HEALTHCARE EXPANSION A SIGN OF BRIGHT FUTURE

E

ast Texas is home to quality healthcare facilities and thriving medical systems. Expansion can be seen across the region, creating better care for residents of both rural and urban areas of East Texas. Beginning in 2020, state and regional leaders and philanthropists joined together to show support for a medical school in East Texas. After meeting a number of milestones and receiving an unwavering amount of support, the UT Tyler School of Medicine broke ground one year ago and started construction this past fall. Now, leaders are looking toward the future with a mission to improve healthcare across Northeast Texas. Additionally, a number of facility expansions have been announced and others already completed, like the new Christus Health NorthPark Hospital that opened in January. The 28,000-square-foot Longview orthopedic and sports medicine center will provide advanced surgical facilities and inpatient

care geared toward musculoskeletal operations. In South Tyler, UT Health East Texas broke ground in September on a 20,000-square-foot building to house orthopedics and sports medicine services, an urgent care, a diagnostic and imaging facility as well as outpatient rehabilitation. It is expected to open mid-to-late this year. In August, Longview Regional Medical Center expanded access to emergency medical care in East Texas with the opening of its new Emergency Center. And that’s not all, as other providers have expansions in the works. If these growth trends of the past few years are any indication, the future is looking bright for our region. We commend our local healthcare entities on their determination to better serve East Texas, and inside you’ll read stories about just that.

A BRIGHT FUTURE

Bethesda Health continues mission of serving patients in need with expanded services, new programs S T O RY B Y R A Q U E L V I L L AT O R O PHOTOGRAPHY BY LES HASSELL

W

hen Bethesda Health Clinic opened in Tyler in 2003, it had shower curtains separating rooms. Now, more than 20 years later, the health clinic has grown exponentially to better serve the community. From expanding into pediatrics to opening a third Hangers of Hope store and starting a satellite clinic in Lindale, Bethesda had a lot to celebrate in 2023 for its 20th anniversary. The growth is thanks to community support, said Megan Riaz, Bethesda Health Clinic Director of Community Relations & Marketing. “We started [counseling] and honestly, this is all because the community supports Bethesda,” Riaz said. “The more that the community supports Bethesda the more that we're able to expand our programs and help more people. The more volunteers we get, the more people we serve; the more donations we get, the more people we serve.” Bethesda Health Clinic serves low-income adults who have no insurance or who are underinsured (those who have deductibles that are high relative to their income). Since its inception, the clinic has served more than 20,000 patients and completed more than 265,000 patient visits.

MORE THAN JUST MEDICAL

In addition to medical services, Bethesda offers mental health counseling, dental and vision. Starting in 2024, the clinic will add another full-time dentist. They added dental around 2008 after realizing there was a need for affordable dental. For those who are on Medicare and Medicaid, dental is not included. Bethesda realized seniors and those on disability were not getting access to dental. At the dental clinic, those uninsured, on Medicaid or Medicare can get dental care. While they focus on pain management, they also fix smiles, Riaz said. “You have people, they might have a missing front tooth, or this or that, and it's preventing them from getting that job they need and providing for their family,” Riaz said. “Or they have a lot of shame because of the way their teeth look. We're able to help transform their smile. I love those stories because they literally leave the clinic and they're so happy. They're just bursting with joy crying because they can smile.” Many of their patients have diabetes or other chronic health conditions. The lack of access to doctors exacerbated or worsened health conditions. They offer a Prescription Assistance Program to help people get the medication they need. “It's just life saving,” Riaz said. As long as they're a patient, they have access to that care, they can get their medicine for free. “It's amazing to see how grateful people are,” Riaz said. They also have a dietitian volunteering and helping 10

patients with their diet. For patients who previously did not grow up going to the doctor, this is educating them on what kinds of foods to eat. Another service Bethesda provides is counseling and therapy. Their counselors offer various therapeutic treatments alongside talk therapy. The co-pay for counseling is the same as the co-pay for medical services. Because many of their patients speak only Spanish, Bethesda has a bilingual counselor. Having someone who can speak their language helps patients feel comfortable, Riaz said. “They really need it, it's not right to say ‘hey, you're gonna go speak with an English counselor and have a translator,’” Riaz said. “It's already very intimate, and personal. A lot of people also have their own preconceived notion of counseling and they might be hesitant to go, especially if that person doesn't speak their own language.”


