“The doctor was on a TV screen. He questioned me and could see me, and I could see him,” explained Evelyn. Dr. Choi specializes in cerebro-vascular disease and is the author of numerous articles on the subject both in the U.S. and in Korea where he received his medical degree.

Evelyn also had symptoms that included dizziness and double vision, which are not typical of a TIA, but can occur, according to Sabrina Nelson, R.N., BSN, director of the Emergency Department/Trauma/ Stroke Prevention Clinic.

TIA – A warning sign Evelyn “Jean” Archer is grateful she could stay local for her TIA treatment and stroke prevention.

It was less than a year ago when MGH&FC began offering a new service to patients – a clinic specializing in evaluating and educating patients who have suffered a transient ischemic attack (TIA), also known as a mini-stroke. The Stroke Prevention Clinic opened just in time to help Shelton resident, Evelyn “Jean” Archer, who experienced a TIA in January of 2014. Evelyn said her symptoms started suddenly, while she was resting.

“Physically, I didn’t feel any different, but I had the TV remote in my hand and I asked my husband, Fred, ‘What do I have in my hand?’ I was confused so I made an appointment with my doctor the next day and was told I’d had a mini-stroke,” said Evelyn.

A TIA can be a warning sign that someone is in danger of having a full stroke, and occurs when the blood supply to the brain is blocked for a short time. The brain malfunctions temporarily and then usually returns to normal with no permanent damage. Sometimes, however, permanent damage to brain functions, such as short term memory loss, can occur – as has been the case with Evelyn.

“When a patient comes to the Stroke Prevention Clinic, a nurse reviews their health history to suggest lifestyle changes that could help prevent a full stroke in the future. A neurological exam is performed and the neurologist reviews any tests the patient may have had done recently, such as a CT scan, carotid ultrasound, or MRI. The patient does all this at MGH&FC’s Stroke Prevention Clinic, where the neurologist and patient talk face-to-face via tele-robot. The neurologist then gives follow-up orders and information.

The Stroke Clinic visit is then followed up by the patient visiting their primary care provider,” said Nelson, adding that “helping to ward off a full stroke is the

Clinic’s main goal, as well as providing services that patients once had to travel to Olympia to receive”.

Nelson explained that a patient will receive follow-up phone calls at one month, three months, six months, and one year following their first visit to the Stroke Prevention Clinic, to see if they’ve had additional TIAs or strokes, and to make sure they are following their plan to reduce the risk of another stroke. It is also a check to see if they need further support or resources to help accomplish their goals.

“So far, we’ve received really great patient feedback for the program because of the one-on-one care and personal education we provide for the patients,” Nelson added. A Stroke Prevention Clinic appointment lasts about an hour and includes a lifestyle assessment.

“We start with small lifestyle changes, choosing one thing to work on so the patient doesn’t get overwhelmed. They decide what to work on and we help them with it,” Nelson said. Suggestions for change may include to stop smoking, lose weight, or to add daily exercise to their routine – all of which help decrease stroke risk.

“I think the biggest advantage of our Stroke Clinic is that people are getting in for care much faster. It’s also cost effective having all of their tests done at once, and in one place. A patient can have their MRI, ultrasound, CT scans, Continued on page 5…

A health care delivery system that started out as a “one physician” family health clinic has now increased to 10 clinics located in Shelton and Hoodsport.

Q: Hospitals aligning with clinics within a coordinated health care delivery system

is a current medical industry trend.

What changes have happened in the health care arena to make this change come about?

The process of adding clinics to our family initially began because local community physicians wanted to remain focused on providing top-notch quality health care.

All community members are invited to attend a “Stroke Prevention” seminar hosted by MGH&FC on Wednesday, September 17, from 6 to 8 p.m in the Pershing and Ellinor meeting rooms at Mason General Hospital.

