Warnings: XEOMIN may cause serious side effects that can be life threatening. Call your doctor or get medical help right away if you have any of these problems anytime (hours to weeks) after treatment with XEOMIN:
• Problems with swallowing, speaking, or breathing can happen within hours to weeks after an injection of XEOMIN if the muscles that you use to breathe and swallow become weak. Death can happen as a complication if you have severe problems with swallowing or breathing after treatment with XEOMIN.
o People with certain breathing problems may need to use muscles in their neck to help them breathe and may be at greater risk for serious breathing problems with XEOMIN.
o Swallowing problems may last for several months, and during that time you may need a feeding tube to receive food and water. If swallowing problems are severe, food or liquids may go into your lungs. People who already have swallowing or breathing problems before receiving XEOMIN have the highest risk of getting these problems.
• Spread of toxin effects. In some cases, the effect of botulinum toxin may affect areas of the body away from the injection site and cause symptoms of a serious condition called botulism. The symptoms of botulism include: loss of strength and muscle weakness all over the body, double vision, blurred vision and drooping eyelids, hoarseness or change or loss of voice, trouble saying words clearly, loss of bladder control, trouble breathing, trouble swallowing. These symptoms can happen hours to weeks after you receive an injection of XEOMIN. These problems could make it unsafe for you to drive a car or do other dangerous activities. Do not use XEOMIN if you are allergic to XEOMIN or any of the ingredients in XEOMIN (see the end of this Guide for a list of ingredients in XEOMIN), had an allergic reaction to any other botulinum toxin products such as rimabotulinumtoxinB (MYOBLOC®), onabotulinumtoxinA (BOTOX®, BOTOX® COSMETIC), or abobotulinumtoxinA (DYSPORT®) or have a skin infection at the planned injection site.

• have a history of swallowing problems or inhaling food or fluid into your lungs (aspiration)

The combination of radio frequency (RF) and microneedling is a powerful one, but the way RF energy and microneedles are placed into the skin can determine whether a treatment is effective or not, and comfortable vs. painful. Every person and every body is different, and being able to tailor an RF microneedling treatment to each p atient is why VIRTUE RF MICRONEEDLING has risen in popularity so quickly.
VIRTUE RF's no-downtime treatment is truly ageless and can be used as a preventive and corrective procedure for light to dark skin tones.
What truly sets VIRTUE RF apart is that it incorporates its pain-free needle delivery and unique energy settings into three handpieces to target multiple areas of the face, neck and body.
Single-needle handpiece targets delicate areas around the mouth, eyes and jawline that require absolute precision.
Improves skin tone and texture, tightens lax skin, and reduces fine lines and the appearance of scars on the face, neck, décolleté, and hands.
Tightens, smooths and revitalizes skin on the arms, abdomen, glutes, thighs, and other large areas of the body.

Having transitioned from crafting products in her bedroom to accessorizing Hollywood, Jennifer Fisher is a self-made superstar. Though life as a high-end jewelry designer may sound all glitz and glam, she assures that, whether it’s dancing in her kitchen or saving for special occasions, she’s just like us. —Isabelle
“My favorite beauty product is Macrene Actives High Performance Eye Cream—I have never used a product as effective. I noticed a difference within a week that my eyes looked brighter.”
Fisher swears by MERIT Beauty, Lawless and Charlotte Tilbury. “I use all Merit Beauty products, as well as Lawless Forget the Filler in Rosy Outlook and Charlotte Tilbury Lip Cheat in Iconic Nude for my signature lip,” she says. “My lips are very pink, so I try to create a beiger look with these products.”
Having struggled with Hashimoto’s, an autoimmune disease, Fisher discovered that the best nutritionist was herself. “I’ve spent so much money on programs—nothing was working for me.” Now, she prefers doing what feels good and cooking her own recipes, like her favorite Tribeca Tuna and “You’re a Goddess, Eat Like One” salads.
We all have that one meal that we save for special occasions, and for Fisher, it’s just as luxurious as she is: “Caviar, oysters, and a great glass of red wine.”
“I never leave the house without my My Scent perfume and my JF charm necklace. If my necklace is not on me, it’s in my bag. It’s my good luck talisman that represents everything in my life that I love. My Scent was also intentionally designed to be used on the go, so I throw it in my makeup bag every day so I can reapply throughout the course of the day.”
“My favorite singer is Khalid, hands down. I always play Khalid in the kitchen while I’m cooking. His songs put me in a good mood and my whole family loves listening with me.”

“Prakti is all about making Indian beauty accessible to everyone. Yes, it focuses on the physical aspect of beauty—hyperpigmentation, acne, brightening, plumping— but it’s also providing these sensorial experiences. Ayurveda is the science of life. If you’re feeling good on the inside, you’re going to be better overall.”
“It’s sad that we call Ayurveda and Indian beauty a ‘trend’ when it has so much history! A lot of people are looking for a more balanced way of living their lives and nurturing their souls. The western market is so open and excited about exploring new cultures and understanding that maybe ‘the unknown’ is something that may seem a little exotic at first, but in reality, it’s just a different understanding and education. Ayurveda has been around for thousands of years and it’s proven—it can really benefit lifestyles.”
“I have so many supporters and I’ve been a supporter of so many brands, especially Dr. Barbara Sturm. I’ve been on beauty panels with Dr. Sturm and I love the way she approaches beauty. Look at Fenty and Rihanna—she started a whole movement around diversity and inclusion. She is really embracing skin tones across the spectrum. It’s important when we talk about inclusivity that it’s the full spectrum. There are so many brands I love: Sunday Riley, Summer Fridays, Chanel...the list goes on.”
“Self-care is very important to me, especially when I travel. I just got back from India and it’s a pretty tough time difference—10.5 hours ahead. You need to take care of yourself or you can’t perform your best. Even if it’s something as simple as drinking enough water or making sure you’re eating three to four times a day. I make an effort to remind myself to get up and drink water because when I am working, the day flies by. I also try to do a couple of extra pampering-hydration treatments when I fly. Then, I take some time at the end of the day to do some meditation and breathing exercises. When you’re on a busy schedule, five minutes makes such a big difference.”
“If you’re feeling good on the inside, you’re going to be better overall.”
The singer-songwriter, EGOT winner and entrepreneur is sending his creative spirit in a new direction: skin care. Here, John Legend opens up about his new brand for melanin-rich skin, Loved01, as well as the high-low mix of products he keeps in rotation. —Brittany Burhop Fallon
“I love our Moisturizer. It’s really creamy, and I like to use it in combination with the Face + Body Oil, which is a nice beard oil, too. We really focused a lot on moisture for this first launch because it’s such a salient concern for so many black and brown people. We’re known to lose moisture two and a half times faster than lighter skin tones, so it’s actually scientifically proven that we need moisturizing products more than others do.” $10–$15, loved01.com
“I think the cool thing Chrissy [wife Chrissy Teigen] and I like about the Bioré Strips is that you see the results right away. I didn’t even know what a blackhead was until Chrissy taught me. She can actually look at my nose and do the extractions herself, but it kind of hurts because she doesn’t do all the steaming and everything that a facialist would do.” $9, ulta.com
“I’ve worked with SK-II in the past, and their products feel very luxurious. Chrissy taught me about this face mask and we both really like it. Having a partner who knows how to take care of her skin has been helpful for me because I’ve learned a lot from her over the years.” $95, sk-ii.com
”Tom Ford Black Orchid is my everyday finishing spray [laughs]. I’m not dressed until I put it on. I discovered it four or five years ago and I’ve been wearing it ever since—I really don’t like anything else as much. Once you figure out what you like, you go with it.” $150, sephora.com
“I like SheaMoisture’s Shea Butter Shampoo and Conditioner, and I usually wash my hair two times a week. I put a little moisturizing gel in it every day, which leaves it kind of nice and glossy. I’m pretty simple with my hair, aside from very frequent haircuts. My brother is my barber, and he does a light trim or a full cut on me at least once a week.” $11 each, target.com
Legend says he’d been thinking about his own line for years, but he didn’t want to create something new just because he’s “a celebrity with nice skin. There needed to be a real reason for it to exist.” That reason, he explains, was the “need for more skin care made for melaninrich skin, and the need for more study about dark skin tones in dermatology and product development.” Another critical factor: “Loved01 needed to be affordable and accessible: something people could buy at their local CVS.” Scan

“Self-care looks like quiet time, proper hydration, travel, adventure, water sports in warm oceans, spa escapes, aromatherapy, dancing, eating of the earth, eliminating consumption of artificial sugars, and detoxing that amazing organ, the liver! Thanks to Angelique Kidjo, I’ll be adding Transcendental Meditation to this list via the David Lynch Foundation.”
“My beauty routine consists of proper hydration, regular facials and moisturizing. My favorite brands are Young Living, yFOY, La Mer, conditionHER, Dermalogica, Soothing Touch’s over-sized lip balms for my beautiful, full lips, and an amazing new line by a friend of mine, Octava.”
“During the pandemic, I took a closer look at my health and leaned into the fact that what I eat is also what I use to repair my body.”
“Proper alignment, strength training, cardio, restorative yoga, and a lot of foam-rolling and stretching make up my fitness routine. I also like to frequent the StretchLab when my schedule permits.”
“I am really into supplements right now. Standard Process has been a constant for me for high-quality supplements, as well as some of Young Living’s and Vitamin D3 by Allergy Research Group. My go-tos for healthy eating are Dr. Sebi’s Alkaline Foods, Medical Medium recipes and Bieler’s broth.”
“The StretchLab and deep-tissue sports massages are my favorite way to indulge.”
“Self-care looks like quiet time, travel, adventure, warm oceans, spa escapes and eating of the earth.”
“Wellness is vitality through mind-body awareness and following practices that allow me to experience true harmony and peace.”
Racking up the highest ratings of any new network comedy in four years, NBC’s Night Court revival has already been picked up for a second season. No one is more excited than comedian-actress India de Beaufort, who plays the assistant DA, which John Larroquette played in the original series. “I have been in this industry for 20 years,” she says. “And I have never heard the words ‘Season 2.’”

