The communities, services, programs, and facilities in this guide are designed with transition in mind: to help you navigate the process and arrive at an improved quality of life. That may mean acquiring home care services to stay in your current home, downsizing to a more manageable house or apartment, or, if you’re a caregiver, using adult daycare services to help you balance the care of your loved one. When a transition in living arrangements becomes necessary for you or a loved one, this publication is a useful aid in determining which options make the most sense for your family. We are fortunate to have abundant housing/ living accommodations, healthcare, and resources in our region. Types of Facilities, Care Options A number of broad categories of care options and facilities are included in this guide. Terminology may vary from facility to facility. Residential Living is for individuals who are responsible for their own basic needs. Nursing services are not available on site nor are residential communities affiliated with nursing care providers. No meal service or activity programs are offered. Active Adult Communities are residential-type communities for mature adults only. Individuals are responsible for their own basic needs. Nursing services are generally not available on site. Meal service is not provided. Outdoor maintenance service may be provided. Continuing Care Retirement Communities/Life Plan Communities To portray the “vibrant, forward-looking nature of the contemporary senior living community,� some continuing care retirement communities have chosen to describe themselves as life plan communities. A life plan community follows the definition of a CCRC set by The National Investment Center: Age-restricted properties that include a combination of independent living, assisted living, and skilled nursing services (or independent living and skilled nursing) available to residents all on one campus. Resident payment plans vary and

include entrance fee, condo/co-op, and rental programs. Retirement Communities are planned for those who are able to care for their own basic needs (or almost so) and choose to live in a community with other seniors. Organized social programs, meal service, transportation, recreational activities, and access to health and shopping are typical amenities. Nursing care is not usually provided. Some do offer assisted living/personal care designed for individuals who function mainly on their own but may need help with medications, bathing, or dressing. Services offered are similar to those provided by assisted living/personal care facilities. Retirement communities can be freestanding or affiliated with a complex offering nursing care services. Personal Care Homes offer food, shelter, and personal assistance or supervision. They are ideal for people who do not want to forfeit independence and do not require the services of a licensed long-term care facility but do require assistance or supervision in activities of daily living (ADL). Some ADL provided are transferring in and out of a bed or chair, toileting, bladder and bowel management, personal hygiene, securing and managing healthcare, self-administering medication, 50plus Living 2019
and proper turning and positioning in a bed or chair. Assisted Living Residences provide an environment that combines housing and supportive services to allow residents to “age in place� without having to move to a licensed long-term care facility when their care needs increase. ALRs have living units with kitchen capacity, private bathrooms, and only one person per unit. An ALR may provide services that would be provided in a long-term care facility (personal care homes may not). As of January 2011, changed regulations regarding licensure for assisted living communities became effective. Nursing/Rehab Facilities provide nursing care and related medical or other personal health services 24 hours a day, seven days a week, to individuals who require full-time care or supervision but do not need more intensive, hospitalbased care. Nursing/rehab facilities may be independent or part of another community. Individuals may be temporarily placed for rehabilitation following an illness, injury, or surgery or reside long term due to illness or aging. Nursing facilities offer skilled medical care under the supervision of licensed nurses, and at least one registered nurse must be on duty during the day. Custodial care is also offered and includes services that assist in the activities of daily living, which include bathing, dressing, using the bathroom, or transferring from a bed to a chair. Nursing centers are licensed by the State Department of Health. Acute Care Facilities are licensed as hospitals. They provide the highest level of care under a physician. provided at an acute care facility include emergency services, critical care, medical/ surgical services, and maternal/childbirth services. Dementia/Alzheimer’s services offer assistance and support to those living with memory issues. These units/communities enable the person living with memory issues to be as independent as possible while enhancing freedom of movement in a safe, secured environment. Adult Day Centers offer programs in facilities or function as independent organizations. The services may include but are not limited to personal care; assistance with eating, using the toilet, and taking medication; and social assistance. Centers offer a protective, supervised setting and generally operate during normal business hours. Respite Care provides caregiving opportunities on a short-term basis. Care may range from personal to nursing care. Home Care Services may be provided in a residential setting or as ancillary services, such as in personal care, assisted living, or nursing care environments. Services include skilled nursing services, therapies, and changing and reinforcing simple wound dressing, as well as nonmedical services, such as light housekeeping, transportation to doctor visits, shopping, respite, and more. Hospice Care is for families living and coping with life-limiting illnesses and provides professional treatment of pain and symptom management with support and counseling for medical, psychological, and spiritual needs. Palliative Care also focuses on providing relief from the symptoms and stress of serious illness but, unlike hospice, can be provided along with curative treatment.