The featured speaker will be Registered Nurse M. Fawn Ross, FNP, stroke/telestroke coordinator for Providence St. Peter Hospital in Olympia. Ross provides support for stroke programs throughout the area through educational outreach to community members, administrative, and medical staff. Her presentation will cover stroke symptoms and what to do when someone is experiencing them. “The seminar will also address what to do once a stroke victim arrives at Mason General Hospital,” said Ross.

Ross will be joined by Youngbin Choi, M.D., from the Neurology Department at Providence St. Peter Hospital, who also serves as the tele-robot physician for MGH&FC’s Stroke Prevention Clinic.

A heart-healthy meal demonstration will be offered by MGH&FC House Supervisor/ Chronic Care Case Manager Terri L. Gushee, R.N., BSN, and heart healthy snacks will be served. The seminar is free, and those wishing to attend are asked to RSVP to Terri at (360) 426-1611, ext 28470.

assume the responsibility of running the medical practice so the physicians could focus on what they love the most -- spending more time with their patients and providing the highest quality health care possible. From our perspective, acquiring the clinic practices was about maintaining provider access in the community.

By owning the clinics, we could do more to help recruit providers. Over time, we realized physicians thought leadership and engagement would be critical to the success of developing a highly coordinated, patient-centric, care delivery system.

Q: How many physicians and supporting mid-level providers are now a part of MGH&FC and what are their areas of specialization?

Combined there are now 25 physicians and 10 mid-level providers who specialize in either ankle/foot, eye care, women’s care, obstetrics, orthopedic surgery, general surgery, pediatrics, family medicine, and internal medicine. All together, there are more than 100 physicians providing medical services within the District.

Q: Your first year as CEO has been a busy one. Within your first year as CEO you and the hospital board of commissioners were able to bring in two new clinics to the “family” – Hoodsport Family Clinic and Olympic Physicians. Can you share how and why these two clinics chose to become a part of MGH&FC? During our initial conversations, it was clear we had strong philosophical alignment around our visions for quality patient care. Without that, it doesn’t make sense to move forward. By joining a larger community of providers, we felt there are greater partnership opportunities for quality improvement.

Q: As a patient – how will this expanded coordinated system of health care delivery improve my access to local health care and access to other medical specialists?

get moving Nancy Samples

Walking helped Nancy lose weight & reduce stress Five years ago, Nancy Samples won the Biggest Loser contest at work. Nancy is a cook and dietary aide for Mason General Hospital & Family of Clinics. Unfortunately, her weight loss didn’t last, mainly she says, because she did it to “beat her boss”, and not to help herself.

Then, seven months ago, Nancy found the drive and determination to start again, thanks to her ailing mother who urged her to take care of herself.

“I’m a rescuer and I’m always doing things for other people, not for myself,” she said. “This time, I wanted to lose weight for me.”

Nine months of stress while caring for her mother, who passed away in May, plus working and caring for her family, were almost too much for Nancy. Walking every other day was just the stress-relief she needed, and today she is 35 pounds lighter.

Nancy started by walking one mile every other day, pushing herself a little more each time until she could walk five or six miles at a time, saying she often walks from Cloquallum Road all the way to Oakland Bay Junior High.

“My dog is my biggest motivator. If I don’t feel like walking, and someone at home asks me if I’m going to walk, Scooter will jump off the couch and drag his leash to me as if to say, ‘Let’s go, Mom’. We rescued Scooter from the pound, and now he rescues all of us,” said Nancy.

As a dietary aide at Mason General Hospital, Nancy understands the importance of eating right, but said she doesn’t deprive herself.

“I still allow myself sweets, but I control my portion size and don’t eat starches. If I have a meal at the Hospital, I’ll eat the meat and vegetables and skip the bread or potatoes, but still have dessert,” she said.

Losing weight, Nancy claimed, has changed everything for her. She can get around better, do her job more easily, wear cuter clothes, and best of all – her weight loss is motivating others to lose, too!

