From acute care and preventative services to long-term health management, the HSBT Health and Wellness Centers has your best interest at heart. Services are provided by Community Health Network providers and medical staff who can evaluate and treat you for a wide variety of ailments.

Is my personal health information protected?

Yes, Community Health Network follows HIPAA Privacy Rules for protecting your personal health information.

What is the cost?

There is no cost to be seen at the clinics.* *Based on your insurance, there may be fees charged by outside vendors for blood work sent to an outside lab.

Is Prescription Medications available?

Select generic medications and refills are available for FREE.

How do I make an appointment?

Call 317.497.6140 to schedule an appointment.

Walk-in appointments are not available.

Do I need to wear a mask to my appointment?

Patients must wear a surgical mask to be seen for an appointment. If you do not have a surgical mask, please wear a cloth mask to enter the clinic and a surgical mask will be provided for you to wear for your appointment.

All clinic staff will be required to wear surgical masks as well.
