ENROLLING IN BENEFITS SELF SERVICE ENROLLMENT THROUGH UKG Before you enroll in your 2022 benefits, keep in mind that your eligibility period is the only time, outside of a Qualifying Life Event (QLE), that you’ll be eligible to enroll in SCS benefits outside of the annual Open Enrollment period. To the right, you’ll see examples of QLEs. Enrollment must happen within 30 days of a Qualifying Life Event.

Your username will be “SCSIN” plus your 5 digit employee ID number. This is the same employee number you currently have.

For example, if your employee ID number is 12345, then your username would be SCSIN12345.

Enter your MMDDYYYY of your birthday as your default password. You will then be prompted to update/change to a new password.

Select Sign In to access the UKG Pro portal.

After logging in, you will be asked to verify your identity via multi-factor authentication. Select the E-Mail option to have your code emailed to you.

You will be sent an email with a numeric code to enter on the next screen. Enter the code, and click the “Do not ask me for codes again on this computer for the next 30 days” to avoid needing to perform multifactor authentication from that computer for 30 days.

HELPFUL ENROLLMENT REMINDERS

QUALIFYING LIFE EVENTS As a new hire, you’re eligible for a mid-year enrollment. New employment, marriage, and having a baby are all examples of qualifying life events. Before selecting your benefits, you will need to navigate to ‘Life Events’ on the left to population information about your dependents. Once you select ‘New Hire’ as your Qualifying Life Events, you can click ‘Next’ to continue.

BENEFICIARIES and DEPENDENTS Before you’re able to elect dependents for your insurance plans, you must navigate to ‘Verify Beneficiary And Dependent Information’ on the left and update your beneficiaries. You will not be able to add dependents to your plans without completing this step. You will also need to include their Social Security Number and Birth Date, AND check the box next to their name. This is an important step!

You will have to actively enroll or waive each benefit and insurance plan to complete your enrollment.

SCS BENEFIT ELIGIBILITY DO YOU KNOW WHO YOU CAN COVER? FOR SPOUSE:

EMPLOYEE ELIGIBILITY

A copy of your marriage certificate AND one of the following: • A copy of the front page of your 2020 federal tax return showing you filed as married with your spouse • A document dated within the last 60 days showing current relationship status such as a recurring monthly household bill or statement of account. The document must list your spouse’s name, the date and your mailing address. Healthcare bills will not be accepted as proof of eligibility as healthcare coverage is being verified.

All SCS employees who are full-time and scheduled to work at least 30 hours per week are eligible to participate in all of the plans listed in this guide, unless otherwise noted under a specific benefit. New hires that are full-time and are scheduled to work at least 30 hours per week are eligible for coverage the 1st of the month following the date of hire.

DEPENDENT ELIGIBILITY If you are a benefits-eligible employee, you may also enroll your eligible dependents. As a reminder, eligible dependents are defined in your benefits summary as: • Your legal spouse • Your qualifying Domestic Partner • Your child up to age 26 (Coverage may be extended to an unmarried child of any age who is physically or mentally disabled and are incapable of self-support due to the disability.)

• A child is defined as your biological child, stepchild or child of your domestic partner, legally adopted child or child placed with you for adoption, a child for whom you or your spouse has been appointed the legal guardian or custodian, or a child for whom you are required to provide health insurance by a Qualified Medical Child Support Order.

FOR CHILDREN UP TO AGE 26 AND DISABLED CHILDREN: • A copy of the child’s birth certificate or adoption certificate naming you or your spouse/partner* as the child’s parent. Please note the document must list the first and last names of the child and parent(s). OR • A copy of the court order naming you or your spouse/ partner* as the child’s legal guardian or custodian. *Note for a stepchild: If you are covering a stepchild or child of your domestic partner you must also provide documentation of your current relationship to your spouse or domestic partner as requested above

• Specialty Coating Systems’ Declaration of Domestic Partnership, AND • TWO Documents* of joint interdependency: • One dated within the last 60 days • One dated at least 6 months prior to this letter’s mail date *Acceptable documents include: a joint bank account, credit account; joint lease, mortgage or deed; joint auto insurance; life insurance, retirement account or will listing your partner as the beneficiary (or your partner’s life insurance, retirement account or will listing you as the beneficiary); document designating your or your partner as the other’s durable power of attorney.

New 2022 Dependents MUST submit the REQUIRED DOCUMENTS to Human Resources. This helps ensure that the medical plans are compliant with applicable plan and regulatory requirements.

All required documents MUST contain the date (including year), employee’s name, and dependent’s name. Personal information such as social security numbers, account numbers, and financial information may be marked out for confidential purposes.

Log in to the member website at myhealth.bankofamerica.com or through your employer’s benefit site.

Manage your account, view your contributions and payment history, and securely store your receipts online.

• Go to the profile tab to verify your email, mailing address and phone number so we can reach you if needed. If your account is through your employer, updates to your mailing address may need to be made through your benefits department.

