Yes, Memphis has its very own It all started innocently enough.

A weekend in New Orleans.

What could go wrong?

It was 2013, and Pathologist Susan Warner traveled with her husband, Le Bonheur pediatric orthopedic surgeon, William Warner, for his Tulane Medical School class reunion. There she discovered The Goldring Center For Culinary Medicine at Tulane, founded by internist Dr. Timothy Harlan. Trained as a chef, at age 22, Harlan owned his own restaurant, Le Petit Cafe in Athens Georgia, before changing direction and a career in medicine, with medical school and residency at Emory. Dr. Harlan adopted the persona of Dr. Gourmet, publishing multiple books, appearing on media outlets, working through his website DrGourmet.com.

But Dr. Warner, Susan that is, wasted no time. Returning to Memphis Susan certified as a Culinary Medicine Specialist (CCMS) and followed that with a 13 month Associate of Applied Science Culinary Arts program through the Adolphe Escoffier School in Boulder Colorado, a professional chef program. Online, it included a 6 week practical apprenticeship with Jose Gutierrez at his East Memphis restaurant Royal Oaks.

She paired with Dr. Joan Han, Le Bonheur pediatric endocrinologist to bring the Culinary Medicine Program to Memphis.

Started in 2016 with three medical students, by 2018 UTCHS offered an elective in Culinary Medicine for fourth year medical students. The program seems firmly established: the 2019 class filled in 45 minutes. As an added jewel, the faculty now includes Nancy Kistler, professional chef that Susan worked with at Royal Oaks.

Susan grew up in East Tennessee. a professed Mountain Girl, in a family that loved to cook. She cooked in her apartment in medical school at UTCHS, met her husband Bill during residency here in Memphis. First date: Christ United Church and then, naturally, lunch‌at Houston’s.

So Dr. Warner, again Susan that is, brings an impressive resume to her program: a love of cooking, professional chef training, experience at the pathology table finding the fatty ravages of lipid and poor diet, a commitment to do something about it, to train the trainers in a new approach to nutrition.

And arguably the most important: for such a healthful diet to be embraced, it must also taste good.

So a reunion in New Orleans.

Nothing went wrong.

And Memphis has its own Dr. Gourmet.

I just have to find a place to hide that foie gras in my refrigerator.

President’s Letter

Greetings!

I am honored to be your new President and thankful to Jesus Christ for allowing me to be in this position. I recognize our roles in health care are an enormous commitment, and I thank all of you for what you’re doing every day.

You may have heard this before, but it’s worth saying again. From the beginning of its creation in 1876, the Memphis Medical Society (MMS) has upheld its mission and been the voice for important issues of the day, speaking on behalf of both patients and physicians.

What does that mean for us in 2020? We will build and modernize.

We'll improve our online activity at mdmemphis.org and look to provide data to our members regarding their online presence, to provide a business guide, and to add member resources. We'll grow MedTemps, our in-house, Joint Commission-accredited medical staffing and recruiting service. We'll increase our community partnerships and public health efforts by exploring avenues for more collaboration. Through our service, we will build relationships, while promoting our organization. As a result, we want more patients to seek our members for care. We want businesses and community leaders to seek our members for advice and expertise. Finally, we’ll continue partnering with the Tennessee Medical Association, particularly addressing hot issues such as scope of practice, which has already been a 2020 legislative win for the physician-led health care model in our state.

Imperative in our modernization is updating the bylaws and focusing on strategic planning. We want strength in numbers and voting from members concerning matters affecting the organization. A handful of docs can attempt to speak on behalf of the masses, but would that be an accurate representation of our physician community? I would say no.

So please join us with your paid/active membership. Moreover, we crucially need political action committee donations. When legislators vote on subjects directly impacting patients and physicians, who will they listen to? Let’s put our money where our mouths are and contribute to the nonpartisan Memphis Political Action Committee (MEMPAC).

In closing, I ask you to contact MMS with your concerns, questions, and suggestions.

Let us know how to represent you as well as be a resource for you. Let's take inspiration from Helen Keller who said, “Alone we can do so little; together we can do so much.”

Methodist Le Bonheur Healthcare, Urban Child Institute Introduce Babies to Books Through Pilot Program at Methodist South Hospital Methodist Le Bonheur Healthcare recently launched Welcome Bluff City Baby, a program designed to encourage reading and literacy from birth.

In partnership with Urban Child Institute and Read901, Methodist South Hospital will give every newborn baby’s family a backpack filled with early developmental reading materials. The hope is to replicate this program at birthing centers across the city.

“Literacy unlocks our potential as a community,” said Jessie Tucker, president of Methodist South Hospital. “We are giving every newborn reading materials to propel a life-long love of learning.”

“Sharing a book with your baby is a powerful parenting tool,” said Katherine Bryan, Urban Child Institute Program Officer. “It creates a positive association with reading, exposes baby to language, encourages imagination and nurtures a loving bond with the parent.”

The region’s high rate of poverty may limit access to books. Since the program launched, all babies born at Methodist South Hospital have received a backpack. For details or to donate, please visit urbanchildinstitute.org/projects. A $30 donation covers the cost of one backpack, but any amount can be donated.

