CEO’s Report Diane Van Dyk Good A ernoon, A warm welcome to our guests, staff, board members and volunteers to 34th Annual General Mee ng. The theme this year in our annual review, “our pictures paints a thousand words”. The picture I would like to paint for you this a ernoon is one in which our South Coast Hospice has come through many difficult mes where we have had to adjust and change. In 2013/2014 we really had to baton down the hatches and weather the storm.

I would like to thank our proac ve board for their guidance, support and dedica on. They are commended for the role they play at South Coast Hospice. With the generosity from the community, successful grant applica ons and staff that understood the task on hand and have persevered; we can now see the light on the horizon. Our anchor throughout has been the firm founda on that was formed by our predecessors. The one fact that we are sure of, is that once emerging from the storm we will never be the same and that change is inevitable, a er all that is what storms are all about.

What has never been compromised both in good and bad mes, is the quality of Pallia ve care when we serve both the pa ents and their families in our community. Each and every person plays a vital role in achieving the best possible outcome. This service delivery also applies to the organisa ons that we partner with. The UGU District Department of Health, Department of Social Development in the UGU region, Broadreach Health Services, Kheth’impilo, HPCA and CaSIPO. Through these good rela onships we manage to access the vital needs of vulnerable people living in the UGU region. We con nue to learn much through our partnerships, both in the medical field, as well as the partnerships with our funders and the generous community.

So, how does this paint a picture? What is the reflec on that we at South Coast Hospice would like you to see. It would be a beau ful canvas painted in the brightest of colours of people in the community coming together for the greater good, each one with an open heart of compassion. What do you see in the centre of this work of art? A never ending dance of the people that we have served.

Mission Statement VISION Leaders in Quality Pallia ve Care MISSION STATEMENT South Coast Hospice Associa on is a non-profit organisa on commi ed to serving the community of the UGU Health District in KwaZulu-Natal, working in partnership and contribu ng to the strategic goals of the Department of Health We provide: •Pallia ve care to pa ents with Cancer, HIV/Aids and Motor Neuron disease •Care and support to TB/MDR-TB pa ents and families •Psychosocial and bereavement support to families and children •Training for our staff and volunteers, other health carers and the community Care is given in the pa ent’s own home, backed up by our Pallia ve Care Support Centre and local health facili es.

Chairman’s Report David Boyd Di Van Dyk and her team of staff and volunteers are to be congratulated for their commitment and dedica on, which brought about a successful opera ng outcome for the year. An important element to this outcome was the Broadreach Project, which involved up to sixtyfour contract employees counselling at thirty primary healthcare clinics within UGU district. Our team con nues to pursue all avenues of poten al donor support, a very ac ve fundraising effort and cost reduc ons. Approximately one third of support funding was derived from overseas sources during the year.

Our board is concerned about factors that could impact future funding opportuni es for Hospice.

These include both recent developments within our own country, as well as changes arising from the new administra on in the USA and Brexit implementa on. At the local level there appears to be a trend towards funding of government ini a ves and the Na onal Development Programme, training and youth development.

During the year we welcomed Nomuso Zulu and Thabo Ngundze to the board and bid farewell to Jean Du Preez, who relocated to Johannesburg. We remain commi ed to corporate governance and human capital development, including succession planning. My thanks to our board members for their contribu ons this past year, par cularly Richard Savage and Hilton Horsley for the pivotal roles they play.

To all of you in the local community and business, our thanks for the ongoing support of Hospice, which is much needed, and greatly appreciated.

Thoughts On Hospice And Pallia ve Care Dr. Hilton Horsley Over the past 50years there has been incredible progress in the field of diagnos c and therapeu c medicine and this has resulted in improved survival mes and cure rates in the face of many different life threatening illnesses. Sadly, although this progress is admirable, there are s ll many unfortunate pa ents who, either due to aggressive disease or late diagnosis, relentlessly progress into the realms of incurable disease and eventually death. This rather neglected field of modern medicine resulted in pioneering work in the six es by St Christopher Hospice in London and resulted in specialised Hospice units being set up throughout the world.

A common term used for this phase of progressive illness is terminal illness and hence the term “Terminal Care”. This is rather an ambiguous and nega ve term. For example: • Does it refer to the last hours, days, weeks or months of a pa ent’s life? Perhaps it refers to the me when cure is no longer an op on. However the pa ent may survive many months or, as in some cases, a year or more, in reasonable health.