This spring, Ali Wentworth is looking forward to sharing the Brooke Shields documentary, Pretty Baby—it streams on Hulu April 3 via her production company, BedBy8—with the world. But, before that, she was a teenager with a Noxema and “wings” obsession.
“I decided one afternoon (my parents were not home) to make myself look like Farrah Fawcett. So, I smothered my body in a fake tan lotion and sprayed Sun-In all over my hair. I laid outside for hours on a tray of tin foil. The result was not Farrah Fawcett, but Carrot Top.”
“One of the first beauty items I ever bought was Sea Breeze. I used to clean my face with cotton balls and Sea Breeze about 10 times a day. And Bonne Bell Lip Smackers. I tried to collect every flavor from Sprite to Strawberry. And Noxema, but that was because all the teenage models for Noxema were so pretty.”
“My mother has always encouraged me to do beauty in a very natural way. She believes in a fresh face. Less makeup, more dewy and moist.”
“When I was a teenager, I cut my hair to have wings. They were always in my way. And when I started cutting my own, they were never even.”
“I love smoky eyes. The 1990’s black-and-gray blended eye! It’s so dramatic and fun.”
“I realized I don’t have to keep up with trends. At my age, I want to look and feel like myself. When I’m not working, I prefer a clean face. Just moisturizer, lip balm and a nice scent. Right now, it’s Malle’s Portrait of a Lady.”

WHEN APRIL WILCOX ENTERS THE ROOM, people look. Partly because of her innate physical beauty and also due to an inner glow that stops time for a moment In December 2022, Wilcox was crowned the Fab Over 40 winner as chosen by the public. The Competition was run as a fundraiser by Colossal to benefit National Breast Cancer Foundation (NBCF) Wilcox, who turned 40 in early 2022, shares why the Competition is so meaningful, what it’s like to be 40 and fabulous, and her tips on aging gracefully (with a little help).
It’s worth shouting from the rooftops that the Fab Over 40 Competition raised over $7.4 million for NBCF! How do you feel about being a part of it?
I am humbled by this win because, being a nurse for much of my adult life, I have walked with so many women through their journey of breast cancer reconstruction. From their first call after they found out to rebuilding with them to feeling the confidence cancer stole from them. We have laughed and cried together.
You’ve realized that each breast cancer journey is different and that those boundaries of grief should be remembered and respected.
It’s important to allow women to be able to process their diagnosis, whatever it may be, and walk that journey in their own way. Many people make comparisons and share their experiences to relate to the situation—
however, with everybody giving their own stories and side notes, it can become exhausting for them. Most of our patients just want friends and family to listen and offer their shoulders to cry on or vent to.
What makes you feel fabulous over 40?
This is the best time of my life. Personally, I have a wonderful 17-year-old son and am so in love with my husband We live on a half-acre property with our three dogs in my hometown of Edmond, OK. Professionally, I am an aesthetic RN at Shadid Plastic Surgery and am on a mission to help women (and men) feel their best by using the tools we have that allow us to age however we feel appropriate Being a nurse injector is a lot of fun and very artistic.
Tell us about the shift in the aesthetics world over the last few years.
It’s gone from just putting filler in your face to balancing your face The best

What is your go-to for looking so flawless in your 40s?
thing about my job is to let everyone still be and look like themselves, but the best version of themselves My surgeon is so science-minded that we don’t jump on bandwagons because a rep says a product is awesome. We do all the research. My newest favorite is DAXXIFY (like botox) It’s the only vegan option and seems to last double the time.
What advice do you have for those struggling with the aging process?
Embrace yourself. It’s more of a mindset than it is a number. We have much more wisdom in our lives from experience. Taking care of your mental and physical health with the right foods and exercise and engaging in the things that bring us joy are all essential to aging gracefully. Botox doesn’t hurt, either.
All the things are my favorite! But in moderation. The goal is not to change our faces; it’s to be ourselves and keep them guessing how old we are So as long as you remain looking like yourself and don’t overdo anything, all the tools in the toolbox are open game Often, people only see the botched stories, and not all the beautiful filler and Botox placed appropriately Don’t be scared of it. If you’re in the right hands that you feel comfortable in, and you’ve seen their work, then you know what they can do.
What do you say about Botox, fillers, and other tools to those who are interested but skeptical?

TAKING CARE OF YOUR MENTAL AND PHYSICAL HEALTH...AND ENGAGING IN THE THINGS THAT BRING US JOY ARE ALL ESSENTIAL TO AGING GRACEFULLY.
Swedish model and actress
Anna Karin is the ultimate optimist when it comes to aging, and she has some great advice on the matter “Try to accept that we all age, and we do so together. You’re not alone,” says Karin. “I would encourage people to count their blessings instead and be grateful that they get to stick around,” she adds
While Karin was fortunate to be born with good genes, she won’t turn down a quality, noninvasive treatment “We have amazing opportunities to not look our age. I’m an Ambassador for an amazing health and wellness company called Q Sciences,” she says. “I love, love, love their medical-grade collagen, which made my hair grow so thick and healthy in just a few months. From innovative in-office procedures to skin-care selections popping up by the (quite literal) second, this past year in beauty can best be described as one that was stacked. Of the more than 10,000 products and treatments we tested throughout the year, and with help from top doctors, experts and celebrities who helped us along the way, 399 submissions stood out from the rest.
Whether it was a key ingredient sourced from a newly discovered botanical, or a delivery system we’ve never seen before, brands really stepped up their game this year, making our job that much more difficult (and fun!).
We’ve also expanded our categories to bring you breakthroughs in sustainability, pregnancy, menopause and men’s grooming (yes, we actually have many male readers!), and our curated mix of both brand-new and tried-and-true does not disappoint.
So here’s to this very special section of award winners who earned our coveted and trusted NewBeauty Seal: the edited of the edited, the curated of the curated, and at the end of the day, the ones that deliver on all their claims to our group of very discerning testers. We encourage all of you to try, test and tell us what you think.

