This elevation represents a predisposition to diabetes that can often be prevented with lifestyle changes and sometimes with medications. Technically, pre-diabetes is diagnosed if an adult has a fasting blood glucose between 100 and 125, an A1c measure between 5.7 to 6.4 percent or a glucose tolerance test (GTT) result between 140 and 199, and a body mass index (BMI) greater than 24 (22 if Asian).

Research by the Centers for Disease Control and Prevention (CDC) have demonstrated that people with pre-diabetes who participate in structured lifestyle change programs can “reduce their risk of developing type 2 diabetes by 58 percent”. MGH&FC’s Diabetes Prevention Program (DPP) provides attendees in a group setting with the tools needed to make adjustments in their daily lifestyles, including healthy eating, physical activity, reducing stress and improving coping skills. The program curriculum is designed by the CDC and facilitated by Duke Vivian, a certified diabetes prevention coach.

This program is different from conventional diabetes education because it runs for a full

year and utilizes experience-based learning in a confidential, small-group setting facilitated by a coach. Each participant is supported to make and sustain personal lifestyle changes, including a healthy body weight lowered by 7 percent, and exercise increased to a minimum of 150 minutes per week.

Stroke Prevention Clinic helps reduce risk of full stroke When dealing with the aftermath of a Transient Ischemic Attack (TIA), also called a mini-stroke, patients need to understand what has taken place in their brain and learn how to prevent a full stroke, according to Liz Hamel, R.N., B.S.N., Trauma and Stroke Program coordinator at MGH&FC’s Stroke Prevention Clinic.

“A TIA is a significant warning sign for the later development of a full stroke. Our goal is to have the TIA patient quickly evaluated by a neurologist, to provide education about what has happened in his or her brain, and to help the patient immediately reduce their risk for another stroke, wherever possible. The highest risk of developing a full stroke is within the first 90 days of a TIA,” said Hamel.

When a TIA patient is identified in MGH&FC’s Emergency Department, the patient is referred to the Hospital’s Stroke Prevention Clinic. Patients are generally seen in the Clinic within 3 to 7 days of the TIA. This visit consists of a neurological assessment and a TeleStroke visit with a neurologist in Olympia. The TeleStroke session allows the neurologist to see and speak with the patient, conduct a physical exam, order tests to find the cause of the patient’s TIA, and assess the patient’s risk based on those findings. The patient may also be referred to MGH&FC’s diabetes educator, or to physical/occupational therapy to assist in managing the resulting physical deficits and risk factors.

Education is one of the most important functions of the Stroke Prevention Clinic.

During the appointment, patients are provided with education about the brain, what happens when a TIA or stroke occurs, how it can affect the brain and how bodily functions can be disrupted by a TIA or stroke. Education is focused on identifying and modifying the Risk Factors for Stroke, which include: History of atrial fibrillation Smoking High blood pressure Heavy alcohol or recreational drug use High cholesterol

Diabetes Lack of exercise High sodium, high fat diet High stress lifestyle Poor medication management Improved management of these risk factors is key to preventing the development of Stroke.

At the TeleStroke clinic, the neurologist may order additional tests including MRI, Holter monitoring, echocardiogram, etc. Follow-up to this testing may be with the Continued on page 10...

Act FAST, time is brain!

Symptoms of stroke and TIA are similar, and can be identified by the acronym FAST:

F - Face.

A drooping or uneven smile on someone’s face is a warning sign.

A - Arms. Numbness or weakness in the arms can be a warning sign.

Ask the person to raise their arms. If an arm drops down or is unsteady, this is a warning sign.

S - Slurred speech. Ask the person to speak. If their speech is slurred, this is a warning sign.

T - Time. Time is brain! If someone is experiencing these symptoms, call 911 immediately. The sooner they receive help, the better their chances are of having no permanent brain damage.