Weight loss: Dream on – it helps Persons starting on weight loss programs

often set overly optimistic goals for themselves that prove to be virtually impossible to reach. Should they set more realistic targets for their weight loss?

Several studies have found that setting a lower target is no more likely to produce lasting results; in fact, those who dream high are slightly more likely to succeed in longterm weight loss. Unrealistic optimism is, in many cases, a healthy adaptive mechanism.

For example, our emergency physicians are able to see a more complete picture of our patients by accessing information across our clinics and hospital. Over the past 20 years, we have seen a fairly significant increase in patients with multiple chronic diseases, which requires more team-based medicine. By aligning clinics within the same organization and creating provider governance across specialties, we are making good progress developing a team-based approach to patient care.

Q: Can you take a few minutes and share the benefits of the new patient portal – MyMasonHealth – and how it has brought the entire MGH&FC health care system together?

Being able to access results quickly has been a major benefit. The portal also allows more communication with the clinic offices through messaging around appointments and questions about medical conditions. In addition, the portal also allows patients to store personal health information to self-monitor things like weight, blood glucose, and blood pressure.

In honor of October being “Breast Cancer Awareness Month” MGH&FC healthcare professionals will offer valuable health guidelines to all women on good nutrition, heart health, diabetes awareness, and more – plus information on the HPV vaccine that prevents most cases of cervical cancer. There will also be free blood pressure screening and diabetes testing, healthy refreshments served, and mini massages offered by volunteers from Kaleidoscope Massage in Shelton. In addition, the first 50 women, aged 40 to 65, who are uninsured or underinsured for mammograms will receive a free digital mammogram screening. The free screenings are funded by the Karen Hilburn Breast and Cervical Cancer Fund; the Washington Breast, Cervical and Colon Health Program; and Susan G. Komen for the Cure of Puget Sound.

With an emphasis on ‘prevention,’ MGH&FC’s TIA Clinic was recently re-named the Stroke Prevention Clinic.

Hydrate to avoid heat problems

When exercising in hot weather, dehydration is your biggest

enemy. Your body’s cooling system works through evaporation of sweat. If you don’t consume enough fluids, you become dehydrated, and your body becomes unable to produce sweat during an extended exercise session.

When body temperature rises, symptoms of heat exhaustion occur. Sometimes cramping is the only symptom; other signs include weakness, nausea and dizziness.

Heat stroke is a more serious condition with body temperature above 104 degrees, an inability to sweat, confusion and sometimes loss of consciousness. These symptoms call for immediate medical attention

The typical symptoms of TIA and stroke – F.A.S.T. FACE Drooping – Does one side of the face droop, or is it numb? Ask the person to smile. Is their smile uneven?

ARM Weakness – Is one arm weak or numb? Ask the person to raise both arms. Does one arm drift downward?

SPEECH Difficulty – Is speech slurred? Is the person unable to speak or hard to understand? Ask the person to repeat a simple sentence, like “The sky is blue”. Is the sentence repeated correctly?

Time to call 9-1-1 – If someone shows any of these symptoms, even if the symptoms go away, call 9-1-1 and get the person to the hospital immediately. Check the time so you remember when the first symptoms appeared.

How to help prevent a stroke If your blood pressure is high, lower it. For people over age 18, high blood pressure is a measurement of 140/90 or higher.

Find out if you have atrial fibrillation (Afib). Atrial fibrillation is an irregular heartbeat that can cause blood to pool in parts of your heart. Pooling blood can cause clots which can break away and cause a stroke.

If you smoke, stop. Smoking doubles the risk of stroke.

If you drink, drink in moderation. Heavy drinking can increase the risk of stroke.

Keep your cholesterol at a normal level. Your total cholesterol should be under 200, and your LDL or “bad” cholesterol should be between 50 and 70.

If you are diabetic, follow your doctor’s instructions carefully and keep your blood sugar levels under control. Diabetes increases stroke risk.

Include daily exercise in your routine, which decreases the risk stroke.