In Ancient Egypt, castor oil and almond oil were often used to promote hair growth.
“They would also use concoctions made of dates, dog paws and donkey feet,” says New York facial plastic surgeon Yael Halaas, MD. In India, they also had a tradition of standing on your head to increase blood flow to the scalp to improve hair loss, adds New York dermatologist Julie Russak, MD.
Spices, beetroot, and some eyelike urine were used
Spices, beetroot, myrrh, and some eyeopening ingredients like urine were used during the Greco-Roman era, when hair was an indicator of youth, wealth and status.
“The famous Greek physician Hippocrates believed that a mixture of spices, pigeon droppings, horseradish, beetroot, and opium could cure hair loss, which he suffered from himself,” says Dr. Halaas.
In 8th century China, safflower oil, rosemary, Fo-Ti root, and various Chinese herbs were ground together with animal parts and used as a hair growth potion. Across Scandinavia, follicularly challenged Vikings would rub goose droppings on their heads in hopes of growing new strands. Saint Hildegard of Bingen, who lived in 12thcentury Germany, wrote of a baldness cure that called for leaving bear fat and ashes on the scalp for “a long while.”
Thankfully, today we don’t rely on antiquated animal-based mixtures, but we are using follicle-stimulating growth factors. “A treatment plan might include platelet-rich plasma injections, topicals, supplements like Nutrafol, nutritional changes, and lasers,” notes Dr. Day. Doctors may prescribe medications like finasteride and topical minoxidil says Dr. Russak. Recently, low-dose oral minoxidil has also emerged as an effective hair growth solution.
Hair transplant surgery evolved in the ‘90s with Follicular Unit Transplants, where “donor follicles are removed as a full strip from an area of healthy growth,” explains Wayne, NJ facial plastic surgeon Jeffrey B. Wise, MD. “Follicular Unit Extraction, a technique whereby grafts are harvested individually using a small punch, emerged in the 2000s.”
Like the Ancient Egyptians, Victorians used castor oil, almond oil and gentle tonics, but also relied on rigorous scalp massages to promote blood flow. Silk nightcaps were also introduced to fight friction and minimize hair loss.
Modern-day restoration stems back to work done in Japan in the ‘30s, but it wasn’t until the ‘50s that hair transplant surgery was born. “A dermatologist named Dr. Norman Orentreich discovered that hair follicles could survive when moved from one site to another,” says New York dermatologist Doris Day, MD. “At first, the outcomes appeared unnatural in that clumps of follicles, called plugs, were placed with little regard to the natural hairline or final look.”
True or false: Natural teeth need to be shaved down to pegs to get veneers?
Learn the answer and more as top dentists demystify the most commonly misunderstood aspects of this popular smile procedure.
Most veneers require minimal to no tooth reduction.
Ask anyone considering veneers: their biggest roadblock is often the thought of needing to have their natural teeth shaved down. Dentists say they hear this concern all the time, but the process doesn’t have to be as drastic as you think. “Patients fear that their own natural teeth will be drastically damaged in order to have veneers placed, but the reality is that most veneers only require minimal to no tooth reduction,” says New York cosmetic dentist Steven Roth, DMD.
“The teeth do not need to be shaved down to pegs,” New York cosmetic dentist Husam Almunajed, DMD adds, noting that porcelain veneers are minimally invasive by design. “When done well with the occasional no-prep veneer, there’s no tooth reduction.” New York cosmetic dentist Jason Kasarsky, DDS calls no-prep veneers “the most conservative treatment for smile-altering aesthetics.”
Porcelain veneers have many levels of quality.
Veneers are not one-size-fits-all.
“There are machine-made porcelain veneers, hand-layered porcelain veneers, and many variations in between, all with varying quality,” says Dr. Almunajed. “It’s important to communicate with your dentist about this so you know the level of quality you’re getting, as it can make all the difference.” Porcelain veneers are not all made equal, which is why some people may get the obvious “Chiclet smile,” and others will get a youthful, seamless natural-looking smile.
When it comes to your smile, you have to think of the big picture. “A rejuvenated smile can help support the face, broaden the smile and make a person appear happier,” says New York cosmetic dentist Timothy Chase, DMD, noting that he thinks veneers give people “superpowers.” If you’ve never felt confident in your smile, veneers can change that. Dr. Chase also points out that studies have shown that people with nicer smiles are perceived as more successful.
When putting time and money into a cosmetic treatment, we hope for a good return on our investment. Los Altos, CA cosmetic dentist Joseph Field, DDS says, “Veneers should last around 15 to 20 years if they are properly maintained.”
Dr. Almunajed adds that “porcelain veneers are the most conservative dental treatment, and research shows they are also the longestlasting dental restoration, over crowns and fillings.”
“One thing people may not know is that the materials used in veneers are stronger now than they have ever been before,” says Sarasota, FL cosmetic dentist Jenifer Back, DMD.
“Lithium disilicate is the material of choice currently, and it is more wear-resistant than our own natural enamel.” This means veneers can make your smile even stronger.
“Veneers are designed to look, feel and act like natural teeth,” says Dr. Field. “As such, they need to be cared for like natural teeth, including brushing at least twice a day, flossing daily and getting regular cleanings with your oral health-care professional.”
Additionally, Dr. Field notes that those with veneers should use a nonabrasive toothpaste.
Veneers aren’t only used to address cosmetic concerns; they can also help relieve pain and discomfort. Dr. Chase says veneers can be used to improve the function of your jaw and can also help eliminate headaches and TMJ issues.
Dr. Roth says patients often come in concerned that their veneers will look obvious and their smile won’t quite match their face. However, he assures them this doesn’t need to be the case. “The reality is that porcelain veneers can be as lifelike and realistic as a patient wants.” To ensure patients get the results they want, Dr. Roth says it’s “critically important that they view the before-and-after cases their doctor has performed.” Dr. Kasarsky adds that “while it’s a challenging task for the operator, when performed by a qualified, skilled practitioner, patients have predictable, long-lasting, beautiful results.”
COFFEEANDMILK/GETTY IMAGES; IMAGE USED FOR ILLUSTRATIVE PURPOSES ONLYAfter COVID-related lockdowns put medical tourism on pause, our desire for improvement came back stronger. The “Zoom Boom” that famously sparked countless cosmetic treatments also launched a thousand flights to exotic locales in search of a better deal, but at what cost?
—Tatiana BidoBodies are a big business abroad, and when it comes to breast and body work like tummy tucks, Mommy Makeovers and curvaceous Jessica Rabbit-esque bo dy contouring, experts say popular de stinations include Mexico, Brazil, Colombia, and the Dominican Republic. “In general, medical tourism is risky,” says New York plastic surgeon Brad Gandolfi, MD. “To protect patients, there is significant physician oversight, training and credentialing in the U.S., which is not replicated in most foreign countries.” If something does go wrong, surgeons caution there’s a lack of recourse while abroad, and sometimes even when you return.
The most common destinations for facial cosmetic surgery include Turkey, Dubai and South Korea. “Because of th e complex nature of these facial procedures, surgery can be more expensive. However, if a patient’s surgery fails to correct a structural problem, creates other anatomical is sues, or looks aesthetically un desirable, then they will end up ne eding a revision,” says Miami plastic surgeon Paul Afrooz, MD, who has seen more than a few patients return home with complications. “A rhinoplasty performed by an unqualified surgeon can lead to excessive swelling, nasal collapse, breathing issues, infection, skin necrosis, and septum perforation.”
According to the Turkish Dental As sociation, 150,000 to 250,000 foreigners travel to Turkey each ye ar for dental work. While it may seem appealing to book an exotic vacation alongside a smile makeover, patients often return with subpar results, says Chicago cosmetic dentist Nathan Hoffman, DDS. “Other countries typically do not have regulatory organizations to enforce a standard that has our well-being at its core,” he explains, noting that it’s a quality-control issue. “Other factors like poor materials lead to dental work that doesn’t last or looks totally unnatural, hence the term ‘Turkey te eth.’ Patients end up spending substantially more to correct, repair or replace their ‘cheaper’ smile makeover in the long run.”
In South Florida, where many pe ople travel for affordable surgery, Miami plastic surgeon Sean Simon, MD says he’s seen hundreds of ca ses where unqualified physicians pe rform surgeries they’re not ce rtified or trained to do. “You have surgeons or people that aren’t eve n trained in plastic surgery pe rforming these procedures with higher risks, higher complication rates and a lack of follow-up.”
Eug ene, OR plastic surgeon Mark Jewell, MD adds that verifying a surgeon’s credentials with the International Society of Aesthetic Pl astic Surgery is crucial. “There are many exceptionally qualified plastic surgeons worldwide that are great at what they do,” he says. “ISAPS is a good resource for vetting international providers.”
When it comes to facilities, many popular plastic surgery destinations don’t follow the same protocols we do in the United States. “The quality of hospitals outside of the U.S. varies tremendously,” explains La Jolla, CA, plastic surgeon Robert Singer, MD. “Some are accredited and inspected; others are not.” Here, our facilities are certified through the American Association for Accreditation of Ambulatory Surgical Facilities to ensure they meet strict safety criteria. In addition to vetting a surgeon, surgical centers and facilities abroad should also be researched. “The lack of regulations just scratches the surface,” says Dr. Simon. “The worst-case scenario isn’t an infection or complications, it’s death. Do your research before undergoing any procedure, anywhere.”
Westborough, MA facial plastic surgeon Min Ahn, MD says timing is also a concern. It’s generally considered safe to fly two weeks after a facelift and one to two weeks after a rhinoplasty. “Flying beforehand can negatively affect he aling due to air-pressure changes, the physical strain of traveling, and potential exposure to illness,” he notes. For body work, surgeons advise waiting four to six weeks. There are major potential issues like “fatigue, dehydration, le g clots called deep venous thrombosis, and clots to the lungs, which may be life-threatening,” adds Dr. Singer.
Once patients are back stateside, there can be a lapse in care due to issues finding a local provider willing to take on a case done abroad. “I’ve seen patients have difficulty finding a physician or surgeon to help them should they ne ed assistance once they return,” Dr. Simon explains. “M any plastic surgeons will not see these patients, es pecially during the early post-op period.”
SOURCE: ISAPS
Dr. Afrooz adds that almost all revisions procedures are more complicated than the original procedure, and this is something traveling patients need to consider. “The surgeon must address the existing incision, make every at tempt to remove it or revise it, and then conceal the revision incision. These considerations add time, money and technical challenge to a revision,” he explains.
Dr Gandolfi recalls a cautionary case of a patient who traveled to Mexico for a cheaper tummy tuck: “She lost all the skin below her belly button and required multiple re constructive surgeries over the course of a few years to be able to return to her normal life.” Stories like this one highlight the long-term concerns with “affordable” cosmetic surgery deals.
This 35-year-old patient wanted to correct a breast augmentation done abroad that left her breasts disfigured. Dr. Simon performed a revision surgery to give her a more natural look.
The destinations with the most cosmetic patients traveling from abroad include Turkey (32%), Mexico (29%), Colombia (26%) & Thailand (23%).
Expert-recommended treatments and products to help get us summer-ready from the neck down.
—Brittany Burhop FallonCome summertime, maintaining a regular shaving routine can be exhausting, not to mention the risk of ingrown hairs right before a beach trip. Laser hair removal can eliminate the issue entirely “by utilizing a wavelength of light and heat to damage the growth area of the hair near the root,” explains Nanuet, NY dermatologist Heidi Waldorf, MD. “Each treatment will destroy 10 to 30 percent of the hair, and it takes three to five treatments at intervals of six to eight weeks to see a 70 to 90 percent reduction.” Although some wavelengths, like the diode, can be used for both light and dark skin tones, Dr. Waldorf prefers the long pulse Nd:YAG laser for darker skin tones, and the long pulse Alexandrite laser for lighter skin tones.
Though the results of laser hair removal last a long time, New York dermatologist Elaine F. Kung, MD says maintenance treatments may be needed once every six to 12 months if small vellus hairs (peach fuzz) grow back. At-home devices are great for maintenance as well, or full-blown DIY hair removal. One innovative device we like is Silk’n Infinity ($429, saks.com), which uses galvanic energy to open up the pores so the Intense Pulsed Light (IPL) can penetrate to the root of the hair follicle.
Another is SmoothSkin Pure Fit ($379, currentbody.com), which employs quick pulses of IPL to permanently remove hair, and can treat the full body in 10 minutes.
When the goal is to reduce fat and tone muscles, noninvasive Emsculpt NEO is the perfect way to prep for a bikini bod (when paired with a healthy lifestyle of course). “It combines two treatments to build muscle and reduce fat in just 30 minutes,” explains Fresno, CA dermatologist Kathleen Behr, MD. “HIFEM—high-intensity focused electromagnetic stimulation—is used to build up to 25-percent more muscle, while radio frequency is used at the same time to eliminate up to 30-percent fat. It is a very comfortable treatment that can be performed on the abdomen, outer thighs, buttocks, arms, calves, and most recently, the flanks. It really helps contour and strengthen an area, but is not intended for weight loss.” (Liposuction is still the gold standard in fat reduction.) Usually
four to eight weekly treatments are performed on patients with a BMI up to 35. Therefore, it’s ideal to begin treatments in early April if the goal is to see full results by mid-June
Another body-sculpting treatment called Cryo T-Shock is a favorite of Paris Hilton and other A-listers, and Miami-based aesthetician Amy Peterson is a fan as well. “It uses a combination of heating and cooling to break down fat cells, which the body will then naturally flush out through the lymphatic system,” she says. “We typically recommend five to 10 sessions spaced 10 to 14 days apart, so beginning in early April with this one is also beneficial.”
To complement these treatments with at-home care, Peterson suggests her clients use Alastin TransFORM Body Treatment ($205, alastin.com). “This cream was designed to be used in synergy with energy-based fatreduction and skin-tightening procedures, and addresses skin laxity and crepiness.”
“It really helps contour and strengthen an area, but it is not intended for weight loss.”
Undoubtedly one of the toughest body concerns to treat is cellulite, which impacts 90 percent of women and demand for effective treatments is greater than ever. This year, noninvasive skin-tightening treatment Sofwave gained FDAapproval for temporary cellulite reduction. It uses high-frequency, low-divergence ultrasound and heat to target dimples. “Sofwave provides a unique balance of efficacy without downtime,” says Houston dermatologist Jennifer Segal, MD. “Unlike more invasive treatments, which are better-suited to colder months when you can be covered during healing, Sofwave can be performed year-round.”
Two other FDA-cleared treatments for cellulite are noninvasive Resonic, which sends high-frequency sound waves into the skin to reduce dimples; and Avéli, a device that is inserted under the skin to release the septa bands causing dimples. Only one session is needed with all three of these treatments, but full results may take up to 12 weeks.
Keratosis pilaris, aka KP, which shows up as tiny red or pink bumps and makes skin feel rough, can be embarrassing enough to keep someone in long sleeves come mid-July. “The bumps typically show up on the backs of the arms and tops of the thighs, and need regular treatment to promote and maintain smoother skin,” Dr. Waldorf explains.
Dr. Kung recommends using “moisturizing creams with chemical exfoliants like glycolic acid, rather than physical scrubs.” Her favorites are lactic acid–based AmLactin KP Bumps Be Gone ($15, target.com), and Glytone Exfoliating Body Lotion ($44, glytone.com), which contains 17.5-percent glycolic acid.
Peterson is a big fan of Environ Alpha Hydroxy Derma-Lac Lotion ($76, shoprescuespa.com) and says it is one of the only body products she’s found that “truly softens the texture of rough, bumpy skin.”
