Cyclospora: Prevention, Recognition, Testing, and Recovery What Is Cyclospora?

Cyclospora cayetanensis is a microscopic, single-celled parasite that infects the small intestine and causes an illness called cyclosporiasis.

People become infected by consuming food or water contaminated with the parasite. The organism reproduces only in humans, and the contamination cycle begins when Cyclospora is passed in human feces.

The parasite may then enter irrigation water, agricultural soil, foodprocessing water, or other parts of the food supply through sewage contamination, inadequate sanitation, or the unsafe use or handling of human waste.

Cyclospora is different from many familiar foodborne bacteria. When the organism first leaves the body in human feces, it is not immediately infectious. It must remain in the environment long enough to mature, usually for approximately one to two weeks. This makes direct personto-person transmission less likely. The greater danger is environmental contamination of water and food, particularly produce that is later eaten raw.

Symptoms usually begin approximately one week after contaminated food or water is consumed, but they may begin within two days or may not appear for two weeks or longer. Without effective treatment, the illness may continue for many weeks and frequently follows a relapsing pattern in which symptoms improve temporarily and then return.

The Current Outbreak CDC case totals are preliminary and should not be read as a real-time count. CDC estimates an approximately six-week lag between illness onset and case reporting, and the true number of illnesses is likely higher because many people are never tested or reported.

CDC's July 28, 2026 surveillance update reported 6,707 laboratoryconfirmed domestically acquired cases from May 1 through July 27, 423 hospitalizations, no deaths, and cases in 45 states. CDC also reported awareness of more than 11,500 additional cases that were not laboratory confirmed or still required investigation and analysis.

Case confirmation requires patients to be tested, laboratories to identify the parasite, healthcare providers to report the illness, health departments to investigate it, and information to be transmitted and classified at the federal level. Many people with prolonged diarrhea are never tested specifically for Cyclospora, and routine stool studies may fail to identify it. These factors produce both delayed reporting and substantial undercounting.

These totals include 1,947 laboratory-confirmed illnesses in a 9-state outbreak linked to recalled iceberg lettuce from Taylor Farms de Mexico. CDC is also investigating other clusters and illnesses not linked to that lettuce outbreak. These data are preliminary and subject to change.

Based upon numbers coming out of Ohio and Michigan these numbers should not be relied upon.

The normal infectious period for cyclospora runs between May and August due to climate and growing season factors.

How Food Becomes Contaminated – Human Feces Cyclospora contamination ultimately originates in human fecal waste.

Contamination may occur when sewage reaches irrigation or washing water, when untreated or inadequately treated human waste is used on agricultural land, when workers lack adequate sanitation, or when contaminated water contacts food during growing, harvesting, processing, or preparation.

The final food served to the consumer may look, smell, and taste completely normal. Cyclospora cannot be seen without a microscope.

Contamination may also be uneven, so one serving from a shipment or food container may cause illness while another does not.

Historically, outbreaks have been associated with leafy greens, lettuce, fresh herbs, raspberries, blackberries, basil, cilantro, parsley, snow peas, snap peas, green onions, and mixtures of fresh produce. The source is not always the same from one outbreak to another, and several contaminated foods or distribution chains may be involved at the same time.

Why Washing Is Not Enough Rinsing is an appropriate first step and should still be done, but it cannot be relied upon to eliminate Cyclospora. FDA advises rinsing produce under clean running water, including before peeling, and scrubbing firm produce with a clean produce brush. Do not use soap or bleach on produce; Cyclospora is resistant to standard chlorine-based sanitizers.

Washing can reduce surface dirt and some contamination, but it cannot guarantee that raw produce is free of Cyclospora. During an active outbreak, washing should be presented as one part of risk reduction, not as a substitute for following recalls, preventing cross-contamination, or cooking.

Cooking produce to at least 158°F (70°C) throughout is the safest household option identified by FDA for inactivating Cyclospora. Surface warming alone is insufficient.

What to Eat During the Outbreak Follow current CDC, FDA, and state recall alerts, and do not eat recalled iceberg lettuce from Taylor Farms de Mexico. Because illnesses unrelated to the lettuce-linked outbreak are also under investigation,

people seeking an additional margin of protection may emphasize freshly prepared, thoroughly cooked foods.

