Gender-Affirming Care and Homelessness: An Annotated Bibliography June 2023
Introduction Gender diversity is a social determinant of both health and housing. 1 2 Trans homelessness has increased 113% since 20163 and trans individuals disproportionately experience unsheltered homelessness, further exacerbating health inequities that exist among this population. As such, gender-affirming care is an important component of Health Care for the Homeless (HCH) services. The phrase “gender-affirming care” often refers to health care sought by trans and non-binary individuals that is trauma-informed and patient-centered. Gender-affirming care can range from medical interventions, including surgical interventions and hormone replacement therapy, to non-medical gender affirmation, including support for social and legal pathways to transition. 4 While data exists exploring the link between gender diversity and homelessness, the connection between trans identity, housing status, and access to gender-affirming health care has not been well documented in the literature. Most studies seek to understand trans identity in relation to housing status or health care separately and research on clinical gender-affirming care outcomes and delivery rarely includes disaggregation based on housing status. People experiencing homelessness face unique challenges to accessing health care. Trans and genderdiverse individuals’ access to gender-affirming care should be understood in this context. Some questions that motivated this publication include: • • •
What are outcomes of gender-affirming care for unhoused trans individuals? What are outcomes of gender-affirming care for unhoused vs. housed trans individuals? How does housing status impact access to gender-affirming care?