Susquehanna/Delaware Valley Profile Weather Average high temperatures are the 80s in the summer months and the 30s in the winter. The warmest month is July, with an average maximum temperature of 85 degrees. The coldest month is January, with an average minimum temperature of 20-23 degrees. Average annual precipitation is 41-47 inches.
Taxes Sales tax is 6 percent. Sales tax exemptions include groceries, healthrelated items, medical services, prescription drugs, and groceries. State income tax is 3.07 percent; retirement income is tax-exempt. Property tax is determined by the local government according to school districts and municipalities.
State Lottery Benefits The Pennsylvania Lottery contributes to programs such as Property Tax/Rent Rebate; the Mass Transit Program; PACE, PACENET, and PACE Plus Medicare; long-term care services administered by the Department of Public Welfare; and the 52 Area Agencies on Aging.
PACE To be eligible for PACE, you must be 65 years of age or older and a Pennsylvania resident at least 90 days before the date of application. You cannot be enrolled in the Department of Public Welfare’s Medicaid prescription benefit.

PACE and PACENET eligibility is determined by your previous calendar year’s income. Effective in 2014, Social Security Medicare Part B premiums are excluded from income. For a single person, your total income must be $14,500 or less. For a married couple, your combined total income must be $17,700 or less. Once you are enrolled in the PACE program, a benefit card will be sent to you, and you will pay no more than $6 for each generic prescription medication and no more than $9 for each brandname prescription at your pharmacy. These co-payments are based on a 30-day supply.
PACENET To be eligible for PACENET, you must be 65 years of age or older, a Pennsylvania resident at least 90 days before the date of application, and you must not be enrolled in the Medicaid prescription benefit program. A single person’s total income must be between $14,501 and $23,500. A couple’s combined total income must be between $17,700 and $31,500. PACENET cardholders that do not enroll in a Part D plan will pay a nominal deductible each month at the pharmacy, which will be calculated through the cost of their medications. If the deductible is not met each month, it will accumulate. In addition, the individual will pay no more than $8 for each generic prescription medication and no more than $15 for each brand name. 50plus Living 2019
Susquehanna/Delaware Valley Profile PACENET cardholders enrolled in one of the program’s partner Part D plans will pay the Part D premium at the pharmacy each month, which will be calculated through the cost of the medications. PACENET cardholders enrolled in a Part D plan that is not one of the program’s partner plans will pay the Part D premium directly to the Part D plan. In addition, they will pay no more than the PACENET co-payments of $8 for each generic prescription medication and $15 for each brand name. PACE Plus Medicare Under PACE Plus Medicare, PACE/ PACENET coverage is supplemented by federal Medicare Part D prescription coverage and offers older Pennsylvanians the best benefits of both programs. Older adults continue to receive the same prescription benefits while, in many cases, saving more money.
Transportation Pennsylvania Free Transit Program – Provides free transit service on bus routes, trolley, and rapid-transit lines. Individuals 65 and older may ride free on fixed-route services at all regular operating times on weekdays, weekends, and holidays. You will need a senior citizen transit identification card to ride free. The identification card is available free of

