Physician Recommends A Specific Service
In order to be a candidate for Nomi Health, there must be valid medical records and a referral order the specific service needed (lab, imaging, physical therapy, surgery, etc.).
Chat With A Care Guide
As soon as you know the medical service needed, call or email Nomi Health to speak to one of our Care Guides. Your Care Guide will find the nearest option for your specific need and determine any out-of-pocket costs.*
Schedule With Physician/Facility
Once we locate a Nomi Health Physician/Facility for the service needed, the Physician/Facility will reach out to schedule your appointment. If pre-authorization is required, a Care Guide will work to get all necessary approvals.
Receive A Voucher
Before your appointment, a Care Guide will provide you with a voucher to present to the front o ce on the day of your visit. This voucher will replace the need to show your major medical ID card.
*If you are on an HSA qualified HDHP health plan, an individual/family must meet the IRS minimum prior to receiving services through Nomi Health at no cost. IRS minimum requirements are based on an annual basis. IRS mandated minimums for 2023 are $1,500 individual/$3,000 family.
