Diabetes

Diabetes is a chronic health condition; however, if self-managed,

someone with diabetes can live a normal, healthy life. Type 1 Diabetes can’t be prevented, but 58 percent of the Type 2 cases can be prevented with a lifestyle change. Type 2 Diabetes used to be considered an “old-age onset” chronic condition. Or, simply described as something you got in your 60’s or 70’s. This, however, is no longer true. Type 2 Diabetes has now reached epidemic levels and is affecting children as young as 10 years of age. An estimated 635,000 adults in Washington have diabetes, and it is estimated that one in every four people with diabetes does not even know they have the disease.

Sue Barwick, R.N., certified diabetes educator, has been director of MGH&FC’s Diabetes Wellness Center for 24 years. She and her staff, made up of many clinical specialists, offer diabetes education daily, Living Well With Diabetes classes quarterly, and Diabetes Support Group Meetings monthly – all teaching and coaching people to selfmanage their diabetes in order to live normal, healthy lifestyles.

The following are commonly asked questions about diabetes.

Q - Sue, let’s start with the basics. What is diabetes?

A - Diabetes is a chronic disease where your pancreas makes little or no insulin, or your body prevents the insulin you do make from working properly. As a result, sugar cannot get into your body’s cells.

This further results in increasing amounts of sugar staying in your blood and your blood sugar getting too high (hyperglycemia). High blood sugar can lead to diabetes complications such as heart disease, blindness, and kidney failure.

Q - What is the difference between Type 1 and Type 2 Diabetes?

A - Type 1 Diabetes is when the body makes little or no insulin, and usually occurs in children and young adults, but can also happen

in older adults. People with Type 1 must take insulin injections every day. Type 2 Diabetes is when the body prevents the insulin from working properly or doesn’t make enough insulin. It usually happens in people who are older or those who are overweight, and accounts for the majority of cases (90 to 95 percent of those with diabetes).

Q - What are the symptoms of Type 1 and Type 2 Diabetes?

A - Common signs and symptoms of Type 1 Diabetes include: frequent urination, excessive thirst, more hunger than usual, unexplainable weight loss, fatigue, irritability, blurred vision, non-healing wounds, and numbness or tingling in the hands or feet. The most common symptom of Type 2 Diabetes is fatigue or weakness. Type 2 symptoms may include the same as Type 1, and some people have no symptoms at all.

Q - How is diabetes diagnosed?

A - Here’s an example of how someone might be diagnosed with diabetes. On October 15 a fasting glucose test is 162. The health care provider orders a repeat fasting glucose test, and a week later it is 150. With both lab results showing elevated fasting blood sugars, the diabetes diagnosis is confirmed.

Q - Can Type 2 diabetes be prevented?

A - Studies have shown that lifestyle changes including healthy eating, weight loss, and regular physical activity can lower your risk of getting Type 2 diabetes by 58 percent!

Q - An increasing number of American children and adolescents are being diagnosed with Type 2 Diabetes.

Who is at risk? What are the warning signs? How can it be prevented?

A - The single greatest risk factor for Type 2 Diabetes in children is excess weight. In the United States, nearly one out of every five

children is overweight. Other factors with increased risk in children are: family history of diabetes, female gender, and ethnicity (i.e.

Hispanic/Latino, Native American, Asian, and African American).

MGH&FC applies to be Accountable Care Organization

Warning signs include: increased thirst, frequent or nighttime urination, blurred vision, unusual fatigue, and dark skin around the neck or armpit.

Mason General Hospital & Family of Clinics has applied to become an Accountable Care Organization (ACO). Once accepted, the area’s only local hospital and family of clinics will become a part of a national model for high quality, patient-centered health care – continuing to put patients first.

Prevention strategies include: Encouraging active play and daily physical activity Limiting time for television and video games Drinking less sodas with sugar, sports drinks, and juices Drinking more water Limiting fast foods and eating healthy foods Eating smaller portions

“Fundamentally, an ACO exists to achieve the Three Part Aim of better patient experiences, lower total cost, and better quality outcomes. This change will have a direct impact upon our community’s patients, and especially those who are Medicare Beneficiaries. Working together, doctors can do more to follow a patient’s health and make sure they get the best coordinated care,” explained Eric Moll, CEO.

Q - What does it mean when a patient hears from their health care provider they are “border-line diabetic”? And what should they start doing differently?

A - Today, most health care providers don’t use the term “borderline diabetes”. Instead you may hear the term, Pre-diabetes or Continued on page 13...

one, are at elevated risk of another ulcer, according to a Dutch study. Of 171 diabetic subjects, all of whom had a previous ulcer at least 18 months before the study began, 71 developed another ulcer, often in the same location.

