equipment is lift-equipped and handicapped accessible. Rides can be booked as far out as two weeks in advance but at least 24 hours in advance. Riders are encouraged to book earlier rather than later. Scheduled riders should be prepared to leave 20 minutes before or after a scheduled ride appointment time. Passengers unable to ride by themselves are permitted to bring a personal assistant for free. Trips will be arranged where logistically possible. Citizens who live within a 3/4 mile of a fixed route are asked to board the bus along the fixed route. Citizens may also complete an American with Disability Act application if they live within the 3/4 mile range and need American with Disability Act services. Service hours are Monday, Wednesday, and Friday from 8:00AM-5:00PM.

Auxiliary Grants Program. An auxiliary grant from DSS is an income supplement for individuals who receive Supplemental Security Income (SSI) and certain other aged, blind, or disabled individuals who reside in a licensed assisted living facility (ALF) or an approved adult foster care (AFC) home. Payment is issued to an individual monthly, to be used with a designated amount of their monthly income to pay an ALF or AFC maximum monthly rate. This rate is determined by the Virginia General Assembly and is adjusted periodically. Eligibility: Prior to admission to an assisted living facility, an individual must be assessed by an adult services social worker at their local department of social services or another qualified assessor. The individual applying for an auxiliary grant, or the individual’s designated representative, should contact an assessor and request the assessment. The Virginia Department for the Deaf and Hard of Hearing (VDDHH) works to reduce the communication barriers between persons who are deaf or hard of hearing and those who are hearing, including family members, service providers, and the general public. This is accomplished through three programs: Virginia Relay (Traditional Relay Service and CapTel Relay), Interpreter Services (Interpreter Coordination for Virginia Court System and state agencies, and VQAS Screening for Interpreters), and Outreach, who provide direct services to Virginians with hearing loss. Rehabilitation Center provides comprehensive, integrated medical and vocational rehabilitation services to enhance an individual’s independence and employability. Michele Arft is the current domestic violence resource specialist providing advocacy and support to victims of domestic violence and stalking. She provides information and resources that are available to victims based on their particular situation (i.e. emergency shelter, transitional housing, support groups, legal aid, etc.) and handles issues pertaining to Protective Orders where victims are assisted with completing petitions and accompanying them to court for support. DSS: FAUQUIER COUNTY DEPARTMENT OF SOCIAL SERVICES Adult Protective Services (APS) 540-422-8400

Anyone living in Fauquier County who is 60 years or older or who is incapacitated and 18 years of age or older may receive protective services if services are needed, which is determined by a DSS social worker investigating reports or suspicions of abuse, neglect, or exploitation. Adult Protective Services are provided regardless of the person’s income or financial resources. APS completes investigations in almost every setting including the home, assisted living facilities, nursing facilities and hospitals. APS does not investigate allegations of mistreatment in jails or prisons. To report suspected abuse or neglect of a vulnerable adult (aged or with disabilities) or to get more information about adult abuse and neglect, call 888-832-3858 any time or the Fauquier office 540422-8400 during office hours

Adult Services

Adult Services coordinates and provides a range of services to maintain safe living conditions for incapacitated or elderly adults. These include assistance with emergencies, home-based companion services, in-home assistance with some activities of daily living, adult foster care and respite care, nursing home preadmission screening for impaired adults, Medicaid communitybased care screenings and arrangement of services, help with Assisted Living placement and nursing home placement, and case management for adults who require coordination of services.

Medicaid has several medical assistance programs that pay for health care visits for children and certain adults who are unable to pay for needed services. Covered individuals may be children, pregnant women, people with disabilities or elderly individuals. This is known as categorical eligibility. There are special provisions and several types of coverage for which you may qualify depending on your income, family size and circumstances. Individuals with severe mental illness may apply for Medicaid through Rappahannock Rapidan Community Services at 540-825-3100 as there are specific screening processes for “GAP” coverage. Call 855-6354370 for more information. Screening for financial eligibility may include an assessment of both income and resources depending on which of the many Medicaid programs for which you may qualify. Child Care Assistance Program 540-422-8400

This program provides funding to enhance the quality, affordability, and supply of childcare available to Virginia’s families. To qualify, you must be a resident of Fauquier County and fall within the income guidelines, and must be working or attending school on a full-time basis. Program goals include providing low-income families with the financial resources to find and afford quality child care for their children, providing childcare to parents trying to achieve independence from public assistance, and promoting parental choice in the selection of child care. It also aims to empower working parents to make their own decisions on the child care that best suits their family’s needs and provides consumer education to help parents make informed choices about child care. Child Protective Services 540-422-8400

