quality Care Coordination helps to keep Mistelle out of the hospital After struggling with Type One Diabetes

for more than 30 years, Shelton resident Mistelle Huskey, with the help of the Care Coordination Team at Mason General Hospital & Family of Clinics (MGH&FC), has a new tool to help manage her diabetes.

At the tender age of only nine years old, Huskey experienced a near-fatal diabetic coma. “The physician who saw me during my coma was diabetic and had a son who had experienced a diabetic coma, so he recognized it for what it was. He said that if I made it through the night, I had a 50/50 chance of survival,” Huskey remembers.

Not only did she live, she grew up, got married, and gave birth to two children. But the road to survival hasn’t been easy. Her appetite is almost non-existent and she has developed other diabetes-related conditions including heart disease and failing kidneys.

In addition, constant trips to the hospital to control her blood sugar and related complications meant that she could be away from her husband and children for as many as twelve days at a time and have as many as 14 hospital admissions in one year.

“I have struggled with diabetes for many years, spending precious time away from my husband and two children,” Huskey added. “The Care Coordination Team really worked together to facilitate the use of a Continuous Glucose Monitoring (CGM) system.

Prior to having the CGM, I was checking my blood sugars 7 to 12 times a day and still not able to keep them within a normal range. My highs were too high and my lows were too low. I now wear the CGM all the time which has an alarm to alert me when I am heading into the danger zones.”

According to Terri Gushee, B.S.N., R.N., director of Care Coordination Services at MGH&FC, a referral was made to her department – Care Coordination Services (CCS) — on behalf

of Huskey to help coordinate gaps in her care.

Care Coordination Services is not a new service line at MGH&FC. The name is new but the resources have always been in place. Care Coordination Services includes Diabetes Wellness Educators, Clinic and Hospital Care Coordinators, Dietitians and Discharge Planners. CCS connects patients with services based on their individual needs such as continued need for rehabilitation services, home health, obtaining needed medical equipment and assisting with referrals to community services such as Area Agency on Aging (AAA) and Community Options Program Entry System (COPES). “Our goal is to put patients at the center of their care. To make this possible we have many talented Care Coordinators that can tap into the many services the Hospital and Clinics offer at bedside such as Pharmacy, Physical Therapy, Speech Therapy, Occupational Therapy and Wound Care. We bring the services to the patient. CCS is a patient– and family– centered team designed to assess and meet the needs of patients, while helping them navigate effectively and efficiently through the health care system,” Gushee explained.

Gushee added that MGH&FC staff members, Sue Barwick, R.N., C.D.E, program director of MGH&FC’s Diabetes Education, and Cindy Mortensen, B.A., program director of Discharge Planning, worked with Huskey to bridge the gaps in her care and to help her obtain the CGM. Sue still has an

ongoing relationship with her.

Huskey commented, “I bring my monitor in to Sue to have my data downloaded and for diabetes education. The monitor measures my blood sugars every five minutes and shows the trend of my blood sugars.

My appetite isn’t what it should be, so I have met with the dietitians to gain insight into what foods trigger changes in blood sugars. Since I have had the monitor I have not been admitted into the hospital for a year. Just being able to be at home with my family and give them spontaneous hugs is so important.”

“Part of Cindy’s job is to look at patients with re-admissions,” said Barwick. “Mistelle was one of those patients. Together, Cindy and I developed a plan to make the goal of lowering hospital admissions for her a reality. We facilitated securing the use of the 24-hour CGM system. This was a new process for Mistelle, but because of her extreme high and low blood sugar counts, we thought it would be very beneficial for her. It measures her blood sugars every five minutes and shows the trend of her blood sugars. New pre-diabetic program in MGH&FC’s Diabetes Wellness Dept.

MGH&FC’s Department of Diabetes Wellness is working to try and reduce cases of Type II Diabetes in Mason County. As part of this effort, a new, year-long program for pre-diabetic patients is being offered beginning Thursday, September 10, 2015, from 5:30 to 6:30 p.m. in the Pershing Room at MGH. Pre-registration is required by calling (360) 427-7332

by Wednesday, September 9. The program runs for a full year, meeting weekly for the first four months, and then monthly. It focuses on diabetes prevention and supporting pre-diabetic patients to be active daily, to eat a healthy diet, and to lose weight.