Cut down on sodium and fat. Eating less salt and fat can lower your blood pressure and risk of stroke.

quality Jean Farmer is glad to stay local for her health care needs!

“The next day I received a call from a nurse at the Hospital just to see how I was doing, and then a call from my doctor. I thought that was really unusual,” Jean said.

Later, the Hospital’s accounting department informed Jean that a portion of her property taxes goes toward supporting Mason General Hospital, and that she could use that tax credit to help pay her hospital bill, which she did.

Jean and Tom Farmer can go almost anywhere for medical care, however, they choose to stay local for the convenience and quality of care at MGH&FC.

The name Jean Farmer is well known in Mason County for two reasons. Jean has served as a Port of Allyn Commissioner for seven years, and she’s part of a family business known as Tom Farmer Oysters.

Her husband Tom owns the business, and also serves as a PUD 3 Commissioner.

This energetic woman also works as an accountant for the Archdiocese of Seattle, and has a generous health care plan through her employer. That means she can go anywhere for her health care; however, Jean chooses Mason General Hospital & Family of Clinics because of the great service she said she receives. Her primary care physician is Christopher Penoyar, D.O., at MGH Family Health.

“He’s a wonderful doctor,” said Jean. It was Dr. Penoyar who referred her to boardcertified Podiatric Surgeon John Rice, D.P.M., at MGH Ankle & Foot, who treated her for plantar fasciitis. “Dr. Rice wanted to use surgery as a last option, so he tried several other treatments first. options didn’t work, so I went in for surgery,” she said.

“I had no prior knowledge about Mason General, so wasn’t sure what to expect.

When I went in that morning the nurses were so attentive, providing one-on-one care before, during, and after my surgery.

Before the surgery they were concerned about my warmth, comfort, and pain level, and they were wonderful during the anesthesia. I was hardly ever left without a nurse, and it was only a day surgery,” said Jean. She relayed that her positive experience with MGH&FC didn’t end there.

“From the day I stepped through the Hospital doors to the day I paid my bill, the whole experience was positive for me,” said Jean. “So I decided to start having my regular mammograms at Mason General. When I got to my first screening appointment I waited ten minutes for my exam, which took about another ten minutes. I was in and out, and now I get my mammogram there every year. MGH’s service is fast and super-efficient, and I’ve had nothing to complain about,” said Jean, adding that she can’t praise MGH&FC enough.

“Mason General is a caring, loving, kind, and compassionate place. I wish more people knew that about it,” Jean stated.

“With this kind of care, why wouldn’t I want to go there for all of my medical needs?”

“The Toshiba scanner also allows for automated side-to-side movement of patients,” said Michael Gray, director of Diagnostic Imaging at MGH&FC. “The larger bore hole and automated movement allows for higher quality exams by placing the patient in the center of the bore.”

“This scanner can image the patient much faster than previous models, and because of this faster scan time, the details of the images are less affected by motion and are more crisp and refined,” said Kevin Roscoe, M.D., chief of Radiology at MGH&FC. “Because of faster scan time and faster reconstruction of images, Emergency Room patients can have results quickly, and more patients can be scanned sequentially.”

Gray went on to explain that in order to enhance safety, the new scanner can automatically reduce radiation exposure to patients.

Radiation exposure is calculated based on the patient’s body size and exam type. The scanner includes dose reduction software that can reduce radiation dose even more than 50%. This is used for specific exams.

These added features provide patients at MGH&FC more diagnostic options without having to travel. And because of the scanner’s unique qualities, MGH now accepts patient referrals from health care providers throughout Western Washington, making MGH&FC a leader in putting patients’ needs and comforts first.

