TRENDS + OUR CHALLENGE AHEAD Nantucket is an island located 32 miles off the coast of Cape Cod, Massachusetts, with a relatively small year-round population and a substantial seasonal influx of visitors. While the island’s permanent population is estimated at approximately 14,000 residents, peak summer days can see the population swell to more than 60,000 people. Annual visitor activity is significant, with estimates ranging from hundreds of thousands of trips to as many as two million visits each year, according to the Nantucket Data Platform. Given this dynamic population flow, Nantucket is deeply interconnected with broader national and global trends. Visitors, part-time residents, seasonal workers, and year-round residents travel to and from the island from across the United States and around the world. As a result, patterns of substance use and misuse that emerge at regional, national, or global levels have the potential to be introduced to, and influence, the local community. Understanding these broader trends is therefore essential to anticipating risks, informing prevention strategies, and protecting the health and well-being of the Nantucket community. EMERGING AND SYNTHETIC DRUG TRENDS • Novel synthetic opioids: In addition to fentanyl, highly potent and dangerous synthetic opioids known as nitazenes continue to spread across the country. They are often mixed with other drugs without the user’s knowledge, and some are so powerful that multiple doses of naloxone are needed to reverse an overdose. • Animal tranquilizers: The veterinary sedative xylazine remains the top adulterant found in the illicit drug supply, especially in fentanyl. Another, more powerful animal tranquilizer called medetomidine has also emerged as an adulterant. These sedatives increase overdose risk and can cause severe skin ulcers. • Polydrug use: The practice of mixing illicit substances, or “drug cocktails,” is becoming more common. For example, forensic labs are finding fentanyl in both cocaine and methamphetamine, dramatically increasing the risk of a fatal overdose for users of stimulant drugs. • Cannabis potency: As regulations evolve, Chinese transnational criminal organizations are increasingly producing high-potency marijuana for the U.S. market, with THC content averaging 25–30%. GLOBAL & UNITED STATES SUBSTANCE USE PATTERNS AND POPULATIONS 2025 • Alcohol consumption decline: In 2025, a Gallup poll reported the lowest rate of alcohol consumption among U.S. adults in 90 years. This shift is particularly evident among Gen Z, who are driving a cultural trend toward “mindful drinking” and lower- or no-alcohol beverages. • Growing screen and gaming addiction: Technology addiction has emerged as a global public health concern. As of 2025: • 18% of global internet users meet the criteria for internet addiction. • 63% of Gen Z users in the U.S. spend over six hours a day on social media. • Young adults face the highest risk of internet addiction. • Blurring of reality through AI: The integration of AI into daily life creates new addiction risks. People are forming deep emotional connections with AI companions, while AI-generated deepfakes can distort reality and encourage escapism into manipulated digital worlds.

ASN ANNUAL REPORT - 2025
NANTUCKET SUBSTANCE USE PATTERNS AND POPULATIONS 2025 Addiction trends in Nantucket show a shift toward higher-than-average cocaine use, which contrasts with statewide efforts to combat the opioid crisis. Wastewater surveillance data is now the primary tool used by local officials to monitor substance use on the island and inform intervention strategies. • Key findings from Recent Wastewater Testing: Initial results from Nantucket’s 2025 wastewater surveillance program offer the most up-to-date look at drug use on the island: • High cocaine levels: For several weeks during the summer of 2025, cocaine levels in wastewater were more than 50% higher than the national average. • Low fentanyl and other synthetic opioid levels: In contrast to the high cocaine levels, fentanyl, methamphetamine, and xylazine were found at levels below national averages. Opioid and Overdose Concerns Historically, opioids have been a significant concern on the island, and local officials continue to monitor the situation carefully. • Potential undercounting: While statewide numbers showed a drop in opioid-related overdose deaths in 2023, local Nantucket officials are concerned that the island’s lowvolume data may not reflect the true picture. • Fentanyl risk: Local healthcare professionals note that even with low reported levels, the presence of fentanyl is dangerous because it can be mixed into other substances, potentially causing deadly effects. • Increased Narcan availability: In response to overdose concerns, the Nantucket Police Department ensures every sworn officer carries Narcan, a life-saving medication that can reverse an opioid overdose.