While we’re still waiting for an in-office treatment that can make KP disappear instantly, Dr. Kung says there have been many small-case reports of lasers that can help improve its appearance.
“I use the Lumenis Stellar M22 Intense Pulse Light, which reduces the redness surrounding the hair follicles after three treatments spaced four weeks apart.” Peterson also favors a no-downtime body peel called BioRePeel Gold, which “dissolves the bonds between skin cells to carefully remove the top layer of skin, revealing the glowing, smooth layer underneath.”
“The bumps typically show up on the backs of the arms and tops of the thighs.”
In 1995, plastic surgeon Dr. Des Fernandes (he’s also the founder of skincare brand Environ) used hypodermic needles to help minimize wrinkles and scars. Soon after, he developed a stamp-like tool with surgical-grade needles on it that could temporarily puncture the skin to promote wound healing, and ultimately, collagen synthesis. Today, everyone from celebrities to soccer moms are opting for microneedling as demand continues to soar.
A deep-dive into the world of microneedling, which has become an in-office staple for head-to-toe rejuvenation.
Prospect, KY dermatologist Tami Buss Cassis, MD says, “The easiest way to explain microneedling is that ‘micro’-size needles create tiny holes, or wounds, in the top layers of the skin, which then promotes wound healing and stimulates the production of collagen and elastin fibers. As the skin heals, fine lines, pore size and overall texture improve to rejuvenate the appearance of the skin.” The treatment is also used to smooth a variety of scars, including those from acne and stretch marks.
Another major benefit of the treatment is that the open wounds serve as delivery pathways for any products applied afterward. “Because microneedling creates microchannels in the skin, we can take advantage of this by applying substances like platelet-rich plasma (PRP) or exosomes to deliver growth factors and peptides deep into the skin for more radiant results,” explains Beverly Hills, CA facial plastic surgeon Kimberly J. Lee, MD. “Even after the microneedling treatment is done, the PRP and exosomes continue to work to stimulate collagen production to enhancelonger-lasting results.”
There are three main types of microneedling treatments: “Electric, handheld devices that move across the skin with needles going in and out like a ballpoint pen; manual devices that require pushing the needles in, like a stamp, or rolling them over the skin; and radio-frequency (RF) microneedling, where RF energy is released through the tips of microneedle electrodes,” explains New York dermatologist Macrene Alexiades, MD.
Dr. Cassis favors a treatment called SkinPen, which falls under the first category (no RF energy), and is very popular for short-term skin-plumping and radiance. “We numb the skin topically, and then the procedure takes about 15 minutes,” she says, noting that there may be some discomfort during, particularly on the forehead. “The skin looks raw and red immediately after, and then it will peel for a few days. It takes about a week to have complete healing, but then you will have an amazing glow. I try to get it done at least twice a year now that I’ve turned 50!” Dr. Lee is also a fan of SkinPen, which she says was “the first FDA-cleared microneedling device,” and is clinically proven to improve the appearance of acne scars with a few treatments.
Chapel Hill, NC dermatologist
Beth Goldstein, MD uses a device called Rejuvapen NXT, which is another no-RF option. “It’s FDAcleared to treat facial wrinkles with little to no downtime, and I’ve seen great results, not only with smoothing fine lines and creases, but also acne scarring, enlarged pores and stretch marks.”
For the past 15 years, Dr. Alexiades has worked to help create and launch the prototype design for radio-frequency microneedling as we know it today. “As my research has shown, you must combine microneedles with radio frequency to get long-term wrinkle and scar reduction, and improvement in skin quality,” she says. RF microneedling is also a go-to treatment for skintightening. “The use of radio frequency adds the element of heat to also enhance the tightening of the skin,” Dr. Goldstein adds. “However, because of this, a higher pain level and more downtime may be necessary.”
Dr. Alexiades’s recent paper in the Dermatologic Surgery journal reveals that RF microneedling may also have a role to play in fat reduction. “We proved that when RF microneedling is delivered deep into the subcutis, it also remodels the fat while tightening the overlying skin. This can be offered as a single treatment and is effective for lax skin above the knee and localized fat deposits.”
A topical numbing cream is applied, which may take up to 45 minutes to kick in, but then the treatment itself is relatively quick. Dr. Goldstein says a full face can be done in around 30 minutes. Although there is numbing beforehand, there may still be some pain and discomfort. You may have seen Kim K.’s viral Instagram post about her Morpheus8 treatment “to tighten her stomach,” which she described as “painful but worth it.” Post-treatment, hydrating serums and other products are applied, and the skin will look like it’s been sunburned.
“Due to inconsistencies of penetration, it’s best to avoid the use of exfoliating products such as retinoids and topical acids for one week prior to treatment,” says Dr. Goldstein, noting that patients should also avoid aspirin, ibuprofen and naproxen during this time, as they thin the blood and increase risk for bruising or bleeding. “If those medications are required, any discontinuation should be discussed with the prescribing provider.”
THE DOWNTIME: This depends on which device was used, but it’s generally minimal with a day or two of redness and superficial swelling. “Within a week, you should notice improvement in skin tone and texture,” says Dr. Alexiades. Patients need to keep their skin dry and clean for 12 hours post-treatment to allow the microchannels or micro-wounds to properly close and heal, adds Dr. Goldstein.
NUMBER OF SESSIONS NEEDED: It varies per device, but for many, it’s recommended to have one to three treatments spaced four weeks apart.
BRANDS TO KNOW: Vivace, VirtueRF, Matrix Pro, Profound, Genius, Morpheus8, Intensif, SmoothGlo, INTRAcel, Secret, and VoluDerm. Profound debuted in 2018 and is still considered the strongest of the bunch with the ability to induce more collagen and elastin, but with that comes more discomfort and a higher price tag (though only one treatment is needed).
To improve the skin laxity and uneven texture on her arms, this patient received one Virtue DeepRF treatment. The “after” picture shown here was taken two months post-treatment.
“Microneedling of any kind is an opportunity to apply anti-aging products like growth factors that will penetrate better through the needle punctures,” says Dr. Alexiades. “When you combine microneedling with topicals, you have a shot at some collagen building. I typically apply a lab-based, highly potent hyaluronic acid with standard microneedling, and PRP with RF microneedling.”
“Stay out of the sun and avoid scheduling any major events for a week,” Dr. Cassis advises. “Use gentle cleansers and great hydrating products that will help stimulate healing. I really like Alastin’s formulas. Also, don’t worry if you look a little scary—it’s normal. The skin heals and you will love your results.”
Those who have active inflammatory acne, active bacterial or viral infections such as cold sores, or active inflammatory skin conditions like psoriasis or rosacea. If you are on a blood-thinning medication, consult with your doctor first. “If you have sensitive skin or already ‘perfect’ skin, I wouldn’t recommend microneedling, as it will incite inflammation,” adds Dr. Alexiades.
According to Dr. Goldstein, the tiny needles do not go deep enough to disrupt the results of fillers or neurotoxins. “However, we recommend that patients wait at least two weeks after neurotoxin injections for microneedling, and about one month after filler,” she says. “At the two-week mark, your toxin is at its best and cannot be manipulated by any swelling that can occur after microneedling. Hyaluronic acid filler patients will actually have the benefit of advanced collagen stimulation with microneedling.”
While microneedling with PRP can be used to address hair loss, Dr. Lee finds it more effective when PRP is injected directly into the scalp, which allows it to penetrate deeper. “Because the hair isn’t shaved with PRP, microneedling can sometimes be a little bit cumbersome and uncomfortable,” she explains. “By injecting PRP into the scalp, it can spread to adjacent areas of the scalp for improved results.”
Though some at-home microneedling tools claim to be safe, most doctors don’t recommend them. “At-home rollers are only 0.1 millimeter in depth and require someone to have the knowledge of cleansing their skin properly before using them,” says Dr. Goldstein.
LASERS,RADIO FREQUENCY, ULTRASOUND, OH MY! EXPERTS GET INTO THE DETAILS OF THE MOST SOUGHT-AFTER IN-OFFICE TREATMENTS RIGHT NOW.
BY ALLIE HOGANThe first FDA-cleared energy treatment for acne, AviClear is a groundbreaking laser that helps treat mild-to-severe acne by targeting excess sebum production. “AviClear works by selectively targeting oil glands and suppressing them so that over time, they produce less oil and less acne,” explains New York dermatologist Jody Levine, MD.
“This technology is revolutionizing the way we treat acne because until now, the only other professional treatment that targeted sebaceous glands was Accutane, which comes with significant side effects,” New York dermatologist Dennis Gross, MD says. “I’m thrilled to finally have a safe and effective longterm treatment option to offer patients who are looking to clear their skin without having to take an oral medication.”
“Ultherapy is an FDA-cleared, noninvasive procedure that lifts and tightens skin on the neck, chin and brow,” says Dr. Levine. “It also improves lines and wrinkles on the upper chest, and results in a firmer, toned, more youthful appearance.”
Ultherapy uses radio-frequency energy to deliver heat to deeper layers of the skin where collagen is most prevalent, explains Omaha, NE dermatologist Joel Schlessinger, MD. “Focused ultrasound waves (similar to those used during pregnancy imaging), heat and break down existing collagen, alerting the body’s natural immune response to create new collagen,” he adds. As a result, over the next few months, the treated area becomes firmer and tighter. “The result isn’t a dramatic improvement, but rather a subtle, lovely change that leads to a more youthful look,” says Dr. Schlessinger.
“Sofwave is a noninvasive treatment for skin rejuvenation using proprietary high-intensity parallel ultrasound waves,” says Miami dermatologist Dr. Deborah Longwill. According to Charlotte, NC dermatologist Gilly Munavalli, MD, the soundwaves are converted to heat in the dermis, causing collagen remodeling and skin contraction. The treatment improves skin laxity around the eyebrows, and on the neck and chin, but that’s not all. It can also reduce fine lines and wrinkles on the face, and cellulite. “The collagen stimulation allows for a tighter framework to support the skin, which retains the skin’s moisture, volume and elasticity,” Dr. Longwill says.
Morpheus8 is a radio-frequency microneedling device that delivers fractionated heat to deep layers of the skin via tiny insulated needles, explains Duxbury, MA plastic surgeon Christine DiEdwardo, MD. “The heat stimulates collagen and elastin production, resulting in firmer, smoother and tighter skin,” she adds. Houston plastic surgeon Olga Bachilo, MD says it’s great for reducing signs of aging, softening acne scarring and improving skin laxity. Morpheus8 has a wide variety of uses, from improving fine lines to tightening loose, crepey skin above the knees, arms and abdomen.
Clear + Brilliant is a nonablative resurfacing laser with the goal of improving skin tone and texture concerns, like dark spots and fine lines. As Dr. Levine explains, “Clear + Brilliant utilizes diode laser technology to remove damaged skin and stimulate collagen growth. This is one of the few lasers that comes with virtually no downtime.”
“The Halo laser combines both ablative and nonablative energy wavelengths that work together to target superficial skin concerns, as well as those beneath the surface,” says Dr. Bachilo.
Aventura, FL dermatologist Dr. Bertha Baum refers to Halo as one of the best resurfacing lasers with very little downtime and excellent results. “It gives a great glow on the skin, treats lines, improves pores, and reduces acne scars. It’s a great option for an antiaging treatment.”
generate heat within specific targets in skin, reducing pigmentation and redness, adds Dr. DiEdwardo. “The treatment also stimulates collagen production in the skin,” she says. “IPL is a good choice for those with skin discoloration, including sunspots, acne, broken capillaries and rosacea.”
Rochester, NY dermatologist Lesley Loss, MD visibly reduced this patient’s melasma with two sessions of Halo paired with a topical cream.
Fraxel is a nonablative laser that resurfaces skin to address signs of aging and/or sun damage, explains Dr. Levine. “The laser pushes out old damaged skin and stimulates the formation of collagen and healthier, new skin,” she says. Dr. Munavalli adds that the microscopic columns of heat can help “dramatically help skin texture, tone and contour.”
Intense Pulsed Light (aka IPL) “uses specific wavelengths of light to selectively cause nonablative destruction of pigment,” explains Delray Beach, FL dermatologist Dr. Janet Allenby. Pulsed light waves
Carbon dioxide (CO2) lasers “vaporize tens of thousands of tiny, discrete channels from treated skin. The selective injury of the vaporized holes stimulates a healing response, but now the skin actually builds back better, thicker, smoother, and tighter,” says Dover, OH facial plastic surgeon David Hartman, MD. Dr. Allenby explains that the carbon dioxide resurfaces the uppermost layers of the skin with a controlled burn, while also heating the underlying skin, stimulating collagen production.
Dr. Hartman says the fractional ablative CO2 laser is regarded as the “most effective laser for treating aging skin and scarring.” The treatment can be tailored to address different conditions by adjusting the depth, the percentage of skin treated, and how much heat gets delivered. But La Jolla, CA plastic surgeon Robert Singer, MD notes that CO2 lasers may produce permanent pigment changes—lighter or darker— and require a longer recovery.
An A-to-Z guide to younger-looking eyes
Are your eyes telling your age quicker than the rest of your face?
If so, you’re not imagining it. “The skin in the eye area is about 10 times thinner than the skin on the rest of the face or body,” says Beverly Hills, CA oculoplastic surgeon Raymond Douglas, MD.
According to Dr. Douglas, we lose some of the natural fat in our ocular region as we age. “This, combined with the breakdown of collagen and elastin, causes fine lines and wrinkles along with diminished elasticity in the area.”
New York oculoplastic surgeon Irene Gladstein, MD says dark circles, volume loss around the tear trough area, “or fat herniation in the area of lower eyelids, also known as ‘bags,’” can also contribute to an aged appearance.
by Danielle Fontana DooleyTHE FIX:
From makeup tips and potent skincare ingredients to in-office tweaks and permanent fixes, experts outline every option when it comes to turning back the clock on the eyes.
“Our eyes are the rst area to show signs of aging— sometimes a decade or more before other areas.”
These application tricks straight from top makeup artists will help give the eyes a youthful look.
Shimmery powder shadows may look pretty in the pan, but celebrity makeup artist Ermahn Ospina says to steer clear of frosty, shiny shadows and powder formulas on the eyes. “Cream and liquid products are your friend,” agrees celebrity makeup artist Jamie Greenberg. “Powder products often accentuate signs of aging, as mature skin tends to be more dry.” Instead, opt for cream shadows and take your time blending them in.
When it comes to wrinkled skin, Greenberg says less is more, including concealer. Start with a small amount on the inner and outer corners of the eyes and blend. Along with proper application, selecting the right formula is paramount. Ospina says using one that is “both concealing and hydrating” will leave you with younger, more even-looking skin beneath and around the eyes.
FOCUS
“Skin care plays an important role in prepping clients with mature skin,” says Greenberg. “For wrinkles, I start by hydrating the skin with a good moisturizer and primer.”
Quality skin care is a tool not to be overlooked.
When shopping for an eye cream, Delray Beach, FL dermatologist Dr. Janet Allenby says to look for one containing antioxidants, flavonoids, peptides, or caffeine. For an instant fix, “optical diffusers help diminish the appearance of dark circles and undereye puffiness, too.”
To plump out the look of wrinkles and lessen hyperpigmentation, creating new collagen is the goal. “One of the most beneficial skin-care ingredients to stimulate collagen is vitamin C,” says Dr. Allenby.
Dr. Gladstein performed an upper blepharoplasty on this 58-year-old patient, along with Sculptra for midface volume correction, and Botox Cosmetic for crow’s-feet. Platelet-Rich Fibrin Matrix was used on the lower lids to improve skin quality.
Anti-aging retinol can also be found in around-the-eye products. Before jumping in, Dr. Allenby says to keep th ese guidelines in mind: “Retinol should always be used at night, especially around the eye area. And, because retinol will make you sun-sensitive, always protect your skin with SPF during the day.”
“Powder products often accentuate signs of aging.”