Virginia statute requires social services departments to investigate all reports of possible child abuse or neglect and to arrange protection for children when protection is needed. The purpose of child protective services intervention is to protect children, to preserve families whenever possible, and to prevent further abuse and neglect. DSS encourages anyone who is aware of possible child abuse or neglect to call the 24 hour child abuse hotline: 800-5527096 at any time. To get more information about a situation you may be unsure meets the definition of abuse or neglect or to report suspicions of abuse or neglect, call the Fauquier office at 540-4228400 during office hours. An abused or neglected child is any child under 18 whose parent, or any other person responsible for the care of the child causes or threatens to cause, a physical or mental injury (except for an accident), who fails to provide adequate food, clothing, shelter, medical care, or caring support, who abandons the child, who fails to provide the kind of supervision necessary for a child’s age or level of development, or who commits, or allows to be committed, any sexual act involving the child.

Children’s Services (CSA) 540-422-8400

Services under the CSA may be available to a child who meets at least one of the following descriptions: youth who require private placement for special education, youth who are in foster care or eligible for foster care services, youth who are eligible for services through a Child in Need of Services determination or youth who have significant emotional or behavioral problems and may require services not available from any single agency, require services of multiple agencies, or may be at risk of residential placement. While most of the children who qualify for CSA services are already under the purview of one of the member agencies (Juvenile Court Services, Social Services, Community Services, or the Public Schools) you may call the Children’s Services Coordinator at 540-422-8400 to find out if you and your child are eligible for services. The department provides limited emergency assistance largely to prevent homelessness. Applicants must be Fauquier residents who are ineligible for federal aid such as TANF, have an emergency need resulting from a recent change in circumstance and meet restrictive income and resource limits. Energy Assistance Programs 540-422-8400

This program has three parts: fuel assistance, crisis assistance and cooling assistance. Each has different application dates: fuel assistance from the second Tuesday in October through the second Friday in November; crisis assistance (for energy related emergencies) from November 1 through March 15; cooling assistance from June 15 through August 15. In addition, the department can help residents access energy assistance programs operated by utility companies, such as Dominion. Fauquier County Department of Social Services, (FCDSS) provides a wide variety of benefits and services. Benefits include Medicaid and SNAP (formerly known as food stamps) Temporary Assistance to Needy Families (TANF), daycare assistance, energy assistance and emergency assistance. Services include protective services to vulnerable adults and children, preventive services to families with children and vulnerable adults and employment services .Application for benefits is made on-line at https://commonhelp. virginia.gov, or the Fauquier office. Eligibility determination is expedited when application is made on-line. If you wish to apply at the office, it is recommended that you call first. To qualify for benefit programs and services, you will be asked to verify such matters as residency, income and expenses, resources such as bank accounts, insurance, motor vehicles owned, etc. Hours: Monday through Friday, 8:00 am-4:30 pm. Foster Care Services 540-422-8400

DSS also provides services to foster children and their foster parents. Foster parents are professional parents who enjoy parenting and are willing to share their home, time, energy and love with children who have special problems. The children range in age from newborn to 18 years. Some have been victimized by abuse or neglect. Some may be physically or mentally challenged; some may have emotional or behavioral problems. Sometimes, as a result of abuse, neglect, abandonment or a death in the family, courts decide that children must be temporarily separated from their families. To find out more about becoming a foster parent, visit http://www. dss.virginia.gov/family/fc/. A child’s stay in foster care may be as short as overnight or longer depending on the permanency plan for the child. The goal is to reunite the children with their own families. When this is not possible, parental rights may be terminated and adoption becomes the goal for child. Often foster parents are the first choice for the child’s adoptive home. In foster care, the department has legal custody of the child and assumes the ultimate responsibility for the child. A family will receive a maintenance payment on behalf of the child to help with expenses while a child is in their home. Children have medical and dental coverage while in foster care.