Temporarily avoiding higher-risk raw produce and pre-cut or pre-washed items is a conservative personal precaution, especially for older adults, pregnant people, immunocompromised individuals, and those with serious chronic disease. It is more restrictive than current CDC and FDA guidance, which emphasizes avoiding recalled or implicated foods, rinsing produce, preventing cross-contamination, and prioritizing cooking.

Cooked vegetables, soups, stews, grains, beans, lentils, eggs, fish, poultry, meat, and thoroughly heated frozen vegetables are practical choices. Canned fruits and vegetables are heat processed and are generally a safer alternative. Fruit that can be peeled and then cooked may also be used.

Watermelon, Pineapple, and Other Thick-Skinned Fruit An intact watermelon or pineapple provides a natural outer barrier, but cutting can transfer contamination from the exterior to the edible flesh.

FDA advises rinsing all produce under clean running water before cutting or peeling, scrubbing firm melons with a clean produce brush, and using clean hands, knives, and boards.

Pre-cut fruit requires additional caution because extra handling creates more opportunities for contamination. Discard any recalled or implicated product. During this outbreak, avoiding commercially cut fruit may be chosen as an added precaution, but current outbreak evidence identifies iceberg lettuce, not watermelon or pineapple, as the source of the 9-state cluster.

Whole fruit handled in a clean home kitchen generally involves fewer handling steps than commercially pre-cut fruit, but no raw food is riskfree. People seeking the greatest margin of protection may temporarily favor canned or cooked fruit.

Traditional Use of Cooking, Herbs, and Spices For hundreds of years, cultures in India, Asia, Africa, the Middle East, and other regions have relied heavily upon thoroughly cooked foods seasoned with generous amounts of herbs and spices. These practices developed in environments where food and water could be exposed to human waste, animal manure, inadequate sanitation, high temperatures, insects, spoilage organisms, parasites, and many different diseaseproducing microbes.

Heat was and remains the principal protective measure. Traditional herbs and spices provided flavor, supported digestion, helped preserve certain foods, and supplied plant compounds with antimicrobial and antioxidant activity. Modern laboratory research confirms that numerous culinary spices can inhibit various bacteria, fungi, yeasts, and food-spoilage organisms.

This does not mean that seasoning contaminated raw produce will make it safe. Herbs and spices should be combined with adequate cooking rather than used as a substitute for heat.

Meals may be prepared liberally with turmeric, garlic, onion, ginger, cumin, coriander, oregano, thyme, rosemary, cinnamon, cloves, cardamom, and black pepper. Dried spices should preferably be incorporated while the food is cooking rather than scattered uncooked over the completed meal.

Turmeric and Curcumin Support Turmeric has a long history of culinary and medicinal use in Ayurveda, Unani, Traditional Chinese medicine, and other healing systems, including for gastrointestinal complaints and conditions traditionally associated with infection or parasites. Experimental research supports antiparasitic activity against several protozoa. In a 2023 study of immunosuppressed mice with experimental Cyclospora cayetanensis infection, curcumin and particularly a curcumin nanoemulsion 6|Pa ge

substantially reduced oocyst shedding and improved intestinal pathology and oxidative-stress measures. These findings constitute promising direct preclinical evidence for cyclosporiasis.

Antimicrobial and Antiparasitic Herbs and Spices Neem (Azadirachta indica) has occupied a central place in Ayurvedic medicine for thousands of years and is one of the most respected medicinal trees of India. Nearly every part of the plant, including the leaves, bark, seeds, flowers, and oil, has been used traditionally to support health. Neem has long been employed for intestinal parasites, skin infections, wound care, fungal conditions, and general digestive health. Modern laboratory studies have demonstrated antibacterial, antifungal, antiviral, and antiparasitic activities against a variety of microorganisms, although the quality of clinical evidence varies depending on the condition being treated. The complex mixture of bioactive compounds, including azadirachtin, nimbin, nimbolide, and quercetin, appears to interfere with microbial growth, reproduction, and survival while also exhibiting anti-inflammatory and antioxidant properties. Neem remains one of the most extensively studied medicinal plants in traditional Indian medicine.

