here’s not a nurse I know who thinks our current healthcare system “works.” ● Too much testing vs. not the right tests; ● too much documentation vs. documentation that misses the boat; ● hotel-style “customer service” scripts vs. from-theheart empathic dialogue; ● rote protocols vs. critical thinking; ● factory-style “efficiency” vs. humane interaction (which takes time); ● cumbersome outdated policies vs. common sense and professionalism; ● virtual management, training and human resource access vs. real, live human beings with personalities who want to help you.

So, why are some folks so terrified of looking at alternatives to this dysfunctional system?

Fundamental change in systems and programs raise the specter of the unknown. We may not like the way things are, but we have figured out a way to manage within their constraints. The question becomes: Is a better way possible?

We look around the world and there are things we know: ● National health systems exist

in every industrialized economy… except the United States. ● Not one country that has established such a system has voted or moved to abandon it. ● The satisfaction ratings in such countries are far higher than those in the U.S. ● They cost nearly half of what the U.S. pays as a percentage of the gross domestic product. ● The health indices in these systems are superior in nearly every standard to those in the U.S. ● These systems are built around a progressive taxation system rather than a “cost” system that discriminates against those with fewer means. ● These systems are not perfect, nor are they identical, and there are variations within systems.

That does happen in a country with a national health system.

The insurance industry has been described as the parasite of the

healthcare industry. It provides no direct service, care, research or materials—and yet it is the key driver in costs and “professional aggravation”. It feeds off of inadequate treatments (no care = no cost to the insurer) and simply raises rates if complex care is required (always with a sweetener that further raises costs).

Big Pharma is referred to as the predator of the healthcare industry.

Hyper-inflation without regulation of drug prices (directly linked to the for-profit nature of the system) has led to increased morbidity and mortality for folks who cannot afford needed medications.

Advertising and marketing has convinced people of the need to over-medicate or “selectively” medicate—with limited scientific basis. Providers lean on medication when confronted with insurancemandated productivity requirements—instead of comprehensive, holistic care. Side effects and multiple medications have resulted in serious complications. And the opioid crisis is directly linked to such practices.

These parasites and predators could be dramatically reduced or even eliminated with a universal system that puts people before

profits. The repetitive checklists of the EMR could be reformatted to serve caregivers and patients rather than the hospital accounting department. The savings generated by eliminating multiple insurers’ share of the healthcare dollar, high drug prices, administrative overhead to manage reimbursement, unnecessary marketing and advertising could be used instead to hire desperately needed staff, allow for more time with patients, raise salaries for healthcare workers—employers would no longer “charge” us for health insurance as part of contract packages—and pay for research and innovative programs.

Professionals could once again exert more control over our practice and avoid time-consuming workarounds to provide the care our patients deserve. Decisions could be based on need and best practices rather than what the insurance will cover.

Clearly there are flaws in many national health systems, but such flaws are over-exaggerated by media controlled by parasites and predators—and the politicians and outlets that receive funding from them. Any new system can learn from the errors of previous or existing ones. What are the problems we can observe when we look at our neighbors in other countries? How can we avoid some of them?

We are smart. Surely if we can figure out how to transplant organs, stop a cerebral or cardiac infarct in mid-attack, and eradicate and prevent fatal infections, we can create an equitable, quality healthcare system that includes everyone who needs care, while simultaneously respecting the caregivers who dedicate our lives to serving our communities.

Improved and expanded Medicare for All is not a dream.

It is a necessity.

WMC needs direct care RNs

T

here will soon be more than 20 new RNs at Westchester Medical Center (WMC) in two essential units that deal with patients in severe situations.

Ten of the yet-to-be-filled positions will go to critical care, while another 10 will go to behavioral health.

However, RNs at WMC say positions need to be created and filled by nurses performing direct care at all times as opposed to many charge nurses.

David Long, RN, an executive committee member at WMC, sees a great need for more critical care nurses in his unit. “The coronary care unit is an eight-bed unit—a small unit for a big center—however, it’s very busy,” said Long.

“We turn over rapidly, sometimes a whole unit in one day. We had a 2 to 1 ratio in critical care, and we had to float nurses in from other areas to help cover. We need nurses who are trained to deal with critical care, not those floated in with different specialties. That’s what we need.”

Prioritizing staffing

Long said the push for more bedside nurses solely focused on direct care has not always been easy. “We’ve been meeting twice a month to go over the needs of the institution and the needs of nursing. Getting more nurses in our unit has been the top priority of our agenda: staffing. We need bedside nurses.” he said.

