You’ve helped us to support mums, dads, brothers, sisters, children, grandparents, friends and carers for over 35 years. Your support has made such an impact to tens of thousands of local people over the last 35 years, in ways that most of us cannot even begin to imagine.

Now, we must focus our efforts on reaching even more people throughout North Devon, transforming what end of life care looks like for local people and their families.

We want to ensure our care effectively meets the needs of our patients and their families both now and in the future. We want to: •

Focus our Hospice to Home on “rapid response” care to support patients in a crisis.

Create a Single Referral “Hub”, clinically managing all hospice referrals to ensure that every patient has access to a range of services at the right time Increase supportive care into the community through use of social care/worker volunteers Provide Rehabilitative Palliative Care though increased use of Allied Healthcare Professionals ensuring that our patients are able to achieve what is important to them Increase services provided on the bedded unit to reduce the need for hospital admissions Ensure our care reflects the needs of and is accessible to all communities across North Devon Pro-actively engage with our beneficiaries to ensure involvement in the development of responsive care Using Public Health data, ensure our care remains responsive to the changing needs of our patients.

By working closely with other healthcare partners, patient care is enhanced and widened to ensure they and their families are supported during the last phase of life. We want to: • • •

Identify joint working projects with non-cancer charities Develop training and support to out of hours services.

Establish a clear patient pathway when working in partnership with District Nurses, Rapid Intervention Centre, Pathfinders and out of hours services.

Work with a broad range of providers, charities and other organisations to ensure equal access to our care.

Establish a formal partnership with Northern Devon Healthcare Trust and Petroc so that North Devon Hospice is a formal training location for the University of Bolton’s nursing degree course Work with GPs to establish support around new End of Life Quality & Outcomes Framework

Provide education opportunities for the health care workforce to enhance end of life care

By promoting a learning environment within the organisation in order to develop and maintain a confident, competent and skilled care workforce capable of delivering a model of care to meet the changing needs of patients.

By being recognised as a centre of excellence for palliative and end of life education by other health and social care providers in North Devon.

By creating an environment of openness, honesty and trust, which values every person as a unique individual.

We want to: • Achieve “One Team” by developing creative ways to

engage with all staff no matter their location • Embed 1:1s and quarterly ‘My Objectives’ conversations,

building on our strengths with a focus on personal and

career development • Deliver an organisational Wellbeing Plan which is personal

and covers everything from Mental Health to Nutrition • Develop recruitment, induction and probation processes,

based on our core competencies, that explore individual

values as well as requirements for the job • Extend leadership development and management skills • Support cross team project working, job swaps, shadowing,

multi-team away days and team events • Extend diversity action plans focusing on age, race, LGBTQ+

and disabilities • Publicise staff stories and staff survey feedback - actively

promote how good it is to work for the hospice • Embrace agile working • Develop flexible work spaces to facilitate collaboration and

cross team working so that people may sit together when

Volunteers are the lifeblood of so much of what we do and quite simply, we wouldn’t be here without their ongoing support. Therefore we want to create an environment where every volunteer is valued and recognised for their contribution to the success of the hospice. To achieve this we want to: • Hold recruitment ‘roadshows’ and ‘open days’ to

showcase what we do, targeting different generational groups across

the community • Engage and empower volunteers to promote the hospice, raise

awareness, dispel myths and promote volunteering at the hospice • Develop a pathway for work experience students to be come

volunteers and engage with younger volunteers.

• Build on Corporate Volunteering to encourage individuals to join us as volunteers • Actively promote how good it is to work for the hospice through

volunteer stories and publicising volunteer survey feedback on the website • Develop volunteer induction, including a ‘buddy system’,

taster sessions, and shadowing opportunities, matching volunteer skills

sets and wishes to roles as far as possible • Enhance communication with volunteers through different ways

recognising that one method is not suitable for all • Develop a volunteer social events programme, including

‘bring a friend’ social events • Raise awareness with incoming populations and new housing developments.

• Create a dedicated video about volunteering at the hospice clearly

articulating the impact that any role has on our care • Develop a learning hub and facilitate access to free learning

opportunities and resources • Complete a volunteer skill, learning and development audit / matrix,

maximizing the skills, knowledge and experience available to support

the hospice.

This has helped us to streamline our processes, engage our teams and enhance our care and support. We plan to continue to move with the digital era and embrace emerging technologies to enhance our care, impact and effectiveness. We want to: •

Create a digital hospice tour to help let people know the hospice is not a scary place. We hope it will support people to feel prepared for when they visit Use virtual reality to enhance the experiences of patients and their families Use technology to enable hospice teams to access hospice systems in a safe, secure and user friendly way Create an on-line shopping experience Create a ‘supporter account’ profile Develop a hospice app specifically for our patients and their families Develop the use of tele-medicine to enhance patient and family care Deliver a digital plan to enhance our income opportunities Review emerging technologies to enhance our environmental sustainability.

Every year, the counter resets to zero and we start again, raising money in a multitude of ways to ensure we can keep being there. We want to achieve financial sustainability to secure our future.
