Why early detection matters to men’s urological health Consultant urologist highlights the importance of early assessment for male urological symptoms — and asks whether better information could support time-limited consultations.

round 40% of men over the age of 50 experience urological issues, such as urinary urgency, weak urinary stream or incontinence.1 Barriers to men’s urology care “Men often delay reporting their symptoms,” Dr Rees explains. “They put things off, hoping they’ll go away. Add to that the embarrassment of talking about urinary issues or sexual function, or fear of what the diagnosis might mean, and many simply avoid going to the doctor altogether.”

Stigma, embarrassment and fear are some of the reasons men may not seek medical attention. Access to healthcare also plays a role, as rising demand for urology treatment contributes to longer waiting times.2 The importance of timely assessment Delaying medical attention for urinary symptoms can allow problems to worsen over time. Early, open discussions with a doctor may enable timely identification of suitable treatment options that can help manage symptoms before they progress.

“Earlier diagnosis might facilitate choosing less invasive treatments and may support faster recovery and improved patient experience for some patients.” says Dr Rees.

While benign prostatic enlargement can cause urinary symptoms, prostate cancer often shows no early signs — making prostate-specific antigen (PSA) testing and early screening important.

He adds: “Following treatment for prostate cancer, men can be left with chronic urinary symptoms, but do not always seek help. There are effective treatments available throughout cancer survivorship.”

Accessing reliable information With the demand for urology treatments rising3 due to an ageing population, it’s more important than ever that meaningful discussions4 between doctors and patients are timely, deep

and educational.

“Accurate, accessible resources can help patients recognise symptoms earlier, and make informed, shared decisions — now the standard in care,” he says. “It is essential to access trusted resources to avoid misinformation.”

W

hen Dr Steve Allen, a 59-year-old retired anaesthetist, underwent prostate surgery after a cancer diagnosis, he was told he might have “some incontinence for a while.” What followed was far worse than he could ever have imagined.

“I remember the day I realised I’d become totally incontinent,” says Steve. “I was shaving, looked down and saw a pool of urine on the floor. My first thought was: ‘Am I going to be in nappies for the rest of my life?’”

Devastating impact of incontinence Throughout his cancer diagnosis and surgery, Steve hadn’t shed a tear. However, after his surgery, the reality of incontinence broke him. “I fell apart,” he says. “Incontinence is not life-threatening, but it affects everything. I felt hopeless, isolated, confused. All I wanted to do was feel normal again.”

Initially, Steve managed his incontinence with pads, but his condition was so severe that he had to change them around six times a day. Finally, when his situation didn’t improve, he underwent another operation a year and a half after his prostate surgery. He made the decision to move forward with a procedure to implant an artificial urinary sphincter, a device that helps control urine flow. It was, he says, a life-changing decision.

A treatment option that can change lives The result was transformational. Six weeks after the operation, at his follow-up appointment, the device was activated. On the journey home from the clinic, Steve realised the pad he was wearing was completely dry for the first time since his prostate cancer surgery. “People talk about life-changing moments,” he says. “That was mine.”

That was 16 years ago, but Steve still remembers that feeling clearly. “I’d gone from being 96% incontinent to 96% continent,” he says. “I felt I had a future. Importantly, it allowed me to have an intimate and close relationship with my wife again, without fear of leaking urine.

Importance of reliable and accessible health information Steve insists on the importance of relying on trustworthy, easy-to-understand information about health issues, to be able to act sooner. For example, he notes that while the sphincter implant operation is available on the National Health Service (NHS), few people seem to know about it.

“Join a patient support group and talk to your GP because this procedure made a meaningful difference to me.” he encourages. “It certainly changed mine.”

he toll a cancer diagnosis takes on so many lives is driving pharma companies around the world to do more and innovate faster in the oncology space.

For example, Accord Healthcare has 75 oncology and oncology-related treatments across its portfolio and provides more than one in four injectable oncology medicines dispensed in Europe. Plus, it has molecules showing huge promise in the fields of lung cancer, breast cancer and prostate cancer.

Engaging with prostate cancer patient groups Prostate cancer has been a focus for the company for over a decade. However, in the last two years, it has sharpened that focus significantly by launching innovative prostate cancer medicines.

