The importance of effectively managing swabs Every day thousands of operations are carried out using thousands upon thousands of swabs. Each one of these operations and the swabs used pose a potential threat not only to the patient but also to the staff handling them.

A patient is at risk of a swab being left inside the surgical cavity, which can result in the surgical wound becoming infected. This has potentially serious consequences and could even result in death.

The staff acting as circulators in theatre are potentially at risk from cross infection due to unnecessary handling of dirty swabs. This could put them in danger of HIV, Hepatitis A, B and C, MRSA, Streptococcus

and Staphylococcus, along with other bacterial infections.

Retained swabs are one of the ‘never event’ threats to patient safety. The National Patient Safety Agency found 92 incidents occurred in 2008 that related to retained swabs.

Add to the patient safety threat the financial impact. A review of the NHS Litigation Authority (NHSLA) claims database identified 186 maternity claims for retained swabs, with an incident date between 1 April 2000 and 31 March 2010.

These claims, which occurred following both operative and vaginal deliveries, resulted in an estimated total value of £3 million (Ten Years of Maternity Claims: An Analysis of NHSLA Data - October 2012).

Although UK statute law does not dictate what system or method of accountable items, swab, instrument and needle counts should be performed within a perioperative environment, the law is quite clear that healthcare practitioners have a ‘duty of care’ to the patient.

Unintended retained objects are considered a preventable occurrence and careful counting and documentation can significantly reduce, if not eliminate, these incidents (Rowlands & Steeves 2010, AfPP 2016). A count must be undertaken for all procedures where countable objects, for

example swabs, instruments and sharps, are used. The Association for Perioperative Practice 2016 Standards and Recommendations for Safe Perioperative Practice recommends there should be standardisation of how countable items are named and referred to across one organisation and referenced into the local policy, to minimise the risk of confusion among staff.

To ensure swabs are not retained and are treated safely, an effective swab management system, backed up with a detailed protocol, is required. Without a swab management system in place, items can be lost or retained in a body cavity, causing the need for expensive, distressing and unnecessary restorative surgery. It also increases the risk of items being misplaced in clinical waste or linen, causing hazards to other healthcare professionals.

The AfPP’s recommendations in Accountable items, swab, instrument and needle count (2012) state that swabs should be in full view of the operating surgeon and anaesthetist, where applicable, throughout a clinically invasive procedure.

Used swabs and packs should be counted off the sterile field. Although no specific technique is stated,

the chosen technique should be safe and incorporate infection control measures in conjunction with standard precautions. All items should be fully opened by the circulating practitioner and placed into an appropriate contained disposal system.

As part of an effective risk management strategy, it is recommended that a suitable contained disposal system is in place. If appropriate, the system used should be sealed and the number of contents recorded on the outside of the disposal system. If there is a discrepancy in the closure counts, all bags or containers should be opened and their contents recounted.

The swab rack is now viewed as an old fashioned concept, which causes more potential problems due to exposure of blood than is proven to be safe in today’s theatre environment (Mumford M. 1991). The Swabsafe™ swab management system meets with the AfPP’s current guidelines for swab counting (AfPP 2016). With NHS budgets under increasing pressure, healthcare providers can’t afford to not have a watertight policy in place.

A duty of care to patients and healthcare colleagues is vital at every level and for every operation.

Rising to the challenges of manufacturing for day surgery In the UK, day surgery has been one of the key strategies used to modernise the NHS, addressing the need to reduce waiting times, improve cost efficiency, and most importantly make health services more patient-centred.

‘True’ day surgery is seen as the admission of a patient to hospital for a planned surgical procedure requiring full operating theatre facilities and/ or a general anaesthetic, who returns home on the same day.

In other words they are cases which would not be considered as traditional outpatient activity.

As a specialism day case has changed and developed with the times, but as it has become the norm for more and more routine surgery the benefits have been increasingly recognised: • Patients receive the appropriate treatment and are able to recover in their own homes. Cancellation of surgery due to emergency pressures in a dedicated day surgery unit is unlikely and the risk of hospital acquired infection is reduced.

• Clinicians are able to provide high quality care for appropriate patients, releasing inpatient beds for more major cases.

• Hospitals improve their throughput of patients and reduce waiting lists.

This enhanced weight capacity is also a significant factor for lifting and handling considerations. In a

traditional operating scenario, a patient would be admitted to a ward where they would get into a bed, before being transferred to a trolley to be wheeled into the operating theatre and transferred to the operating table. After the operation, the whole process would be reversed, meaning the patient would need to be transferred between equipment four times. A true day surgery trolley removes the need for any such physical transfer of the patient.

A choice of head and foot sections are available with the system which can also be used with most operating table accessories so, in addition to general surgery, it is now widely used in gynaecology, urology, endoscopy, orthopaedics, dental and ENT. Over time, other specialist accessories have also been added, such as for shoulder arthroscopy and ophthalmics.

Patient trolleys For any patient having to undergo a stay in hospital they will probably have to climb up onto a trolley at some point during their experience and here we will concentrate on theatre trolleys.