Baptist Memorial Hospital-North Mississippi recently won Baptist Memorial Health Care’s President’s Quality Award for its sepsis quality initiative.

The award is the health care system’s top quality award and is given out annually to the hospital that develops an initiative that transforms and influences quality of care across the entire health care system. “It was my honor to award Baptist North Mississippi the 2019 President’s Quality Award,” said Dr. Henry Sullivant, vice president/chief medical officer for Baptist Memorial. “This initiative has elevated the quality of care at the hospital and saved many patients’ lives. I believe it will have a substantial impact on the quality of care within the Baptist system, and I look forward to seeing other Baptist Memorial hospitals implement this initiative and achieve great outcomes.”

Sepsis is the body’s extreme response to an infection and can happen when an infection you already have — in your skin, lungs, urinary tract or somewhere else — triggers a chain reaction throughout your body. According to the CDC, nearly 270,000 Americans die as a result of sepsis each year. Baptist North Mississippi’s Sepsis Champions quality team developed a protocol, or process, to ensure the consistent and early identification of sepsis followed by immediate and effective treatment. It included ensuring patients’ antibiotic therapy was appropriate and necessary. The initiative has saved 118 lives since being implemented in 2015. “We are very proud of this team of experts who are improving the outcomes of our patients who are fighting sepsis,” said Bill Henning, CEO and administrator of Baptist North Mississippi. “The team’s hard work is saving lives and will advance patient care.”

Hospital Updates

Regional One Health has opened Kirby Primary Care, at 2725 Kirby Road in East Memphis. The new location offers primary care services for the whole family, including pediatrics and midwifery services. Kirby Primary Care is the sixth primary care site at Regional One Health, adding an important eastern location to the current sites that span the city from north, south, central and downtown Memphis.

The Kirby Primary Care team includes a family medicine physician, three family nurse practitioners and six certified nurse midwives providing care for the entire family, from babies to seniors. Kirby Primary Care provides preventative medicine, physical exams, and treatment of acute and chronic conditions.

“Kirby Primary Care brings a team of health care providers together to care for the whole family in a new general medicine practice. We are a neighborhood family practice, and we look forward to welcoming our neighbors of all ages,” said Kim Bell, vice president of ambulatory services at Regional One Health. “Adding this location to our primary care network gives us the opportunity to expand our services in this area. We were able to bring primary care providers under one roof while also adding pediatrics and midwifery in this community.”

Saint Francis Hospital - Bartlett earned The Joint Commission’s Gold Seal of Approval® for Total Hip, Knee, and Spine Certification by demonstrating continuous compliance with The Joint Commission’s performance standard. The distinction marks Saint Francis-Bartlett as the area’s only hospital holding certifications in all three areas, and only one of two in the state. Over the last 12 months, orthopedic surgeons and neurosurgeons affiliated with Saint Francis have performed more than 600 spine and joint replacement surgeries at Saint Francis - Bartlett and have performed more than 1,500 joint replacement and spinal procedures at Saint Francis - Memphis.

What’s Your Reopening Plan?

Many of you have been chomping at the bit to get your practice back to work and taking care of patients. Your usual patient care responsibility is further escalated with the responsibility of prescreening those who enter your practice, or even the building where the practice is located, so that safety can be a top priority. This is our opportunity to get our community back to t0 business, and you do not want to mess that up!

Here are some thoughts on prioritizing reopening. Each of these is an opportunity to work with MedTemps to make sure you are adequately staffed.

· Pre-screening patients via phone – are you adequately staffed to do that? If not, will this only be a temporary need? MedTemps can supply the personnel to help you focus on operations inside the clinic. · Temperature screenings at entrance – this is vital. If you share a building with multiple tenants, consult your co-tenants and/or building owner. Perhaps each could share in the cost of a temporary associate to screen all who enter the building. · Getting going quickly – you will need experienced and competent staff to get you up and going as quickly as possible.

New Award Highlights an Outstanding Member At the Gala, Dr. Mancell announced a new award titled “Beyond Call.” The Beyond Call Award is given to a Society Member who has shown extraordinary leadership, initiative and courage to impact organized medicine beyond the traditional oath of service and duty. It is given at the discretion of the Board of Directors. Dr. Mancell's speech follows.

We all know the importance of recruiting and nurturing young leaders in our profession.

This future physician is not only one of those young leaders, she is already well on her way to nurturing her colleagues to be active in organized medicine. She has served in a variety of leadership roles, most notably as the Society’s student representative on our Board of Directors and for TMA as Chair of the Student Section. But where she goes “Beyond Call” is in the story of Graduate Medical Education funding. Simply stated, until 2019, increasing GME Funding was considered a dead issue by many, including the leadership of TMA. Our own Legislative Chairman, Dr. George Woodbury (who deserves his own credit for mentoring our winner and his steadfast pursuit of all Society legislative priorities) had championed this cause for years.

