If you believe that you are being subjected to harassment, you must immediately report the incident to an O-pa staff member. Any reported incident will be investigated promptly, including incidents reported after a person’s separation from O-pa. Complaints and actions taken to resolve harassment will be handled as confidentially as possible, given O-pa’s obligation to investigate and act upon reports of such harassment. Violation of this policy may result in discipline, including termination of employment, relationship with Opa and/or dismissal from a class/camp. All individuals may bring complaints, ask questions, and raise concerns under this policy without fear of reprisal. All participants, parents, teaching artists, staff and volunteers are responsible for understanding, adhering to and strictly enforcing this policy. Page 21 of 35

HARASSMENT POLICY (continued from previous page) Investigation and Recommendation: O-pa will, upon receipt of a report or complaint alleging harassment or other inappropriate conduct, authorize an investigation, which may result in discipline. In determining whether an alleged conduct constitutes harassment or other inappropriate conduct, O-pa may consider the surrounding circumstances, the nature of the alleged statements or conduct, the relationships between the parties involved and the context in which the alleged incidents occurred. Whether a particular action or incident constitutes harassment or other inappropriate conduct requires consideration of all facts and surrounding circumstances. The investigation may consist of personal interviews with the complainant, the individual(s) against whom the complaint or report is filed, and others who may have knowledge of the alleged incident(s) or circumstances giving rise to the complaint or report. The investigation may also include any other lawful methods deemed pertinent by the investigator. In addition, O-pa may take immediate steps, at its discretion, pending completion of an investigation. If the conduct continues, this should also be immediately reported to O-pa staff. Prohibition against Retaliation O-pa will discipline any individual who retaliates against any person who complains of or reports alleged harassment or other inappropriate conduct or who retaliates against any person who testifies, assists or participates in an investigation, proceeding or hearing relating to a harassment complaint. Discipline and Other Appropriate Action O-pa will, in its discretion, take any appropriate action it deems necessary in response to complaints which are confirmed by investigation. Such action may include but is not limited to: discipline, such as verbal or written warnings, suspensions, counseling or other required conditions for retaining employment or relationship with O-pa, final warnings or separation, as well as general reminders of its policy, orientation, and training. I have read, understand and agree to the harassment policy:___________________

INFECTIOUS DISEASE CONTROL POLICY Omaha Performing Arts will take proactive steps to protect staff, participants and volunteers in the event of an infectious disease outbreak. It is the goal of O-pa during any such time period to strive to operate effectively and ensure that all essential services are continuously provided and that participants, staff and volunteers are safe at O-pa. O-pa is committed to providing accurate information based on health authorities’ information about the nature and spread of infectious diseases, including symptoms and signs to watch for, as well as required steps to be taken in the event of an illness or outbreak. Preventing the Spread of Infection Op-a will ensure a clean environment, including the regular cleaning of objects and areas that are frequently used, such as bathrooms, breakrooms, conference rooms, door handles and railings. We ask all participants, staff and volunteers to cooperate in taking steps to reduce the transmission of infectious disease including frequent hand washing, hand sanitizer use, covering sneezes and coughs and discarding used tissues in wastebaskets. O-pa may also implement other policies or procedures, such as distancing requirements, to limit the spread of infectious disease in accordance with directives from relevant agencies. During an infectious disease outbreak, it is critical that participants, staff and volunteers do not attend classes/camps while they are ill and/or experiencing symptoms of the infectious disease. Requests for Medical Information and Confidentiality If you are out sick or show symptoms of being ill, it may become necessary to request information from you and/or your health care provider. In general, we request medical information to confirm your need to be absent, to show whether and how an absence relates to the infection, and to know when it is appropriate for you to return. As always, we expect and appreciate your cooperation if and when medical information is sought. Any medical information is considered confidential. Any disclosure of medical information is in limited circumstances with relevant staff, first aid and safety personnel with a need to know, and government officials as required by law. I have read, understand and agree to the infectious disease policy:___________________