• It is rather nega ve terminology, sugges ng that nothing else can be done. In fact, when a pa ent enters the final stages of their progressive illness, be it hours, days, weeks or months, there is always something that can be done. In fact, this is the me when the Hospice Team swing into ac on with intensified vigour the focus moves even more to the pa ent and less on the pathology. The pa ent and family take “centre stage”. A very difficult me for all concerned. Another term o en used is “Pallia ve Medicine”. Technically this refers to the medical staff’s prescrip ons, (i.e. medicines prescribed by doctors). So what would be the most appropriate term?

The world health organisa on (W.H.O) refers to this specialised field of medicine as “Pallia ve Care” an expanded defini on by the W.H.O include the following salient points.

• Pallia ve care affirms life and regards dying as a natural process • It neither hastens nor postpones death • It provides relief from pain and other distressing symptoms and integrates psychological and spiritual aspects of care • It offers a support system to help pa ents live as ac vely as possible un l death and offers a support system to help the family to cope during the pa ent’s illness and their own bereavement Pallia ve care unques onably grew out of the pioneering work of Hospice Care and resulted in increasingly skilled symptom control, support system, home care and mobiliza on of community services. The principles contained in the defini on encompass the holis c care all Hospices including South Coast Hospice, endeavour to achieve for all pa ents. There is li le doubt that appropriate treatment can make a radical difference to a pa ents’ life and death.

It is important to respect the fact that each individual pa ent is special and unique and may have diversified problems, o en referred to as total pain. This may include, Physical pain, Social Pain, Psychological/emo onal pain, Spiritual pain and o en Financial pain.

It therefor becomes clear that the Hospice holis c approach can only be successful with a well integrated mul -disciplinary team.

This cons tutes the Hospice Pallia ve care team and each team member has their own field of exper se. We also realize that a vital aspect of team work is communica on. Communica on is central to the success of Pallia ve care and this include communica on between team members, pa ent and the family.

It is not necessary for everyone to die in a specialized unit (Inpa ent unit). The Inpa ent unit’s major func on is for difficult symptom control, when intensive nursing and close observa on is necessary. The pa ent, if possible should always be party to the decision where they should be looked a er in their final days. Ideally home, in a familiar environment surrounded by loved ones, pets etc. is the ideal place to be. The Hospice team makes this possible with a well respected and loved home care team.

Finally: “You ma er because you are you, and you will ma er un l the last moments of your life and we will do all that we can not only to help you die peacefully, but also live un l you die.” Dame Cicily Saunders No ma er how challenging the final walk, we will endeavour to walk every step with you.

Home care nursing programmes

Inpa ent Unit

The majority of our services take place in the home environment. A dedicated team comprising of professional nurses, home based care givers, grief and bereavement counsellors and social workers. Care for pa ents and their families in the comfort of their own homes. There is a great need in our community as many of our pa ents lack resources and have li le support due to fragmented family situa ons. Sr. Sheila Smook our dedicated professional home care nurse, is a great inspira on to our hospice team; and is much loved in our community. We receive many le ers of thanks from family and friends expressing their gra tude and apprecia on.

Our Kingfisher inpa ent unit, in Port Shepstone, con nues to provide much needed intensive care and support to pa ents who require pain and symptom control as well as offering respite for the pa ents families. End of life care offers compassion, dignity and support at the most vulnerable of mes. Colleen Baxter ensures quality standards are maintained and we are extremely grateful for her long service and dedica on.

Pa ents living with Mul -Drug Resistant TB are provided reliable care in their homes. We commend the nursing staff and the drivers who ensure uninterrupted service to pa ents. This programme is done through Murchison Hospital and is supervised by our programmes manager, Karin Warman.

Psychosocial Our psychosocial team provides incredible support to pa ents and families facing very trying mes. Diane Christensen, our psychologist on our grief and bereavement team with her long serving volunteers are an integral part of our interdisciplinary team.

Day clinic Provides valuable cancer screening and Sr. Heidi Thom provides health and wellness services in the community. We are privileged to have her on our team.

CCMDD The Department of Health has approved South Coast Hospice as a CCMDD site. Where people requiring chronic medica on can collect their prepacked medica on from us. This eliminates long queues in over burdened clinics, whilst offering good service to our community. Nolwazi Noyingana, is in charge of this value service.

Support Groups

Mobile Clinic

Our day care group has been in existence for many years and con nues to provide care and support and friendships that have endured over many years.

The mobile clinic donated by The Victor Daitz Founda on, con nues to work alongside DOH Primary Healthcare Clinics at Marburg, Margate, Gamalakhe and Mbunde. When requested we work alongside DOH on health outreaches. Care giver/driver, Patricia Magubane, and Sr. Nobantu work relessly suppor ng the clinics. In support of the strategic plan of DOH outreaches are conducted throughout the year.

