quality Care Coordination Services now available Case Manager will work directly with the patient, their family, and the rest of their care team to maximize the health care experience.

Q. What can the Case Manager do for the patient?

A. The patient’s physician and Case Manager want to make sure the patient has the tools and support needed to achieve and maintain the best health possible. The Case Manger supports the patient in making the best choices for their health by helping them become a more active partner in improving their overall well-being.

Mason General Hospital & Family of Clinics (MGH&FC) recently joined an innovative program called the National Rural Accountable Care Consortium, an initiative intended to improve the health of people with Medicare fee-for-service via access to a Care Coordinator.

“We are excited to be a part of this program, as data show that patients with two or more chronic health conditions, who are in this program, have better outcomes,” said Terri Gushee, R.N., director of Care Coordination Services at MGH&FC. Program activities include training Care Coordinators to provide additional Terri Gushee, R.N. services to the most vulnerable patients, promotion of population health and wellness, and improving quality, customer service and patient satisfaction to those 65 years of age and older.

“This collaboration will help to keep our senior patients in their homes,” added Eric Moll, CEO, MGH&FC. “This is possible because the Care Coordinator works closely with doctors and community resources to assist these patients in staying healthy, securing care when needed and avoiding repetitive tests. In general, it helps patients navigate the complex health care system.”

Q. What is Care Coordination Services?

A. Our Care Coordination Services actually begin within the hospital and they transition along with our patients to meet their needs in various care settings. The goal of Care Coordination is to provide the patient with focused, one-on-one attention to help them become an active participant in their own health care. This program will also help enhance the patient’s relationship with their physician.

As a participant in the Care Coordination Services program, the patient will receive focused support from the Case Manager. The 2 S C O P E

Case Management includes: Identifying and overcoming barriers to getting care needs met Arranging care and connecting the patient to important community resources Providing education, resources or materials about a chronic disease or other medical condition the patient may have Acting as a patient advocate with providers Supporting the patient in becoming a self-advocate and active participant in their own health care Providing information about advanced directives or assisting the patient in designating a health care power of attorney Q. How will this program affect the patient’s relationship with their doctor?

A. The patient/physician relationship is very important. The Care Coordination program has been designed to support and strengthen that relationship by working with the patient’s doctor in developing the best plan of care.

becomes official department offering patient-centered care

Rehabilitation Services Director Cheryl Woods, PT, claims, “One of the values focused on in ‘Patient First’ care is Accessibility. We are putting that value into practice in our department by increasing staff in order to increase accessibility to patients with rehabilitation referrals.”

Colorectal cancer on decline in U.S. Thanks to screening with colonoscopy

and other tests, both incidence and mortality from colorectal cancer have been declining steadily since 2001. The decrease in incidence averaged 3.4 percent a year; the decrease in deaths, 3.0 percent a year.

The biggest declines have been among Americans age 65 and over who have high screening rates. Medicare has covered colonoscopy screening since 2001.

Under the direction of Cheryl Woods, PT, a long-time area licensed physical therapist and veteran Director of Rehabilitation Services (RS) at MGH&FC, Mason General Hospital’s Rehabilitation Services recently became an official department of MGH. “Until now rehabilitation services at MGH were contracted through a private company,” said Cheryl.

“We are all very excited to be a part of MGH&FC as it is a progressive, patient-centered organization.”

Cheryl went on to explain how the Hospital and Clinics are implementing core values that emphasize ‘Patient First’ care. “It is a very exciting time to be a part of a team of health care professionals who work daily to improve patient care,” she added.

In addition, Cheryl said she has seen lots of changes at MGH&FC during her tenure, which started in 1986. “I am so pleased with the direction MGH&FC is heading. I am very happy and honored to be a part of the Hospital and Clinics’ progressive culture and to be a member of the leadership team.”

Cheryl served as director of the Physical Therapy (PT) department (now known as Rehabilitation Services) from 1986 to 2000. She then stepped down to work part-time. In 2012 Cheryl was reappointed as Director of Rehabilitation Services for MGH&FC. Cheryl grew up in Shelton, graduating from Shelton High School in 1980 and going on to receive her Bachelor of Science degree in Physical Therapy from the University of Puget Sound in 1984.

One of the first things Cheryl plans to do is hire more staff to cover the ever-growing patient load. “Currently we have a full staff of highly-trained professionals,” said Cheryl.

“Treating our patients as individuals with specific needs is what we do best.”

Rehabilitation Services treats patients of all ages for a wide variety of diagnoses including orthopedic musculoskeletal conditions, neurological diseases, vertigo, sports injuries, pediatric developmental delays, speech and language problems and swallowing issues.

They also provide treatment for post L&I injury, post motor vehicle accidents, and teach balance and fall prevention. “We value one-on-one treatment and care,” added Cheryl.

The staff at Mountain View Women’s Health (MVWH), a part of Mason General Hospital & Family of Clinics, is working hard to provide this environment for women of all ages. In doing so they have created “Women’s Services” in collaboration with The Birth Center at MGH.