Overprescribed antibiotics weakens their effectiveness

Employee Wellness Committee organizes “Road Trip! Discover your National Parks”

In 1928, the theory of antibiotic use to fight deadly infections in human beings first emerged, however, the actual use of penicillin as an antibiotic was not fully realized until 1941. (www.acs.org/flemngpenicillin).

This breakthrough cure for infections was the first in a long line of antibiotics that have been developed over time. The need for new and stronger antibiotics became apparent as physicians discovered people can develop resistance to previously used antibiotics, as well as adverse reactions. This means that no single antibiotic is effective for all people, or for all time.

Eventually, antibiotic use increased to treat many non-bacterial infections such as severe colds, and as a result, a threat to the curative power of antibiotics has developed.

MGH&FC’s Employee Wellness Committee, under the leadership of Sarah Fulkerson, CD, RD, CDE, organized a wellness walk for the Hospital District and Shelton School District. The event ran six weeks and had 29 teams consisting of 8 to 15 members each. Nearly 300 people participated from both MGH&FC and Shelton School District for the 2,700-mile event.

The rapid emergence of resistant bacteria is occurring worldwide, endangering the efficacy of antibiotics, which have transformed medicine and saved millions of lives.

The event, called “Road Trip! Discover your National Parks,” took participants on a virtual tour of six national parks, from Mount Rainier to Glacier, Yellowstone, Yosemite, Redwoods, Crater Lake, and back home to Mount Rainier. Participants wore fitness trackers and teams reported their physical activity progress each week.

In recognition of this growing resistance, patients should work with their health care provider to limit the prescribing of antibiotics.

Working together, they can come up with more effective ways to treat the condition, i.e. irrigation of sinuses for a few days to eliminate any infection vs. prescribing anti-biotics. By limiting antibiotic use to conditions that are not likely to heal on their own, physicians can help prevent antibiotic resistance from increasing.

Patients can help by not requesting antibiotics for some conditions they may have had prescriptions for in the past, such as a sinus infection. As patients and physicians work together to limit antibiotic use, it may be possible to preserve the life-saving qualities antibiotics were intended for.

Employee Teresa Holcomb gives back by volunteering at Saints Pantry

Holcomb has also taken on the task of keeping her co-workers and supervisor involved with Saints Pantry. “After the MGH&FC challenge ended, I picked up the ball and got my co-workers to continue donating food and helping in other ways. The Hospital has supported me by working around my schedule at Saints Pantry. My boss and co-workers have donated a lot of food, and they listen to me talk at meetings to let them know what the current needs are at Saints Pantry,” said Holcomb.

She explained that summer is a low donation time for Saints Pantry because people are out enjoying the weather and donating is not always on their minds.

Saints Pantry’s Assistant Director Patty Russell and volunteer Teresa Holcomb display some of the many donated items handed out twice weekly in Shelton.

When MGH&FC began an initiative to help employees find a place to give back to their community through volunteering, Registration Specialist Teresa Holcomb was drawn to Saints Pantry Food Bank in Shelton.

She began volunteering in February by helping to organize supplies and fill food boxes. Since that time, her duties have expanded twice.

Now she operates a satellite food bank at the Shorecrest community clubhouse on Saturday mornings and helps manage a summer feeding program there for students whose parents can’t always provide daytime meals at home.

She explained the philosophy at Saints Pantry is to not look down on those who need services. “No one judges you. It doesn’t matter if you are poorly dressed, unwashed, or whatever – you are a person – a human being worthy of dignity and respect,” she said.

“Saints Pantry always needs to be rebuilding its food stock, even after a major food drive,” said Holcomb, adding that other needs include toilet paper and personal hygiene supplies.

“The problem isn’t going to go away, so we need people to continue donating food, especially in the summer,” she said.

For Holcomb, volunteering has become a way to honor her father, who stepped in and raised her after her mother left. “My dad always said, ‘Put your hand down and pick someone else up.’” Giving back to the community by volunteering at Saints Pantry is her way to do just that.