GROWING AND SERVING PEOPLE

Inside the Journey Center, a nonprofit in Lindale with various ministries providing different resources from food to health, Bethesda began offering primary care services this January on Mondays and laboratories and admissions on Wednesdays. The goal with the Lindale satellite clinic is to reach the surrounding rural areas such as Mineola and Van who may not be able to get to Tyler. Dr. John English, Bethesda CEO, will be the doctor on site. The clinic will be staffed with volunteers, nurses, admissions and a phlebotomist. “It's just like Tyler,” Riaz said. “It's the community coming together and saying, ‘hey, we see a need, what can we do to help meet that need?’ They come alongside us and it's a partnership, which I think is what makes it really successful.” On Sept. 1, 2023, Bethesda and St. Paul’s Children announced Bethesda Pediatrics, which was huge news for the community and the clinic as a whole. A lot of the same families seen at Bethesda Health Clinic are also treated at Bethesda Pediatrics, so the partnership was a no-brainer. It came together due to the organizations’ faith-based missions and passion to serve a similar client base such as the uninsured. Bethesda serves people from 85 different cities surrounding Tyler. They have saved people around $1.5 million in emergency room costs. If the patients can get there, Bethesda will serve them, Riaz said. She is proud of the growth of Bethesda and its impact on people. “It makes me so proud because I really do love our patients,” Riaz said. “I love them, we all do, and to see their joy and to look in their faces and know how grateful they are and how they feel better and that they can live that abundant life. It's like the best thing ever. It makes me really, really proud to be a part of Bethesda. It's like something bigger than yourself. It's amazing to see this happen right before my eyes.” As the organization enters its 21st year, the future is bright. “It’s exciting to think about not only where we’ve come from, but where we’re going, as we look forward to the future, just to really build upon this,” English said. How a hospital in Tyler built one of the best heart attack programs in the country S T O RY B Y S A M U E L S H AW PHOTOGRAPHY BY LES HASSELL

W

here are the best places to get treated for a heart attack in Texas? You might guess a hospital in Austin, Dallas, Houston, or perhaps a hospital connected to one of the state’s leading medical schools. Those would all be good guesses, but unless you pay attention to hospital awards and accreditations in your spare time, it might come as a surprise that one of the nation's leading institutions for heart attack treatment resides in Tyler. The Louis and Peaches Owen Heart Hospital resembles a glass spaceship docked at the Christus Trinity Mother Francis medical complex. The facility became the first dedicated heart hospital in East Texas when it opened just 11 years ago. It’s still the only one. By 2018, five years after opening, Tyler’s heart hospital was the first in the country to receive the American College of Cardiology’s National Distinction of Excellence. In 2021, the American College of Cardiology awarded the hospital a Platinum award for

chest pain and heart attack treatment, putting the Louis and Peaches Owen facility among an elite group of around 200 hospitals in the country that received the designation. Time governs everything about heart attack survival and the staff behind the facility's precipitous rise in cardiology circles brought a similar level of urgency to East Texas heart care.

‘TIME IS MUSCLE’

The hospital’s Cath Lab (short for catheter) is an operating room specialized for the insertion of heart stents that expand blocked arteries, effectively stopping heart attacks. The team inside works like a rapid response team, always on call and patched into EMS operators who give the Cath Lab advanced notice of serious heart attack cases called STEMIs or ST-elevated myocardial infarction, said Jamie Moore, a nurse and the hospital’s clinical director of the Cardiovascular Center for Excellence. FROM PG. 13 shepherd the patient to the facility where they skip past the ER and arrive straight at the Cath Lab operating table. “The urgency for treatment is different with a STEMI,” said Dr. Oscar Paniagua, the heart hospital's chief medical director overseeing heart attack treatment. “It's a very acute event. So the benefit of intervening very early on and very quickly is more important than with other kinds of heart attacks.” The window from first contact with EMS personnel to the patient hitting the Cath Lab operating table is 90 minutes or less. As soon as the heart attack begins, so does damage to the heart making every minute critical to survival. “That’s why they say time is muscle,” said Moore. The clock stops only once the blocked artery is open and blood flow is restored — sometimes with a stent or an inflatable catheter called a “balloon” — in the Cath Lab. Despite the hospital service area covering all of Northeast Texas or roughly 1.5 million inhabitants, with many living deep in the Pineywoods, Paniagua and his team are achieving “first contact-to-balloon” times of less than 50 minutes, according to Moore. EDUCATION AND THE AIRBORNE AMBULANCE