“The aim is to make a woman’s continuing health care seamless, and for patients to have easier access to services,” said Marcia Rohlik, R.N., BSN, director of Women’s Services.

MVWH currently provides coordinated care for women’s obstetric, gynecological and urological health needs, with future plans to include women’s breast health, and other services unique to women.

“Women are different at a cellular level, and they need a different kind of care. We want to bring awareness to this fact,” said Rohlik. “In this collaborative approach to care, a woman should never feel like she is going through health and wellness issues alone.”

Marcia went on to explain that MVWH office staff, board-certified physicians, and Birth Center Manager Kris Gaa, R.N., have coordinated patient care for years, and how their “team approach” to care is proving to be an effective model. “We want to choose the best information possible from all providers to keep us all working together from ‘the same sheet of music,’ so to speak,” said Rohlik. Mountain View Women’s Health’s Office Manager Misti Webb is a big part of this process, too.

Dr. Vassall studied medicine at Meharry Medical College in Nashville, TN. He is a board-certified obstetrician/ gynecologist who has been at Mountain View Women’s Health in Shelton since July of 2009. Dr. Vassall likes caring for pregnant women and being around babies, and really appreciates sharing in the entire birth and delivery process. He enjoys working at Mountain View Women’s Health for the excellence in quality of its office staff, out-patient services, and follow-up care. Dr. Vassall graduated from Meharry Medical College in Nashville, TN, in 1978; did his internship at Erie County Medical Center, Buffalo, NY; and his residency at SUNY, Buffalo, NY in 1985.

Mercedes Goebel, M.D. Board Certified Obstetrician/Gynecologist Mercedes Goebel, M.D., recently joined the women’s health care team at Mountain View Women’s Health and the medical staff at Mason General Hospital. She is board-certified in obstetrics and gynecology and highly qualified in all aspects of gynecological care and surgery, as well as contraceptive

Dr. Goebel obtained her medical degree from SUNY Downstate Medical Center, New York in 2001; and completed her internship and residency at Jackson Memorial Hospital/Jackson Health System, Jackson, FL. With a special interest in adolescent female care, Dr. Goebel welcomes younger patients who may have a gender preference for their personal health issues.

Carley A. Jacobs, PA-C Board Certified Physician Assistant Carley joined the medical staff at Mountain View Women’s Health in November, 2012.

She obtained a Bachelor of Science degree in biology with a Spanish language minor from Western Washington University in 2002; and in 2007 completed her Master’s degree in Physician Assistant studies at Shenandoah University, Winchester, VA. Carley worked as a certified nursing assistant (CNA) at the Cordata Healthcare and Rehabilitation Center and St. Joseph’s Hospital in Bellingham, WA in 2002; and in 2004 joined Woodstock Internal Medicine Specialists, Inc. in Woodstock, VA as a physician assistant. She is board-certified with the National Commission on Certification of Physician Assistants (NCCPA). Carley’s experience is mainly in primary care rural medicine, and she has a particular interest in women’s health and preventive medicine. She and her husband are both originally from Washington State, and have relocated to the west coast where they enjoy the rural setting.

Birth Center Hat Donations

Fourteen of the 22 members of MGH&FC’s team, Scrambled Legs and Achin’, recently gathered in MGH’s Bistro for a team photo prior to Seattle’s “Big Climb.” The Climb will take place Sunday, March 22, 2015 at the Columbia Center in downtown Seattle. This is the third year MGH&FC has entered a team. The Washington/ Alaska Chapter of the Leukemia & Lymphoma Society (LLS) hosts the annual event. All proceeds will go towards the LLS mission to cure leukemia, lymphoma, Hodgkin’s disease and myeloma, and to improve the quality of life for patients and their families.

Ladies from the “Knit and Pray” group enjoy creating unique baby hats for newborns arriving in MGH’s Birth Center, while praying for friends and family. The group meets bi-weekly at Alpine Way Retirement Center in Shelton. Nearly 6,000 people are scheduled to be a part of the challenge of climbing 69 flights of stairs in Seattle’s highest skyscraper.

Participants had the choice of either running up the stairwell (in a timed race) or walking up (untimed). Someone walking will take about 20 minutes to climbed 1,311 steps (788 feet of vertical elevation) to reach the top. Each team member is asked to raise at least $100 for the climb. Last year the MGH&FC team raised more than $7,000. “This is a great team-building exercise,” said Shannon Barnhart, captain of the team and director of central registration at MGH. “We all enjoy the challenge of doing this and helping to raise money for a great cause. Preparing for the Climb also motivates you to stay healthy.”

Babies born in The Birth Center at MGH go home wearing something unique: a handmade, knitted or crocheted hat donated by members of the community.

Susan Petty, a parishioner at Saint David’s Episcopal Church in Shelton leads “Knit & Pray” – a group of ladies who meet at Alpine Way Retirement Center every other Monday to make little hats for newborns in Mason General Hospital’s Birth Center. “We knit and pray. Our prayers are usually for people on our prayer list. We started this two years ago when we took on the project of knitting baby hats for the Hospital’s Birth Center. In two years, we’ve knitted over 200 hats,” said Petty, explaining that group members buy and share the yarn they use, but each member creates her own designs.