charge from participating local transit operators. Shared-Ride Program – Provides reduced-fare transportation services to persons 65 and older utilizing shared-ride transportation services. Reduced-fare transportation is provided to medical appointments, senior community centers, and adult daycare centers. Mature Driver Safety Program – There is a mandated auto insurance discount for older drivers who complete a mature driver improvement course. Courses are offered through AARP, AAA, and the National Safety Council. It is intended for drivers 55 and older. Low-Vision License – Persons who do not meet the vision standards for a driver’s license may qualify for a restricted (lowvision) license. A restricted license allows the driver to drive during daylight hours on roadways other than freeways. A person with visual acuity of less than 20/70 combined vision, but at least 20/100 combined vision with best correction and 120 degrees’ field vision in the horizontal meridian, may qualify with a recommendation from an optometrist or physician. Retired Status Registration – Customers who qualify for retired status registration pay a $10 per year processing fee instead of the full $36 registration renewal fee. Total income must not exceed $19,200 and vehicles must weigh less than 9,000 lbs.

Caregivers Can Take Steps to Prevent Depression
By Lisa M. Petsche Family members and friends provide practical assistance and enhance the quality of life for ill or frail seniors who might otherwise require longterm care. The help they provide ranges from chauffeuring, shopping, running errands, and paying bills to housekeeping, preparing meals, managing medication, and assisting with personal care. Other typical caregiver responsibilities include coordinating care and advocating for the ill person’s needs. If the care receiver is cognitively impaired, they may also provide supervision to ensure safety. In addition, caregivers provide companionship and emotional support. Although it has its rewards, the caregiving role can be physically, psychologically, emotionally, and financially demanding. It is particularly challenging when it continues over a long period of time, and when the care 10

receiver has complex needs, a demanding personality, or mental impairment. While a certain degree of stress is inevitable, when left unchecked, it can lead to the caregiver becoming physically, mentally, and emotionally run down. Depression may result. According to the Family Caregiver Alliance, “a conservative estimate reports that 20 percent of family caregivers suffer from depression, twice the rate of the general population.” The rate is even higher for those caring for someone with dementia. If you are a caregiver, consider the following strategies for keeping stress manageable and reducing your risk of depression. • Accept the reality of your relative’s illness so you can appropriately plan for the future. • Learn as much as possible about the illness and its management. • Keep positive. Focus on what your relative can rather than can’t do and on your strengths and successes as a caregiver. • Do things that bring inner peace, such as meditating, writing in a journal, or listening to music. • Create a relaxation room or corner in your home — a tranquil spot that you can retreat to in order to rejuvenate. www.onlinepub.com

• Develop a calming ritual to help you unwind at day’s end. • Make a conscious effort to look after your health: eat nutritious meals, get adequate rest, exercise, and get regular medical checkups. • Stay connected to people whose company you enjoy. • Simplify your life. Set priorities and don’t waste time or energy on unimportant things. If finances permit, hire a housecleaning service or a companion for your relative to free up some of your time and energy. • Be flexible about plans and expectations. Take things one day at a time. • Give yourself permission to feel all emotions that surface, including resentment and frustration, which are normal. Remind yourself that you are doing your best and are only human. • Don’t keep problems to yourself — seek support from a relative, friend, religious leader, or counselor. Join a caregiver support group in your community or on the internet. • Accept offers of help. Ask other family members to share the load, and be specific about what you need. • Get information about community support services and take full advantage of them. Information can be obtained from your local office on aging. Depression: An Overview Contrary to what some may believe, depression does not stem from personal weakness, and the depressed individual cannot “buck up” or “snap out of it.” Clinical or major depression is a medical illness involving a chemical imbalance in the brain. The most common symptoms are as follows: • A change in appetite, which may result in weight loss or gain • A change in sleep patterns (needing more sleep or experiencing insomnia) • Feeling tired or lacking energy • Noticeable slowing of movements, speech, or thinking 12