Then, tonight’s winner got involved, working with Dr. Woodbury, Dr. Womack, she mobilized her medical student colleagues in Memphis, and then those across the entire state. She worked to polish the message and energize her colleagues about this potential boost in funding, despite many saying it still would never happen. This all culminated with our annual Day on the Hill, which saw record turnout from medical students, which dramatically multiplied the number of voices tied to the GME Funding issue.

Thanks to this, the governor included an increase in GME funding in his budget proposal and our state received its first increase in GME funding in almost 30 years.

Spend about 2 minutes in the presence of our winner and you’ll understand why so many have applauded her enthusiasm, energy and leadership. I am pleased to present our first-ever Beyond Call award to Jennings Dooley.

Protecting your investment portfolio Q. I plan to retire next year when I turn 65 years old. I’ve heard the new SECURE Act has some significant changes that might impact my retirement situation. Should I be concerned, and how will it affect me?

A. Whenever new legislation is approved and signed into law, a period of uncertainty regarding the potential impact begins. Officially, the new legislation is titled the Setting Every Community Up for Retirement Enhancement (SECURE) Act and it became effective January 1, 2020. The bill introduced changes that will encourage retirement savings for small employers, adjust lifetime income provisions, and modify retirement plan contributions and distributions. Based on the description you provided, there are three retirement plan changes that will likely have the largest impact for you.

First, the new law moved the starting age for Required Minimum Distributions (RMDs) from age 70.5 to age 72. This change should provide retirees some extra time to prepare for RMD’s and it eliminated the confusing half year calculation. Unfortunately, the new starting age only benefits those that have not reached age 70.5 as of 12/31/2019.

Those scheduled to receive or already receiving RMDs must continue distributions as planned.

Another important change removed the maximum age limit for contributions to a traditional IRA. This is good news for those working later in life and still receiving earned income as it will allow normal and catch-up IRA contributions to continue well past the previous 70.5 age limit. The law also added an anti-abuse provision that ties traditional IRA contributions to Qualified Charitable Contributions (QCD), so that any QCD will first have to be reduced by an IRA contribution made after age 70.5.

Perhaps the most significant alteration made by the SECURE Act was the new treatment for inherited IRA distributions. The previous law provided opportunities for multi-generational estate planning by allowing beneficiaries of qualified retirement plans (non-Roth) to “stretch out” distributions over an inheritor’s individual life expectancy.

The SECURE Act now applies a 10-year distribution rule for qualified plan assets (non-Roth) inherited by non-spouse beneficiaries after 12/31/2019. Fortunately, surviving spouses are excluded from the 10-year rule. They are members of an exempted beneficiary group called eligible designated beneficiaries (EDB) that qualify to receive distributions under the stretch option. While the new law does not require non-spouse beneficiaries to take minimum distributions each year, it does require the entire account balance to be withdrawn from inherited IRAs no later than 10 years after the original account holder’s death. Sadly, this forced payout schedule decreases the opportunity for tax-deferred growth and potentially increases the income tax burden on distributions. That is a huge disadvantage for the nonspouse inherited IRA owner.

Based on all of the provisions in the SECURE Act, I recommend that you revisit your current plan with a qualified financial professional to assess your retirement strategy, craft an approach to take advantage of applicable changes, and track progress toward your financial goals and objectives.

Asbell Leads National Ocular Antibiotic Resistance Study, Publishes in JAMA The results of a 10-year ongoing study led by Penny A. Asbell, MD, FACS, MBA, FARVO, chair of the Department of Ophthalmology and director of the Hamilton Eye Institute (HEI), have been published recently in the Journal of the American Medical Association (JAMA) Ophthalmology. This surveillance study examines isolates from eye infections across the United States and analyzes trends in antibiotic resistance.

The Antibiotic Resistance Monitoring in Ocular micRoorganisms (ARMOR) surveillance study evaluated in vitro antibiotic resistance rates in ocular bacteria collected from 41 states (88 medical centers) from 2009 to 2018. The study, titled Trends in Antibiotic Resistance Among Ocular Microorganisms in the United States From 2009 to 2018, found that antibiotic resistance was prevalent in ocular bacteria (staphylococci), and bacteria that were resistant were also likely to be multi-drug resistant. Antibiotic resistance was also found to be higher in the southern and western parts of the United States, as well as more common in older patients.

“This surveillance study has given us an incredible and unique view of eye infections across a 10-year period,” said Dr. Asbell.

“We are conducting the only ongoing surveillance study of eye infections across the U.S. These data will enable clinicians to better choose the best antibiotic for each patient and scenario.”Dr. Asbell and her colleagues will continue the nationwide study, which is sponsored by Bausch + Lomb, a global eye health business. She hopes that the coming years of research will answer more of their questions, leading to even better methods for treating patients with serious eye infections and ultimately, preservation of vision.

“With over 6,000 isolates collected from eye infections, we have a wonderful opportunity to learn more about antibiotic resistance,” said Dr. Asbell. “Among research questions to be considered include possible genetic differences among the samples – information that may lead to improved diagnostics and therapies, the genetic drift of these pathogens among different areas of the U.S. and how that affects spread and resistance, and possibly understanding why some bacteria cause intra-ocular infections that are associated with permanent loss of vision, while others do not.”