Rosa GonzalezGonzalez, certified medical assistant and office manager at MGH Surgery Clinic, has worked at the Clinic for three years; the first year and a half as a medical assistant before being promoted to Office Manager. “I love working here because of the contact I have with patients. I am able to help them by calming their fears and letting them know they are in good hands and will get the best care possible,” said Rosa. Rosa is originally from Puerto Rico and fluent in Spanish. “Since I know the language I am able to help our Spanish community communicate with the doctor, which helps to put them at ease,” she added. As Rosa is a certified medical assistant and a certified medical interpreter, she wanted to maintain patient contact after she was promoted to Office Manager. “It was important to me to be able to continue with patient care,” she said.

Dr. Dightman obtained his medical degree from the University of Washington School of Medicine, Seattle in 2007, and did his internship at the St. Peter Family Medicine Residency Program in Olympia.

He also completed a Fellowship in Rural Family Medicine - Obstetrics at Tacoma Family Medicine, and is board-certified in Family Practice. With a special interest in preventive care, diabetes management, and obstetrics, Dr. Dightman enjoys helping his patients achieve and maintain the best health they can. MGH Mountain View Women’s Health (MVWH) is open late on Wednesday evenings until 7 p.m. Daytime office hours are Monday through Thursday from 8 a.m. to 5 p.m., and 8 a.m. to 2 p.m. on Fridays. MVWH is a full-service medical clinic that provides medical treatment for women of all ages with gynecological and obstetrical needs, and is staffed by board-certified Gynecologist/ Obstetricians Nkem Nwosa, M.D. and Alford N. For the seventh consecutive year, MGH&FC has received the prestigious, “Most Wired” Hospital award, this year competing with nearly 2,000 hospitals surveyed across the nation in information systems capability.

The Hospitals & Health Networks (H&HN) Most Wired Survey identified MGH&FC as one of the top hospitals in the country that were given this designation. MGH&FC received the “Most Wired”- Rural Hospital award in 2008 and 2009, and then in 2010 it was reclassified to be included in not just rural, but all hospitals, nationwide. Every year since then – 2010, 2011, 2012, 2013 and now 2014 – MGH&FC has received the Most Wired Hospital award for information technology (IT) that implements systems to ensure quality and improve efficiency in the delivery of patient care. The award is based on four categories: infrastructure, business and administrative management, clinical quality and safety, and clinical integration. It also focuses on how hospitals use information technologies for quality of care, customer service, public health and safety, business processes, and workforce issues. Most Wired hospitals have better drug formulary checks and drug interaction alerts than hospitals in general, and are better prepared for security breaches, employing more tools to protect patient data. They monitor their IT performance more closely, and monitor their health system’s overall performance more thoroughly, as well as engaging more patients online in social media sites.

MGHF Auxiliary officers elected Mason General Hospital Foundation Auxiliary Chapter held its annual end-of-the-year luncheon in May. Every year at this time, the annual report is given and the officers for the next year are elected and installed. Auxiliary awards scholarships

At its Annual Meeting in May, the MGHF Auxiliary awarded six medically-related scholarships at the $2,300 level for a total of $13,800!

This outstanding accomplishment was all due to the wonderful support of the new Auxiliary Gift Shop. MGH&FC’s team participated in the annual American Cancer Society’s Relay for Life event June 27 and 28 at the Shelton High School track. The team raised more than $4,000 for cancer prevention and research. This year’s Relay for Life theme was “Animation Nation: Joining Forces to Fight the Evils of Cancer,” and Team MGH&FC’s theme was “Pink Panther.” Mason General was a Gold Sponsor of the Event and this is the team’s 13th year participating.

When the end was near, Sally even stayed well past the end of her shift in order to be bedside with us and my mom at the time of her passing, rather than hand us over to a new nurse in ICU. My husband and I determined, the next day, that it almost seemed like Sally had been an angel placed there for us that day. It is my hope that she will be recognized at MGH for how amazing she is. I can only hope that other families will have Sally there for them in their critical time of need. Additionally, Dr. Neal is equally amazing and his kindness and attentiveness to us during this time was incredible as well. I am so grateful for everything that he did to try and save my mom’s life.”