For transformative results, experts say going under the knife isn’t your only option.
“Most mild-to-moderate concerns regarding the lower eyelids are often best managed with nonsurgical approaches,” Dr. Gladstein says. “More advanced cases may benefit from surgery as a first line of treatment, but may still require regenerative techniques for the best outcomes.”
By stimulating collagen production, microneedling treatments such as Vivace RF and Morpheus8 can make a significant improvement in the wrinkles and firmness around the eyes. “Vivace RF can be effective for the eyelids by improving fine lines and wrinkles, pigmentation issues, as well as skin tone.”
Volume changes around the eyes should be managed by assessing the volume in the midface, says Dr. Gladstein. “Often, other parts of the face require correction prior to improving volume around the eyes.” According to Dr. Douglas, another area that can benefit from dermal fillers (used off-label) are the temples, “which can become depressed and showcase the bone around the ocular region, often a sign of age-related fat loss.” Due to the vasculature of the temples, it is vital to see a properly trained expert injector for this treatment.
If dynamic wrinkling around the eyes, aka crow’s-feet, is a concern, neurotoxins such as Botox Cosmetic, Dysport, Xeomin, Jeuveau and Daxxify of fer an easy fix. By temporarily blocking the nerve signals responsible for these constrictions, wrinkling is bypassed. Neurotoxins can also sometimes be used on the forehead and glabella (between the brows) to help give a mild lift to the eye area.
Collagen-boosters such as TIXEL and CO2 lasers are also used for skin-tightening and anti-aging around the eyes. By transferring thermal energy into the skin, Dr. Douglas says TIXEL doesn’t just tighten; “it also increases lubrication and reduces tear evaporation for an overall more youthful appearance with improved eye comfort.”
For low-lying lids or ptosis
(a detachment of the eyelid muscle that can happen with age), Upneeq is a prescription daily eye drop that quickly opens up the eye. Utilizing 0.1 percent oxymetazoline hydrochloride, when placed in the eye, it causes the Muller’s muscle to contract and lift about 3 millimeters.
“The result is a brighter, wider, bolder, more awake look,” says Dr. Douglas. “It’s like a great night of sleep and a cup of coffee combined.”
For a relatively quick, low-downtime solution to aging eyes, consider a surgical approach.
An upper blepharoplasty, or eyelift, addresses the upper eyelid by removing excess fat, skin and muscle to eliminate the sagging skin, excess fat or puffiness on the upper eyelids.
A lower blepharoplasty removes the excess skin or “bags” under the eyes. This can be done via an incision just below the lower lash line or on the inside of the lower lid.
In addition to addressing “bags” and loose skin, a canthopexy procedure may be necessary to tighten the lower eyelid to prevent it from pulling away from the eyeball after surgery.
Ptosis correction surgery—Dr. Douglas calls the procedure “relatively simple”— tightens the Muller’s muscle of the lid for a wider, more awake look.
BEFORE
• Talk to your surgeon about your current medications and adjust accordingly
• Stop smoking and/or alcohol consumption
• Avoid taking aspirin, anti-inflammatory drugs or any herbal supplements, which may increase bleeding and bruising
AFTER
What to expect: Mild swelling, bruising, some irritation at the incision site, and dry eyes for a week is normal. Expect the healing process to last about 14 days and the small incision to fade into the eyelid’s crease.
What to do: Continue applying lubricating ointment, cold compresses and gauze
• Wear sunglasses daily to protect the area
• Prioritize quality sleep
• Avoid physical exercise
• Continue to steer clear of smoking and alcohol
Gone are the days of the super tight, windblown, exaggerated facelifts once reserved for those proud members of the upper echelon of society ages 60-plus. In their place comes the modern facelift, a natural iteration suitable for women and men of all ages. The once hush-hush surgery has had a facelift itself, with far better results and myriad options—there’s a facelift solution for everyone.
A new demo and new techniques fuel the gold standard of surgery.
A facelift is centered around and elasticity issues repositioning the tissues to a more elevated and position, lost volume as needed and excising loose skin. For the face to appear natural and lifted without inferences of “work,” Beach, FL facial plastic surgeon MD believes the muscles must be released. “The of the face have to be to put the face back
A comprehensive facelift around correcting laxity issues by repositioning and muscles to a more youthful position, replacing volume as needed and skin. For the face to appear and lifted without inferences Delray Beach, FL facial Miguel Mascaró, MD believes underlying muscles must “The ligaments of the be repositioned to put where it belongs.”
Even facelift techniques have and may now include the option for anesthesia for faster and diminished complications, the surgery is still not one to take “It’s not a matter of paint-bynumbers, but rather and facial anatomy because individualization is says La Jolla, CA, surgeon Robert Singer, MD.
Even though facelift techniques progressed and may now option for lighter anesthesia healing and diminished the surgery is still not one lightly. “It’s not a matter numbers, but rather understanding and analyzing facial anatomy individualization is key,” CA, plastic surgeon Robert
precursor facelifts to what’s practiced now were skin-tightening procedures aimed at removing fine and wrinkles in older patients. Releasing and stretching the skin tightens the muscle a bit, but the technique notoriously stretched out mouth and eyes. “That’s why so patients looked strange and sported that fake, ‘wind tunnel’ face,” Mascaró says. Besides short-lived results, he adds that stretching the does not counteract aging because it doesn’t remedy the underlying muscles. “Horizontally

