Fortunately, you have UMR GenerationYou CARE Support on your side to help you understand all your medical care options.

GenerationYou CARE Support is a personalized experience with a focus on your health and the health of the family, resulting in a more positive experience for you.

UMR CARE Support has a team of experienced and caring nurses (RNs), social workers, pharmacists and nurse practitioners.

This team works together to provide members and their families with medical support and assistance in navigating their benefts and the entire health care system. By working with you, your doctors and other medical advisors, it ensures you will get the services that best meet your needs.

Our new approach allows our CARE nurse managers and CARE Guides to get involved earlier and with more members, which leads to better and closer relationships. Our CARE Team provides support and resources for members across the full spectrum of needs, focusing on members who are at-risk and high-risk. This program will provide outreach to members, family support and a whole person approach to support.

Helpful support in any situation Our CARE team can assist with a wide variety of services, including helping with lack of transportation; connection to providers due to a lack of options/services in certain areas or counties; and access to community resources such as counseling, food banks, housing shelters and more.

Whether you’re having a baby, have an emergency hospitalization or need non-emergency care, our CARE team is here for you.

Here for you in times of crisis Hopefully, you or a family member never experience a serious injury or long-term illness.

But if you do, UMR CARE nurses will be on the case, at no cost to you. Our CARE Support team of CARE nurse managers and CARE Guides will assist with your medical care and treatment by: Communicating directly with patients and/or families and their providers to address specifc medical or psychological needs Determining needs and requirements of the member Establishing collaborative long-term and short-term goals

Assisting the member and family to understand what to expect during the course of treatment Mobilizing appropriate resources for patient care over the course of the patient’s illness and recovery Interfacing efectively with providers Consulting with health care professionals qualifed in specialized care Negotiating care in an efort to reduce claim costs Identifying potential cost-efective alternatives for care while optimizing quality care Assisting and overcoming any barriers to care

Our CARE Support Team The CARE Support team is comprised of CARE nurse managers and CARE Guides.

Our CARE nurse managers are registered nurses and Certifed Case Manager (CCM) credentialed. They are required to have a minimum three years of acute-care clinical experience and expertise in one or more of the following areas: Critical care l Medical/surgical l Neonatology/ obstetrics l Oncology l Organ transplant Rehabilitation/trauma l Mental health/ substance abuse To better address member needs, UMR has created specialty teams for transplants, oncology, neonatal intensive care, hospitals and behavioral health. These subteams are comprised of registered nurses with extensive clinical practice and expertise in their respective areas.

The CARE Guides consist of both registered nurses and social workers. Our CARE Guides have many years of experience spanning multiple specialties including general surgery, behavioral health /substance use disorder, critical care and pediatrics. Our CARE guides assist members who do not need the long-term clinical or treatment coordination. If the patient is stable and support is in place, the nurse closes the case after one call (with the option of one follow-up call). However, if the patient is in crisis, the CARE Guide will refer the member to a CARE nurse manager.
