Getting Started Welcome to the Nippon Life Benefits online portal registration process.

Primary Administrator Responsibilities and Access The Primary Administrator is responsible for managing Portal access for their business unit. This includes determining which users may be granted access, such as Secondary Administrators and Brokers, and assigning appropriate permission levels.

Eligibility

• The Primary Administrator must be an employee of the Policyholder

Managing User Access

• Primary Administrators may grant Portal access to Secondary Administrators • Secondary Administrators cannot receive permissions that exceed those assigned to the Primary Administrator

Changes to Primary Administrator Access

• Only Nippon Life Benefits Customer Service may modify a Primary Administrator's access • To appoint a new Primary Administrator, the administrator must provide the signed web access form from an authorized officer of the Policyholder. For more information, contact admin-info@nipponlifebenefits.com

Portal Registration and Activation

• Primary Administrators are granted access to Employer and Member Support Tools based on permissions granted on the signed web access form

• All users, including Primary Administrators, must register for Portal access • After registration, user credentials are verified through a security authentication process • Access is activated once verification is successfully completed • Please allow up to three business days for authentication and activation

Account Registration: Employer / Broker

Nippon Life Benefits online portal registration process Step 1: Visit the Employer Registration/Login page

Step 2: Begin Registration • Select Login/Register → Choose I’m an Employer → Select Create My Account Step 3: Verify Your Email • Enter your email address → Select Start → Enter the verification code sent to your email → Select Verify, then Next Step 4: Create Login Credentials

Enter the following: • Username (minimum 4 characters) → Password (6–32 characters, case-sensitive, at least one non-letter character) • First and last name → Email address • Three security questions and answers

Select Submit, confirm your password, then select Next.

Step 5: Enter Account Number • Enter the 6-digit Unit/Location Group Number • Multiple accounts may be separated by commas • Once submitted, your registration request may be activated immediately. If additional verification is required, you will receive an email confirmation once the account is activated.

Broker Registration Broker registration follows the same process as Client registration, with the addition of: • Option to use Manage Access to add additional groups after initial registration • Permissions must be set for each client group individually MFA (Multi-Factor Authentication): Adds an additional layer of security

• Setup (1st time): Enter email/phone→ Enroll→ choose email/text→ enter code→ (optional) trust device 30 days→ Submit • Ongoing Login: Unless trusted for 30 days, you will complete MFA verification at each login

Password Recovery • Forgot password: Enter username → verify → set new password • Forgot username: Enter email → follow email instructions

Reports

Users with enrollment permissions will see an Enroll button in the “Find a Member” box. Selecting “Enroll” opens the New Member Enrollment workflow.

• For existing members, select “Enrollment” from the top-level navigation after locating the member to update enrollment details

• Enrollment features may vary based on assigned permissions and account settings The Enrollment tab allows users to: • Add plan participants • Add dependents • Update personal information • Update coverage • Update Flexible Spending Accounts • Terminate coverage

• Reinstate members • Void coverage entered in error

All enrollment actions require:  Date of Change  Reason  Signature Date

Enrollment: Adding a Plan Participant • Users with enrollment permissions will see an “Enroll” button in the “Find a Member” box. Click “Enroll” to start the New Member Enrollment process

• To update an existing member’s enrollment, first locate the member, then select “Enrollment” from the top navigation • Enrollment options and functionality vary based on user permissions and account settings Step 1: Complete the Event Section

Please complete the appropriate steps below to enroll a plan participant and dependent(s). The “Date of Event” could be the date of hire, date of marriage, date of loss of other coverage, etc. The “Signature Date” is the date the member signed the enrollment form.

 Use the Enroll button to initiate a new enrollment only  To update an active member’s enrollment, select Enrollment from the navigation menu

All enrollment actions require entering the following:

• Date of Change • Reason • Signature Date: Eligibility is processed based on the “Signature Date” provided.

Enter the following information: 1. Name 2. Date of Birth 3. Social Security Number 4. Gender

5. Address 6. Phone Number 7. Date of Hire 8. Employment details (salary, hours, occupation, etc.)

Please Note:

Social Security Number is a required field. Non-U.S. employees without a Social Security Number must enter 99999-9999 to continue the enrollment process. All other employees MUST enter their VALID Social Security Number.

9. Citizenship/Rotational Status is required for all enrollments. Select one:

 U.S. Citizen  Permanent Resident/Green Card Holder (Long-term Legal Residency)  Expatriate/Rotational Employee (working outside their home country on assignment)

All required fields must be completed before moving to Coverage.

Enrollment: Coverage Options Step 3. Coverage: Available options depend on the selected location and group configuration.

• Select the appropriate Action (Not applicable for new enrollments) • Enter the Effective Date (the first date of coverage) and select the Location/Division • Choose the applicable Benefit Plan • Elect coverage options (Medical, Dental, Vision, Life, STD, LTD, etc., as applicable)

Step 4. Life Elections (If Applicable)

If Life or AD&D coverage is selected, complete: • Election status • Coverage amounts (if applicable) • Supplemental elections (if applicable)

Final Step: Review & Submit • Select “Next” • Review the “Summary of Enrollment Changes” • Confirm all information is accurate • Select “Submit” to complete the enrollment request Adding Dependents

Find the Plan Participant.

• Use “Find a Member” to locate the plan participant • Select the “Enrollment” tab to request an eligibility change • Complete the “Event” fields and the “Personal Information” section

Access and view forms and documents, such as the formulary drug list, administration guide, claim forms, and other forms relevant to specific user group(s).

2. Nippon Broker and Client User Guide

View, print, and download the portal user guide.

3. Premium Statement History

Access to view monthly billing statements. You receive an email notification when a new statement is available. New invoices are available by the 25th of each month. There are two types of premium statements:  Original - The initial list bill for a specific billing period

 Revised - A full reissue of the list bill for a specific billing period, which replaces

the original bill

4. Print Temporary ID Card

A member must be selected in the “Find a Member” box to view, download, or print ID cards.

5. Reports

View, print, or download experience and/or wellness depending on group size.

6. Request a New ID Card for a Member

Request new ID cards for members.

7. Special Delivery Prenatal Program

Access the Prenatal Program Brochure.

Online Portal Payment Guide (Employers Only)

The Online Payment Portal (OPP) allows you to make one-time payments or set up AutoPay using a bank account (ACH) or credit/debit card.

Credit/debit card payments incur a separate fee, which will appear as a separate transaction on your statement.

To set up online payments, follow the steps below. This feature is available only to employers.

Complete and Submit the Form

Before Online Payment Registration • Fill out the Web Access and Online Payment Portal Form (link to change permissions form) • Have the Primary Administrator sign the form • Email the completed form admin-info@nipponlifebenefits.com OR • Attach the form to Messages in the Employer Portal

Authentication • Once the form is authenticated, access to the Online Payment Portal will be granted • Additional authentication is required at login: select Send Code to receive a verification code via email or text message

• Enter the code and choose either Private Browser or Public Browser to complete the login Accessing the Portal • After login:

 The Online Payment Portal link will appear in the My Client Links tile (above the Premium Payment History link)  Click the Online Payment Portal link to go to the home page

Payment Options • On the portal home page, make payments by selecting:

 Auto-Pay  Pay My Statements  Allows one-time payments and recurring scheduled payments

Recurring Scheduled Payments • Complete payment method details:

 Credit card, debit card, or banking information • Click Recurring Schedule under My Profile to:  Pick payment date  Set other options

Important Notes • Check with your bank to ensure there is no Debit Block on the account  If a Debit Block exists, the account cannot be accessed  ACH ID is Company name: NIPPON LIFE BENE, ID: 0000063576