• Unexplained physical aches and pains, or exacerbation of chronic health issues • Difficulty concentrating, remembering things, and making decisions • Feeling anxious, irritable, or angry • Feeling sad, and perhaps also crying easily, without any particular reason • Feeling guilty, incapable, unlovable, or hopeless • Lacking interest in activities that normally bring enjoyment • Withdrawal from social contact • Preoccupation with death or recurring thoughts of suicide If any of the above symptoms persist for more than a few weeks and interfere with your ability to carry out activities of daily living, make an appointment to see your primary physician. In the case of suicidal thoughts, help should be sought immediately through community crisis resources. Depending on the severity of symptoms, treatment may involve counseling, antidepressant medication, or a combination. The sooner help is obtained for depression, the easier it is to treat. Lisa M. Where Seniors Can Get Help with Home Chores and Small Jobs By Jim Miller Dear Savvy Senior, What’s the best way to find good, trustworthy, qualified people who can help seniors with home chores or small jobs? – Looking for Mom Dear Looking, Getting help at home for any number of household tasks is a lot easier than it used to be, thanks to a number of webbased tools that can quickly and easily connect you and your mom to a wide variety of skilled, carefully vetted workers. Here’s what you should know. Finding Qualified Help One of the best ways to find qualified, reliable, trustworthy people who can help with home chores and other small jobs is through referrals from people you trust. But if your friends or family don’t have any recommendations, there are a number of online companies you can turn to now, such as TaskRabbit 20
(www.taskrabbit.com) and Takl (www.takl.com). These are on-demand service companies that can quickly and easily connect you to skilled workers to handle a wide variety of household chores and small jobs, such as cleaning and housekeeping, moving and packing, lawn and yard cleanup, handyman tasks, grocery shopping, running errands, furniture assembly, picture hanging, closet organizing, and much more. TaskRabbit currently has more than 60,000 Taskers (workers) in 47 U.S. cities, while Takl currently serves 75 U.S. cities with around 35,000 workers. All you need to do is download their app, or go to their website, select the service your mom wants to have done, and set a time when she would like the worker to show up. The software then matches your request and provides you a list of qualified, feedback-rated workers (including their hourly rate) from which to choose. Once the job is complete, payment is done through 50plus Living 2019
the company’s app. You should also know that all TaskRabbit and Takl workers have to go through a thorough vetting process before they can join their respective company, including extensive background checks. If, however, you can’t find a skilled worker through TaskRabbit or Takl, or if they don’t serve your area, another option is Amazon Home Services (www.amazon.com/services). Like TaskRabbit and Takl, Amazon will connect you to qualified workers who handle dozens of household chores and other small jobs. Amazon also screens all workers through media searches, online interviews, reference checks, and background checks. And all experts need to have licenses and insurance. To purchase and book a service, you can either buy a prepackaged service with a fixed price (like two hours of cleaning), or you can submit a custom request and receive estimates. When booking, you select three different dates and time frames, and the pro confirms an appointment. All payment is done through your Amazon account. Need a Tradesman If your mom primarily needs a tradesman, such as a plumber, electrician, painter, roofer, or carpenter, for home repairs or remodel projects, you should also check HomeAdvisor (www.homeadvisor. www.onlinepub.com
com) and Angie’s List (www.angieslist. com). Both of these sites can connect you with prescreened, customer-rated service professionals in your area for free. Senior Specific Another option you should know about is Umbrella (www. askumbrella.com), which is a feebased membership service for seniors 60-plus that provides qualified, vetted workers to do small jobs in and around the house for only $16 per hour. Currently available in New York, they are expanding nationally over the next year. Lower-Income Option If your mom is on a tight budget, you should also contact her nearby Area Agency on Aging (call (800) 6771116), which can refer you to services in her area, if available. For example, some communities have volunteer programs that provide chore and handyman services to help seniors in need. And some local nonprofits provide residential repair services that offer seniors minor upgrades and adaptations to their homes.