And 41 of these did not involve any significant trauma or injury.

Subjects who wore shoes customized to pressure points on their feet had a 57 percent lower risk of developing a new ulcer.

In a diabetic patient, a foot ulcer is a serious matter.

Because of nerve damage and impaired blood flow associated with the disorder, foot ulcers often develop into severe infections requiring, in some cases, amputation.

‘Whole-patient’ wound care service

MGH &FC lead Grounds Maintenance worker Bill Perreault is passing on his love for mountain hikes to 13-year-old son, Samuel.

After not being able to walk, staying fit is Bill Perreault’s passion For MGH&FC lead Grounds Maintenance worker, Bill Perreault, staying fit isn’t just

a job requirement – it’s a reminder of what he nearly lost.

“At 22, I was in a motorcycle accident and couldn’t walk for a month. Then I was on crutches for three months. I had lost my God-given ability to walk, and so when I regained it, I never took it for granted. Within a month of getting off crutches, I had already run a mile. I gave my doctors ulcers, but within a year I was running 12 miles a day and was in the best shape of my life,” said Perreault.

Being athletic wasn’t new for Perreault. Growing up in California, he surfed, was involved in multiple sports, and by age nine, was hiking in the High Sierras with his dad.

“My father was an Assistant Scout Master who spearheaded High Adventures for Scouts, which are multiple-day backpacking trips that range from 65 to 85 miles in length,” he said.

Perreault continues his father’s scouting and backpacking legacy with his own son, and now serves as the Boy Scouts’ Charter Organization Representative for Troop 110. “That’s just a fancy title for being the liaison for the troop with the charter organization,” Perreault said.

But having a title hasn’t stopped him from joining in on what he enjoys most about scouting – outdoor adventures.

“I will always be out in the field with the boys because that’s what scouting is all about,” he said, adding that he’s been with this troop since they were Cub Scouts, and now they are seasoned outdoorsmen who can do it all.

“Boy Scout Troop 110 is boy-led and these trips are their own idea. Next year, they plan to do a 50-mile canoe trip. Fundraising helps with costs Continued on page 15…

Wound Care Certified Registered Nurse

At the end of December Sara will be the new team leader at WCS, replacing Lena McGinnis, R.N., CWCN-RN, who is moving to a consultant position.

“We see on the average of 8 to 9 patients a day – inside the Hospital or as an outpatient – who have wounds that won’t heal on their own,” said Sara.

Many of the patients WCS sees have chronic wounds caused by diabetes or decreased circulation to the lower extremities. A portion of them are from local nursing homes. “We truly want to be an asset to the community, so we reach out to all areas that need our support,” said Sara, adding that the WCS staff doesn’t just look at the wound, but at the whole patient, to determine the cause so treatment will prevent a recurrence.

Doctors Rice and Penoyar celebrate a collaborative “25 years of caring” for our community MGH&FC Physicians John V. Rice, D.P.M., and Christopher Penoyar, D.O., celebrated 25 years of service - and working collaborative - this past summer at a staff barbecue held at the lake house of Dr. Rice and his wife, Gretchen. The event focused on staff appreciation and included members of both doctors’ staff and their families.

“It was a nice, relaxing, sunny day to sit around and visit without the distractions of work,” said Gretchen Rice, MSW, who also works at MGH Ankle & Foot Clinic.

MGH Ankle & Foot and MGH Family Health are clinics housed in a building Dr. Rice and Dr. Penoyar opened across the street from Mason General Hospital in 1989.

Although the two physicians share a building, their practices are completely different. Dr. Rice is board-certified in Podiatric Medicine and surgery, specializing in problems of the ankle and foot and related foot surgery. He is also a diabetes educator, along with fellow board-certified Podiatrist James A. Wright, D.P.M. They both offer their expertise in diabetic foot care to patients, through “Living Well With Diabetes” classes offered by MGH&FC’s Diabetes Wellness Center.

MGH Eye Clinic has the best equipment and technology available today to diagnose and treat eye disease, including Optical Coherence Tomography (OCT), a non-contact diagnostic imaging device which allows the doctor to view a cross-section of the eye.

“This machine uses light to obtain cross-sectional images of the eye to help identify the problem. In ‘wet type’ macular degeneration, a damaging, abnormal growth of blood vessels are detected in the retina at the back of the eye. The OCT scans help direct therapy for patients who may need an Avastin injection to prevent further damage.

Cancer patients: avoid supplements

Doctors usually advise their cancer patients to avoid all dietary supplements while they are in treatment.

Antioxidants might be good for a healthy person, but they can also interfere with anything designed to kill cancer cells.