Turmeric (Curcuma longa) has been used for more than 3,000 years in Ayurveda, Siddha medicine, and Traditional Chinese Medicine as both a food and a medicine. Traditionally it has been prescribed for digestive disorders, inflammatory diseases, liver support, wound healing, respiratory illnesses, and gastrointestinal infections. Curcumin and numerous related curcuminoids contribute to its broad spectrum of biological activity, including anti-inflammatory, antioxidant, antibacterial, antifungal, antiviral, and antiparasitic effects demonstrated in laboratory and animal studies. Although high-quality clinical trials for specific parasitic infections remain limited, turmeric has an extensive history of empirical use throughout South Asia where intestinal parasitic diseases have long been endemic. Its ability to support immune function,

reduce inflammation, and influence microbial growth has contributed to its enduring role as one of the foundational herbs of traditional medicine.

Garlic (Allium sativum) has been valued in both Traditional Chinese Medicine and Ayurveda for thousands of years as a powerful medicinal food. Traditionally it has been used to combat intestinal parasites, respiratory infections, digestive disorders, and circulatory disease. The sulfur-containing compounds formed when garlic is crushed, particularly allicin, exhibit broad antimicrobial activity against numerous bacteria, fungi, viruses, and protozoa in laboratory investigations. Garlic has also been traditionally employed to help expel intestinal worms and to support recovery from gastrointestinal infections. Its immunesupporting, antioxidant, and anti-inflammatory actions have made it one of the world's most widely used medicinal plants.

Ginger (Zingiber officinale) has served as a cornerstone of Asian herbal medicine for over two millennia. In Ayurveda it is regarded as a universal digestive remedy, while in Traditional Chinese Medicine it is valued for warming the body, improving circulation, and supporting digestive function. Ginger has traditionally been prescribed for nausea, diarrhea, intestinal discomfort, foodborne illness, and various infectious conditions. Modern research has identified gingerols, shogaols, and related compounds with antibacterial, antifungal, antiviral, and antiparasitic activity in experimental studies. Its ability to stimulate digestion and reduce intestinal inflammation has made it an important companion herb in traditional treatments for gastrointestinal infections.

Holy Basil (Ocimum tenuiflorum or Tulsi) is revered in Ayurveda as the "Queen of Herbs." Beyond its spiritual significance, Tulsi has been used traditionally for respiratory infections, digestive disorders, fevers, skin diseases, and intestinal parasites. The essential oils and phenolic compounds found in Tulsi possess antimicrobial activity against numerous bacterial and fungal organisms in laboratory research while also demonstrating immunomodulating, antioxidant, and antiinflammatory properties. Tulsi is commonly incorporated into traditional 8|Pa ge

formulas designed to strengthen resistance to infection and promote recovery from illness.

Clove (Syzygium aromaticum) has been an important medicinal spice throughout Asia for centuries. Rich in the essential oil eugenol, cloves have traditionally been used to preserve food, relieve toothache, support digestion, and combat intestinal parasites. Laboratory studies have demonstrated antibacterial, antifungal, antiviral, and antiparasitic activities, while traditional practitioners have valued cloves for their ability to reduce digestive discomfort and discourage intestinal pathogens. Their aromatic oils also contribute to their long-standing use as a natural food preservative.

Cinnamon (Cinnamomum verum and Cinnamomum cassia) has been employed extensively in both Chinese and Ayurvedic medicine for digestive weakness, respiratory illness, circulatory disorders, and gastrointestinal infections. Traditional practitioners have long recognized its warming, digestive, and preservative qualities. Modern investigations have shown that cinnamaldehyde and related compounds possess antibacterial, antifungal, antiviral, and antiparasitic activity in laboratory settings. Cinnamon also exhibits antioxidant and antiinflammatory properties that may complement its traditional role in supporting recovery from infectious illness.

Black Pepper (Piper nigrum) has occupied an important position in Ayurveda for thousands of years, where it is considered a digestive stimulant and an enhancer of the effectiveness of other medicinal herbs.