Liesl Van Ledtje, RN, a new member of the executive committee, works in the behavioral health unit at WMC and believes not having a full staff of bedside nurse support is a matter of safety for her unit. “Safety is a huge concern when you’re working in behavioral health,” said Van Ledtje. “We’re looking for no less than two nurses per unit for safety and the building as a whole. If there is a code blue, we are the first responders, so if there is only one nurse per unit, that means we either can’t respond to the blue code or our patient has no care.”

heart issues, while behavioral health touches everything from adults suffering from bi-polar disorder to teenagers experiencing suicidal or violent thoughts.

“These nurses are needed on the front lines—especially in this time when mental health is at the forefront,” emphasized Van Ledtje.

“We see so many awful things that happen from lack of care. We need to support this program.”

“With more bedside nurses, patients in my unit will know they’re being cared for by a trained cardiac nurse who understands their issues and understands their own role as a registered nurse,” Long explained.

“There now will be great confidence with patients knowing every nurse in the unit is trained to address their needs. It will make a difference.”

Both nurses are hopeful that the administration will hear their calls for bedside nurses.

A public health platform NYSNA

’s policies constitute a public health platform. As nurses, we see that proposals and laws have a fundamental connection to the health and safety of our patients and communities. In that light, wages, pollution, gun violence, safe staffing and universal healthcare are inextricably linked to public health.

With this month’s New York Nurse, we begin a months-long series highlighting leading candidates in the race for president of the United States. We start this month with the Democrats. (See pp. 7-15.)

Candidates will be gauged based upon their commitment to NYSNA’s positions on the most significant issues.

Because safe staffing saves lives. The Nurse Staffing Standards for Hospital Patient Safety and Quality Care Act of 2019, sponsored by Sen. Sherrod Brown in the Senate and Rep. Jan Schakowsky in the House, would provide for minimum nurseto-patient ratios and “amend the Public Health Service Act to establish direct care registered nurse-topatient staffing ratio requirements in hospitals, and for other purposes.”

Because healthcare is a human right. We must fight to keep important aspects of the Affordable Care Act because to take them away will cost human lives and constitute a setback in our campaign for true universal healthcare.

Because we believe in patients over profits. We support the Medicare for All Act of 2019, introduced by Senator Bernie Sanders and 14 of his colleagues. In the U.S. House of Representatives, it is sponsored by Rep. Pramila Jayapal (D-WA) and Rep. Debbie Dingell (D-MI). Medicare for All

represents a major step forward in lives of the vast majority of Americans who would receive quality care with no co-pays, no deductibles and no cost for prescription medicines by eliminating the insurance middleman.

Because we believe that opioid use is a major public health crisis.

We support Congressional action to further investigate the role of Big Pharma and we further support increased funding for the treatment of the addicted and public health programs to educate the most vulnerable, all paid for by Big Pharma.

Because union jobs matter. We support the Green New Deal that was recently introduced by U.S. Representative Alexandria OcasioCortez (D-NY) and Senator Edward Markey (D-MA). With 102 supporters in Congress and 11 of the Democratic presidential candidates speaking in its favor, it is a boost to organized labor. In its current form, the Green New Deal includes a federal jobs guarantee, workforce retraining, strengthening of collective bargaining rights, retirement security, and universal healthcare. (See p. 16 for our reportage on the Green New Deal.)

Because without decent wages, people cannot live healthy lives.

We support the Raise the Wage Act, which would raise the federal minimum wage to $15/hour by 2024. It’s sponsored by Bernie Sanders (D-VT) in the Senate and Bobby Scott (D-VA) in the House.

Because a healthy planet matters to public health. All Democrats seeking the party’s nomination agree that the U.S. must sign back on to the Paris Climate Accord. When discussing the paramount issue of pollution and its climate and public health consequences, many have invoked the Green New Deal as the fix.

A slew of litigation has also challenged the Trump Administration’s rollbacks to the Clean Air Act. Because workers’ rights matter.

Support the Protecting the Right to Organize Act (“PRO Act”) to strengthen protections for employees engaged in collective action, to facilitate collective bargaining, and

to provide for stronger remedies for employees under the NLRA. Because gun violence is at epidemic levels putting the public health in jeopardy. We are submerged in a culture of gun violence and our commitment to help stop it is a top priority. The Help Empower Americans to Respond (HEAR) Act was introduced in June by Senators Bob Menendez (D-NJ), Dianne Feinstein (D-CA), Richard Blumenthal (D-CA) and Tim Kaine (D-VA). It bans the importation, sale, manufacture, transfer or possession of gun silencers or suppressors like the one used by the gunman in the recent VA Beach shooting.

Because trickled-down economics do not help alleviate poverty.

All Democrats seeking the nomination see the Trump tax cuts as a giveaway to the nation’s elites.