Petty said what she loves most about the project is how different each hat is, as well as the nurses’ reactions when she delivers the hats.

Kathy Mix, R.N., works in The Birth Center and has been an employee at MGH&FC for nine years. She said the nurses give away about 300 hats a year, and look forward to receiving donations from other groups as well. “We really enjoy being able to offer hats to the parents of new babies. It’s great to get a selection of colors and sizes and to try and match them with donated quilts,” she said.

Homemade quilts are donated by Christmas Town Quilters and Ladies of the Lake Quilters, who also crochet hats.

MGH New Year’s Baby Fifty-eight minutes into “New Year

2015” on January 1, Olivia Lee King was born in The Birth Center at Mason General Hospital. The 2015 New Year’s Baby weighed 8 lbs., 13 oz. and was 20-½ inches long. Olivia and her parents, Cyle and Holly King, received a gift bag filled with many items, including toys, diapers, and sparkling apple cider – all made possible by The Birth Center staff, MGH&FC and MGH Foundation’s Auxiliary Gift Shop. The Gift Shop contributed to the festivities by giving a generous discount on many items for the new baby, such as baby care items and adorable toys and accessories. The Shelton Group will meet on Thursday, March 19, 2015 from 1 to 2 p.m. in the Washington Meeting Room at Mason General Hospital; and the Allyn Group will meet on Thursday, March 26,

The Shelton and Allyn Diabetes Support Groups are affiliated with the American

Diabetes Association. The Shelton Group meets every third Thursday of the month and the Allyn Group meets every fourth Thursday of the month from September to June. The classes are free and open to people with type 1 and 2 diabetes, their friends, and family. First Aid/CPR/AED courses offered in April, May and June The First Aid/CPR/AED course size is limited to 16 per class, and an early, nonrefundable registration fee of $35 (exact cash or check) is required to cover the cost of books and materials. It is open to the entire community and a Spanish interpreter can be made available upon request. Comfortable clothing is recommended.

Cancer Support Group Mason General Hospital and Harmony Hill are co-sponsoring a Cancer Support Group for anyone who has been affected by a cancer diagnosis, including caregivers. Petty went on to say that some hats have flowers or pom poms on them, and some are red and green for Christmas or red and white for Valentine’s Day. The group uses only acrylic yarn, in case a baby has an allergy to wool.

“We have a lot of fun and we’ll continue to knit until the Hospital says they don’t need any more hats,” said Petty, adding that anyone is welcome to join the project.

“I appreciate whoever wants to make hats,” she said.

“Being a rural clinic doesn’t mean we have to offer a lesser quality of service; but we have to do it right – and our goal is to do it better,” said Rohlik. She offered an example of a woman scheduled for a C-section who had significant other health concerns. “Misti and I were in contact about her needs for over a month.

We collaborated with all departments to make sure her delivery went smoothly,” she said.

While delivering babies is only one service they provide, a good birthing experience for patients is a priority. “We have a goal, and we are working towards it – and the birth rate at MGH is picking up.

We are constantly asking our patients, ‘Why would you choose to have your baby elsewhere?’ We ask this question because we want to know why women would go out of the area for delivery. It is important for us to listen to their needs and then make adjustments,” Rohlik said. Flashes have other causes

Perceived flashes of light in the field of vision and/or recent onset of floaters are two major symptoms of retinal tears and detachment. Some flashes, however, are not caused by retina problems.

A patient with migraine often experiences visual disturbances, such as a poorly formed pattern of lights or jagged lines and colors. These may occur with or without a headache.

Migraine flashes tend to occur in both eyes whereas retinal flashes occur only on the eye affected by retinal detachment or tear.

Other possible causes of flashes include seizures and postural hypotension – low blood pressure caused by rising too quickly from a seated or lying position.

Retinal detachment linked to certain antibiotics

were linked to retinal detachment in a case control study published in the Journal of the American Medical Association +[2012]. The study found that current, but not past, users of fluoroquinolones were 4.5 times more likely than non-users to develop retinal detachment.

Previous studies found that these commonly used antibiotics were associated with rupture of the Achilles tendon and other problems involving collagen and connective tissue. The eye contains both collagen and connective tissue.

If you want an egg for breakfast...

If you’d like to have an egg with whole grain toast for breakfast, go for it. It’s a healthy choice.

Concern about the cholesterol content (212 milligrams per egg) has haunted health conscious egg lovers for decades, but the American Heart Association says that healthy persons can eat one egg a day without worry.

Eggs are also good for weight control. A study that compared overweight women eating either two eggs or a bagel for breakfast (identical in volume and calories) found that egg eaters lost 65 percent more weight, had 83 percent greater loss of waist circumference and had higher energy levels. There was no significant difference between the two groups in total cholesterol or triglycerides.
