Spin-off company commercializes simulation education

SimTunes, an MUSC spin-off company, announced it will offer patient simulation educational material developed at MUSC’s Health Care Simulation of South Carolina (HCSSC) beginning this April through SimStore on the Internet.

This development marks the advance of better, standardized simulation technology training offered worldwide that will result in a better-trained health care workforce. Training through patient simulation is thought to reduce medical errors and improve patient safety overall, similar to the effects of its use in other industries, such as commercial aviation.

John J. Schaefer III, M.D., holder of the Lewis Blackman Endowed Chair at Health Sciences South Carolina and MUSC Simulation Center director, said what he really likes about the project is that licensing simulation learning systems serves the main missions of improving patient safety and the education of health care students, all while generating resources and jobs in South Carolina.

Charleston Mayor Joseph P. Riley, Jr. also supported the development.

“I am thankful for the vision and support of the leadership in the Health Sciences South Carolina and S.C. Legislature in supporting the endowed chairs program," he said. “Today’s announcement is a most extraordinary example of MUSC’s research activity creating a health sciences company that will deliver groundbreaking, life-enhancing services to the world.”

SimStore (http://simstore.com/) will sell a wide range of medical simulation educational material. It was established by SimVentures, a joint venture between HealthStream Inc., a leading provider of learning and research solutions for the health care industry, and Laerdal Medical, the world’s leading provider of resuscitation education products and patient simulators.

SimStore will offer for purchase more than 1,000 scenarios—with thousands more to follow—from some of the world’s leading health care organizations and associations that together form a leading developer network.

Academic medical institutions that use simulation education will benefit from access to scenarios that are standardized to equalize training, instruction material needed for the simulation teacher, and extensive debriefing sections that enable researchers to study simulation training effectiveness, and eliminate the need for a full-time simulation specialist because individual educators can master the content to pass

onto students.

By April 1, SimTunes expects to have more than 300 simulation scenarios available for sale through SimStore.

Heyward Coleman, president of SimTunes, said SimStore is going to be very effective in distributing educational material developed at HCSSC to centers that employ patient simulators around the world.

“HealthStream, the leader of online education to hospitals in the United States, and Laerdal, the largest producer of patient simulators worldwide, are the ideal partners to make SimStore a dominant factor in the field of educational material for the simulation industry,” he said.

Nurse uses past lessons to inspire hope for others

I

n children’s author Lynne Barasch’s book, “First Come the Zebra,” the story focuses on the cultural struggles of two Kenyan boys, who grew up as enemies from rival tribes, but who work together and overcome conflict by realizing their differences and coexisting in harmony, hope and friendship.

It’s a theme that resonates loudly for Kenyan-born neonatal nurse educator Kathleen White. Drawing from her multicultural childhood in East Africa, White wants to be a catalyst for positive change among students and practitioners in health care focusing on respect, collaboration and acceptance. For the past year, White has worked with campus leaders, students and individuals to address prejudice, discrimination and conflict by developing dialogue skills through MUSC’s National Coalition Building Institute (NCBI) program.

White is among four women at MUSC honored as part of the 2011 National Women’s History Month celebrating leadership and excellence. This year’s theme, Our History is Our Strength, recognizes 100 years of women’s achievements around the world.

White is the youngest of four children born to American career missionaries affiliated with the Africa Inland Mission International, and grew up with the Kikuyu people in the central highlands of Kenya, East Africa. The Kikuyu tribe led the uprising against British colonialism resulting in Kenya gaining its independence in 1963. White has seen first-hand the results of the country’s tribal tensions, affecting the government, land and everyday lives of the Kenyan people.

As a child, she remembers watching visiting missionary doctors and nurses caring for and treating the men, women and children in the Naro Moru and Kijabe communities where she grew up. Their presence and influence had such an effect on her that her mother made her a child’s nursing outfit to play in. It was her first introduction to nursing and health care.

