Keeping Central Oregon Seniors Fed, Safe, and Connected Every day, the Council on Aging of Central Oregon connects older adults and family caregivers with the support they need to age well wherever and however they choose. Our programs are designed to help people remain independent, stay engaged, and navigate the opportunities and challenges that come with aging. Information and Resource Specialists answer questions, listen to your concerns, and refer you to the appropriate agencies for the services you need. More information, page 13 Nutrition Resources include Meals on Wheels and Community Dining, which provide nutritious meals and vital social connection to seniors at home or at our six community dining sites. More information, pages 50 and 55 Options Counselors listen, assess, create a plan, and offer you options on ways to move forward on the aging path. More information, page 84 Oregon Project Independence Medicaid (OPI-M) delivers in-home services and personal care needed to help eligible Central Oregon seniors and adults with disabilities live safely and independently. Senior Health Insurance Benefits Assistance (SHIBA) offers free, unbiased Medicare counseling to help older adults understand their benefits, compare health plans, and save on prescription costs. More information, page 74 The Health Promotions Program provides and sponsors evidencebased classes, such as Tai Chi and Chronic Disease Self-Management, to help older adults improve mobility and manage long-term health conditions. More information, page 65 The Family Caregiver Support Program can provide respite grants, assistance finding services, and support groups. More information, page 24 Pet Pals coordinates free pet food, supplies, and veterinary care to Meals on Wheels clients in Bend, La Pine, Madras, Prineville, and Sisters to help their pets stay healthy and at home.
Community Impact by the Numbers In 2025, we were grateful to have: • delivered 153,409 Meals on Wheels • served 45,465 senior lunches at Community Dining • answered 7,382 calls with information and referrals • provided 1,073 hours of Medicare counseling • served 1,100 clients with case management • provided 822 hours of paid respite support for caregivers

Advance Directive
Palliative care is specialized medical care for people living with a serious and life-limiting illness, such as cancer or heart failure. It focuses on relieving symptoms and easing the stress of the person, their family, and their caregivers that often comes with serious illness. While individuals may continue to receive treatments aimed at managing their condition, the primary goal of palliative care is to improve comfort and overall quality of life.
Advance directives are written legal documents that identify who will be your healthcare representative and outline your preferences for medical care if you are ever unable to make decisions for yourself. They help guide doctors and caregivers in providing care that aligns with your wishes in situations such as a serious illness, coma, latestage dementia, or end of life.

Physician Portable Orders for Life-Sustaining Treatment (POLST)
Hospice care focuses on comfort and quality of life for a person with a serious illness who is nearing the end of life. It is intended for individuals with a terminal diagnosis when a doctor believes they may have six months or less to live if the illness follows its expected course. Hospice supports the patient and their family with a team of interdisciplinary professionals who not only address physical comfort, but emotional and spiritual needs as well. The goal of hospice is to provide dignity, comfort, and support during the final stages of life, often allowing a person to remain at home surrounded by family and loved ones.

UNDERSTANDING SOCIAL SECURITY Social Security Benefits
Social Security is a federal safety net program that provides benefits to retired workers, their spouses and dependent children, and people with disabilities. Benefit amounts are based on lifetime earnings and the age at which benefits begin. Social Security is funded through payroll taxes, while Supplemental Security Income (SSI) is funded through general federal revenues.
Supplemental Security Income (SSI)
SSI is a monthly benefit paid by the Social Security Administration (SSA) to people with limited income and resources who are blind, age 65 or older, or have a qualifying disability. SSI is designed to ensure that eligible individuals and couples have enough income to meet basic needs for food, shelter, and clothing. SSI is not contingent on a person's work history. Eligibility is determined by financial need and criteria related to age, disability, or blindness.