The Democrats would abolish the Tax Cuts & Jobs Act of 2017 and return the country to a progressive tax system. Some have called for the elimination of the capital gains tax, invoking the argument that the sale of assets, particularly in the trading of stocks, should be taxed as regular income.

Because every woman should have the freedom to choose.

Reproductive Rights affect us in several ways. Most of us at NYSNA are women and hold firm beliefs about the decisions that affect our bodies. As public health and patient advocates, we know that limiting abortion poses serious harm to most women, who must seek services outside licensed clinics and hospitals. The Women’s Health Protection Act of 2019 would protect abortion access from state-level bans and restrictions that threaten to eliminate access around the country. It has been introduced in the House by U.S. Representatives Judy Chu (D-CA), Lois Frankel (D-FL), and Marcia Fudge (D-OH), and in the Senate

by Senators Richard Blumenthal (D-CT) and Tammy Baldwin (D-WI). The Democratic candidates for president all have various policy platform proposals around a woman’s right to choose and should be scrutinized.

Martha Wilcox: paragon of public health

O

n June 7, NYSNA lost a very special nurse colleague, board member, committed public health professional, wife, mother and devoted community advocate: Martha Wilcox.

She passed away in her sleep at her home in Roscoe, New York.

Martha was 57.

First and foremost, Martha distinguished herself for the work she carried out as a public health nurse in Sullivan County. The county is rural, where family farms dot gentle hills. Many of these small agricultural enterprises are dairy farms, others grow organic fruit and produce. Lamb, beef and fowl are also raised. Much of what’s grown and raised is served in New York City’s finest restaurants. Martha and her husband, Daniel, operated a farm that has been in Daniel’s family for several generations.

On June 12, on the steps of City Hall, City Council members and borough presidents, alongside the NYC public advocate, stood tall with NYSNA nurses demanding safe staffing. The enthusiasm was palpable: nurses, elected officials, public health experts

and community advocates all pressing for a standard of safe care: equality for all patients, in all hospitals, with enough nurses to do their jobs.

These pronouncements came on the heels of the release of a new study published in the Journal of Nursing Administration that provides additional empirical evidence to the common-sense argument that nurse understaffing puts patients at increased risk: infection rates for patients in understaffed units were 15% higher than those safely staffed.

Why the Green New Deal matters to labor and for the public’s health

P

resident Franklin D. Roosevelt (FDR) assumed office in 1933 to a country in grave trouble. It was The Great Depression and more than 60 percent of the U.S. was living in poverty. Banks were shuttered, the future was bleak.

Hardest-hit were working people: construction workers, farmers, manufacturers, families.

Roosevelt knew the country needed a fundamental change—

that the usual political solutions wouldn’t be enough to turn things around. With help from Congress, his administration implemented The New Deal: a series of public programs that would bring the country back to life.

An important legacy

Upon assuming office, the FDR administration launched a variety of programs in short order. They launched a Civilian Conservation Corps to put men back to work in rural areas. They created the National Labor Relations Board to protect workers’ rights by growing labor unions. They passed the Social Security Act to support the retired, elderly, disabled, and survivors of loss.

These policies, and many more, were among the most ambitious

government projects in American history. They not only brought the American people and the economy back to life, they also created an important political legacy that still resonates.

An ambitious agenda

Today, as we tackle the fundamental crises facing our country— income inequality and climate change—members of Congress have looked back to FDR’s approach and proposed the Green New Deal.

Like Roosevelt, the Green New Deal focuses on what historians call the “three Rs”: 1) relief for the unemployed and the poor; 2) recovery of the economy; and 3) reform of the financial system.

Within the next 10 years, the Green New Deal aims to provide everyone in the country with: ● High-quality healthcare ● A job with a family-sustaining wage, family and medical leave, paid vacations, and retirement security ● Affordable, safe housing ● Access to clean water, clean air, healthy and affordable food, and nature ● Resources, training, and highquality education, including higher education It will do this by creating jobs and government programs that will: ● Move 100 percent of the power demand to clean, renewable, zeroemission energy sources ● Repair and upgrade infrastructure ● Spur massive growth in clean manufacturing ● Ensure that all Green New Deal jobs are union jobs that pay prevailing wages and hire local ● Upgrade buildings to maximal energy efficiency, water efficiency, safety, affordability, comfort, and durability ● Ensure affordable access to electricity ● Overhaul transportation systems to eliminate pollution from transportation ● Work with farmers and ranchers to eliminate pollution from agriculture

Though the ideas behind the Green New Deal have been around since the 1970s, only in recent years—as inequality balloons and the planet changes drastically before our eyes—have its policies gained real political support.

Driven by U.S. Representative Alexandria Ocasio-Cortez (D-NY) and Senator Edward Markey (D-MA), the Green New Deal now has 102 supporters in Congress and 11 of the Democratic presidential candidates touting its importance on the campaign trail.