• Set up your direct deposit information. After you’ve linked your bank account, you’ll receive distributions electronically without the hassle of paper checks, and you also can easily make contributions to your HSA. • Enroll in investments.

• Designate your beneficiaries and dependents.

• Request a debit card for your dependents. You’ll need to have their dates of birth and social security numbers handy.

• Complete your Authorization Form to allow others to contact Customer Care or make transactions on your account.

WHAT IS A FLEXIBLE SPENDING ACCOUNT? A Flexible Spending Account (FSA) is a type of account, provided by your employer, that allows you to put aside money to pay for eligible healthcare and dependent daycare expenses. The money going into the account is tax-free (no payroll tax is withheld) which can save you money on items and services you are already purchasing.

By enrolling in a flexible spending account, you may save up to 40% on your healthcare and dependent daycare costs.

WHAT ARE THE DIFFERENT TYPES OF FLEXIBLE SPENDING ACCOUNTS? A healthcare FSA is designated for eligible medical expenses such as copays, coinsurance, deductibles along with things like prescriptions, medical supplies and procedures.

A limited purpose FSA is designated for eligible dental and vision expenses only. Since you can’t have a healthcare FSA if you are contributing to a health savings account (HSA), you can have a limited purpose FSA to spend on only vision and dental expenses and have less money coming out of your HSA. A dependent daycare FSA is designated for expenses incurred to care for your children age 12 and younger, as well as your adult tax dependents who are unable to care for themselves, while you are at work.

WHAT ARE FLEXIBLE SPENDING ACCOUNT ELIGIBLE EXPENSES? Since the IRS is allowing you to put money in your FSA before it is taxed, they determine what expenses are eligible for reimbursement from the account. Reimbursements are only issued for eligible expenses incurred by you, your spouse or your eligible tax dependents and must be incurred during the plan year.

HOW DO I ACCESS MY FLEXIBLE SPENDING ACCOUNT FUNDS? You can always request a distribution by filing a claim, but the Chard Snyder Benefit Card provides an easy, simple way to use your FSA funds to pay for eligible items and services. It works just like a debit card, but because of smart technology, it can only be used to pay for expenses that are eligible according to the IRS guidelines.

The Benefit Card eliminates the need to pay out-of-pocket, submit a claim form and wait for your reimbursement. Simply swipe your card at the doctor’s office, the dentist’s office or at the pharmacy, for example, and the funds are automatically deducted from your flexible spending account.

Fluoride treatments are payable twice per calendar year for people up to age 19.

Bitewing X-rays are payable once per calendar year and full mouth X-rays (which include bitewing X-rays) are payable once in any five-year period.

Sealants are payable once per tooth per lifetime for occlusal of first permanent molars up to age nine and second permanent molars up to age 14. The surface must be free from decay and restorations.

BASIC LIFE AND AD&D Specialty Coating Systems provides Basic Life and Accidental Death and Dismemberment (AD&D) insurance in the amount of 2 times your salary up to a maximum of $600,000 at no cost to its employees.

This benefit will terminate when your employment terminates or upon retirement, and benefits will reduce according to age.

Employees and spouses who are currently enrolled can maintain their existing coverage amount. 401(K) PLAN ADMINISTERED THROUGH FIDELITY There are many benefits of the Specialty Coating Systems 401(k) Plan. One of the primary benefits is that you will receive help reaching your financial goals for retirement. By reviewing the important information in the guide and visiting Fidelity NetBenefits at www.401k.com, you can take advantage of what your company and Fidelity have to offer.

ELIGIBILITY You will become eligible to participate in the Plan after 3 months of service, and enter the Plan on the first day of the month.

SAFE HARBOR MATCHING CONTRIBUTIONS SCS has elected to make matching contributions to all eligible Participants in an amount equal to 100% of the first 6% of your eligible compensation, contributed to the Plan as Deferral Contributions.

AUTOMATIC ENROLLMENT FEATURE If you are subject to automatic enrollment, you will be notified approximately 30 days prior to when SCS will begin to automatically deduct 3.00% from your pay on a pre-tax basis as a Deferral Contribution for you to the Plan. You may stop or change this automatic contribution by following the instructions provided in the notice.

WHY TIME IS MONEY!

ANNUAL INCREASE PROGRAM Annual increase will gradually raise your contribution rate each year. If you have not established an annual increase program for yourself, SCS will automatically increase your contributions annually by 1% until your contributions reach a maximum of 6.00%. You may stop or change this automatic increase by accessing your Fidelity account.

HOW MUCH CAN I CONTRIBUTE? Through automatic payroll deduction, you may contribute between 1% and 100% of your eligible pay on a pretax basis, up to the annual IRS dollar limit. You may change your deferral percentage as applicable at the beginning of each payroll period.

The Roth 401(k) contribution option is available to you. A Roth 401(k) contribution to your retirement plan allows you to make after-tax contributions and take any associated earnings completely tax free at retirement.
