Health Matters:

Caregivers Can Take Steps to Prevent Depression

By Lisa M. Petsche Family members and friends provide practical assistance and enhance the quality of life for ill or frail seniors who might otherwise require longterm care.

The help they provide ranges from chauffeuring, shopping, running errands, and paying bills to housekeeping, preparing meals, managing medication, and assisting with personal care.

Other typical caregiver responsibilities include coordinating care and advocating for the ill person’s needs. If the care receiver is cognitively impaired, they may also provide supervision to ensure safety. In addition, caregivers provide companionship and emotional support.

Although it has its rewards, the caregiving role can be physically, psychologically, emotionally, and financially demanding. It is particularly challenging when it continues over a long period of time, and when the care 10

receiver has complex needs, a demanding personality, or mental impairment.

While a certain degree of stress is inevitable, when left unchecked, it can lead to the caregiver becoming physically, mentally, and emotionally run down.

Depression may result.

According to the Family Caregiver Alliance, “a conservative estimate reports that 20 percent of family caregivers suffer from depression, twice the rate of the general population.” The rate is even higher for those caring for someone with dementia.

If you are a caregiver, consider the following strategies for keeping stress manageable and reducing your risk of depression.

• Accept the reality of your relative’s illness so you can appropriately plan for the future.

• Learn as much as possible about the illness and its management.

• Keep positive. Focus on what your relative can rather than can’t do and on your strengths and successes as a caregiver.

• Do things that bring inner peace, such as meditating, writing in a journal, or listening to music.

• Create a relaxation room or corner in your home — a tranquil spot that you can retreat to in order to rejuvenate. www.ResourceDirectoryPA.com

• Develop a calming ritual to help you unwind at day’s end.

• Make a conscious effort to look after your health: eat nutritious meals, get adequate rest, exercise, and get regular medical checkups.

• Stay connected to people whose company you enjoy.

• Simplify your life. Set priorities and don’t waste time or energy on unimportant things. If finances permit, hire a housecleaning service or a companion for your relative to free up some of your time and energy.

• Be flexible about plans and expectations. Take things one day at a time.

• Give yourself permission to feel all emotions that surface, including resentment and frustration, which are normal. Remind yourself that you are doing your best and are only human.

• Don’t keep problems to yourself — seek support from a relative, friend, religious leader, or counselor. Join a caregiver support group in your community or on the internet.

• Accept offers of help. Ask other family members to share the load, and be specific about what you need.

• Get information about community support services and take full advantage of them. Information can be obtained from your local office on aging.

Depression: An Overview Contrary to what some may believe, depression does not stem from personal weakness, and the depressed individual cannot “buck up” or “snap out of it.” Clinical or major depression is a medical illness involving a chemical imbalance in the brain.

The most common symptoms are as follows: • A change in appetite, which may result in weight loss or gain • A change in sleep patterns (needing more sleep or experiencing insomnia) • Feeling tired or lacking energy • Noticeable slowing of movements, speech, or thinking 12

• Unexplained physical aches and pains, or exacerbation of chronic health issues • Difficulty concentrating, remembering things, and making decisions • Feeling anxious, irritable, or angry • Feeling sad, and perhaps also crying easily, without any particular reason • Feeling guilty, incapable, unlovable, or hopeless • Lacking interest in activities that normally bring enjoyment • Withdrawal from social contact • Preoccupation with death or recurring thoughts of suicide If any of the above symptoms persist for more than a few weeks and interfere with your ability to carry out activities of daily living, make an appointment to see your primary physician. In the case of suicidal thoughts, help should be sought immediately through community crisis resources.

Depending on the severity of symptoms, treatment may involve counseling, antidepressant medication, or a combination. The sooner help is obtained for depression, the easier it is to treat.

Palliative Care Provides Comfort and Support alongside Cure-Seeking Treatments

By Megan Joyce Although you may often see the words “palliative care” listed in conjunction with “hospice care,” the two terms are not entirely interchangeable.

“Palliative care can take on different meanings for different people,” Sandy Sferrella-Taylor, public relations manager for Visiting Angels York, said.

“Palliative care seeks to prevent, relieve, or soothe the symptoms of disease, disorder, or aging without effecting a cure. It is a resource for anyone living with a serious illness where doctors feel they can provide treatment in the hopes of a cure.”

Both palliative care and hospice care provide symptom management and comfort care for people with serious illnesses, but palliative care — which also supports a patient’s emotional, social, and spiritual needs — is not restricted to those with a life-limiting illness.

“Some people are more comfortable with the term ‘palliative care’ if the explanation of support and comfort is provided,” Sferrella-Taylor said.

“Many people associate the term ‘hospice’ with ‘giving up’ or death in general. The expectation of death to be coming soon does not need to exist to receive palliative care.”

Hospice care is a type of palliative care, but not all palliative care is hospice care, which is reserved for individuals diagnosed with a terminal illness and with a life expectancy of six months or less. Patients who choose hospice have chosen to end curative treatment, but “diagnoses and life expectancy are not part of the need for palliative care,” Sferrella-Taylor said.

There is no time limit for palliative care; it is appropriate at any stage of a serious illness and for patients of any age.

Palliative care can occur in a hospital, in outpatient settings, in assisted living facilities, or at home. It is provided by an interdisciplinary team assembled based on the patient’s particular needs: palliative doctors, nurses, dieticians, pharmacists, social workers, financial advisers, and other specialists.

This team works in conjunction with a patient’s physicians to provide “an extra layer” of comfort and support, according to the Center to Advance Palliative Care (www.getpalliativecare.org).