Medicaid for Long-Term Care (LTC)

LTC Medicaid covers long-term care in a community-based setting for individuals whose mental or physical condition requires nursing supervision and assistance with activities of daily living. Medicaid requires both a functional and financial screening process prior to paying for long-term care services. A preadmission screening is required prior to admission to a nursing facility or waiver service if Medicaid is expected to provide payment. You may call Social Services to request a long-term care screening. Ask for Adult Services. Supplemental Nutrition Assistance Program (SNAP)

SNAP (formerly the Food Stamp Program) helps families buy nutritious food and seeds or plants to grow food. The benefits are distributed to families electronically. To apply for SNAP, go online to https://commonhelp.virginia.gov or call the Fauquier office.

Temporary Assistance for Needy Families (TANF) TANF provides temporary financial assistance for eligible families with minor children in the home, who are financially needy and meet other requirements as well. An interview is required. TANF payments depend on the size of the family; a family may qualify while receiving income from other sources. Applicants must cooperate in naming the parents of all eligible children and help establish paternity. Some individuals qualify for TANF may be required to participate in VIEW, the Virginia Initiative for Employment not Welfare. VIEW participants can earn an income in addition to their TANF benefits but the total income may not exceed the federal poverty level for their family size. The Work Place – Career Resource Center

The Work Place is open from 10 am–3 pm and is available to all county residents to assist in finding employment. The Work Place is not an employment agency but encourages independent job search with multiple resources at your disposal. Staffed by the department and Literacy Volunteers of Fauquier it offers the following one-stop services free of charge: job postings direct from employers, various Web pages and local papers, assistance with establishing new email accounts and job search Web pages, computer and Internet access, fax and copy machine use, resume and job preparation assistance by appointment (call 540-422-8422), and referral and registration for free computer classes.

Fauquier County Fire, Rescue, and Emergency Management is dedicated to providing fire protection, emergency medical services, and related life safety functions with fiscal integrity while maintaining a balanced volunteer and career service. The Department of Fire, Rescue and Emergency Management (DFREM) strives to reduce threats to life, safety, property, and the environment of Fauquier County residents through education, prevention, and effective response to fire, medical, environmental, and other emergencies. Fire and rescue services in Fauquier County are divided into two organizational groups: DFREM and the Volunteer Fire Rescue Association (VFRA). DFREM has five divisions: 1) The administrative division provides emergency planning and disaster coordination, as well as coordination of


services supporting the VFRA. It includes volunteer recruitment, orientation and retention, budget preparation, procurement, management, service supplementation with career firefighter, paramedics, insurance and risk management, mandatory annual medical physicals, and human resources support. 2)The operations division consists of the firefighting, emergency medical service and hazardous materials responses. 3) The prevention division consists of fire prevention and life safety, including code enforcement, life safety inspections, fire investigations, plans reviews and hazardous materials cleanup. 4)The training and logistics division is responsible for training and recertification of over 600 career and volunteer members, EMS compliance, mandated exposure control programs, medical direction and quality assurance at all levels. 5) The emergency management division maintains emergency operations, hazard mitigation and hazardous materials transportation plans. This division develops incident-specific plans and coordinates responses to emergencies by mobilizing personnel and resources. This division also facilitates training and exercises, manages long term mitigation and recovery programs following a local disaster, provides public education to citizens and groups, and maintains /operates the Emergency Operations Center.

PATH Resource Center

People Helping People (PHP) was incorporated in 1990 as a nonprofit (501) (c) 3, all-volunteer organization. The mission of PHP is to provide financial emergency assistance to Fauquier County residents undergoing hardship due to circumstances beyond their control, i.e. medical issues, loss of employment or death in the family. We assist with paying utility bills to avoid disconnection of service; rent/ mortgage to avoid eviction from home and thereby preventing homelessness. We also provide gasoline vouchers to occupants of the local family shelter. We are funded by local businesses, local churches, private citizens, Piedmont United Way, and other grants. Hospice care focuses on maintaining quality of life during the last chapter of a person’s life. In most cases, it is paid for by Medicare. A physician’s referral is required to start hospice care. Several hospices serve Fauquier County; families can choose which they prefer. Hospice provides support to the patient and family by keeping the patient as comfortable as possible, while maintaining his or her dignity and quality of life and allows the patient to remain at home as long as possible. Hospice also serves people living in institutions. When an individual is stricken with a debilitating disease such as dementia or cancer, families may need in-home assistance. In some cases, a doctor may write a prescription qualifying a patient for in-home medical visits. In other cases, families may turn to a nonmedical personal care agency, or to a company that provides both. The 20W Court Services Unit (CSU) is governed by the Department of Juvenile Justice and covers Fauquier and Rappahannock counties. It handles complaints concerning minors accused of violating a criminal statute, as well as child support, custody, and protective order complaints. CSU staff supervise youth that have been placed on probation, parole or require some type of monitoring supervision. CSU provides public protection in