The Kingfisher tea club meets on a Thursday where and open invita on is given to the community to join us and it is an opportunity for hospice to educate the community on the benefits of pallia ve care as an holis c approach to pa ents living with life limi ng illness.

Social Workers The social workers headed by Tarryn Smith, Offers comprehensive social support to both adults and children, by working consistently together ensuring that all our pa ents needs are addressed where possible. This team has a par cular focus on orphans and vulnerable children, and Tarryn has a true passion and calling for their needs.

The Memory Box Workshops con nues to make a difference in the lives of young children who have experienced many losses. Group work has been held in different schools with various objec ves, (behavioural change, sexual health, puberty, hygiene, alcohol and drug abuse, HIV/AIDS and delinquent behaviour). This team faces many challenging cases and as with all the clinical and care staff, debriefing and sharing is of paramount importance to the work that we do.

The mobile clinic has been of great service to the community, especially regarding women’s health.

The team con nues to conduct cancer screening and many are referred for specialised care.

Duduza, the isiZulu word for comfort - each child attending our Memory Box Workshop receives a Duduza doll, a special comforter, who has big ears to listen and a closed mouth that never tells created with compassion by members of our community.

Broadreach Health Services (PEPFAR)- in partnership with DOH – June 2016 – April 2017. The primary focus for this project was HIV counselling and tes ng for treatment. Working towards the 90-90-90 strategic goals of DOH. Kheth’impilo (Global Fund) – December 2016 - March 2019. The focus is on adherence counselling and the forma on of adherence clubs to fast track service delivery with the suppor ng DOH. ViiV Healthcare – 6 Memory Book Workshops – Grief and Bereavement support for the youth infected and affected by HIV/AIDS Khula Genesis Centre – 12 months, one healthcare and support visit by our mobile clinic per month for adults and children #Ya2i ‘Youth in the know’ – Changing behaviours through promo ng healthy lifestyles

Training Department Dirk Van Reenen South Coast Hospice is an HWSETA Accredited Training provider and in 2016 we registered two skills programs with HWSETA for implementa on.

The good quality training we offer takes teamwork and we are extremely proud of our highly qualified team of Facilitators, Assessors and Moderators.

Behind The Scenes Support Services OPERATIONS Headed by Shamila Clothier who manages the opera ons of SCH. To make sure that, not only the human resources are managed well but ensuring that the vehicles are maintained, general maintenance is carried out on grounds and buildings and that policies and procedures are followed to the le er. This mul -skilled individual can definitely keep things on track.

FINANCE Alf Gore has managed this department for 11 years. Under his guidance his team’s mission of good financial governance has been proven over the years. With impeccable audi ng results and compliance, he is a great asset to our organisa on.

MONITORING AND EVALUATION M&E are cri cal for building a strong evidence base around the programmes that we offer.

Through the HDMS we are well placed to assess the wide, diverse range of interven ons that are implemented. At South Coast Hospice it is a tool for iden fying and documen ng our programmes, and tracking our progress towards common indicators across our relevant programmes.

The capture of quality sta s cs helps with iden fying the most valuable and efficient use of resources. It is cri cal for developing objec ve conclusions regarding the extent to which programmes can be judged a ‘success’.

M&E together provide the necessary data to guide strategic planning.

Support Funding RETAIL CHARITY OUTLETS In this last year the charity outlets Have had poor result. This being noted by the board, Mrs Esme Grey, Has been employed to supervise the 3 charity outlets; Kingfisher Charity shop at Sebenza Village, The Hospice Shop at Umtentweni and The Mike Northard at The South Coast Hospice Premises.

Contribu ng factors to this is reduced donated items to sell in the shops and an increase in other charity outlets being opened.

Not withstanding this, these shops have made a slight profit and it is to be noted that the outlets themselves, serve as a public rela ons support for South Coast Hospice, which is very necessary in our community.

A special thanks to the staff, volunteers, donors and of course all those who shop in our stores.

Support Funding FUNDRAISING BT and Darryl King make a formidable fundraising team. BT’s passion for raising funds to support the programmes at Hospice is seen and heard in every fundraising event. From her selling her own home made cup cakes at the Shelly Centre to the substan al Halfway Toyota/Lake Eland peddle for pa ents, she gives it 100 % ++. What a great asset she is to this hospice. Her heart for hospice shines out for all to see.