In January, White and her husband, Bryan, and 10year- old daughter, Amelia, returned to her native Kenya and visited towns and villages from the Great Rift Valley to Mount Kenya and Kenya’s eastern coast.

“It was a trip of a lifetime for me and my family,” said White, about her homecoming after a 26-year absence.

“Although Kenya has progressed and seen growth in its cities and towns, there are still dramatic socio-economic contrasts.”

Embracing her past, White has always shared an intercultural spirit and non-judgemental attitude with others as a nursing student studying at Columbia University in New York’s inner city and throughout her career as a NNICU nurse in Michigan, Illinois, Virginia and South Carolina. She came to MUSC in 1990 to

Deb Browning, R.N. manage the Children’s Hospital’s neonatal intensive care unit (NNICU) services. Then, she received her master’s degree in nursing from MUSC’s College of Nursing and worked as a neonatal nurse practitioner.

She spent two years with facilities management as a nurse consultant and project manager before returning to the NNICU as nurse educator.

Deb Browning, R.N., Perinatal Service Line clinical director, works closely with White in the NNICU. “Kathleen’s an organized, dependable and genuinely caring person who has great empathy for others,”

Browning said, of her friend and colleague. “She is focused on improving neonatal outcomes by use of evidence-based nursing practices. She brings a lot to the table within Perinatal Services.”

Wanting to do more beyond the clinical setting, White got involved with the Office of Diversity’s NCBI Welcoming Diversity training and communications workshops to promote campus diversity and reduce prejudice and conflict. She found it easy to teach students, faculty and employees behaviors she was passionate about. She’s collaborated with NCBI campus director DaNine Fleming, Ed.D., and other colleagues as an active trainer and mentor with the NCBI Campus Leadership Team.

The Office of Student Diversity’s associate director Laurie Charles, Ph.D., remembers how White voluntarily completed the three-day diversity training workshop and other education required by leadership team participants. “Kathleen easily stepped into the role of leader throughout the program. She was extremely thoughtful in her planning and cited from both personal and professional experiences to emphasize specific information. It was a delight to watch her interact and connect with others,” said Charles.

White continues to make a difference whether she’s guiding neonatal nurses in the units or teaching the value of diversity and interprofessionalism with physicians, nurses and other health care professionals in training.

“This level of training and skills building is so important in health care today,” White said. She believes hospitals and higher learning institutions are in a vulnerable time. “With diminishing resources and greater demands in terms of quality and compassionate care for our patients, health care professionals need to work effectively together now more than ever. It is essential that we use collaborative-building skills to work through the relational struggles that role diversity brings. If not, people look to their own self interests, naturally pulling away from one another, which leads to struggles reminiscent of those amongst the tribes in Kenya. It doesn’t have to be that way.

“My hope is that if we choose to do this work, our interprofessional relationships and clinical outcomes will both improve,” she said.

It’s a hope-among-struggle belief that White has grown up with and passes on to daughter, Amelia.

f C. David Geier Jr., M.D., has a spare minute, he’s probably tweeting. Or perhaps researching the topic for his next blog or checking his website’s Google analytics.

Basically, he’s just having fun. “I love doing it,” he said, grinning like a boy with a new toy.

He pulls up his website page to show his latest posts.

His website has had hits from all 50 states and 76 countries. Geier, an orthopaedic surgeon and director of MUSC Sports Medicine, blogs about sports injury prevention and treatment and writes on wellness topics for online publications and a column for The Post and Courier newspaper.

“I’ve learned I enjoy writing, and one of the things I’m trying to do is develop more of a media presence, and it’s been fantastic for that,” he said.

“Take the pulse. Listen first. See what people are saying and what information you have to offer that they are looking for and that may be unique to what others are doing. What I love about social media in media relations is that it allows us to connect with the people we serve in unprecedented ways. We don’t have to rely only on news stories to get good news out. We would love for more people here to get involved and serve as ambassadors of MUSC in this way.”