A boost to labor

The Green New Deal also includes a boost to organized labor and a national reminder that unions protect workers and the economy as a whole. In its current form, the Green New Deal includes a federal jobs guarantee, workforce retraining, strengthening of collective bargaining rights, retirement security, and universal healthcare.

But perhaps most important are the positive health effects these policies will have on everyday Americans. Our members have seen the impacts of poor environmental conditions on our patients and in our own families. We have seen how drastically our country’s income disparity has grown, and how critical union representation is to protect our jobs, our patients and our communities.

Well-being for generations to come

Whether it’s an agricultural community in the Finger Lakes or an urban center in the heart of NYC, the Green New Deal will fundamentally and positively impact New York State.

NYSNA supports moving the U.S. economy to clean energy, reducing pollution to the very best of our technical abilities, ensuring access to clean water, air, food and nature, and doing so by growing good-paying union jobs for all Americans.

These measures will strengthen America’s health and well-being for generations to come.

NYSNA celebrates diversity

T

his June, NYSNA participated in multiple parades in New York City celebrating the cultures of its diverse membership and patients.

Nurses marched up Fifth Avenue in the 62nd Annual National Puerto Rican Day Parade on June 14th for the fourth time. Flandersia Jones, RN, at BronxCare Health System, was one of a group of members who danced with students from a Bronx elementary school.

“The nursing community is a diverse community. Becoming a dancer this year was the best way to celebrate and support our Puerto Rican members,” she said.

Along the route, parade-goers expressed their exuberant support for the nurses who work tirelessly to care for New York City residents every day. “Every time we walked by, everyone would stand up and cheer,” said Jones. “They were very happy, saying thank you for your service and all that you’re doing.”

Jones said NYSNA’s presence at the parade sends a message about the union’s priorities. “Of the thousands of nurses in NYSNA, many are Hispanic. So for NYSNA to participate in a parade like this shows recognition for the members and support for their culture and Puerto Rico itself along with its development. Puerto Rico needs more healthcare services.”

Celebrations continue

At the celebration, Jones met several nurses who recently returned from a New York Recovery Network (NYRN) medical mission trip to Puerto Rico, where residents are still struggling to fully recover from Hurricane Maria. “Most of the people who danced with me were people who went on the mission trip to Puerto Rico,” she said. “Their stories of the trip brought more meaning to the parade this year.”

NYSNA members marched as millions lined the streets of NYC to celebrate LGBTQ+ people. It was the world’s largest annual Pride march. All people should live as themselves with dignity, just as all people deserve healthcare as a matter of human rights.

A

fter an impressive career that began in the summer of 1981, Colleen Vokes, BSN, RN, SPHN and current WIC Coordinator at the Livingston County Department of Health, is embarking on the next chapter: retirement. In a Q&A with the New York Nurse, Vokes reflected on what made her nearly 40-year career so special and shared advice for up-and-coming nurses.

How are you feeling about retirement?

I’m very excited about it, but I’m also conflicted. There are a lot of things I’m going to miss, and I’ve been coming to the Livingston County Department of Health— except when I had my kids—every single day for 35 years. It was a huge part of my life, but I’m excited for the next chapter. I have a new grandson due in July and a two-year-old grandson already, so that will be my retirement right off the bat.

Where did your nursing career begin?

When I first came to Livingston, I was a public health nurse and I was out on the road doing home care.

Starting out, public health nurses did everything, we did the home care, maternal child health visits, a well-baby clinic, orthopedic clinic and end-of-life care. Everybody did everything.

As a public health nurse, what have you learned about the people you serve?

People will come together to help accomplish whatever needs to be done. For instance, there was a presentation about emergency pre-

paredness recently, and more than 50 people attended. We got several volunteers for medical reserve core.

These were community volunteers who would step up in the face of a real emergency to support the public health team. People are willing to help if you give them the opportunity.

After 40 years in the profession, what have you learned about being a nurse?

I learned to never underestimate the value of kindness, listening and meeting people where they are and not projecting what you think they need. Rather, listening to the person, seeing what is important to them, then supporting and moving them along.

What piece of advice would you give to new nurses?

I would say to treat your patients

and co-workers the way you would want to be treated.

What are you going to miss the most about being a nurse?

I will miss the people we provide a service to and also the people I work with. I’ve worked with some awesome, kind, smart, and professional people in the world here at the Health Department who do great work for a lot people.

What’s one word you would use to describe your time as a nurse?

A blessing. I know it’s two words, but it truly was a blessing. I’ve learned so much and met some of the most wonderful people. Being able to help people in their own homes and give care that was invaluable. A blessed journey for sure.
