RELEASE AND WAIVER We, the undersigned, participant (and parent if under 18) in the _________(name of activity) to be held at Flandreau, South Dakota, during the 2020 season, do hereby acknowledge and understand the City of Flandreau provides no medical coverage as a result of any injury that may occur during this activity. I voluntarily assume full responsibility for any risk of property damage, property loss, or personal injury that I may sustain, as a result of participating in this activity. This assumption of the risk applies whether the loss or damage is due to the active or passive negligence of the parties hereby discharged or released. Furthermore, I agree to indemnify and hold harmless the City of Flandreau from any and all claims arising from my participation, or my child’s participation, in this activity or as a result of any injury or illness that I or my child may sustain during the activity. We discharge, release and waive the City of Flandreau, its council members, employees, and volunteers from any and all claims for damages, expenses and losses, including but not limited to attorney’s fees, that the undersigned may have as a result of losses, injuries, and/or illnesses arising out of, or resulting from, participation in this activity. 2020 was a fabulous year because the Flandreau community has given the Flandreau Hospital Auxillary the largest support ever for our scholarship program. Now we are looking for students in any medical field that are half-way through their studies so we can award them a scholarship. We accept one-year programs to our scholarship awards which would be paid halfway through the year. The other qualification is that they had to have graduated from Colman-Egan or Flandreau High Schools. Applicants are required to voluntarily disclose their grade point average and provide a brief description of career goals, college attending and its financial office address. You may obtain a scholarship form by emailing electric@vastbb.net or duncana@iw.net or call 997-2786. You must submit the scholarship form by July 1, 2020. Selection of eligible applicants will be made by the Auxillary Scholarship Board.