Naturally, this type of advance in disease treatment doesn’t happen in a vacuum, explains Joe Dunford, Senior Vice President of Specialty Brands EMENA, Accord Healthcare.

To bring better, more relevant medicines to the marketplace, it’s vital for the company to engage with patient groups and advocates. “They are our first port of call because we need to understand the patient’s point of view clearly,” says Dunford.

“They provide us with valuable insights that can help improve health outcomes for patients while improving disease management for healthcare professionals.”

Because it can be so difficult for men to talk about prostate cancer, the podcast has also created its own digital Unmentionables discussion guide. This provides a list of prompts and questions that patients can personalise and use when talking to their clinical teams.

“Patients, who could be feeling stressed, want to get the most value from the limited time they have with their healthcare professionals,” says Dunford. “The guide is designed to structure the conversation in a way that’s most useful for them.”

Men must be willing to talk about prostate cancer The company also supports prostate cancer patients by sponsoring a patient-led podcast called The Unmentionables, which launched earlier this year and is hosted by prostate cancer advocate and sports broadcaster, Jeff Stelling.

“The podcast is a resource for those affected by prostate cancer,” says Dunford. “It talks about things that men don’t want to talk about, like practical and sometimes embarrassing problems that can be a side effect of treatment, such as

A patient’s experience of prostate cancer inspires support for a national screening push A prostate cancer patient hopes to educate and empower people of Asian heritage to recognise the signs and symptoms of prostate cancer and seek early assessment.

“I

n many Asian communities, there exists a cultural taboo around men’s health – especially if it involves discussing anything below the waistline, a ‘Bury your head in the sand’ mentality,” explains Sanjiv Kanwar, haematologist and prostate cancer patient.

Prostate cancer cultural barriers While approximately 150 new cases of prostate cancer are being diagnosed in the UK daily, men of Asian heritage may be less likely to seek medical advice or screening due to associated stigma, lack of awareness and reluctance to discuss health concerns – especially those thought to relate to sexual organs. This cultural barrier can lead to later diagnosis and poorer clinical outcomes.

“There’s a common misconception that prostate cancer is an ‘elderly white man’s disease’, but this is not the case.

We are seeing men of all backgrounds, as young as 40, being diagnosed with advanced cancer.”

The need for national screening Kanwar has first-hand experience of the prostate cancer journey and is now championing for improvements in prostate cancer awareness. With a family history and a

medical background, Kanwar was able to identify the signs and take early action, but he recognises the difficulties many others face in following suit. He says, “Prostate cancer can often be treated successfully if caught early. Without a national screening programme, many men, especially those in higher-risk groups or from minority communities, may not seek help until it’s too late.”

“If the NHS needs more resources to support screening, then that’s what must happen,” implores Kanwar, who calls for increased access to prostate-specific antigen (PSA) testing from the age of 40 for men known to be at higher risk, as an indicator of prostate health.

Advocacy and awareness Working alongside Accord Healthcare, Kanwar was part of the patient steering committee, which helped to shape the design of the ‘Pro-state of mind’ campaign, empowering patients to get educated, get screened and get involved.

“Reflecting on the prostate cancer journey takes understanding from different perspectives,” says Kanwar. “Healthcare professionals diagnose and treat; pharmaceutical companies develop the drugs; but the patient is at the heart of it all.”

Accord offers a range of resources, including informative videos, podcasts and doctor discussion guides, designed to improve access to prostate cancer information to enhance screening rates, particularly for those who feel hesitant to seek help in person.

“Early awareness and screening are critical,” concludes Kanwar. “The goal is simple: empower men to get educated regarding prostate health, get screened and get involved in their own healthcare

Inequitable burden of prostate cancer That system is failing thousands — and it is failing them unequally. Black men are twice as likely to develop prostate cancer and twice as likely to die from it, while men with a family history of the disease face a similar, heightened risk.

Men in poorer areas are also less likely to be diagnosed early or offered the best treatments. The result is a deepening inequality that costs lives.

Darren knows this too well. Diagnosed with late-stage prostate cancer in his 40s, this father of two young boys now fears he won’t see them grow up. His story is tragically common – and entirely preventable.