“More than in other regions, the whole South and Southeast has a high prevalence of strokes and heart attacks,” Paniagua said. “That can come down to lack of exercise, poor diet, delays in diagnosis or receiving medical care.” Though medical technology, team coordination, and transport times are vital to improving heart attack outcomes in a region wrestling with the disease, so is education, according to Paniagua. Outreach is one of the investments his teams have made in the rural and farflung pockets of East Texas. “That's why we do a very heavy educational program, at least in East Texas,” said Paniagua. “We’re going to the communities and organizing events, trying to educate people about what the signs and symptoms of a heart attack are and all the important steps for prevention early on.” His team has seen the results of that effort from patients who used the educational material to quickly identify what they were feeling in their chest, Paniagua said. But no amount of education will stop time from being 16

a factor when a STEMI call does come through. And when it does, Chritus’s growing air wing is ready to pick up even the most remote patient and keep treatment times below the all-important 90-minute mark. “Our five helicopters are comprehensively equipped air ambulances,” said Shawn Salter, director of Christus EMS and Flight for Life. “And each one of our helicopters is staffed by a flight registered nurse and also a flight paramedic.” None of the aircraft are kept at the heart hospital. Instead, they’re dispersed across Northeast Texas to further decrease reaction time and distance traveled. “Statistically cardiac care accounts for about 20% of our total request for service,” added Salter. The helicopters also include emergency ultrasound equipment and life-saving medications. t the Heart and Vascular Institute of Longview Regional Medical Center, cardiac advancements and specialized care are important pieces to treat patients in East Texas. LRMC’s institute focuses on providing minimally invasive surgeries, providing a second-opinion for patients and providing the best postoperative care. For East Texans, having care close to home is important. It reduces stress on planning, traveling, figuring out where to park, and making arrangements with family. Hospital systems in bigger cities can also be more

expensive, according to Dr. David Jayakar, director of cardiovascular services at Longview Regional. The Longview Regional Cardiac Care team works to keep patients in East Texas. The cardiovascular team is made up of 10 cardiologists, five anesthesiologists, and one surgeon. In addition, they have a huge team of providers and nurses dedicated to the cardiac ICU. In contrast, Longview Regional patients will see a nurse and surgeon every day, Jayakar said. “We are able to keep them here, treat them appropriately and then get them to the stage in which they can have a better experience than going through to a university hospital,” Jayakar said. Jayakar said patients often come to Longview Regional for a second opinion regarding procedures such as open-heart surgeries. “Let's say you see a surgeon (elsewhere) and they say you're a high surgical risk and you're not a surgical candidate,” Jayakar said. 20

PROVIDING MINIMALLYINVASIVE SURGERIES

Heart disease is the No. 1 leading cause of death in Northeast Texas. The rate for heart disease is 27% above rates in Texas overall. If Northeast Texas were a state, it would rank 47th in heart disease, according to the Health Status of Northeast Texas 2021 report. Aortic dissections — an extremely fatal condition — requires a whole team approach for high-risk patients. This high mortality leads community hospitals to sometimes transfer patients to university facilities,

Jayakar said. “But we’ve eliminated that [at] Longview Regional,” Jayakar said. “We are now the go-to center for dissection.” Coronary heart disease — caused by plaque buildup in the arteries restricting blood flow — is the most common type of heart disease. In Northeast Texas, the rates for CHD were higher than rates in Texas, at 8.3% in 2019, according to the 2021 report. Coronary aortic bypass surgery, or beating heart surgery, is done with the heart still beating. Traditional surgeons cut the whole breastbone and stop the heart with the heart valve machine. “We leave [the breastbone] alone


so our patients can use their hands right away,” Jayakar said. “They can drive their car within a week without restrictions.” The traditional method is considered an invasive surgery. The beating heart surgery is considered minimally invasive because the heart doesn’t stop, Jayakar said. “We don't cut the press valve completely,” Jayakar said. “We don't stop the heart. We keep the whole thing beating. So recovery is faster and you get better faster.” Many of the surgeries Longview Regional performs are done without pressing the valve on the heart machine, meaning they are less invasive. Some of the patients Jayakar has seen work in the oil field, for example, and were able to go back to work after a few weeks. In addition, Longview Regional performs valve surgeries. The team repairs valves instead of replacing them. Valves are like doors. If your door is broken, it needs to be fixed, Jayakar explained. “The problem is that there is no real good replacement valve,” Jayakar said. “What if I can repair you? That means you don't have to replace it with a man-made valve. That is what we do.”

PROVIDING THE BEST POST-OPERATIVE CARE

After surgery, if the patient still needs support, the team will keep checking on them. After they get better, the team will transfer them to a rehab center with more involved care. Longview Regional is passionate in providing minimally invasive care along with providing experienced care so people can get the care in Longview, Jayakar said. “We want you to improve,” Jayakar said. “We want people not to be traveling away and be away from loved ones.” In addition to heart care, Longview Regional offers a number of services and specialized care for East Texans. Additionally, the emergency room’s average wait time was just nine minutes as of Jan. 3. or the UT Tyler School of Medicine, 2024 will be a year full of growth, Founding Dean Brigham Willis said. From building on research efforts to continuing construction to welcoming their second class and celebrating the inaugural class’ first year, the school has a big year – and bright future – ahead. The 40 students of the school of medicine’s inaugural class will successfully complete their first year in 2024, and the second cohort will start classes. The school of medicine also has plans to expand research, residencies and grow its community efforts, Willis said. In 2025, the school’s construction is expected to be finished and Willis said the school will also seek provisional accreditation that year.