ADDICTION SOLUTIONS CLINIC In our experience helping those suffering from addiction, we have witnessed true recovery. Dr.Timothy Lepore and our staff have seen men and women of all ages come back from the brink and rejoin their friends and family as productive, clean, sober survivors.
Addiction Solutions is a Medication Assisted Treatment (MAT) program. ASN uses many medications to help individuals stop using illicit drugs or alcohol by substituting prescription medication in a controlled manner to prevent the discomfort of withdrawal, on-going urges to use drugs or drink, and for maintenance therapy. Some of the key medicines include:
SUBOXONE (Buprenorphine/Naloxone) A medicine used to control opioid withdrawal, illicit opioid use, and maintenance therapy. Suboxone blocks the effects of getting high, and pain control.
SUBUTEX (Buprenorphine) A medicine that is a different formulation, because the Naloxone is not included. Subutex is used for individuals who may be facing surgical procedures or who may be pregnant. Subutex can be used with other medication for pain control.
NALTREXONE A medication used to treat both opioid or alcohol use disorders by helping to block the effects alcohol or opioids, helping control the urge to use, as well as preventing intoxication.
VIVITROL (Naltrexone) This is an injectable form of Naltrexone that is given in a shot every twenty-eight days. Vivitrol works to control the urge to drink or use opioid drugs, blocks effects of alcohol or opioids, and helps to prevent intoxication.
ANTABUSE (Disulfiram)
This medicine is effective in controlling the urge to drink by blocking an enzyme involved in breaking down alcohol. Antabuse produces very unpleasant side effects when it is combined with alcohol.
CAMPRAL (Acamprosate) This medicine is effective in controlling the urge to drink Alcohol by stabilizing certain brain chemicals.

Race for Recovery Photos by Bill Hoenk for Nantucket About Town
RACE FOR RECOVERY Addiction Solutions of Nantucket held its annual Race for Recovery on September 28, 2025, bringing together over 300 runners and walkers—with kids running free— to raise awareness and support for community members facing addiction. With the help of our donors, volunteers, and sponsors, the event raised over $97,000. These funds will directly support the clinic with additional medical staff and help offset rising medication costs, allowing us to expand care for the growing number of patients seeking recovery services. Thank you to everyone who participated and contributed. Your support strengthens our mission and helps create meaningful pathways to recovery for individuals and families across Nantucket.

EDUCATION & OUTREACH
Top Image: Race for Recovery Adl. Lower Image: Reels promoting our website translation abilty in Spanish, Portuguese, and Chinese.
The following films, Public Service Announcements (PSAs) and resources are made available from Addiction Solutions of Nantucket. The films and website were designed to 1) serve discussion groups led by teachers, mental health councilors, and anyone willing to facilitate a discussion about this topic, and 2) appeal to anyone struggling with addiction letting them know we are here to help them. As Dr. Timothy Lepore, President of ASN, states, “Awareness and taking action to seek help are the first important steps to battling Substance Use Disorders (SUD). The Boy Next Door film and the Q&A session after the film are intended to raise understanding of addiction as an illness rather than a weakness. In addition, it should enlighten viewers that addiction does not discriminate. With 1 in 14 Americans touched by addiction, we cannot look the other way.”
The Boy Next Door Nothing impacts us more than when tragic events happen in our community. This powerful and poignant 13-minute film takes the heartbreaking story of addiction and its impact and turns it to hope that breaking the negative stigma surrounding addiction can bring. The Q&A film is informative and sheds a light on the challenges that the Nantucket Community faces. Click on the images above to watch the films.

These PSAs are part the outreach efforts to parents, family, friends, and co-workers in learning more about substance use and engage viewers in learning about the stigma of drugs/alcohol addiction and the help that is available.
Radio Script It’s no secret that drugs and alcohol can get out of control during the summer season on Nantucket. The added stress of working multiple jobs during the busy season or summer residence escaping from the reality of life on the mainland can create a dangerous environment for individuals in recovery—or become a slippery slope to those on the borderline of addiction or overdose. In light of these harsh reality trends, it’s essential to bring attention to the services offered by organizations like Addiction Solutions of Nantucket, offering a wide range of services to help individuals facing substance abuse disorders, including Medication Assisted Treatment Programs, networks for counseling, support groups, and rehabilitation programs. We encourage you to make a meaningful difference in someone’s life. This summer season, let’s remember that our awareness and empathy can be the greatest support to those in need. Help break addiction stigma. Together, we can positively impact and bring hope to those who need it most. Have a great summer, and remember, we are here if you need us. Save the date for Race for Recovery on Sunday, September 29! Registration is now open. Details at addicitonsolutionsnantucket.org.

Future Expansion We will continue to support our medical team by expanding staffing and service hours as needed. Our staff is actively building a network of appropriate community partners, which is essential to extending the reach of our services and enabling direct, face-to-face engagement with community members. This personal approach is critical to connecting with individuals who are most in need of our healthcare services.
2026 GOALS
02
Education & Outreach Over 8,000 individuals—90% of them on Nantucket—have meaningfully engaged with our website and social channels. Yet our patient base has remained at roughly 1,000 residents compounded annually. This gap reflects the sensitive nature of seeking recovery and the essential role of personal connection with trusted medical professionals. Our outreach and education efforts will adapt and utlize newer technologies to provide privacy as they consume information and meet individuals where they are through one-on-one engagement, community presence, interactive technology and targeted multilingual communication outreach initiaves.
03
Building for the Future To ensure the long-term strength and sustainability of our mission, we will be establishing an endowment. This permanent fund will provide a stable source of support for our core programs and services, helping to safeguard our work through economic changes and evolving community needs. By investing in the future today, we are strengthening our ability to deliver consistent, high-quality prevention, treatment, and recovery support for the Nantucket community for generations to come.
Where there is help, there is hope.