these areas. Its biggest goal is to target services to high-risk youth and identify the right interventions at the right amount. Michele Arft is the current domestic violence resource specialist providing advocacy and support to victims of domestic violence and stalking. She provides information and resources that are available to victims based on their particular situation (i.e. emergency shelter, transitional housing, support groups, legal aid, etc) and also handles issues pertaining to Protective Orders where she can assist victims with completing petitions and accompanying is an educattonal 501 (c)(3) nonprofit organization them to court for support. PDRC provides mediation services on a wide array of issues including parent-teen, landlord-tenant, consumer-merchant, elder care, divorce, education and business. Mediators do not take sides or tell people what to do. They are neutral and assist people in discussing what’s important to them and how best to resolve the issues. PDRC mediators are certified by the Supreme Court of Virginia. Fees are on a sliding scale and services are intended to be affordable by all who seek them. The Center also offers training and workshops in conflict resolution, effective communications and negotiations for children, youth and adults. PDRC’s Restorative Justice Program works with victims of crime and offenders, providing a safe place to meet face-to-face to talk about the harm caused and repairing the harm. MEDICAID AND MEDICARE

Despite the similarity in names, Medicaid and Medicare are two completely different programs. Contact DSS for Medicaid issues. For Medicare issues, including signing up for Part D coverage, contact Rappahannock-Rapidan Community Services at 540-825-3100 and ask for Poppy Foddrell, Aging Support Coordinator at ext 3427. Medicare is a federal health insurance program for people age 65 or older, people under age 65 with certain disabilities, and people of all ages with end-stage renal disease (permanent kidney failure requiring dialysis or a kidney transplant). Medicaid provides health coverage to low-income children and provides assistance for some low-income adults, such as pregnant women, parents of dependent children, blind or disabled individuals, individuals under hospice care, individuals age 65 or older, or other medically needy criteria. In Virginia, the Governor ‘s Department of Medical Assistance Services’ (DMAS) is the agency that administers Medicaid and the State Children’s Health Insurance Program (CHIP). In Virginia we call our children’s program FAMIS. Check out www.famis.org to learn more. Part D Prescription Drug Coverage - most people will pay a monthly premium for this insurance; private companies provide the coverage. Beneficiaries choose the drug plan and pay a monthly premium. Like other insurance, if a beneficiary decides not to enroll in a drug plan when they are first eligible, they may pay a penalty if they choose to join later. Previously called Rappahannock-Rapidan Community Services Board-Area Agency on Aging, RRCS is a large, multi-faceted agency serving Fauquier and four other counties. Its main office is on Bradford Road in Culpeper but it also runs facilities in Fauquier County, including the senior center and adult day care at the Fauquier Community Center on Shirley Avenue, and Behavioral Healthcare Services on Hospital Drive, as well as Fauquier Bridges. RRCS offers a wide variety of programs to Fauquier residents of all ages, including transportation services for the elderly or disabled, home-delivered Meals on Wheels for seniors, the adult day care program and the Infant Toddler Connection early intervention program. In addition RRCS programs offer protective money management, intellectual disability services, mental health services, and substance abuse services. RRCS operates supportive housing for those with mental illness or intellectual disabilities. In addition to the senior center and its hot meal program, a variety of aging services for seniors are available from RRCS. In particular, the agency has a staff of experienced caseworkers called “aging support coordinators” who respond to calls from seniors and their families and help seniors resolve problems, answer questions, and connect with community resources. Support Coordination Services are the entry point for all RRCS residential and day support programs. Support Coordinators evaluate, link, monitor, and advocate for services to meet the needs of our clients in a variety of settings. Intellectual Disability Support Coordination provides services to children or adults who have a diagnosis of intellectual disabilities prior to the age of 18. Expanded or waiver services are offered through the Intellectual Disability Waiver Program for those who meet the additional eligibility requirements of the waiver program. Behavioral Health Support Coordination provides services to adults 18 and older who have been diagnosed with a mental illness and need supports to maintain stability in the community and work towards recovery.