Piperine, its principal alkaloid, not only exhibits antimicrobial activity but also significantly improves the absorption of numerous phytochemicals, including curcumin from turmeric. Traditionally black pepper has been prescribed for digestive disorders, intestinal parasites, respiratory infections, and poor circulation. Its inclusion in many Ayurvedic formulations reflects both its direct medicinal actions and its ability to increase the bioavailability of companion herbs.

Long Pepper (Piper longum) is one of the oldest medicinal spices described in Ayurvedic literature and forms part of the classical formulation known as Trikatu together with ginger and black pepper.

Traditionally it has been used for respiratory infections, digestive disorders, intestinal parasites, and impaired metabolism. Experimental studies have demonstrated antimicrobial and anti-inflammatory activities, while its piperine-related alkaloids may enhance the absorption of other botanical constituents in combined herbal formulas.

Andrographis (Andrographis paniculata), often called the "King of Bitters," is widely used throughout India, China, and Southeast Asia for fever, respiratory infections, digestive disorders, and infectious diseases.

Traditional practitioners have valued the intensely bitter herb for its ability to support immune function and promote recovery from illness.

Modern research has demonstrated antibacterial, antiviral, antifungal, and antiparasitic activities in laboratory studies, along with significant anti-inflammatory and immunomodulatory effects.

Chinese Goldthread (Coptis chinensis) is an important herb in Traditional Chinese Medicine and is rich in the alkaloid berberine. For centuries it has been prescribed for infectious diarrhea, dysentery, gastrointestinal infections, and inflammatory conditions. Berberine exhibits broad-spectrum antimicrobial activity against bacteria, fungi, protozoa, and certain parasites in laboratory studies, while also influencing the intestinal microbiome and reducing inflammation.

Closely related berberine-containing plants such as Goldenseal (Hydrastis canadensis), Oregon Grape (Berberis aquifolium), and Barberry (Berberis vulgaris) have been used similarly in Western herbal medicine.

Wormwood (Artemisia annua) has been used in Traditional Chinese Medicine for more than two thousand years, primarily for recurrent fevers and infectious diseases. The discovery of artemisinin from this plant revolutionized the treatment of malaria and earned the 2015 Nobel Prize in Physiology or Medicine. Beyond malaria, Artemisia species 10 | P a g e

have demonstrated antimicrobial and antiparasitic activities in laboratory investigations, and traditional practitioners have employed them for a variety of febrile and parasitic illnesses.

Pomegranate (Punica granatum) has a long history of medicinal use throughout India, Persia, and China. The bark, root bark, peel, and fruit have all been used traditionally to treat intestinal parasites, diarrhea, dysentery, and digestive disorders. The tannins, polyphenols, and alkaloids present in the plant exhibit antimicrobial and antioxidant activities, and the bark was historically regarded as a traditional vermifuge for tapeworm and other intestinal helminths before the advent of modern anthelmintic drugs.

Collectively, these herbs and spices illustrate the depth of traditional botanical medicine in Asia. While modern research has confirmed antimicrobial, antifungal, antiviral, and antiparasitic activity for many of these plants in laboratory and animal studies, the strength of clinical evidence varies considerably between herbs and specific diseases. Their continued use across centuries reflects not only direct antimicrobial effects but also their broader roles in supporting digestion, modulating inflammation, enhancing immune function, improving nutritional status, and promoting the body's natural resistance to infectious disease.

Symptoms of Cyclosporiasis The most characteristic symptom is frequent watery diarrhea. The diarrhea may be sudden, severe, explosive, persistent, or intermittent. A person may begin to feel better and then experience another period of diarrhea several days later.

Other common symptoms include nausea, loss of appetite, abdominal cramping, bloating, excessive gas, belching, fatigue, weakness, muscle aches, headache, and unintentional weight loss. Vomiting and low-grade fever may occur but are less consistent.

Cyclospora can interfere with normal intestinal absorption. Prolonged illness may therefore lead to dehydration, sodium and potassium depletion, loss of muscle mass, nutritional deficiencies, weakness, dizziness, low blood pressure, and substantial weight loss.