Safe path to screening today Historic concerns about screening for prostate cancer centred on fears that it would pick up harmless cancers and lead to unnecessary treatments with life-changing side effects. However, science and medicine have moved on. MRI has been a game-changer, allowing doctors to distinguish aggressive cancers from those that can safely be monitored.

Screening will save lives, and it’s safer than ever before.

It’s time for the UK to introduce a prostate cancer screening programme, starting with those men at highest risk. It is time the public demands it.

Prostate cancer in Black men: why cultural competence matters

Black men often receive a prostate cancer diagnosis later. Building trust, improving communication and training clinicians in culturally inclusive care could help close the gap.

lack men face more than twice the risk of prostate cancer diagnosis and death compared to White men. Yet, despite this stark difference, Black men are often diagnosed later, limiting their treatment options and survival chances.

More than genetics While biology may partly explain these differences — for example, Black men tend to develop more aggressive prostate cancers and, on average, have slightly higher prostate-specific antigen (PSA) levels — these factors alone do not account for the unequal outcomes. A significant and often overlooked barrier lies in the relationship between healthcare providers and Black patients. Mistrust of the healthcare system, shaped by past negative experiences and systemic inequities, discourages many Black men from seeking early screening or openly discussing prostate health.

Healthcare systems must prioritise practical training in communication skills, shared decisionmaking and address unconscious bias.

Representation and inclusion To improve prostate cancer care, the ability of healthcare professionals (HCPs) to understand and respect cultural differences must be a priority.

This is often referred to as ‘cultural competence’ — meaning the skills and awareness needed to communicate effectively and respectfully with patients from diverse backgrounds. In practice, this

involves inclusive communication, active listening and shared decision-making (SDM) that values each patient’s perspective.

Training clinicians for inclusive care HCPs of all cultural backgrounds can benefit from culturally responsive practices that foster trust and support informed patient choice. Building this trust goes beyond clinical expertise; it requires clear, respectful communication that acknowledges patients’ backgrounds, experiences and concerns.

However, some clinicians still struggle to meet these levels of understanding. Healthcare systems must prioritise practical training in communication skills, shared decision-making and address unconscious bias to close this divide. Ultimately, cultivating genuine trust through honest and empathetic dialogue is key to empowering Black men and improving survival outcomes.

The European Association of Urology (EAU) has been taking steps to address these challenges. One such initiative is the patient-reported outcome measures (PROMs) study BALANCE: Embracing Diversity in Prostate Cancer led by the EAU Patient Office, focusing on prostate cancer care for Black men of African and Caribbean descent.

Helping men make sense of prostate cancer decisions

Every year, more than 52,000 UK men are diagnosed with prostate cancer. Many find themselves having to make life-changing treatment decisions quickly — often without enough information or support, which can be frightening and confusing.

ake Sense of It’ is a new pilot project from Tackle Prostate Cancer (Tackle) that aims to change that. The programme puts newly diagnosed men and trans women back in the driver’s seat, helping them feel informed and confident about their treatment choices.

Personal advice in navigating treatment options Through a practical, evidence-informed model, trained Treatment Decision Buddies offer free, one-to-one coaching. They help people understand treatment options and side effects, explore their personal values and priorities and feel supported throughout their journey.

Each Buddy draws on their own lived experience of prostate cancer. Tackle matches individuals with a compatible Buddy — someone of a similar age, background, sexuality or location — who can listen and share information in a non-directive way. The goal is simple: to help each person feel empowered to choose the treatment that’s right for them.

250 newly diagnosed men and trans women. The pilot is initially running in Yorkshire and the West Midlands, as well as across the UK’s LGBTQIA+ community — areas where people are more likely to face barriers in accessing prostate cancer support.

With further investment, Tackle hopes to expand the project nationally, creating a sustainable model for peer coaching in prostate cancer care.

Get involved. Tackle is looking for: • Men, trans women and their partners recently diagnosed with prostate cancer (who are based in Yorkshire or the West Midlands) • Men and trans women across the UK who identify as LGBTQ+ • Volunteer peer coaches with lived experience of prostate cancer (especially from the West Midlands, Yorkshire or LGBTQ+ communities) Clinical and community partners who can refer newly diagnosed individuals for support