Some supplements cause severe reactions when taken during radiotherapy. And supplement/drug interactions are a possibility during chemotherapy.

Terri Gushee, R.N., BSN, director of Care Coordination Services, came to Mason General Hospital 21 years ago and elected to stay because of her love of the community and hospital. “I am so pro-hospital and community,” said Gushee. During those 21 years, Terri has worked tirelessly to bring new programs to MGH&FC, and has done the same as a volunteer in the community to support the Hospital and Clinics.

Stroke seminar a HUGE success On September 17, MGH&FC hosted a FREE Community Stroke Seminar to educate the public on stroke and mini stroke (TIA). They were taught the signs and symptoms of a stroke, what types of tests are administered in the ER, medications that may be used, risk factors, and lifestyle changes that can be made to prevent the recurrence of a stroke or TIA. “The feedback from the community was outstanding”, said Sabrina Nelson, R.N., director of the Emergency Department and Trauma Services. “There was a great dialogue between health care providers and attendees. Several community members stated how wonderful it was to be engaged on a personal level during the presentation.”

Colonoscope

A colonoscope is a long, thin, lighted flexible tube with a camera on the end. It is used to look inside of the large intestine during routine well health screenings (colonoscopies) or in the diagnosis of abnormalities such as polyps. Video images from the camera are displayed on a screen in the examination room for the physician to review. Still images may also be taken to allow the physician a closer examination and to make a comparison to earlier or later colonoscopy images. The colonoscope can also be used to take tissue for a biopsy or to remove a polyp using small disposable devices that are passed through it. The goal for the purchase of the colonoscope is $33,650.

ICU bed and mattress

MGH Foundation funds ICU beds and mattresses for Intensive Care Unit Mason General Hospital Foundation aims to raise enough funds to purchase two ICU beds and mattresses for the Intensive Care Unit. The new beds will improve comfort and safety for patients and the nurses caring for them.

The InTouch ICU bed represents a higher standard of care in the ICU. All bed functions are controlled with an easy, intuitive, touch-screen interface in a powerful, on-bed computer. The bed allows the nurse to document therapies at bedside. With a touch, a nurse can see a patient’s last 50 weights on their weight history graph. And with InTouch the nurse can customize the bed surface to the patient’s skin safety and comfort by adjusting mattress surface zones to improve circulation. The bed also has a turn-assist feature to make the maneuver easy for nurses.

With pre-recorded reassurances and reminders in 12 languages, the computer is able to communicate with patients and keep them safe during times of increased anxiety or confusion. In addition, the Sound Therapy software provides patients with a soothing environment of relaxing music or nature sounds. The cost for two InTouch beds is $78,000.

Los pacientes diabéticos que tienen una úlcera del pie, aun una menor, tienen un riesgo

más alto de desarrollar otra úlcera, según un estudio holandés. De los 171 sujetos diabéticos, de los cuales todos habían tenido una úlcera previa al menos 18 meses antes del comienzo del estudio, 71 desarrollaron otra úlcera, frecuentemente en el mismo sitio. Impaired Fasting Glucose. Pre-diabetes or impaired fasting glucose means that your blood sugar is elevated, but not enough to be diagnosed with diabetes. The keys to preventing diabetes are healthy eating, weight loss, and regular physical activity!

Q - Once someone is diagnosed with Type 2 diabetes, what can they expect?

A - Your health care provider will start the ball rolling by talking with you about your diagnosis, ordering a variety of services, and depending on your blood sugar, may order diabetes medications. All of these services are aimed at getting you started with your diabetes self-management plan. One of these services is DIABETES EDUCATION! Q - What should you expect to be included with Diabetes SelfManagement Education at MGH&FC? A - Self-management will include: education about diabetes; a physical activity plan; a meal plan; and a diabetes medication plan, if indicated. You will also learn how to test your blood sugar daily, and a plan for testing related to appropriate blood sugar goals. There will also be referrals to specialists (i.e. for eye checks, for feet checks, for depression) when indicated; plus coping strategies and a stress management plan.

Q - What classes and support groups are available in Shelton and Allyn to help patients and their family members manage diabetes in order to prevent its complications?

A - The Diabetes Wellness Center at MGH offers individual and group diabetes education – including Living Well With Diabetes, an 8-hour class (over 2 days) taught by a multi-disciplinary faculty.

Diabetes education is covered by most insurance.

Case management is a usual part of what your provider expects when Diabetes Self-Management Education is ordered,

and is usually done by outpatient phone assistance providing diabetes medication adjustments and coaching support with the other aspects of your selfmanagement plan.

The Diabetes Wellness Center also offers free Diabetes Support Groups each month (except during July and August).

