With a staff full of people called to provide end-of-life care, this hospice makes sure patients and their families receive support, wherever and whenever it is needed.
Foundations Hospice has a true passion and calling to fulfill the company’s mission: to serve people at the end of life with honor and dignity.
The entire team makes sure families feel supported and informed every step of the way. Communicating with empathy and compassion, each patient’s care is individualized, guaranteeing that their preferences and wishes are honored.
“Did you know that studies show people on hospice live longer than people with similar conditions who aren’t receiving hospice care? What we do really matters.,” said Mark Malkemus, owner and Administrator of Foundations Hospice.
Along with doctors and nurses, the team includes social workers who provide emotional and social support while also helping with practical needs. Chaplains are also available who support the patient and families spiritually. Also highly important, the hospice staffs nurse’s aides, a dietician, and volunteers who do everything from providing meals, running errands, and visiting patients. Foundation’s Volunteer Coordinator is great at matching volunteers with families. Volunteers also help the very busy administrative staff, who handle operations and intake.
When commenting on the staff, Mark said, “They are the best. Many of our nurses have experience in many areas of medicine, but they will all tell you they felt called to work in hospice.”
The team works hard for patients and families, standing out by always willing to go the extra mile. More than providing standard hospice services, staff have helped families move furniture, cooked them
meals, helped rehome pets, assisted with estate planning advice, and more. Whatever is needed, the team is there.
“Working for Foundations Hospice is incredibly meaningful to me because we’re not just providing care,” said Hailey Florane, Director of Nursing. “We are helping patients and families find comfort, dignity, and peace during one of life’s most difficult times. What makes Foundations different is the heart behind the work. It’s a team that leads with compassion. We care deeply for both our patients and their families.”
From the moment a patient and their family meet one of the community liaisons, they can expect honesty, answers, and empathy. No one on the team treats their work just like a job. Families know they can call the owner or company 24/7.
Foundations is not another corporate owned hospice company; Mark Malkemus, does it all and he leads by example, taking a hands-on approach. He works every day in the field, delivering meals, making welcome visits, or picking up medicine.
Mark started volunteering as a hospice worker almost 20 years ago and he brought his daughter, Molly, with him to visit patients. That began a lifelong interest for her, and she now works as a social worker at Foundations Hospice.
“I believe if I take care of my team, they will take care of
our patients,” said Mark. “The employees are valued and heard at our company and that translates to their care for the patients and families.”
After only three years in business, Foundations Hospice has won Bronze, which the team credits to Mark’s hands-on local ownership and the dedication Mark and the entire team has to providing patients and their families with the respect and dignity they deserve during end-oflife care.