There are various requirements for a theatre trolley: • Manoeuvrability to help reduce moving and handling risks • Durability, weight • Electronics to assist with ensuring that the patient surface is rigid and stable • Comfort

Some trolley systems are both a patient trolley and an operating table, which can provide a solution for day care providers.

Most patients when undergoing a surgical intervention will walk to theatres. They will then be helped onto a trolley for their anaesthetic intervention; once asleep they will be wheeled into theatres to undergo their operative care.

As patients spend an increasing amount of time on trolleys, manufactures are developing trolleys that make the patient experience as comfortable as possible. If the patient is to be on the trolley for any length of time then providing them with access to any controls that help them to reposition the trolley can help maintain their independence without requiring involvement from a practitioner.

Some trolleys have been developed to assist with and simplify clinical procedures, particularly in a clinical emergency i.e. drop down sides to allow easy access to the patient. Many trolleys will have bumpers to cushion any knocks during travel.

Infection prevention is theatres is essential and trolleys are part of the point of care risk for a patient, therefore most surgical trolleys now have plastic elements that include an antibacterial additive to help fight bacteria. The majority of the trolley structure is steel with an epoxy resin that allows for easy cleaning.

Standard precautions are the basic infection prevention and control measures necessary to reduce the risk of transmission of microorganisms from recognised and unrecognised sources of infection. AfPP Standards and Recommendations for Safe Perioperative Practice (2016) states that the operating table or trolley should be decontaminated between cases. Cleaning with detergent and water and drying thoroughly is usually adequate unless there is extensive contamination.

Operating table/trolley mattresses that are torn or damaged in any way should be repaired immediately and not taped in order to prevent contamination with blood or other body fluids and potential pathogens. Obviously, if this is not possible the mattress should be decommissioned and removed from circulation.

When speaking to colleagues the main things they look for in a trolley are: • Opportunity to buy necessary attachments such as IV drip poles, safety rails etc.

Bender UK secures exclusive deal to offer world-beating LED surgical lights to hospitals.

Ulverston-based Bender UK has become the exclusive supplier of the advanced range of Q Flow™ LED surgical lights, developed by Merivaara of Finland.

The new range was formally launched at the Operating Theatre Conference 2017: The Future of Surgery, in Manchester on May 23rd 2017, and Bender

Merivaara lights now form part of Bender UK’s comprehensive range of healthcare products and services which includes critical care IPS/UPs power solutions, surgical equipment, turnkey theatre packages, service and technical support.

Merivaara is an established supplier of medical equipment for operating theatres but the Q-Flow™ surgical lighting range is the first to be manufactured by the company. The new lights provide advanced performance in a number of key areas, including bestin-class colour rendering which is vital for surgeons and theatre staff to monitor the health and well-being of patients.

IntraTemp at Princess Alexandra Hospital NHS Trust ™

Princess Alexandra NHS Trust provides acute services to a population of around 350,000 in Harlow and the surrounding areas. Its main facility is the 500-bed Princess Alexandra Hospital.

Susan Lord is the Team Leader for General, Vascular and Laparoscopic Surgery at Princess Alexandra Hospital.

She is also the Educational Lead for the Association for Perioperative Practice (AfPP) Anglia regional team. Sue approached Ecolab at the AfPP residential exhibition in 2016 to discuss irrigation fluid warming using IntraTemp™.

Irrigation fluid is a frequently overlooked factor in prevention of inadvertent perioperative hypothermia.

Failure to maintain perioperative normothermia has well-documented negative clinical effects (Frank et al 1997, Sessler 2001, Rajagopalan et al 2008).

NICE guidance states that “…all irrigation fluids should be warmed in a designated cabinet, thermo-controlled to 38-40°C (NICE 2008).

Ecolab’s local Key Account Manager visited Sue at Princess Alexandra to consult on their current fluid warming practice.

They created a trial and implementation plan to address the issues they’d identified using the IntraTemp™ fluid warming system. The plan identified which theatres and procedures most required accurate irrigation fluid warming. These included laparotomies, bowel resections, breast augmentation and various gynaecological procedures.

Practice at the trust reflected current NICE guidance.

Irrigation fluid was warmed in cabinets, then brought to theatre and decanted into an unheated standing basin to cool. However, the warming cabinets were located a few minutes’ walk from the main theatres, which meant retrieving fluid was timeconsuming and disruptive.

There was a clear need for a system to deliver and maintain irrigation fluid at a known temperature at the point of use.

With this plan in place, two IntraTemp™ units were supplied for a 3-week evaluation period.

The Key Account Manager was available throughout the evaluation process to collect feedback, monitor implementation and answer questions. He delivered training to the entire team at various times as required, certifying them in safe and effective use of IntraTemp™.

IntraTemp™ was so popular that the trust requested an additional unit immediately after the trial.

Staff praised its ease of use, ability to quickly and accurately verify fluid temperature and the convenience of having irrigation fluid readily available.

The IntraTemp™ systems are now being used by the trust for around 300 cases per year.

Ecolab continues to work with Sue and the trust, supporting the IntraTemp™ system and the team with continuous improvement initiatives.