What to Do If Symptoms Develop Do not assume that persistent diarrhea is simply a stomach virus or food sensitivity. Contact a healthcare provider and explain that Cyclospora is circulating and that you may have eaten contaminated produce.

Keep a record of all salads, herbs, berries, restaurant meals, garnishes, precut fruit, and raw produce eaten during the two weeks before the symptoms began. Include grocery stores, restaurants, dates, brand names, package labels, and loyalty-card purchases when available. This history may help local health officials identify a common source.

Begin replacing fluids promptly. When diarrhea is frequent, plain water alone may not replace the sodium, potassium, and glucose being lost. A commercially prepared oral rehydration solution, mixed exactly according to its label, is preferred for mild to moderate dehydration. Sip small amounts frequently if nausea is present. Inability to drink, severe dehydration, shock, or altered mental status requires urgent medical evaluation and may require intravenous fluids.

After rehydration begins, resume a normal, age-appropriate diet as tolerated rather than maintaining a highly restrictive diet. Smaller cooked meals may be easier to tolerate. Eliminate alcohol, very greasy foods, and large amounts of concentrated sugar. Temporary lactose intolerance can follow intestinal illness, so milk and some dairy products may need to be reduced if they worsen symptoms.

Consider using a “Rescue” diet which is based upon rice or “congee”.

What Test to Request Ask the healthcare provider to request, and the laboratory to confirm: 12 | P a g e

Stool testing specifically for Cyclospora cayetanensis. When available, a molecular gastrointestinal PCR panel that explicitly includes a Cyclospora target is a useful option; not all gastrointestinal PCR panels include it.

Routine bacterial stool cultures do not detect Cyclospora. A routine ovaand-parasite examination may not reliably detect it unless the laboratory is specifically asked to look for Cyclospora and uses appropriate concentration and detection methods.

When PCR is unavailable, diagnosis may be made by concentrating the stool specimen and using modified acid-fast staining, modified hot safranin staining, or ultraviolet autofluorescence microscopy.

A single negative specimen does not rule out infection. Symptomatic patients may shed low numbers of oocysts, so several specimens collected on different days may be needed. CDC laboratory guidance states that three or more specimens collected at two- or three-day intervals may sometimes be required.

Depending on the clinical picture, stool testing may also evaluate Giardia, Cryptosporidium, Cystoisospora, bacterial pathogens, and Clostridioides difficile. Blood tests do not diagnose Cyclospora, but may be used to assess dehydration and complications, including electrolytes, blood urea nitrogen and creatinine, glucose, a complete blood count, and other tests as clinically indicated.

The FDA, in public statements has stated that the current testing methods available are frequently unreliable and generate false positives that results in unnecessary treatment or treatment with the wrong medication.

Standard Medical Treatment Trimethoprim-sulfamethoxazole (TMP-SMX; Bactrim, Septra, or Cotrim) is the CDC treatment of choice for cyclosporiasis.

For immunocompetent adults with confirmed cyclosporiasis, the typical CDC regimen is TMP 160 mg plus SMX 800 mg (one double-strength tablet) by mouth twice daily for 7 to 10 days. Children 2 months to 18 years require weight-based dosing; use under 2 months is generally not recommended. People with HIV or other immunocompromising conditions may need a longer course. Pregnancy, breastfeeding, kidney or liver impairment, folate deficiency, prior drug reactions, and interacting medicines require individualized prescribing and monitoring.

Oral rehydration remains the primary supportive treatment. After adequate hydration, an antimotility drug such as loperamide may be considered for selected immunocompetent adults with watery diarrhea, but it should be avoided when fever, blood in the stool, or suspected inflammatory diarrhea is present. Children and high-risk patients should use these medicines only under clinician direction.

What If You Cannot Tolerate the Standard Medication?

No highly effective medical or pharmaceutical alternative to TMP-SMX has been identified. A previous reaction must be accurately characterized before any further exposure, and patients should not attempt a selfdirected rechallenge.

Nausea, headache, or gastrointestinal upset may represent intolerance, while hives, facial or throat swelling, wheezing, or difficulty breathing can indicate an immediate allergy. Blistering or peeling rash, mouth sores, fever, jaundice, unusual bruising, marked weakness, kidney injury, hepatitis, or blood-cell abnormalities may signal a severe reaction and require urgent medical evaluation.