Social Security Disability Insurance (SSDI)
SSDI provides financial assistance to individuals who are unable to work due to a qualifying disability. Unlike SSI, SSDI is based on prior work history and contributions to the Social Security system. To be eligible for SSDI, individuals must have worked and paid Social Security taxes, earning enough work credits based on their age at the time they became disabled.
Representative Payee
If a person becomes unable to manage their own funds, the SSA may appoint a Representative Payee to manage Social Security, SSI, or SSDI benefits on their behalf. The Representative Payee must use benefits to meet the person’s needs, keep accurate records, and report any changes in living situation to the SSA.

FEEDING OUR COMMUNITY The Council on Aging of Central Oregon offers two main food services for tri-county seniors: Meals on Wheels and in-person senior Community Dining. Medicare Advantage
Medicare Advantage (Part C) plans are offered by private insurance companies approved by Medicare. These plans provide Part A and Part B coverage and usually include Part D prescription drug coverage. Many plans also offer additional benefits, such as vision, hearing, and dental coverage.

KEY LEGAL TERMS AND ESTATE TOOLS Conservatorship: A court-supervised arrangement where a judge appoints a person or entity (a "conservator") to manage the finances of someone who is mentally or physically incapable of doing so themselves. A conservator can be a family member, a friend, a private professional, or a corporate entity like a bank or trust company. Guardian: A person appointed by the court to make healthcare and personal care decisions for an individual who lacks the mental capacity to make those decisions for themselves. Intestate: Legal terminology for dying without a valid will. When this happens, state intestate succession laws dictate how the deceased person's estate assets are distributed, typically prioritizing the closest living relatives. Living Trust: A legal structure created during a person’s lifetime to hold and manage their assets. It is fully amendable and revocable by the creator during their life. Assets held within a living trust bypass the public, time-consuming probate process upon death and are distributed directly to beneficiaries. While efficient and private, they can be more expensive to establish than a standard will.

Personal Representative: A personal representative can be named in a will to ensure that the decedent’s estate is distributed according to their wishes. If no personal representative is named, the court will appoint an administrator. Powers of Attorney (POA): A legal document granting a trusted agent the authority to manage someone's financial and legal affairs. In Oregon, financial powers of attorney are automatically durable, meaning they remain valid even if the creator becomes mentally or physically incapacitated. All powers of attorney expire immediately upon the creator's death and cannot be used as a substitute for a will or trust. Probate: The formal court-supervised process used to validate a deceased person's will, settle their outstanding debts, and legally transfer title of their assets to their heirs. Representative Payee: A person or organization appointed to receive Social Security, Disability or Supplemental Security Income benefits on behalf of someone who can't manage their own benefits. Will: A legal document that specifies exactly how assets held solely in your name should be distributed after your death.

CONTINUUM OF CARE Adult Day Services: Planned programs that provide care, supervision, meals, activities, and social connection in a professional setting during the day. Adult day services can support older adults who need assistance or supervision while also providing respite and support for caregivers. Adult Foster Homes: Licensed residential homes that provide care and support for a small number of individuals, typically five or fewer. These homes offer assistance with activities of daily living, supervision, meals, and a shared living environment. Care providers often live in the home, and services are generally nonmedical. Assisted Living Facilities: Residential settings that provide support with daily activities while allowing residents to maintain as much independence as possible. Services may include meals, personal care, medication assistance, housekeeping, laundry, transportation, social activities, and 24-hour support. Residents typically have their own apartments or rooms and share common areas.

Independent Living: Residential communities designed for older adults who are generally able to live independently but want the convenience, social opportunities, and amenities of a community setting. Services vary but may include meals, activities, transportation, and maintenance support. Assistance with activities of daily living is typically not provided. Memory Care: Specialized residential settings designed to support people living with Alzheimer’s disease or other forms of dementia. These communities provide structured activities, trained staff, and enhanced safety features, such as secure entrances and exits, to support residents’ needs. Skilled Nursing Facilities: Licensed facilities that provide 24hour medical care, rehabilitation, and support from trained health professionals. Some people stay in a skilled nursing facility temporarily to recover after a hospitalization, surgery, or illness, while others may receive long-term care and support.