A palliative care team may help someone with a serious illness — such as cancer; heart, lung, or kidney disease; Alzheimer’s; multiple sclerosis; and others — to better carry on with their daily life.

The types of relief received under palliative care depend on the patient’s individual needs and can include medication or other treatments for physical discomforts such as pain, fatigue, sleep disruptions, nausea, www.ResourceDirectoryPA.com

constipation, or appetite loss.

It could also include trained caregivers and social workers to help a patient better understand their diagnosis, clarify treatment goals and options, and offer emotional support as they deal with the stress and anxiety that arise from the management of their illness. “[Palliative care] can be for anyone needing comfort care … they might not be nearing the end of life or willing to put off any life-sustaining measures,” Sferrella-Taylor said. “They might still be receiving aggressive treatments for certain illnesses. Therefore they wouldn’t necessarily qualify for hospice, but they can get palliative care.”

For someone already facing the costs Palliative care is of treating their disease, the addition of appropriate at any comfort care, while appealing, may not stage of a serious at first seem financially viable. But many illness and for private insurance companies and health maintenance organizations (HMOs) offer patients of any age. palliative and hospice benefits, according to PalliativeDoctors.org, the patient site of the American Academy of Hospice and Palliative Medicine.

For people over 65, Medicare Part B offers some palliative care benefits; Medicaid’s coverage of palliative care varies by state.

Sferrella-Taylor said the nature of the palliative care being provided often determines whether or not Medicare will pay for it. Non-medical care is not usually covered, but if the palliative care is categorized as nursing care or therapy, it may be covered.

“An example of such care may be nursing visits from a skilled homecare agency to the end-stage congestive heart failure patient to manage them at home with the goal of avoiding repeat emergency-room visits,” SferrellaTaylor said. “This type of care also provides comfort and support, but the inclusion of the licensed professional may make it a Medicare-covered benefit.”

How does someone diagnosed with a serious illness obtain the care of a palliative team? By asking for it. Hospitals often have a palliative specialist, or personal doctors and nurses can refer a patient to a palliative doctor.

There are also numerous online resources that can serve as starting points for assembling a palliative-care team.

The essential goal of palliative care is to alleviate physical suffering and mitigate the emotional and psychological burdens that accompany longterm medical treatment.

“Many individuals, especially our clients, receive palliative care on an ongoing basis,” Sferrella-Taylor said. “The key is support and comfort for individuals who need it, and that, as mentioned, can span several years.” www.ResourceDirectoryPA.com

– Looking for Mom Dear Looking, Getting help at home for any number of household tasks is a lot easier than it used to be, thanks to a number of webbased tools that can quickly and easily connect you and your mom to a wide variety of skilled, carefully vetted workers.

Here’s what you should know.

Finding Qualified Help One of the best ways to find qualified, reliable, trustworthy people who can help with home chores and other small jobs is through referrals from people you trust.

But if your friends or family don’t have any recommendations, there are a number of online companies you can turn to now, such as TaskRabbit 50

(www.taskrabbit.com) and Takl (www.takl.com).

These are on-demand service companies that can quickly and easily connect you to skilled workers to handle a wide variety of household chores and small jobs, such as cleaning and housekeeping, moving and packing, lawn and yard cleanup, handyman tasks, grocery shopping, running errands, furniture assembly, picture hanging, closet organizing, and much more.

TaskRabbit currently has more than 60,000 Taskers (workers) in 47 U.S. cities, while Takl currently serves 75 U.S. cities with around 35,000 workers.

All you need to do is download their app, or go to their website, select the service your mom wants to have done, and set a time when she would like the worker to show up.

The software then matches your request and provides you a list of qualified, feedback-rated workers (including their hourly rate) from which to choose. Once the job is complete, payment is done through

the company’s app.

You should also know that all TaskRabbit and Takl workers have to go through a thorough vetting process before they can join their respective company, including extensive background checks.

If, however, you can’t find a skilled worker through TaskRabbit or Takl, or if they don’t serve your area, another option is Amazon Home Services (www.amazon.com/services).

Like TaskRabbit and Takl, Amazon will connect you to qualified workers who handle dozens of household chores and other small jobs.

Amazon also screens all workers through media searches, online interviews, reference checks, and background checks. And all experts need to have licenses and insurance.

To purchase and book a service, you can either buy a prepackaged service with a fixed price (like two hours of cleaning), or you can submit a custom request and receive estimates.

When booking, you select three different dates and time frames, and the pro confirms an appointment.

All payment is done through your Amazon account.

Need a Tradesman?

If your mom primarily needs a tradesman, such as a plumber, electrician, painter, roofer, or carpenter, for home repairs or remodel projects, you should also check HomeAdvisor (www.homeadvisor.

com) and Angie’s List (www.angieslist. com).

Both of these sites can connect you with prescreened, customer-rated service professionals in your area for free.

Senior Specific Another option you should know about is Umbrella (www. askumbrella.com), which is a feebased membership service for seniors 60-plus that provides qualified, vetted workers to do small jobs in and around the house for only $16 per hour.

Currently available in New York, they are expanding nationally over the next year.

Lower-Income Option If your mom is on a tight budget, you should also contact her nearby Area Agency on Aging (call (800) 6771116), which can refer you to services in her area, if available.

For example, some communities have volunteer programs that provide chore and handyman services to help seniors in need. And some local nonprofits provide residential repair services that offer seniors minor upgrades and adaptations to their homes.
