Develop a sustainable, self-paced, asynchronous, patient education program that allows for the patient population of the client (a private clinical practice group with multiple medical specialties) to access reliable, physician-vetted educational modules that will reinforce conversations they had with their doctor at the time of diagnosis that may have been challenging to store in long term memory as a result of the emotional response to the discovery of their condition.

Better patient outcomes result in better reimbursement rates for the practice. o

Reimbursement rates for physician time spent providing patient education are limited, both in the amount of time they account for and the amount of compensation offered for the time the provider spends explaining the details of a patient's diagnosis.

o

Many publicly available online resources can often cause patients or their family members extreme distress due to misunderstanding the risk of medication side effects, the potential progression of their diagnosis, or incorrect assumptions about the actual diagnosis for a given patient.

The resulting time spent with repeat office visits, phone calls from patients, patient messages in the electronic medical record, or the emotional and traumatic experiences of some patients as their family members lead them on a wild goose chase on WebMD can cost healthcare providers time and resources that are extremely limited in many instances. For a small private practice, just a handful of patients placing calls to the clinic repeatedly to seek reassurance that the diagnosis they have spun from thin air by spending a paranoia-fueled afternoon scrolling through medical information on the internet can upend the schedule of a clinic full of providers very quickly.

Providing credible, informative, and accessible patient education resources can address concerns that might lead to disruptive and unnecessary uncompensated care demands falling upon clinic staff.

Learner/Patient Personas will balance the necessity of specificity with the constraints of providing intervention promptly rather than attempting to create thousands of modules that would provide a more granular patient education experience (and cost exponentially more to develop). o Much of the value of patient education is a function of the specificity of the information to the circumstances of the individual patient's diagnosis. o Each module will be based on a particular Learner/Patient Persona with a moderately specific diagnosis/symptom profile to provide as much detail of relevance as possible without attempting to create an entirely personalized patient education intervention.

Provider availability: Part of the need for the intervention under development is to free up providers from the overwhelming volume of calls and electronic patient messages through the electronic medical record system requesting patient education counseling more than the amount of instruction covered by the insurer of the patient's care (whether Medicaid, Medicare, Private Insurance, etc.). o Suppose the development of the intervention becomes burdensome to the provider. In that case, the project sponsor will potentially halt the project to manage the limited resource of provider time for providing patient education. While this investment will pay dividends into the future, the client is particularly aware of the potential for burnout among medical providers after extended high-stress periods during the pandemic, and the long-term benefits may become insufficient to justify the short-term impact on provider mental health, morale, and its corresponding impact to patient care.

External Dependencies: (project success dependencies such as coordination of efforts with other teams) • Potential disruptions in the form of Increased clinical responsibilities for providers from the practice assisting with the project in the event increasing numbers of COVID-19 patients change the availability of the staff assigned to assist with the project.

Initial Proposal When patients are first informed of a new chronic diagnosis, it can be an overwhelming experience between whatever symptoms brought them to seek care initially, the emotional toll of the discovery, and the emotional reaction of friends and family. Physicians are often hurried in their visits to see more patients waiting for their care.

This creates a scenario where the patient may not have considered any questions they should raise with their doctor, or if the cognitive load of the experience was excessive, they may have little to no recollection of what the doctor even told them about their condition or how they can best participate in their recovery.

To enable the patient to access sound information about their situation at their own pace and when they are ready to cognitively engage with the knowledge, a medical practice has requested assistance in developing a patient education intervention for the different chronic conditions they treat among their patient population.

Depending upon the diagnosis and individual details of a case, the physician will be able to assign the modules to their patients after an initial diagnosis to provide relevant and accurate information in cognitively manageable portions to help provide alternatives to calling the physician for Content Creation Guide | SME and Sponsor Approved Not for external use.

information and to deter any inclination to explore WebMD for details that would confuse or unnecessarily distress the patient or their family members.

The project sponsor is a neurologist working in the practice, and the first course to be developed will be for neurology patients who recently experienced their first seizure.

The learners being provided with access to this course will come from a diverse range of cultural, socioeconomic, and educational backgrounds. The intervention must clearly define any terms used that are specific to the condition and provide multiple perspectives or examples of how to relate the diagnosis of epilepsy to more commonly understood scenarios that will be easily comprehended. The learners will have been provided an explanation of their condition when first receiving their diagnosis, but the emotional toll or recent symptoms of their condition often make retaining the information from that conversation challenging. This intervention will act as a retrieval exercise and a review of the relevant information.

The intervention needs to be designed for multiple browser types so that desktop or mobile devices will be able to present the material in a functional and aesthetically appealing manner.

Physical Setup

Module should include a prompt for the learner to find a quiet location to complete the module. Close a door, put in headphones that are noise cancelling, or otherwise indicate to others in their environment that they will need some time uninterrupted in order to complete the module.

Patients will have no control over whether they will have received their diagnosis, but by participating in the intervention they will be able to gain the knowledge and skills to participate in their treatment and regain some sense of control over the terms under which they will face their diagnosis. Learning Objectives and Task Analysis: Terminal Objective: Patients who complete the intervention will be able to describe the condition they have been diagnosed with and construct a plan for how they can participate in the management of their care. These objectives consist of declarative and procedural knowledge, whereby verbal information will be synthesized in the context of their diagnosis to enable the learner to develop the attitude and cognitive strategies to properly manage their care.