Besides short-lived results, he adds that the skin does not counteract aging because it doesn’t the muscles. “Horizontally
pulling the skin as tight as possible won’t address problems underneath the skin.”
There’s been a revolution in how plastic surgeons view the face, shifting from a 2-D approach to a 3-D one, and using surgical methods that achieve more natural outcomes. Skin tightening aside, volume replacement is equally important. “A young face is like a plump, round grape that turns into a shriveled raisin as it ages,” says Beverly Hills, CA facial plastic surgeon Kimberly J. Lee, MD. “To make a face youthful again you can’t just remove skin, you must also revolumize.”
It’s not a matter of paint-bynumbers, but rather understanding facial anatomy...
“The facelift is better yet different than it was 20 years ago. The fear of looking stretched or unnatural has been put to rest,” says New York facial plastic surgeon Konstantin Vasyukevich, MD. He explains that externally, the surgery still utilizes incisions around the ears to pull everything up, but on the inside, it’s different. “We don’t pull just the
The way surgeons pull and lift the skin has changed from a horizontal approach to one that’s vertical and upward. Pulling back creates a distorted appearance.
A facelift without scars doesn’t exist, but today’s are smaller and less visible. Dr. Mascaró explains that how the face is lifted necessitates incisions hidden along the sideburns, the ear and the hairline. Neck lift incisions are made under the chin, with barely detectable ones that wrap around the ears. “Older techniques would lift the sideburns so high that they would look fake. We’ve modified the incisions along that area, which has changed our understanding of tension.” Less tension on the skin equates to smoother, more inconspicuous scars with considerably less bunching.
The neck deserves equal footing to create the best results. Therefore, most plastic surgeons recommend a neck lift in addition to a facelift for optimal rejuvenation. “Rejuvenating the neck is a common reason for a facelift because the options to rejuvenate the neck otherwise are limited,” Dr. Lee says.
“When I started doing facelifts, most patients were in their 60s,” Dr. Singer says. “Now, they’re in their 40s and 50s.” And younger patients choose surgery once they see the first signs of a loose jawline and minimal excess skin on their lower face. A facelift for a 40- something patient focuses more on tightening the muscles at the jawline and neck rather than removing a lot of skin because, typically, there’s not much loose skin.
Fillers, microfat and injectables play a heavy role in most anti-aging routines and are often a patient’s firsthand account of correcting negligible aging before a facelift. “Many patients fathom the limitations of injectables, threads and radio-frequency treatments,” Dr. Mascaró says. “Or, they don’t get the results they want with these options. When younger patients admit they are ready for a facelift, they want to know what to do to achieve the results they want.”
Dr. Lee says patients in their 40s seek mini-facelifts to look refreshed instead of a more dramatic facelift when they are older. “The shift from correction to prevention is appealing to younger patients.”
One significant upside to a facelift early on is a faster recovery. “It’s safer have surgery when you’re younger because, generally, younger patients healthier,” Dr. Lee says. “They to enjoy their results and have face match their inner youth.”
With so many types of facelifts, it isn’t easy to choose which one is best. So instead, make an informed decision with your plastic surgeon. “Artistry and individualization create good results says Dr. Singer. “There is no one specific technique that will produce good results in everyone.”
THE DEEP-PLANE FACELIFT
With the deep-plane approach, tissue, skin and muscle are lifted vertically as one unit instead of as separate layers, for a natural, youthful rejuvenation.
THE ENDOSCOPIC FACELIFT
This version of a standard facelift relies on an endoscope for additional visualization and elevation of the skin.
THE SMAS FACELIFT
The SMAS lift is the gold-standard. Instead of lifting everything as one unit, the SMAS layer and skin are lifted separately to resuspend the tissues and provide lasting correction.
THE PONYTAIL FACELIFT
This less-invasive facelift requires smaller incisions behind the ears and into the hairline to minimally lift the upper face.
Like a ponytail lift, a mini-facelift is best for younger patients with slight laxity in their lower face.
A facelift is good for solving some problems, like laxity, but it can’t correct everything. You may want to also consider correcting other features that have succumbed to the hands of time, too. These procedures are considered add-ons.
Dr. Singer says not everyone needs fat transfer, but those who do will get a smoother, more natural facelift result. “If you need volume but don’t add it, there can be areas of indentations after a facelift,” he explains. “Some surgeons try to overcorrect by pulling without volume, which leads to an unnatural, fake look.”
Injecting neurotoxin precisely above the brow can give some lift and open up the eyes. When more space needs to be created, a browlift is often preferred because it resets and lifts the brows.
The chin often recedes with age and requires a bit of definition to create harmony and balance.
To shorten the distance between the base of the nose and the upper lip, which lengthens with age, a sliver of skin is removed from the area to lift the lip.
Freeing up the extra skin above and underneath the eyes gives a more awake and complete look.
Dr. Mascaró performed a lower blepharoplasty and a deep-plane facelift with fat transfer on this 62-year-old patient to give her a natural-looking rejuvenation.
Surgery alone doesn’t improve the texture or quality of the skin, which is why a laser treatment may also be recommended.

