MY VOICE MATTERS

World Environment Day is celebrated every year on 5th June. That being said, our concern for nature shouldn’t be limited to just one day! We must work for a cleaner future, not just hope for it! Infertility To Motherhood with IVF
A Journey with right companion and support
By: Dr Mayur Pai (Consultant Infertility, Obstetrics and Gynaecology Pai Hospital, Vasco)
What is Infertility?? What are the treatments for Infertility??
These are some of an important question asked by many couples trying hard for pregnancy. Undoubtedly This is also one of the frequently asked questions on Google as many couple hesitate to speak openly about infertility or conceiving issues leading to misunderstandings in their relationships . This article is a brief introduction to infertility and its treatments.
What is infertility?
Infertility is failure to conceive within one or more years of regular unprotected sexual relation. Primary Infertility refers to those patients who nave never conceived. Secondary Infertility refers to those patients who have become pregnant before but now have difficulty in conceiving at present.
When should a couple visit the infertility doctor?
As per statistics, 80% of the couples can achieve pregnancy in one year of marriage if they keep trying. Of the remaining 20%, 10% will conceive spontaneously in the second year. About 10% Of the couples will remain Infertile by the end or the second year. A few recent statistics say that the incidence of infertility is increasing in recent years. In spite of the above statistics, a couple may visit the infertility doctor if a pregnancy is desired early. A couple in which a partner is above the age of 35 years, should visit the infertility doctor if they do not have conception within 6 months of trying. In case of repeated miscarriages, couples should seek help early. If a couple has done infertility investigations by self in any lab and the results are not normal or show decreased fertility , couple may seek help earlier than one year.
Investigations and Role of Husband:
There are the different tests that the doctor does to diagnose the cause of infertility. Chlamydia test, Ultrasound scan ,Sperm test ,Hormone tests, Hysterosalpingogram, Laparoscopy, Hysteroscopy and Cervical mucus test . These tests will help the doctor decide which treatment might work best in that particular couple. There is a myth that only the woman is the cause for infertility and hence she should be the only target of treatment. This is not true. In up to 30% cases, the cause is the male factor. In infertility evaluation, a couple constitutes a biological unit and therefore, investigations of both partners individually and collectively is essential to plan a rational approach to treatment. This also ensures couple counseling and improves success rates.
What are the routine tests that a doctor will do?
The usual basic investigation protocol includes blood and urine tests for both husband and wife, semen analysis for husband and ultrasound for wife. As per the individual needs of the couple, the doctor may need added studies like tubal patency tests and diagnostic endoscopy for the wife and further testing of husband. The investigations are tailor made as per the clinical diagnosis of the infertility doctor.
A basic idea of the treatments in infertility. Only the most common treatments are discussed here. Ovulation induction (Ol): This is the back bone of infertility treatment. Here, some medicines are given to the woman to produce eggs. The progress of the eggs' growth is monitored by a special ultrasound scan called a follicular study or folliculometry. Once the eggs become sufficiently mature, the doctor usually gives an injection to trigger egg release or ovulation. The doctor then counsels the couple about the most fertile days. The couple is advised intercourse on those days.
Intra uterine insemination (IUI): Ovulation induction may be combined with |UI. IUl is a procedure that involves placing sperm inside a woman's uterus to facilitate fertilization of egg. The goal of IUI is to increase the number of sperms that reach the fallopian tubes and subsequently increase the chance of fertilization. This procedure further increases the couple's chance of conception.
In vitro fertilization (IVF): This is commonly called test tube baby treatment. This is one of the major treatments of infertility and is done when other treatments have failed. IVF is a process by which an egg is fertilized by sperm outside the body. This fertilized egg is further matured to form an embryo (an early development stage of baby). This embryo is transferred into the woman's uterus. There are a few uncommon treatments which are not discussed here.
What are the success rates?
The success rates vary as per the diagnosis, couple's age and type of treatment offered. Typically success rates vary from 10 to 30% in most centers. These rates are now going up with newer techniques in reproductive technology
Useful information for early conception.
Timing: The most fertile time of a woman is between day 10 to day 20 of her menstrual cycle. During these days, a regular intercourse improves pregnancy rates markedly.
Diet: Have plenty of water to drink (2 to 3 L per day). Avoid high fat and high starch foods. A balanced diet which also includes fruits and vegetables helps to keep excess weight under control.
Lifestyle: Avoid alcohol and tobacco. Avoid hot temperatures. Regular exercise like walking helps in keeping weight under control. Couples doing exercise as a pair tend to remain healthy. Keep stress and anxiety away. Always feel free to contact an infertility specialist for expert opinion.

ARZ Launch ‘WISH’ an initiative for dignified life

The ARZ in association with Goa Police, Department of Women & Child Development, and Office of the Sub-Divisional Magistrate has been actively involved in Rescue, Rehabilitation, Reintegration and Repatriation of the victims of sex trafficking in Goa. In 2006, the ARZ had set up a mechanised laundry unit, Swift Wash, to provide employment to the victims of sex trafficking.

The Anyay Rahit Zindagi (ARZ) has started a Paper Products Production Unit for the economic betterment of ex-sex workers. Approximately, 15 ex-sex workers are enrolled with ARZ. The aim of the unit will be to provide financial stability to the workers and their families. The Director of ARZ, Mr. Arun Pandey mentioned “we were running a laundry to rehabilitate the Baina sex workers since 15 years. In that process, we were dependent mostly on hotel industries. But, during the pandemic we faced lot of issues and the unit had to be closed down as we were not getting customers. So, we have now come up with the idea of Paper Products Production, which is an independent business.”

DENGUE, A VECTOR-BORNE DISEASE IS GRABBING A LOT OF ATTENTION THESE DAYS DUE TO ITS INCREASING PREVALENCE IN AND AROUND THE CITY. AS PER THE STATISTICS PUBLISHED BY THE NATIONAL CENTRE FOR VECTOR BORNE DISEASE CONTROL, THE NUMBER OF CASES IN GOA HAS RISEN FROM 293 IN 2015 TO 649 CASES IN 2021, 2019 HAVING THE HIGHEST NUMBER OF 992 CASES.
How is it spread? Four days after being bit by an infected Aedes aegypti mosquito, a person usually develops a condition in which there is a high level of the dengue virus in the blood. It lasts for approximately five days, but can last as long as twelve days. The mosquitoes that spread dengue bite during the day, especially during early morning and late afternoon hours. Dengue fever is not contagious, so it can't spread directly from person to person. Directorate of Health Services under National Vector Borne Diseases Control Programme has adopted preventive measures to restrict dengue virus transmission in the state. Awareness programs are currently been held by the Panchayat and the Primary Health Centres to educate the people before the onset of monsoons. The most common symptoms to Dengue are fever, severe headache, pain behind the eyes, muscle and joint pains, nausea, vomiting, swollen glands and rash.mu Early detection of disease progression associated with severe dengue, and access to proper medical care lowers fatality rates of severe dengue. Like every other disease, dengue is both mild and severe. If you get the proper treatment in the initial phase, you won’t be severely affected by the condition. But, late interruption may lead to complications like breathing difficulty, internal bleeding, liver failure, etc.