Healthy food drive benefits Matlock Food Bank

Holcomb said her interest in Saints Pantry stems from her own childhood struggles.

“When I was growing up, my mom abandoned my sisters and me, and for three months we were on our own. We had to figure out how to feed ourselves, so I have a connection with those who are hungry and need help.”

She said what she likes most about her work at Saints Pantry is that people who come in really need the food and are appreciative of all they receive.

“Everyone is so grateful, and they don’t take more than they need. If they are offered something they already have enough of, they will say, ‘I’m good; save that for someone else

“Living Well With Diabetes” class in November

Early in July, MGH&FC was named a “Most Wired Hospital” for 2017. This is the tenth straight year MGH&FC has received the prestigious designation from the American Hospital Association’s (AHA) Health Forum. The Most Wired Survey, conducted between January 15 and March 15, 2017, was recently reported in Hospitals and Health Networks, a publication of the AHA. MGH&FC’s own Chief Information Officer Tom Hornburg was on the cover pictured with a Manke Lumber truck. The Most Wired Survey and benchmarking study is a leading industry barometer measuring information technology (IT) use and adoption among hospitals, nationwide. The 2017 survey represented an estimated 2,158 hospitals – more than 39 percent of all hospitals in the U.S.

New Navigator Marcia Stauffer is the new Patient Navigator in MGH&FC’s Care Coordination Services. She has been with MGH&FC since October 2012, working first at MGH Eye Clinic and then in the Anticoagulation Clinic. Marcia Marcia Stauffer Patient Navigator received her Bachelor of Arts degree from the University of Arizona, Tucson.

She helps women navigate the many steps in their diagnosis and treat-ment of breast cancer, connecting them with the many resources available to them in the local medical community.

Support, encouragement and informal education is provided to anyone in the community and meetings are held in two locations, free of charge. MGH&FC Diabetes Support Groups are affiliated with the American Diabetes Association and meet every third Thursday in Shelton or every fourth Thursday in Allyn from September to June.

Marissa Hubble is a MGH&FC employee and two-time previous recipient of the Auxiliary’s Renewable Scholarship in the amount of $2,000. She is currently enrolled as an online nursing student at Indiana State University. Marissa will complete her degree at the end of 2017 as a registered nurse and plans to continue her employment at MGH&FC.

Mason General Hospital Foundation’s 23rd Annual ‘Quality Care Open’ Golf Tournament held in June at Alderbrook Golf & Yacht Club in Union was another huge success! More than $96,000 in cash and in-kind contributions were raised.

Funds raised at the 2017 Tournament will go toward the purchase of eight Zoll R Series ACLS defibrillators to be placed in five locations at MGH&FC – Medical/ Surgical/Pediatrics, Emergency Department, ICU, Surgery Department, and the Treadmill Room.

Hamel went on to explain, “We have many people come to the ED or their doctor’s office days after their TIA or stroke symptoms appeared, which limits the interventions we can take to stop the development of a full-blown stroke. We urge patients to come to the ED as soon as TIA or stroke symptoms are noticed. Patients are urged to call 911 instead of having someone drive them to the ED, to reduce the risk of the patient driving or riding in a car.

Patients are safer in the care of EMS providers who are well trained in TIA and stroke care.

Gayle Weston, Hospital commissioner; Whitney Jones, MGHF president; Tanya Frazier, MGHF vice president and Fantasy Forest chair; Tom DiDonna, MGH&FC director of education; and Sherry Curtis, MGH&FC education assistant hold a “mock check” representing funds donated to MGH&FC by the 2016 Fantasy Forest Committee.

Proceeds in the amount of $64,308.47 were raised during the 2016 Fantasy Forest fundraising event held at the beautiful Alderbrook Resort & Spa in Union. The money will be used to purchase Zoll R Series defibrillators in addition to proceeds from the MGH&FC 2017 Quality Care Open Golf Tournament.