SEXUAL ABUSE & MOLESTATION PREVENTION POLICY Omaha Performing Arts does not tolerate sexual abuse or molestation. O-pa provides procedures for teaching artists, employees, volunteers, victims of sexual abuse or others to report such abuse, and disciplinary action for those who commit such acts. No teaching artist, participant, employee, volunteer or other third party, no matter their title or position, may allow the commission of sexual abuse or molestation. O-pa has a zero-tolerance policy for any sexual abuse committed by anyone. After the completion of an investigation, O-pa will take disciplinary action up to and including termination of employment (or other relationship with O-pa), as well as criminal prosecution for violations of this policy. Sexual abuse or molestation takes the form of inappropriate sexual contact or interaction for the gratification of the individual who is functioning as the caregiver and is responsible for the client’s and/or child’s care. Sexual abuse includes sexual assault, exploitation, molestation or injury. It does not include sexual harassment, which is another form of behavior not tolerated by O-pa. Such conduct is addressed in O-pa’s Harassment Policy (see page 21-22). Any incidents of sexual abuse reasonably believed to have occurred will be reportable to the appropriate law enforcement and regulatory agencies. Reporting Procedure: Everyone has a responsibility to report suspected abuse or neglect. State law requires any person who suspects that a child has been abused or neglected to report their concerns to the Child Abuse and Neglect Hotline is (800) 652-1999. If the suspected abuse is to an adult or elder, you should report the abuse to Nebraska Adult Protective Services and Elder Abuse Hotline at (800) 652-1999. Any employee who learns of an incident of sexual abuse being committed must immediately report it to their supervisor, the President or HR, in addition to contacting the abuse hotlines. I have read, understand and agree to the sexual abuse/molestation policy:_________________________

I have read, understand and agree to the substance abuse policy:_______________________

VIOLENCE PREVENTION POLICY Purpose Omaha Performing Arts is committed to preventing violence and to maintaining a safe environment. O-pa has adopted the following guidelines to deal with intimidation, harassment or other threats of or actual violence that may occur onsite or offsite during O-pa activities. Prohibited Conduct All participants, teaching artists, volunteers, and staff should be treated with courtesy and respect at all times. All individuals expected to refrain from fighting, “horseplay” or other conduct that may be dangerous to others. Conduct that threatens, intimidates or coerces another person will not be tolerated. O-pa resources may not be used to threaten, stalk or harass anyone. O-pa treats threats coming from an abusive personal relationship as it does other forms of violence. Indirect or direct threats of violence, incidents of actual violence and suspicious individuals or activities should be reported as soon as possible to O-pa staff and security personnel. When reporting a threat or incident of violence, the individual should be as specific and detailed as possible. Individuals should not place themselves in peril, nor should they attempt to intercede during an incident. Participants should promptly inform an O-pa staff member of any protective or restraining order that they have obtained that lists O-pa as a protected area. O-pa will not retaliate against individuals making good-faith reports. Investigations & Enforcement O-pa will promptly and thoroughly investigate all reports of threats of violence or incidents of actual violence and of suspicious individuals or activities. The identity of the individual making a report will be protected to the extent possible without impairing the ability to conduct a thorough investigation. O-pa will not retaliate against individuals making good-faith reports of violence, threats or suspicious individuals or activities. WELLNESS, SELF-INJURY AND SUICIDE PREVENTION POLICY Omaha Performing Arts is committed to protecting the health, safety and welfare of our participants. Selfinjury occurs when an individual chooses to inflict wounds upon themselves because of psychological distress. Although it is difficult to understand, this behavior becomes a coping mechanism for some people. Feelings of anxiety and distress, being “outside” one’s body, and a need for self-punishment are among the reasons self-injurers cite for their behavior. All acts should be immediately reported to a teacher, staff member or administrator and will be kept strictly anonymous. All reports will be taken seriously. In cases where a participant has engaged in a self-injury incident, including a suicide attempt, immediate care for any physical injuries will be assessed by medical personnel. The parents/legal guardians of any participant who through investigation is found to have engaged in self-injury will be notified immediately. This ensures the participants’ overall safety and can prompt additional help outside of O-pa. I have read, understand and agree to the content of wellness policy:___________________