John Steinbeck once wrote, “To be alive at all is to have scars.” Accumulating over time, they are the body’s way of repairing from accident, illness or surgery. Here, experts share novel and tried-and-true ways to conceal, minimize and revise these markers of life.
01 ACNE
The result of inflamed blemishes, Richmond, VA plastic surgeon Ruth L. Hillelson, MD says 20 percent of acne sufferers will end up with resultant scars.
02 HYPERTROPHIC
These types of scars are raised and correspond to a wound. Washington, D.C. dermatologist Tina Alster, MD says they do not extend beyond the area of injury.
03 FLAT
Flat scars will even out and become the color of surrounding skin.
04 KELOID
These thick, raised scars occur where there is a skin injury and a significant collagen overgrowth. “They typically spread beyond the borders of the initial injury,” says Dr. Hillelson.
05 ATROPHIC
Appearing as indented or depressed, atrophic scars are the most common type from acne.
06
These scars are wrinkled, often indented, or atrophic lines in skin that has been stretched beyond its elastic limits.
Understanding color theory is key to camouflaging acne marks or darker scars. “To minimize redness, use a green color-correcting shade,” says celebrity makeup artist Tim Quinn.
“If the scars have darker pigments that show through foundation, use an orange or pink color-correcting shade underneath to offset it.” Follow with a hydrating, high-coverage concealer and foundation. “Always layer with setting spray or translucent powder to make it stay put all day,” he adds.
PRO TIP:
For visible body scars, use the same color-correcting concepts under full-coverage foundation and set with translucent powder, says Quinn.
To lessen the look of a fresh scar, start with meticulous wound care. “Use ointments like Aquaphor and CeraVe, or silicone bandages to enhance healing,” says Monroe, LA dermatologist Janine Hopkins, MD. “Once healed, use over-the-counter scar creams and gels containing silicone and sun protection daily for up to six weeks.” Beverly Hills, CA oculoplastic surgeon and founder of Skinuva Scar, Christopher Zoumalan, MD says that in addition to silicone, “selective growth factors have been found to play a role in improving the appearance of old and new scars.”
If the scar is very red energy-based treatments, like BroadBand Light or pulsed-dye lasers, can help reduce the redness and thickness. “Softtissue filler injections can temporarily improve indented acne scars,” says Dr. Hillelson. “Steroid injections can lessen the look of thick keloid scars by encouraging collagen breakdown.”
PRO TIP:
Daily massages and red LED light are also effective for new scars or scars undergoing revision, adds Dr. Hopkins.
Older scars tend to be white or less red, says Dr. Alster, so ablative and nonablative fractional lasers that can remodel the scar tissue work best. “Microneedling can also be done to stimulate new collagen production,” she adds. For pitted acne scars, the recommended surgical solution is something called “punch excision,” whereby the old scar is cut out and the area is stitched, explains Dr. Hopkins. “If necessary, a laser can be used six weeks later to blend the skin. Keloid scars can also be surgically removed.” For stretch marks, the name of the game is retexturizing skin to stimulate healing with either a fractionated laser, radio-frequency microneedling or both.
PRO TIP:
“For all scar treatments, realistic expectations are so important,” says Dr. Hillelson. “Scars can be improved, but they may never fully disappear.”
To prevent scarring after an accident, Dr. Hillelson recommended this 72-year-old patient use Iridesse Omega growth factor serum.
From how we gain weight to where we hold it, our sex plays a major role in how our bodies change as we get older. Whether it’s post-baby or post-“dad bod,” getting back to fighting weight can be an uphill battle. Experts say, no matter your gender, finding the right balance is the key to a tighter core.
and a weaker core post-baby. “During pregnancy, the abdominal wall, which includes the abdominal fascia and the six-pack muscles, is basically stretched for about nine months,” says Denver plastic surgeon Philippe A. Capraro, MD. “The muscle and fascia weakens, separates and never goes back the way it was. Even very thin women will say their stomachs feel ‘floppy’ after pregnancy, no matter what they do.” While more common in women, diastasis recti can also occur in men who have gained significant weight that has stretched the ab muscles and fascia. “This is sometimes caused by a ‘beer belly,’ and losing that visceral fat is very hard,” adds Dr. Capraro. Because visceral fat is firmly embedded inside the abdominal cavity, it cannot be suctioned out with liposuction.
These handy tools help boost metabolism, combat stress and strengthen the core.
No matter your gender, managing nutrition and balancing hormones helps to trim waistlines. “Managing our blood sugar and stress levels are key,” says Hanway. “To keep blood sugar balanced and insulin steady cut out sugar and refined carbs and eat protein, veggies and healthy fats.” Studies have shown that higher cortisol reactivity and poorer coping skills in women can lead to “muffintop” fat. Breathing exercises, cutting caffeine, meditating and getting more sleep can naturally lower stress.
“Men also go through dips in testosterone levels,” explains Pittsburgh plastic surgeon Brian Vassar Heil, MD. “When this occurs, we accumulate fat instead of burn it.”
Skip the spot-training, says fitness trainer and Pilates expert Grace Albin. “Target the rectus abdominis, obliques and lower back. Many people only work those showy ‘six pack’ muscles but maintaining proportion with the adjoining muscles will improve posture.” Albin suggests trying Pilates moves like the Criss-Cross for obliques, and the Superman for the lower back.
Added options for abdominal weight loss include injectable drugs like Semiglutide and Tirzepitide which have been shown to produce results that rival gastric bypass surgery.

