Stock management in theatres The supplies department at Nuffield Health York Hospital procures, delivers and manages all the medical consumables used throughout the building. We basically buy in everything except food and drugs!

We are a team of three and our working schedules are designed (and flexed) to reflect the activity of each clinical department. My main goal in my working life is to make things easier and simpler for everybody, by working efficiently and streamlining processes.

The theatre department comprises three theatres and a recovery area, and performs a range of surgeries including orthopaedic, general, ophthalmic, spinal and urology.

There is one sterile store room for the stock that is topped up, plus a prosthesis store room and trolleys for each speciality.

We follow the Nuffield Group Procurement processes for all our stock management, using Lawson as our software system.

There are standardised products (swabs, surgeon gowns etc.) that Group Procurement have negotiated on national contracts, that we are expected to default to, and ranges of nonstandardised products. These can be ordered as required based on clinical need and surgeons’ preferences.

Successful stock management in theatres is wholly reliant on clear and trusted communications between supplies and theatre staff. The trust has to be there for the theatre staff, so that they know what they want will be where they want it, when they want it.

For supplies staff, it’s that the theatre staff will remember to tell us what they’ve used, when they’ve used it and on whom it’s been used! I am also reliant on them checking their requirements the day before a procedure, so if anything has been missed, it can then be ordered.

The supplies department at York is on the same floor as theatres and I wear scrubs so that I can go into each theatre as required to check stock levels, do a stock take, and answer queries and so on while theatre staff are scrubbed. It also means I am visible to theatre staff, adding to their reassurance that supplies are an integral part of their team.

The Nuffield processes for managing stock are straightforward and based on the principle of an upcoming demand allocating stock on the system. If this allocation results in the stock reorder point being triggered, an order is created and the stock allocation fulfilled once the stock is received. The upcoming demand is created by the theatre clerk extracting information from the Patient Management System, which lists all inpatient procedures per theatre, per surgeon, and per date. This information is used to create the pick sheet on Lawson. This is the list of stock required by the surgeon to complete the procedure and can range from a single procedure

pack, through to three pages of individual items. We receive notification that these have been put on Lawson at least a week in advance of the procedure date, allowing us to both pick the stock and replenish items that are currently unavailable in time for the list.

We pick the required sterile consumables (swabs, drapes, syringes, hernia mesh etc.) and bag up into individual clear plastic sacks about a week before the procedure date. These are stored in our main stores, and taken through to theatre the day before the list. Theatre staff then collate them with the instrument trays, prostheses, sutures, bone cement, saw blades etc. that they will need for each patient.

Stock physically held in theatre falls into various different categories, depending on the item’s value, type (e.g. prosthesis) or frequency of use.

Their stock requirements are reviewed and agreed between the supplies manager and theatre manager /speciality team leaders. Supplies agrees to supply the patient and procedure specific stock the day before, to replenish used consignment stock within 48 hours, to keep agreed levels of “inventory” stock (£25+ in value or an owned prosthesis), replenished also within 48 hours, and to top up their clean utility store on a daily basis. >>

>> The store room contains a wide range of items from ophthalmic forceps, to intubating laryngeal mask, to surgeons’ gloves. The stock levels are based on average daily usage, with a bit of leeway to allow for particularly busy lists or anaesthetists’ specific requirements. When calculating stock levels, I’ll also take into consideration pack size versus the fiddle factor of counting out a specific number of units from a box of 50. Often it’s more efficient to put the full box into the store, especially with low value items such as syringes, examination gloves and needles.

Our prostheses fall into two categories; owned and consigned. The owned

prosthesis includes things like low value forefoot screws and plates, and grommets. The consigned stock includes orthopaedic, breast and ophthalmic implants. This stock is counted on a monthly basis to ensure compliance with consignment agreement, and to capture any missed charges that should have been costed to a patient’s charge account. All consignment and inventory stock is marked up with a removable yellow sticker, comprising a bar code, product description and identifying code as to it being consignment or inventory.

This information is used for both stock decrementation and stocktaking.

The inventory stock captures all the higher value items used, such as bone cement, laparoscopic scissors, saw blades, orthopaedic anchors etc. This is managed in a similar way to consignment stock.

Post procedure, theatre staff return the pick sheets to supplies, having attached any yellow bar code sticker from products they have used. This stock is decremented to trigger a re-order, and the cost linked to the patient account. All costs for each patient can therefore be totalled and reviewed; including the costs for the pick bags, additional consignment or inventory stock, any pharmacy

charges, surgeon and anaesthetist fees and so on.

We try and ensure the smoothest and most efficient of supply processes for the theatre staff and the operating teams; what we all do in healthcare is hugely important, but in having efficient and simple processes to follow, it needn’t be overly stressful!

Is inadvertent hypothermia putting your patients at risk?

Even mild hypothermia - when the patient’s core temperature drops below 36°C - can increase post-anaesthesia recovery time, morbidity and length of stay1.

Maintaining normothermia is a key element of the surgical pathway, and theatre staff are familiar with measures like warmed IV fluids and forced air warming. But irrigation fluid is often overlooked.

NICE guidelines state that fluids should be warmed to 37°C and that all irrigation fluids used intraoperatively should be warmed in a thermostatically controlled cabinet to a temperature of 38– 40°C2. However, fluid temperature drops by 1°C per minute after removal from a warming cabinet.

Peskett Solutions Is a lubricant a contaminant or a crucial necessity for maintaining the proper operation of a surgical instrument?

Most instrument manufacturers recommend the use of lubrication to ensure the correct functioning of their surgical instruments and to minimise wear and tear, thus extending the life of the instrument.

Unfortunately, there is a trend

away from the use of lubricants on surgical instruments due to the fact that a lubricant is considered a contaminant.

This definition does not apply to a water-soluble lubricant; a residue would be a more precise definition.

You can use a water-soluble lubricant before sterilization; however the use of oil-based lubricant is discouraged. The ideal location for using the lubricant is the packing room before sterilization. However, if the lubricant dislodges dried contaminates, the instrument will require returning to the decontamination area for reprocessing, which may include soaking in a lubricant bath until all traces of the soil lodged in the joint are removed.

By introducing the following safeguards into your lubricating routine, you will be able to reduce surgical instrument failures and extend their life span.

By ensuring that only the minimum amount of lubricant required is used, the level of lubricant residue will be totally harmless. By not using a lubricant we are running a greater risk of the instruments failing in use, than the risk from lubricant residue on the instrument causing contamination of the operation site.

• Only use a watersoluble lubricant.

• Ensure the product has been tested to be totally steam sterilizable and steam penetrable.

• Check the lubricant’s COSHH(SDS) sheet, section 3 -Hazard Identification Signs/ Symptoms of Exposure, to ensure that the product will not react with tissue.

• Restrict the use of the lubricant to the area of the instrument requiring lubrication.

• Test the action of the lubricated joint to ensure penetration.

• If the lubricant dislodges dried contaminants from the joint, return to the decontamination /wash area for reprocessing.

• When introducing a lubricant into the department, ensure that the manufacturer of the lubricant supplies a method of residue testing following steam sterilisation.

• Most importantly ensure the manufacturer backs up any claims with independent test results.