In Mason County it is estimated there are about 16,500 people with pre-diabetes. Approximately 10,000 are 20 years or older, and 6,500 are 65 years or older. MGH&FC’s program is a proven one that can lower the risk of progression to diabetes by 58 percent in the early years after completion, and by 34 percent ten years after completion.

The cost is only $50 for patients and they will receive annual monitoring of their A1C or glucose levels. For a referral to the program, pre-diabetic patients should contact their primary care physician.

Seniors get in shape for “Freedom From Falls” in October Are you a Senior at risk for losing your independence due to a fall?

Mason General Hospital, in partnership with the Mason County Senior Activities Center, will host its third ‘Freedom From Falls’ event – to promote awareness and prevention of falls. It will take place on Tuesday, October 27 from 9:30 a.m. to 2:30 p.m. at the Pavilion at 190 Sentry Drive in Shelton.

Falls are the biggest factor that threaten the independence of senior citizens – and most of them are preventable. MGH&FC health professionals and guest speakers will present topics that cover falls prevention awareness, medication management, nutritional guidance, and brain fitness. There will also be free blood pressure checks and eye glasses adjustments. MGH’s Physical Therapy department will provide free testing to participants to measure their risk of falling with the Get Up & Go test, Functional Reach, and Tug Test. The Center will also demonstrate fun ways of staying fit with activities such as Tai Chi, gentle Yoga, line dancing and Zumba – and a special performance from the Center’s Spicy Ladies Can-Can group.

At noon, attendees are invited to stay for the Senior Nutrition Lunch, provided by Senior Services for South Sound. A donation of $3 to $6 is suggested (but not required) for the lunch.

Getting rid of your sugar belly Don’t call it a beer belly. That bulging abdomen is more likely a result of eating too much sugar. Americans eat on average 22 to 28 teaspoons of added sugar a day. To get a slimmer waistline and to lower your risk of heart disease, diabetes and cancer, nutritionists recommend no more than 6.5 teaspoons (100 calories) for women and 9.5 teaspoons (150 calories) for men.

For the eighth consecutive year, Mason General Hospital & Family of Clinics has received the prestigious, “Most Wired” hospital award, this year competing with more than 2,000 hospitals surveyed across the nation in information systems capability. The Hospitals & Health Networks (H&HN) Most Wired Survey identified MGH&FC as one of the top hospitals in the country that were given this designation. The Most Wired Survey is conducted annually and publishes its findings and the names of the nation’s Most Wired hospitals and health systems in H&HN magazine, a publication of the Journal of the American Hospital Association.

MGH&FC first received the “Most Wired”-

Rural Hospital award in 2008 and 2009, and then in 2010 it was reclassified to be included in not just rural, but all hospitals, nationwide.

Every year since then – 2010, 2011, 2012, 2013, 2014 and now 2015 – MGH&FC has received the Most Wired Hospital award for information technology (IT) that implements systems to ensure quality and improve efficiency in the delivery of patient care.

That safe place in Mason County is Mason General Hospital, or a staffed, local fire station. “This is an occasional need in our area,” said Marcia Rohlik, BSN, MSN, director of Women’s and Children’s Services at MGH. “No questions are asked. No judgement is passed.

We are here to help mother and baby stay safe and be healthy.”

Immunity from specific criminal liability is given to a parent who transfers a newborn to any MGH employee at the Hospital or personnel at a fire station. The parent must be given the opportunity to provide family medical history anonymously. Within 24 hours the baby is turned over to Child Protective Services. In addition, Hospital or fire station staff and volunteers are immune from criminal or civil liability for accepting a newborn.

Mason General Hospital Foundation’s Annual Quality Care Open Golf Tournament held at the Alderbrook Golf & Yacht Club in June was another terrific success! This year’s tournament hosted a full-field of attendees, with 200 golfers and volunteers. Funds raised this year will support the purchase of medical equipment for Oakland Bay Pediatrics, Olympic Physicians, and the Hospital’s Emergency Department.

