SCHOOL IDENTIFICATION CARD APPLICATION FORM
FOR IN-STUDIO CARD ORDERS
IMPORTANT INFORMATION
This form is to be completed by the Student's school administration and authorized by a school official.
A Student that attends an in-studio session to request a SCHOOL IDENTIFICATION CARD must provide an authorized Application Form with proof of age (passport, birth certificate or BC CareCard).
ELIGIBILITY
To be eligible for a SCHOOL IDENTIFICATION CARD, a Student must meet all the following criteria:
• Attend classes full time or part time at a public or private school

SCHOOL IDENTIFICATION CARDS are available year-round in person at The Artona Group Studio:

Cost per card to Student is $28 ($25 plus $3 tax)
Students must not wear hats, wigs or sunglasses in photographs submitted for identification purposes. Incomplete applications cannot be processed and will be returned.
REQUIREMENTS
A SCHOOL IDENTIFICATION CARD Application Form must be completed, authorized and presented with all card requests initiated by a school on behalf of, or from:
• New Students now attending classes at a public or private school; and/or
• Students who missed an initial photo session and/or re-take photo session at their school; and/or
• Students requiring a replacement card
PRIVACY NOTICE The personal information collected, used and disclosed is necessary for the administration of the Student ID Card program and is in accordance with the provisions of Part 3 of the Freedom of Information & Protection of Privacy Act.