The Shelton Diabetes Support Group usually meets at MGH on the third Thursday of the month at 1 p.m. The Allyn Diabetes Support Group usually meets on the fourth Thursday of the month at 1 p.m. in the Port of Allyn building. Family members and friends are always welcome! To be placed on our email or USPS mail list for notification, call (360) 426-1611 and ask for the Diabetes Wellness Center.

Q - What do the patients think about MGH&FC’s Diabetes SelfManagement Education programs?

A - Here are several quotes from our patients: “After struggling to keep my blood sugar levels controlled without help, I started working with Sue and Debbie. This changed my whole outlook on the importance of keeping my diabetes controlled. Their help enabled me to maintain good eating habits, exercise, and proper dosing with my diabetes medications, including insulin.”

Q - Tell us about the staff and physicians who make up the diabetes educators and clinical specialists for these classes and support groups.

A - Our two registered nurses – Debbie McGinnis, RN, and Sue Barwick, RN, and two registered dietitians – Candy Mattson, RD, and Sarah Fulkerson, RD, have also achieved Certified Diabetes Educator (CDE) status by successfully completing the Certification

Examination for Diabetes Educators.

Candidates must meet rigorous eligibility requirements to be able to take the examination. Achieving certification status demonstrates that the health care professional possesses distinct and specialized knowledge, providing exceptional quality of care for those with diabetes.

Mark Schlauderaff, M.D., has served as medical director of the Diabetes Wellness Center and faculty of Living Well With Diabetes since 1993. Dr. Schlauderaff has been board-certified in internal medicine since 1983.

Also included in the faculty are John Rice, D.P.M., and James Wright, D.P.M., both board-certified in podiatric medicine; Gretchen Rice, MSW, ACSW, social worker; Cheryl Woods, MGH’s director of Physical & Occupational Therapy; and Doug Lindahl, D.O., board-certified in internal medicine.

Q - Recently, MGH&FC’s Diabetes Wellness Center received a Recognition award. Please tell us more about this.

A - On September 18, 2014 the Diabetes Wellness Center’s Diabetes SelfManagement Education program was awarded another Recognition from the American Diabetes Association. Our program was originally Recognized in September of 2001. The Recognition is effective for four years, at the end of which it will be re-applied for. This is a voluntary process that assures approved education programs have met the National Standards for Diabetes SelfManagement Education Programs. This Recognition gives professionals a national standard by which to measure the quality of the services they provide and helps consumers to identify these quality programs. We are very proud of this continuing achievement.

“Our staff members are professional and know our patients by name,” said Gretchen.

The other half of the building belongs to Dr. Penoyar of MGH Family Health. Dr. Penoyar is a board-certified Family Practice physician, and also a Doctor of Osteopathic Medicine (DO), with advanced training in the musculoskeletal system and a thorough understanding of how all body systems work together.

“What impresses me most about Dr. Penoyar is his compassion and availability to his patients,” said the Clinic’s Office Manager Peggy Moore, CMA. “He is dedicated to his

practice and you see that in his patients’ loyalty.”

What makes a DO different from an MD is a method of treatment that focuses on the ‘whole person,’ often integrating osteopathic manipulative treatment (OMT) which uses gentle pressure to manipulate muscles and joints.

Moore added that Dr. Penoyar has a ‘country doctor’ style that makes people feel really at ease with him, and even though he’s a DO who uses manipulative therapy for some, he also fully embraces traditional medicine.

“More than being a DO, he takes pride in being a family practitioner who serves multi-generational families,” said Peggy.

“The stability that Dr. Rice and Dr. Penoyar have brought to the community has been highly beneficial. Their loyalty to our community for so many years has entrusted their patients to them, and both practices have many long-time patients,” she said.

Perreault said he stays physically fit because his kids are 11 and 13 years old. “I’m 60 years old and want to be around to see my grandchildren,” said Perreault. For people wanting to change their personal fitness levels, Perreault offers this advice: “Find something you love to do. Gyms and treadmills help, but there’s nothing like getting outside in the fresh air, no matter the weather, and doing what you love. If you have something you love, you’ll get out and do it. At 60, I still do the outdoor things I’ve always done,” Perreault added.

“I enjoy my practice because I can really change people’s lives by preserving their sight. Some people come to me nearly blind.

And if cataracts are the problem, I can remove their cataracts and change their entire visual world.

Eyesight is valued by my patients, and they’re grateful for having it restored,” said Dr. Vuong.

Dr. Vuong added that she enjoys working at MGH Eye Clinic because of her staff. “I love our staff. The front desk staff greet patients and put them at ease.

The technicians improve my efficiency, and everyone is able to work together as a team to accomplish our goals in quality patient care”.