Fund’s First Annual ‘Tour de Mason Lake’

Riders, along with Karen Hilburn (front and left-of-center), prepare for the Karen Hilburn Breast & Cervical Cancer Fund’s (KHB&CCF) first annual ‘Tour de Mason Lake’ bicycle ride in Shelton this past June. The course was a scenic 20, 32 or 44-mile circuit around Mason Lake with medium and rolling hills. More than $1,200 was raised for KHB&CCF, a chapter of Mason General Hospital Foundation. All proceeds went to KHB&CCF to help uninsured or underinsured women who live in Mason County with expenses for the diagnoses and treatment of breast and cervical cancers. Event sponsors included: Anytime Fitness, Black Star, Heritage Bank, Life Fitness, McCarty & Associates “a marketing firm”, and Olympic Mountain Ice Cream. Tour de Mason Lake is proposed to be held every second Saturday in June.

New course records set at Goldsborough Creek Run

The Karen Hilburn Breast

& Cervical Cancer Fund (KHB&CCF), a chapter of Mason General Hospital Foundation, was presented – for the fifth year in a row – with a check for $30,000 from the Charitable Trust of Margery Jones, a former Shelton resident and cancer survivor. Miss Jones left her estate to be donated to charitable non-profits. This is the fifth consecutive year the KHB&CCF has been a major recipient. Over the last four years Miss Jones’ estate has donated a total of $120,000. “To-date our Fund has helped more than 180 ladies over the past 12 years. Margery Jones’ donation has helped to make this all possible,” said Founder Karen Hilburn. KHB&CCF helps Mason County women that are underinsured or uninsured with their diagnoses and treatment of breast and cervical cancer. Two new course records were set in the 7-mile run at the annual Centennial Guild Goldsborough Creek Run, a part of Forest Festival, held in May. There were a record 238 participants, across the board in age, abilities, and interests. Some walked, some ran and walked, and some ran the entire course. More than $5,000 was raised to benefit MGH&FC. Centennial Guild is a chapter of MGH Foundation. Title sponsor was Regence BlueShield.

In the male division, Andrew Prentice of Olympia, set a new record of 38:51 for the 7-mile, beating his previous record set in 2008 of 39.22. Sarah Johnson of Shelton, set a new record in the 7-mile female division, with a time of 41:06, beating the previous record of 45:08 set in 2008 by Caroline White of Tacoma.

The course record for the 2-mile run is currently held by Jacob Smith of Wenatchee in the male division, with a course record of 9:41 set in 2010. The female course record for the 2-mile run is 10:46, set in 2012 by local resident, Sarah Johnson. Overall winners for the 2-mile run were 11-year old Aiden Gregg of Shelton, with a time of 13:22 for the female division. For the male division, the winner was 19-year old Darius Burke of Shelton, with a time of 10:50. The Goldsborough Creek Run & Walk is held annually on the Saturday of Forest Festival weekend.

The Scholarship Fund consists of profits from the MGH Gift Shop (which is maintained and staffed by Auxiliary members) and from Memorials made to the Auxiliary. The amount of each Scholarship Award is determined annually by the Fund’s total, and the number of selected recipients. This year’s recipients each received $2,000 for the 2015-16 academic year. community Support for local pool benefits health and education Last spring MGH&FC teamed with the Shelton School District because of the High School pool’s importance to the health of our community. Since then, SPLASH of Mason County (Support & Preserve Lifelong Aquatic Safety & Health), a nonprofit organization, has been working with MGH&FC and the School District in keeping this important facility in our community. To this end, SPLASH has been working to raise money to aid in the cost of capital repairs needed at the Shelton High School pool.

According to the Centers for Disease Control, swimming is the fourth most popular sports activity in the United States and is enjoyed by people of all ages, providing a variety of health benefits. Swimming, as an aerobic exercise, increases heart function, muscle strength, and endurance, and is one of the lowest impact sports on joints. Swimming and water recreation is one of the top activities in Mason County. Living in Mason County with many lakes and miles of shoreline allows many people easy access to the water. In addition to being a lot of fun, swimming and other water sports provide many health and wellness benefits. For these reasons, the pool at Shelton High School plays an important role in terms of health as well as providing water education and safety for our community.