PRODUCING MORE PHYSICIANS

In 2023, the school of medicine had its most successful quarter for research funding and welcomed the inaugural class. The class, made up of 40 students, has completed their first semester, received EMT certifications and are doing “fantastic,” Willis said. “If you meet our students, you'd be amazed at the quality of character and the dedication to mission and just overall what amazing people they are,” Willis said. “So we wanna keep that going.” When selecting applicants for their first year, they looked for applicants with roots in East Texas. Of the inaugural class, 95% are from East Texas and 5% have family or connections in the region and intend to practice in the area. The inaugural class is made of people from all around East Texas including Longview, Hallsville, Tyler and Nacogdoches. The students come from various counties including Kaufman, Bowie, Henderson, Gregg, Harrison, Angelina and San Jacinto. The school looked for specific qualities in applicants: service, leadership, humanism and their ethics. For the second class of applicants, Willis noticed more students applying from outside of East Texas. It received more applications for the second class with 6,000 as of September 2023, compared to 4,000 for the inaugural class, Willis said. “Our mission is to produce physicians who are dedicated to East Texas, are going to stay here and practice to improve health outcomes,” Willis said. “And to do that, I | CONT. MORE PROGRAMS AND FELLOWSHIPS

In order to increase physicians, the school of medicine is establishing new fellowships, residencies and programs. Some of the fellowships it plans to establish include oncology and hematology. It also plans to establish a residency in anesthesia and programs in OB-GYN, pediatrics and emergency medicine, Willis said. “Among all counties in Texas, 26 of the 35 counties in the Northeast Texas region ranked in the worst two quartiles on the health outcomes score and 26 of the 35 ranked in the worst two quartiles on the health factors score,” according to the Health Status of Northeast Texas 2021 report. Cancer is the second leading cause of death in Northeast Texas. Cancer rates in Northeast Texas are higher than statewide rates, according to the 2021 report. The fellowship in oncology would help address the need for cancer care. Also in the region, kidney disease mortality rates are 8% higher than statewide rates and 25% higher than U.S. rates, according to the 2021 report. A fellowship in nephrology, the care of kidneys, could help meet those needs. “We're really trying to look at every specialty that has a need in East Texas and establish training programs so that we can start providing physicians,” Willis said.

CONTINUING COMMUNITY PARTNERSHIPS

An important pillar of the school of medicine is community engagement, Willis said. This is encouraged through the school’s house system, which is split into four, where students can get points for their house. One of the new partnerships the school of medicine has is partnering with the East Texas Food Bank. At its recently opened Tyler Resource Center, the school of medicine will offer health screenings. In addition, the med school volunteers will continue doing Clinic Under the Bridge once a month in Tyler, Willis said. These opportunities allow the students to get real-life community experience in addition to their regular core classes. “We've had them go out for a number of clinical experiences and the feedback is that they seem like third or fourth year students | CONT. The school of medicine is working on building centers to better its research efforts and be able to exchange and share information through the various places in East Texas focused on health. The centers include a center for rural health and informatics, according to Willis. Informatics would focus on analyzing big data — the massive amount of data they get from hospitals — through data processing and AI in partnership with the School of Nursing, School of Pharmacy and School of Health Professionals. The center for rural health would help “synergize” and bring everyone together and better utilize their skills. This would bring together the efforts of the Southwest Center for Agriculture Health and the various schools dedicated to health at UT Tyler, according to Willis. “We're focusing really strongly on expanding 26

UT Tyler School of Medicine is set to complete its $308 million Medical Education Building in July 2025. Construction kicked off this past fall and the last beam is expected to be put up mid-2024. The completed building will include five stories, the third story which is currently being built. The state-of-the-art facility will be the first of its kind in Northeast Teas and is expected to be home to a top-notch education system for the region. Willis is proud of the achievements the school has made, including a fully operational simulation lab. They also have one of the biggest collections of plasticine models. The school of medicine is American Heart Association accredited. They also are providing ongoing opportunities for physicians. In 2025, they will be up for provisional accreditation, Willis said. “We’re going to be the No. 1 rurally focused medical school in the country,” Willis said.