Geier said he’s gotten very favorable reaction to his website both from MUSC colleagues and others across the country, and given the state of the economy, he hopes See Digital on page 7

more MUSC health professionals will take advantage of new media. “It makes the university look fantastic. We need to be doing more of this.”

It does take doing some homework before just jumping in. Geier said he was surprised how much time it takes to keep a site updated. Though it’s easier for him now than when he first started, writing his blogs requires a substantial time commitment. It also takes patience in the beginning to build up a following.

“It’s a lot of work, but it’s well worth it,” he said of the discussion that new media can generate. He also likes to clear up misconceptions that are in the press or raise awareness on issues that are important to him, such as preventing sports injuries.

Geier, who found he was frustrated by the poor information available online about sports injuries, said he enjoys getting out good, quality health news in layman’s language that will raise awareness about wellness. It helps to build relationships with people. “I felt it was a good opportunity to get good discussion out there. I realized pretty quickly that I was enjoying doing it, and I got more efficient at it.”

Becoming involved with social media is not for everyone, he said. Health professionals need to understand what’s involved before taking the plunge.

Woolwine agrees.

“Not everyone feels comfortable taking the bull by the horns like Dr. Geier, and that’s OK. There are other ways to get involved. For example, staff can comment on Facebook pages that relate to their expertise. On the MUSC Facebook page, our audience loves to read comments from the folks who work, train and study here, and they really love to see our humor, humility, and transparency,” she said.

“Social media allows the MUSC community to get a better feel for the people we serve and in turn, for those we serve to better know us.”

Geier said he’s gotten significantly better in the last six months that he’s been doing it, but it takes practice. He didn’t realize how much he would enjoy writing until he started to do it. He recommends writing four or five unpublished blogs before deciding if it’s right for you.

MUSC’s social media efforts, including the new community blog, give consumers a chance to share, question and ultimately engage with MUSC, she said.

“The relationship between patients and their health providers is a unique one and is frequently an emotional one.

The social media outlets really give them a chance to express how MUSC and the people of MUSC have affected their lives and the lives of their families.”

The MUSChealth Twitter account, with more than 3,800 followers, provides snippets of interesting health information that may be useful to specific segments of the population.

MUSC’s Twitter page offers real time news and information in 140 words or less. Visit http://www.twitter.com/ muscpr.

It generates frequent comments and re-tweets, as others share posts with their own followers, she said. “The community blog is a much more personal experience for people. The entries may contain valuable health information, but they generally share patients’ personal accounts of how MUSC has changed their lives and their gratitude for the life they now have.”

These websites and social media outlets can generate patients for doctors or clinics, but that’s not what drives Geier in using social media, he said.

“Eighty percent of the questions I get aren’t from the immediate area, so I’m not going to see them as patients. But if it helps them go to their doctors or it helps them to question and make better decisions, then there’s value in that.”

Geier said he’s beginning to understand the exponential or viral power of social media. “I’ll use Twitter or Facebook to drive traffic to a post I wrote, and based on re-tweets or comments people make, I can see how many times my post was viewed by both my followers and the followers of them. One post might be seen by 2,100 people.”

Health professionals becoming more active online can help bridge an enormous information gap, he said.

Some specialties lend themselves better than others. “I think sports medicine is so broad because everyone runs or plays sports or lifts weights. Women’s health, for example, or pediatrics would do very well.”

2. Expect a time commitment.

Some health professionals work with ghost writers, but even so it takes time to review the posts and keep up with them.

“The worst thing you can do is start a blog that doesn’t look good and isn’t updated regularly.”

3. Go through proper channels.

If representing MUSC, be sure to take time to clear how the online information is set up to make sure it’s compliant with branding guidelines set by MUSC. For more information, visit: http://www.tiny. cc/muscmediaguide.

4. Know the legal guidelines.

Learn how to field questions to avoid legal issues and not get into the territory of giving information that’s too personal and could be seen as medical advice. The information needs to stay general and Geier includes a disclaimer in every post.