Patients should not take leftover TMP-SMX, split doses, or alter the schedule on their own. The prescriber may decide whether mild nonallergic intolerance can be managed safely or whether evaluation by an allergist or infectious-disease specialist is needed.

CDC notes that desensitization may be considered only for selected patients who require treatment, have been evaluated by an allergist, and do not have a life-threatening allergy. It must never be attempted at home and is generally inappropriate after Stevens-Johnson syndrome, toxic epidermal necrolysis, DRESS syndrome, severe organ injury, or a serious blood disorder.

CDC states that ciprofloxacin showed only modest activity in a small study among people with HIV, while substantial anecdotal experience suggests that it is ineffective in immunocompetent patients. CDC also lists albendazole, azithromycin, doxycycline, metronidazole, tetracycline, tinidazole, and several other drugs as ineffective on the basis of anecdotal or unpublished data. Herbal antiparasitic products have no established clinical role. Observation and supportive care may be selected for some otherwise healthy patients, but illness can be prolonged and relapse; specialist guidance is appropriate when TMPSMX cannot be used and treatment is needed.

When Immediate Medical Care Is Needed Prompt or urgent medical care is necessary when diarrhea is accompanied by an inability to keep fluids down, very little urine, dark urine, severe weakness, fainting, confusion, rapid heartbeat, low blood pressure, severe abdominal pain, blood in the stool, persistent vomiting, high fever, or rapid weight loss. Blood in the stool and high fever are not typical features of cyclosporiasis and warrant evaluation for another or an additional cause.

Older adults, young children, pregnant patients, immunocompromised people, and individuals with kidney, cardiovascular, or serious chronic disease should seek care earlier because dehydration and electrolyte loss may become dangerous more quickly.

Preventing Household and Food-Service Contamination A person with diarrhea should not prepare food for others. Hands should be washed thoroughly with soap and water after using the toilet and before touching food. Bathroom surfaces that are visibly contaminated should be cleaned promptly, and soiled clothing or bedding should be handled carefully.

Because Cyclospora must mature in the environment before becoming infectious, immediate direct transmission between household members is considered uncommon. Nevertheless, illness involving fecal contamination always requires careful hand hygiene and sanitation. No EPA-registered disinfectant has been demonstrated to inactivate Cyclospora. Visible soil should first be removed with detergent and thorough scrubbing; routine cleaning and disinfection remain important for reducing contamination and controlling other pathogens.

Raw produce and cooked food should be kept separate. Do not return cooked food to a plate or cutting board that previously held raw produce.

Knives, utensils, cutting boards, countertops, and hands should be cleaned before they contact cooked food.

The Practical Message Cyclospora reaches people through contaminated food or water. Rinsing reduces some surface contamination but cannot guarantee removal.

During the 2026 increase in cases, avoid all recalled or implicated products and follow current CDC, FDA, and state notices. People seeking additional protection, especially those at higher risk of complications, may temporarily emphasize cooked or canned foods and avoid higher-risk raw or pre-cut produce; that broader avoidance is a conservative personal choice beyond current official requirements.

When appropriate, cook produce throughout to at least 158°F (70°C).

Turmeric, garlic, ginger, onions, and other spices may be used as 16 | P a g e

culinary ingredients, but they are not proven preventive or antiparasitic treatments for Cyclospora. Concentrated turmeric or curcumin supplements do not replace hydration, diagnostic testing, or appropriate medical treatment and may worsen gastrointestinal symptoms or, rarely, cause liver injury.

Anyone who develops persistent or relapsing watery diarrhea, marked fatigue, loss of appetite, or unexplained weight loss should request stool testing that specifically includes Cyclospora. Not all gastrointestinal PCR panels include it, and routine cultures or ordinary ova-and-parasite examinations may fail to detect it. TMP-SMX is the treatment of choice for confirmed cyclosporiasis, with individualized care for children, pregnancy, immunocompromising conditions, kidney or liver disease, drug interactions, and prior reactions.