When diet and exercise take you only so far, Dr. Heil recommends Emsculpt NEO treatments to build muscle using HIFEM electro magnetic stimulation and melt fat with radio frequency.
“It’s been shown to reduce up to 30 percent of fat and increase muscle mass by 25 percent,” he says. Recent studies have shown a 14 percent decrease in visceral fat as well.

Dr. Matthew Elias targets stubborn fat with CoolSculpting Elite, which has shown a 30 percent reduction. A rare side effect called Paradoxical Adipose Hyperplasia can occur (it happens in 0.025 of patients), so review possible risks and benefits with your doctor.
After losing over 150 pounds, this 35-year-old patient underwent a tummy tuck with Grand Rapids, MI plastic surgeon David Alfonso, MD to complete his transformation.
Nonsurgical modalities are a good option for some patients, but liposuction remains the gold standard for fat removal. Las Vegas plastic surgeon Mary C. Herte, MD adds that loose skin or stretched abs are best treated with a tummy tuck. “The struggle to address these problems can be very frustrating for both men and women. Learning that the only solution is liposuction or tummy tuck surgery is a validation for those who have struggled for contouring or narrow look abdomen,” may il
According to The Aesthetic Society, the number of breast augmentations performed in the U.S. has increased by 89 percent since 2020, with 375,000 performed in 2021 [2022 stats were not yet released at time of press]. Though trends have shifted over the years, demand for more fullness with the help of implants is greater than ever. Need more proof? More than 1.3 million people liked TikTok “it girl” Alix Earle’s video detailing her breast surgery, which she shared after being flooded with questions from followers. Here’s what the experts want you to know about the procedure.
THE CONSULTATION: There is a lot of information to cover during a consultation for breast augmentation, and the surgeon should take their time with the patient, says New York plastic surgeon B. Aviva Preminger, MD. “I ask what bothers the patient about the appearance of their breasts, what expectations they have, and what look they are trying to achieve. I perform a breast exam and take precise measurements, photos and a 3-D image. We talk about options, and the patients touch and feel different types of implants and learn about the risk and benefits of each. They also try on different sizes of implants with a sizing bra.”
THE TIMELINE: Glen Carbon, IL plastic surgeon Ryan Diederich, MD says many top surgeons have significant wait times, so it’s best to plan your procedure timeline backward from your goal date to ensure you can meet it. “At my practice, we often plan a tentative procedure date during the first phone call, even prior to consultation, to make the process as seamless for the patient as possible.” Depending on the patient’s goals and schedule (and the surgeon’s), the surgery may take place a few weeks post-consult, or a few months.
THE PREP: “Prior to breast augmentation, it’s important for patients to optimize their health to make sure they are positioned for the best possible outcome while limiting the procedural risks,” Dr. Diederich says. “This includes a healthy lifestyle with a well-balanced diet, avoidance of all nicotine exposure, and maintaining a stable weight.” San Diego plastic surgeon Larry H. Pollack, MD advises patients to avoid ibuprofen, which can thin the blood, for two weeks prior to the procedure, and to increase their intake of vitamin C to boost their immunity.
INSIDE TIP:
Breast implants do not typically alter the size or position of the nipple, though the increased volume may widen the areola. Plan to discuss these changes with your doctor during your initial consultation.
According to Houston plastic surgeon Henry Mentz, MD, silicone implants are pre-filled with a silicone gel that is similar in texture to natural breast tissue, while saline implants are filled with sterile saltwater after they have been placed in the breast. “Many patients prefer silicone because they think the gel, which is contained within a solid silicone outer shell, provides a more natural feel and look,” he says. “However, if a silicone implant ruptures, it can go easily unnoticed because the silicone tends to remain inside the capsule (intracapsular rupture) and the breast changes little in appearance. If leaking should occur with a saline implant, it is usually quickly apparent be cause the implant deflates.” 02
Implants come in either round (the more traditional and popular option) or teardropshape. “My preference is to use round, smooth implants, which come in various profiles,” says Dr. Mentz. “For instance, a low-profile implant is a bit wider with less projection than a ‘high-profile’ implant that will project outward more. There are also several options in between.”
Breast implant manufacturer
Natrelle offers smooth silicone implants in five profiles, ranging from low to extra-full projection. For those who want more cleavage, doctors typically recommend lowprofile implants.
Breast implant sizes are measured in ccs (cubic centimeters), which describes how much space an implant fills. “Ten years ago, the average volume was much larger,” says Dr. Pollack. “Today, the trend is a smaller, perkier look or a more athletic one. We are also seeing a trend in ‘downsizing’ for women reaching their 50s. After having large implants for many years, they are choosing to go smaller to avoid feeling top-heavy as they age.”
Dr. Mentz agrees, noting that the average implant size in the ‘90s and 2000s was 375 to 400-ccs (a full C or D cup), but now patients tend to lean more toward the 250 to 275-cc range (a small C/full B cup).
Three unique incisions, or entry points, can be used to insert implants. Dr. Diederich says the inframammary fold (the crease below the breast) is the most common approach based on safety data; however, this can vary depending on the patient’s needs. “The inframammary fold incision allows the scar to be covered by the breast in most cases,” adds Dr. Preminger. The periareolar incision involves cutting around part of the nipple, which lets the scar blend into the surrounding tissue. Lastly, the transaxillary approach involves placing the implants through the armpit to help avoid scars on the breasts. This does result in a small scar in the armpits, however. Regardless of the entry point, the scars are usually fully healed within six to 12 months.
Depending on the amount of breast tissue the patient has and how active they are in their lifestyle, implants can be placed in three different pockets: above the muscle, under the muscle, or more of a hybrid known as subfascial. “Subfascial means placing the implant between the covering ‘fascia’ of the pectoralis muscle and the muscle,” says Dr. Diederich. “Like scar location, many surgeons are passionate about the pocket choice, but it depends on the patient.”
Eugene, OR plastic surgeon Mark Jewell, MD says the subfascial approach provides excellent outcomes. “It avoids breast movement that leads to implant malposition when the pectoralis muscle contracts.”
In collaboration with the FDA, the Plastic Surgery Foundation created the National Breast Implant Registry (NBIR) in 2018. “It’s an important quality improvement initiative and safety surveillance registry that collects clinical, procedural and outcomes data at the time of operation, as well as any subsequent reoperations for all patients who receive breast implants in the United States,” Dr. Mentz explains. “To date, more than 1,500 physicians have registered to participate, and more than 78,000 breast implant cases have been entered.”
The Aesthetic Society’s free Aesthetic One app, a HIPAA-compliant platform, allows doctors to share breast implant ID cards, operative summaries, photos, and more with their patients in a quick, easy-to-use format.
“For the first few days, your chest will feel tight and you will feel pressure until the pocket starts to stretch,” Dr. Pollack says, noting that swelling also occurs, and pain medication can be prescribed to help with any discomfort. Arnica and bromelain supplements can also help minimize swelling. “During this period, lots of rest and little movement of the upper body, other than range of motion, is recommended,” he adds. “I also emphasize sleeping on your back slightly elevated and not on your side, as this could cause swelling or bleeding.”
Expect to take at least one week off work for desk-jobs and longer for more physically demanding roles. A one-month hiatus from exercise is also standard for most patients.

Though implants are still the most popular choice for adding fullness to the breasts, fat transfer (the fat is usually taken from the patient’s thighs or stomach via liposuction; donor fat can also be used) can increase their size as well. One key advantage of fat transfer, as Dr. Diederich points out, is that “it utilizes your own tissue—no foreign body—and once the fat has survived, it is yours forever. This is assuming you don’t experience a major weight fluctuation that could change the breast volume.”
Dr. Preminger loves using fat to augment the breasts, but says it often isn’t enough alone to achieve patients’ larger size goals, particularly if they are thin. “In this case, I will recommend a composite augmentation using a small implant and fat together for a natural result,” she explains. “It is also important to remember that not all of the fat that is transferred to the breasts lasts long-term, and that overfilling the breasts with fat may actually cause less of the fat to survive and last.”
Dr. Diederich adds that “other disadvantages are if you desire a significant volume increase, it may take multiple fat transfer procedures, and fat typically does not resist gravity over time in the same way a breast implant does.”

Some patients begin to have what Dr. Diederich calls “implant fatigue,” which is when they’ve had their implants for decades and are just ready to live life without them. “As a result, we are seeing an increased number of implant removals, with or without complementary procedures such as a breast lift or fat transfer,” he says. In other cases, “explants” are performed to relieve patients of symptoms associated with Breast Implant Illness (BII), such as joint pain, fatigue and hair loss. Last year, a study on BII conducted by The Aesthetic Society and The Aesthetic Surgery Education and Research Foundation (ASERF) revealed that 94 percent of subjects showed improvement of at least some of their symptoms within the six months after their implants were removed. However, multiple studies reveal that more than 90 percent of patients who have had breast augmentation with implants are happy with their results.