Enabling Objectives and Tasks: 1.1 - Given examples and non-examples of descriptions of epileptic symptoms, the learner will be able to accurately identify the type of seizure from among the descriptions presented.

1.2 - Given examples and non-examples of medications used to treat seizure disorders, the learner will be able to accurately identify the medications prescribed for their condition from among the listed medications.

1.3 - Given a scenario where the patient must navigate through several situations where they must make choices about how to proceed with daily activities, the learner will be able to correctly choose the options which will reduce the likelihood of their scenario-persona experiencing a seizure.

eLearning Method Lessons 1-3 will be tutorial based videos, interactive slideshows and quizzes that will serve as retrieval exercises to reinforce the material.

Lesson 4 will be a branching scenario where the learner will be presented with a series of everyday occurrences where they will be prompted to make decisions with the goal of not increasing their risk of experiencing a seizure in the scenario. The module will end with specific feedback about what choices they made that may have increased their risk, and why.

Technical Requirements The module will be built using Articulate Rise with Storyline elements incorporated for interactions as detailed in the content summary and storyboard sections.

Learning Activities Lesson 1 Introduction and general information about the patient/learner’s recent diagnosis of Generalized: Tonic-Clonic Seizure (GTCS) with a Sorting Activity (Rise Assessment Feature) Knowledge Check for identifying which characteristics of those listed are associated with GTCS.

Lesson 2 Identifies the main pharmaceutical treatments available for seizure disorders, what specific characteristics of seizure they are used to treat and provides links to reliable websites created by the drug manufacturer for more detailed information about each medication. The knowledge check is a Matching Exercise where the learner pairs each medication with the associated symptoms it relieves.

Lesson 3 Guides the learner regarding other changes necessary to adjust their lifestyle to manage their care in a way that will reduce their risks of additional seizure incidents.

Each Step of the Process will be one of the medications available and each card will include:

Trade Name of Medication, the different conditions or specific diagnosis that it is used to treat, and a link to the website of the manufacturer of the medication for additional information.

Side Effects May Vary Some patients have limited, if any, side effects from their seizure medications, but just as many have more severe reactions to some or all of the available treatment options.

Regardless of how you may react to the drug, communicate with your physician about any issues you may experience with the medication you are prescribed and, more importantly, continue to take it until you find an alternative in this module that works for you. Consult with your physician before discontinuing the medication you have been prescribed for any reason to limit your risk of experiencing a preventable episode.

Client Practice is in the state of Georgia, thus the specific legislative imperatives regarding motor vehicle operation are emphasized in this section of the intervention.

One of the biggest changes is a requirement under Georgia law for anyone who has experienced a seizure. Under GA. COMP. R. & REGS. 375-3-5-.02(2)(c) (2020); GA. CODE ANN. § 40-5-35(a) (2020) anyone who has experienced a seizure in the last six months is ineligible to operate a motor vehicle.

Lesson 3: Knowledge Check – Summative Assessment Quiz for entire Module, plus an acknowledgment that as a newly diagnosed seizure patient, they will be unable to drive until they have been seizure free for at least six months.

Summative Assessment: Scenario Based Learning Exercise This exercise is a branching scenario that walks the learner through a day in the life of another patient who was also recently diagnosed with GTCS and prompts them to make decisions based on the lessons they just completed and provides feedback as to whether or not their choices would reduce or increase the risk of Maria experiencing another seizure.

Additional Developer Notes Things to Remember: Sustainability is crucial in the design of this intervention at every level. Whether by using modular components for video, text, or eLearning content the project budget includes resources for making certain that the final deliverables will all maximize the interoperability of the components in order to minimize the time required to create subsequent courses across the four medical specialties in the practice.

The tone of the intervention should be empowering while also aware of the limitations of current treatments to fully control the symptoms of all seizure patients. With diligence in following their medication regiment instructions, and any other instructions from their physician – the patient is creating the most optimal conditions for their having minimal seizure activity for their particular diagnosis.

The client should be presented as a reliable resource, a guide to help the patient navigate a confusing and distressing experience that can bring tremendous changes to the patient’s life and the lives of their families. This intervention is learner/patient centered, and aims to equip the patient with the knowledge they need to be as active a participant in their care as possible.

Also, while perfect recall of the details of every word we capture in the multimedia assets of this program, the more realistic and optimal outcome is for the patient to learn where they should and where they should not go to look for information about their condition.

Misinformation in the guise of science is a major contributor to the performance challenges observed in the performance assessment of the practice.

Modules will need to be much more diagnosis-specific than a broad category for epilepsy – and any other condition that will eventually be included in the program once completed.

Many modules will significantly overlap features and content. Rather than obtaining redundant audio or video for the intervention with each diagnosis, we will develop modular sections of content that will be reusable across the entire range of courses.

Update Storyboards and Learning Objectives to reflect the new course focus – Generalized Tonic-Clonic Seizures (GTCS).

All Storyboards, Scripts for Content, and file storage structures will reflect the same coding system for identifying and implementing the modular content structure.

Write Scripts Coded for Modular Assembly from existing Audio or Video Assets with editing instructions.