5. Enjoy the nature of it.

“You have to be passionate about it.

You just have to be.” He loves the interactive, free-flowing aspect of social media and blogging. He’ll have people ask questions, which will give him an idea about something he’d like to write.

“It turns more into a discussion board and the blogger only has to put up a comment every so often to keep that discussion going.”

Social media opens the door to a new way of delivering health information that’s exciting for those who are drawn to it, he said. “Medicine is not just opening your doors and saying, ‘come in and see me.’ It can be, but we’re evolving away from that. This is one way to be proactive about getting out there and being involved with the community.

“We need to capitalize on our academic expertise. We’re MUSC. If this article encourages a couple of people to get involved with social media and blogging, that’s good. It’s good for MUSC. It’s good for the university.”

We are continuing to make progress with drilling down on our 5 & 5 plans as discussed in previous Currents newsletters and the town hall meetings. The plans are intended to reduce the cost of care by 5 percent this year and 5 percent next year while improving quality of care. We are using our IMPROVE performance improvement tool to structure and track our cost savings plans. Our goal is to reduce cost within two years by 10 percent, which amounts to approximately $100 million, while providing quality care and protecting jobs. We are compelled to reduce costs of care due to Medicaid funding reductions expected in the immediate future and because it is anticipated that health care reform at the national level could adversely affect reimbursement levels later.

Of significance is Medicare’s public report card of hospitals’ core clinical measures and HCAHPS (consumer assessment) results which will have a direct bearing on our reimbursement levels in the future. While all details have not been resolved, it is expected that hospitals’ performance beginning July 2011 and later will impact their Medicare reimbursement levels.

On a related note, we continue to make progress with an array of MUSC Excellencerelated initiatives which complement and support our efforts to reduce costs while improving quality of care. We are now in the process of updating our yearly MUSC Excellence work plan and priorities. In the near future this newsletter will include updates on the various teams. We have more than 15 active Service Excellence teams in place and numerous other work groups that are doing very impressive work.

For example, the centralized discharge call program pilot project involves seven clinical units and is doing an excellent job. The discharge call program has achieved a 99.8 percent call rate and 86.4 percent reach rate for discharged patients, with documentation and follow up on clinical issues identified. Also, a First Impressions Team was created several months ago to implement training and standards to ensure for an excellent patient and visitor experience. This team consists of employees from a number of service areas which play key roles in creating first and lasting impressions including: patient transportation, outreach clinic receptionists, parking attendants, valet parkers, information desk, patient and family shuttles, retail dietetic services, environmental services and other areas.

Finally, I want to thank everyone for your work toward enhancing our HCAHPS results. For the period of October 2010 to January 2011 our overall HCAHPS rating, as reported through the AVATAR patient satisfaction survey, indicated 81 percent of survey respondents rated the Medical Center a 9 or 10 on the ten point rating scale. We have opportunities for improvement within the various categories of the survey, but our overall results have improved from a 77 since the April 2009 to March 2010 time frame. Our current results position us very favorably compared to other hospitals throughout the state and nation, but we realize all hospitals are improving.

Infection Prevention Awards Linda Formby, R.N., manager for Infection Prevention and Control, and Cassandra Salgado, M.D., hospital epidemiologist, joined the medical center’s infection preventionists in recognizing excellence in reduced infection rates for 2010. Awards were presented to HCC—7W; MSK—10W and 2JRU; SACC—6W and STICU); Children’s—PICU, PCICU, 7E and 7A; Neurosciences—9W and NSICU; MACC—8W and MSICU; Heart & Vascular—A3WX, CCICU, CTICU, A5EX; and DDC—A6EX and A6WX.

Achievement Award. MUSC achieved the AHA’s Gold Sustained Plus Achievement Award in stroke, the program’s highest level.

MUSC’s performance and best practices will be recognized in U.S. News and World Report magazine’s best hospitals edition.