Treasurer’s Report 2016/2017 This has been my sixth year as Honorary Treasurer of South Coast Hospice. It is an utmost privilege to be associated with and to serve an organisa on as professional and caring as this. On a monthly basis, financial reports are prepared and presented to the Board. I have pleasure in presen ng my report and the audited Annual Financial Statements for the year ended 31 March 2017.

Subsequent to the mee ng a copy of these statements will be available for perusal. The results for the year under review reflect an opera ng surplus of 1 449 724. The opera ng income for the year amounted to 6 042 711 while the opera ng expenses were 4 592 987. Other income received was in the form of interest and a profit on the sale of fixed assets to the amount of 519 254.

Taking this income into account the net surplus for the 2016/2017 year was 1 968 978.

INCOME Total income for the year was 10 669 053 and is accounted for as follows:

The income generated by these divisions contributed 20.9% (2016: 19.6%) of the total income. In a very difficult year, once again the Fundraising Commi ee has excelled, contribu ng significantly to the annual income.

Once more this year the training division has contributed substan ally. The service that they provide not only contributes significantly financially but also provides a valuable service to the community.

During the current financial year training fees were 70 211 (+29.8%) higher than in the previous year.

Bequeathed funds are always highly appreciated and fluctuate annually. During the current financial year bequests were 1 323 524 (62%) lower than in the previous year.

EXPENDITURE Total expenditure amounted to 8 700 075 and included project funding expenditure of 3 767 025. Opera ng expenditure was 4 592 987, an increase of 320 781 (7.5%) over the previous financial year. All expenditure is carefully analysed and assessed. Only essen al services and goods are authorized. Heads of department are congratulated on their con nued efforts to limit expenses in all areas, despite the difficult economic mes.

Costs have been well contained and ended the year 53 847 (1.16%) under budget. It is very rewarding to be involved with a professional organisa on such as South Coast Hospice. Every person associated with South Coast Hospice, from Employees, to the Board Members, to the Volunteers, give so much of themselves. I would like to thank Mrs. Di van Dyk and Mr. Alf Gore, along with all involved in the finance department for maintaining records of such a high caliber. Once again it has been an absolute pleasure and privilege to be associated with and to serve South Coast Hospice.

Cindy Maritz In 2002 I had my Larynx removed a er T4 stage cancer was found on my vocal chords. A er I was discharged from hospital we approached South Coast Hospice for a ercare treatment, even though I was not terminal. Dr. Nell and the South Coast Hospice staff were amazing; so compassionate and prac cal.

I needed pain management for about 6 months and received professional and vital assistance, which was not available anywhere else.

This enabled me not only to heal physically but also emo onally. I had weekly visits from the psychological counsellor, Di Christensen, Sister Sheila Smook and a volunteer helper. I only went for a check up at the hospital a er I had completed my radia on and was well, I con nued with my life.

In 2014 my breathing became very difficult because my stoma was narrowing dras cally. I went back to South Coast Hospice in 2016 and once again was listened to, advised and assisted beyond compare.

The doctor, (the same Dr Nell from 2002, big up Dr Nell!) referred me to the Port Shepstone hospital, with this referral I received prompt care. I had to revert back to using a trachea pipe to keep my stoma open. The pain was excrucia ng and again I was given professional and compassionate assistance and care from South Coast Hospice.

Now, even though I am healthy I s ll a end the Day Care support group with wonderful women who have survived cancer as well as those figh ng to beat it. The fellowship is a great way to stay in touch and share encouragement and joy with each other. In March 2017 I married my awesome husband, who along with my mom was pivotal in ge ng me to go back to South Coast Hospice. A er 2 years of ba ling to breath, South Coast Hospice is a lifeline and a service beyond compare!

Inspiring Hope Memory Box Workshops for children – The children who par cipate in the South Coast Hospice Memory Box Workshops are referred by schools, hospitals, clinics, our Rural Home Care teams, and the community. Children are invited to par cipate in a 5day workshop under the supervision of a Counselling Psychologist, two Social Workers and two Caregivers.

Special emphasis is placed on memory work, with modules dealing with family stories and family trees, and memory boxes which the children can decorate. The stories of their deceased parents, photographs, important life documents, as well as various precious objects and tangible treasures pertaining to their history are placed in their boxes. Our Memory Box workshop provides HIV-infected and affected children with life skills that will enhance their resilience and enable them to cope in the face of adversi es.

Memory Book Workshops for youth and adults – Memory books are the perfect way to remember a loved one who has died. They can hold some of the most precious memories and life stories of a special person in your life. The principles and the format are followed in accordance with the Memory box workshops.

Fundraising for these workshops are ongoing as there is a dire need in our community to address unresolved grief and loss in impoverished rural areas.
