An ‘Advanced Directive’ (AD), sometimes called a ‘Living Will,’ supports patient autonomy and patient-centered care with a document specifying medical care. With such a document family members and / or health care professionals can know with certainty what your values are. A second type of AD is a Health Care Power of Attorney (HCPOA), allowing a designated person to make such health care end of life choices for you. Finally, your wishes do not necessarily need to be documented in an AD or HCPOA form, they may simply be recorded by your physician in the medical record.