Palliative Care Provides Comfort and Support alongside Cure-Seeking Treatments By Megan Joyce Although you may often see the words “palliative care” listed in conjunction with “hospice care,” the two terms are not entirely interchangeable. “Palliative care can take on different meanings for different people,” Sandy Sferrella-Taylor, public relations manager for Visiting Angels York, said. 32

“Palliative care seeks to prevent, relieve, or soothe the symptoms of disease, disorder, or aging without effecting a cure. It is a resource for anyone living with a serious illness where doctors feel they can provide treatment in the hopes of a cure.” Both palliative care and hospice care provide symptom management and comfort care for people with serious illnesses, but palliative care — which also supports a patient’s emotional, social, and spiritual needs — is not restricted to those with a life-limiting illness. “Some people are more comfortable with the term ‘palliative care’ if the explanation of support and comfort is provided,” Sferrella-Taylor said. “Many people associate the term ‘hospice’ with ‘giving up’ or death in general. The expectation of death to be coming soon does not need to exist to receive palliative care.” Hospice care is a type of palliative care, but not all palliative care is hospice care, which is reserved for individuals diagnosed with a terminal illness and with a life expectancy of six months or less. Patients who choose hospice have chosen to end curative treatment, but “diagnoses and life expectancy are not part of the need for palliative care,” Sferrella-Taylor said. There is no time limit for palliative care; it is appropriate at any stage of a serious illness and for patients of any age. Palliative care can occur in a hospital, in outpatient settings, in assisted living facilities, or at home. It is provided by an interdisciplinary team assembled based on the patient’s particular needs: palliative doctors, nurses, dieticians, pharmacists, social workers, financial advisers, and other specialists. This team works in conjunction with a patient’s physicians to provide “an extra layer” of comfort and support, according to the Center to Advance Palliative Care (www.getpalliativecare.org). A palliative care team may help someone with a serious illness — such as cancer; heart, lung, or kidney disease; Alzheimer’s; multiple sclerosis; and others — to better carry on with their daily life. The types of relief received under palliative care depend on the patient’s individual needs and can include medication or other treatments for physical discomforts such as pain, fatigue, sleep disruptions, nausea, www.onlinepub.com

constipation, or appetite loss. It could also include trained caregivers and social workers to help a patient better understand their diagnosis, clarify treatment goals and options, and offer emotional support as they deal with the stress and anxiety that arise from the management of their illness. “[Palliative care] can be for anyone needing comfort care … they might not be nearing the end of life or willing to put off any life-sustaining measures,” Sferrella-Taylor said. “They might still be receiving aggressive treatments for certain illnesses. Therefore they wouldn’t necessarily qualify for hospice, but they can get palliative care.” 34

For someone already facing the costs Palliative care is of treating their disease, the addition of appropriate at any comfort care, while appealing, may not stage of a serious at first seem financially viable. For people over 65, Medicare Part B offers some palliative care benefits; Medicaid’s coverage of palliative care varies by state. Sferrella-Taylor said the nature of the palliative care being provided often determines whether or not Medicare will pay for it. Non-medical care is not usually covered, but if the palliative care is categorized as nursing care or therapy, it may be covered. “An example of such care may be nursing visits from a skilled homecare agency to the end-stage congestive heart failure patient to manage them at home with the goal of avoiding repeat emergency-room visits,” SferrellaTaylor said. “This type of care also provides comfort and support, but the inclusion of the licensed professional may make it a Medicare-covered benefit.” How does someone diagnosed with a serious illness obtain the care of a palliative team? By asking for it. Hospitals often have a palliative specialist, or personal doctors and nurses can refer a patient to a palliative doctor. There are also numerous online resources that can serve as starting points for assembling a palliative-care team. The essential goal of palliative care is to alleviate physical suffering and mitigate the emotional and psychological burdens that accompany longterm medical treatment. “Many individuals, especially our clients, receive palliative care on an ongoing basis,” Sferrella-Taylor said. “The key is support and comfort for individuals who need it, and that, as mentioned, can span several years.” www.onlinepub.com