Students in the Shelton School District have the option of taking swimming PE classes and lifeguarding classes as well as participating on swim teams. All third graders in the District participate in water safety classes, an important educational activity in our water-rich county. In addition to District-sponsored programs, there are also swim lessons, water aerobics, open swim, pool rentals and club swim team programs offered to the whole community. The pool is a much used facility and your continued support of our fundraising efforts are still needed.

The Fund drive is ongoing. To participate, donations can be made to MGH Foundation either by mailing a check payable to MGH Foundation, and marked for the “Pool”; or through online donations found on the Hospital’s website (www.

MasonGeneral.com). Donations can also be mailed to SPLASH, PO Box 1982, Shelton, WA 98584. All funds collected by either the MGH Foundation or SPLASH go directly to capital repairs at the pool. With your help, we can keep our pool facility open and in good condition for our kids and community.

The American Cancer Society’s annual Relay for Life was held in June at

MGH&FC CEO Eric Moll a cheer to cut the ribbon to celebrate the HOPE Garden Expansion Ribbon Cutting & Open House held in August. The additional ¼-acre garden is adjacent to the current HOPE Garden across from MGH&FC on Mountain View Drive. HOPE Garden works with CHOICE High School students to cultivate a community garden while teaching employment skills, nutrition, cooking, and other life skills.

“HOPE staff provides a supportive environment for students who often don’t have that support elsewhere in their lives,” said Jayne at the ceremony. “The students have been working very hard for the past six months to transform a very rocky piece of land – allocated from Mason General Hospital – into a bountiful, beautiful, fruit and vegetable garden.” HOPE is supported by MGH&FC, Mason Matters, St. David of Wales Episcopal Church, Green Diamond Resource Company, Skookum Rotary, Community Foundation of South Puget Sound, and the Federal SNAP-Ed program.

become critical and she winds up in the hospital.” Barwick added that when someone has diabetes for as many years as Huskey has, they may not have the early warning symptoms like they did at first and super high or low blood sugar levels put them at extreme risk. Preventing these extremes will lessen complications.

“Prior to using CGM, I was being admitted to the hospital two to three times a month. I had tried for several years to get CGM, but was always told by my insurance company that it wasn’t medically necessary. Now, with CGM, I’ve only been admitted to the hospital once, and that was for an infection in my toe, not for diabetes,” said Huskey. Life is still a rough road for Huskey, but she’s making the best of every day.

The theme for this year was “Dropping the Anchor on Cancer” and MGH&FC participated by dressing as the “Popeye Gang”.

“Her numbers aren’t perfect, but by all of us working together, compared to where she was, she’s come a long way. She’s able to take care of her highs and lows at home, before becoming critical. Now she has a presence in her family and home life that she didn’t have before because she was in the hospital so often. This is extremely important for them all; that’s the difference CGM has made,” said Barwick.

Pregnant women need screening About 15 percent of pregnant women have hypothyroidism, and proper levels of thyroid hormone are necessary for normal development of the child and good health of the mother. One study found that only 23 percent of pregnant women were tested for hypothyroidism.

Women who are already taking medication for hypothyroidism need higher doses, particularly in the first and second trimesters. One study recommended a 30 percent increase on confirmation of pregnancy and later adjustments in line with blood levels of thyroid stimulating hormone (TSH).

Psoriatic arthritis: varied forms Psoriatic arthritis typically attacks only a few joints at first, causing stiffness, pain, redness, swelling and warmth.

Inflammation in the fingers causes swelling and a sausagelike appearance.

This type of arthritis can also cause inflammation of tendons in the feet, causing pain similar to that of Achilles tendinitis or plantar fasciitis. Occasionally, psoriatic arthritis involves several joints on both sides of the body in a symmetrical fashion, similar to the pattern of rheumatoid arthritis. And the arthritis may also affect the joints of the spine (spondylitis), causing back stiffness and pain.
