THE GREATEST GENERATION LIVES ON TODAY IN HEALTHCARE Streets lay deserted, businesses and schools shuttered, and social gatherings, weddings, and holiday festivities postponed indefinitely. A pervasive fear seized our nation, and the COVID-19 pandemic permeated the very atmosphere we breathed.

Loved ones were dying daily, and as hard as we tried, we couldn’t look away from the news. This elusive adversary left us as a nation grasping for solutions, and healthcare found itself pushed to the precipice once again. History, however, taught us that as a people we have the capability of greatness.

During World War II, our troops were engaged in a battle against a known enemy. In contrast, the COVID-19 pandemic introduced an invisible adversary that healthcare workers couldn't see but tirelessly struggled to combat. As Franklin D. Roosevelt ardently stated, “Let the strong help the weak to survive.” They displayed staunch dedication, never wavering in their commitment. These healthcare heroes made significant sacrifices to ensure the nation's care and protection from this unseen threat. World War II witnessed the magnitude of a global conflict previously unprecedented, while the COVID-19 pandemic also unfolded its assault on a global scale with extensive ramifications, including loss of life and societal upheaval. The term "the Greatest Generation" coined by Tom Brokaw is commonly used to refer to those individuals who grew up during the Great Depression and went on to serve in World War II. They are celebrated for their resilience, sacrifices, and commitment to a greater cause. While drawing direct comparisons between generations can be complex, there are some striking parallels between the values and qualities exhibited by the Greatest Generation and the modern pandemic healthcare professional.

The indomitable spirit of the Greatest Generation and the unwavering dedication of healthcare workers during the pandemic can only be described by Winston Churchill's poignant words: "I have nothing to offer but blood, toil, tears, and sweat."

The Greatest Generation heeded the call to serve their nation in a time of worldwide turmoil, while healthcare workers bravely answered the call to provide care during a devastating healthcare crisis. They bravely jeopardized not just their own well-being but also the well-being of their loved ones to care for the sick and preserve lives.

During the first year of the pandemic alone, 1,140 nurses died. More nurses than any other segment of healthcare workers that died during the COVID-19 pandemic. The mask-etched bruised faces of embattled nurses still haunt us today.

While the nature of their service is different, both groups played a crucial role in defending and preserving fundamental freedoms. The Greatest Generation defended freedoms during a time of war, while healthcare workers defended the right to life and health by combating the spread of a deadly virus. As an unknown nurse in Columbus OH distressingly expressed, “We here, at the hospital, are no longer the frontlines. We’re the last line of defense.”

Winston Churchill proclaimed, “If you’re going through hell keep going.” Both groups did just that and displayed resilience and tenacity in the face of adversity. Through it all and even today nurses, physicians and other clinicians continue to join the greatest generation of healthcare professionals.

As of January 1, 2023, the World Health Organization reports over 1 million individuals had succumbed to COVID-19 in the United States. The highest death rates were amongst our most vulnerable those aged ≥85 years. Tragically, those members of the original Greatest Generation, adding a poignant layer of irony to an already heart-wrenching reality.

While the challenges faced by the Greatest Generation and modern post-pandemic healthcare professionals are distinct, both groups share core values such as dedication to service, adaptability, resilience, sacrifice, commitment to community, and a strong work ethic. These qualities underscore the enduring spirit of individuals who rise to the occasion in times of crisis, demonstrating that the legacy of the Greatest Generation lives on in the modern healthcare professionals who continue to make a difference in the world.

EMPOWERING BREAST CANCER AWARENESS October is Breast Cancer Awareness Month, a time when we come together to raise awareness about a disease that affects millions of lives. In the United States, there are over 3.8 million breast cancer survivors, but the harsh reality is that one in eight women will be diagnosed with breast cancer in their lifetime. To shed light on this critical issue, we had the privilege of sitting down with two inspiring individuals who are making a difference in the fight against breast cancer: Dr. Alexa Elder, a specialist in breast surgical oncology, and Cathy "Chick" Halloran, a breast cancer survivor turned activist.

American Society of Breast Surgeons, practices at the Lieselotte Tansey Breast Center at Ochsner in New Orleans, LA. She shared her insights into the challenges and triumphs faced by breast cancer patients.

According to statistics published by the National Breast Cancer Foundation, "On average, every 2 minutes a woman is diagnosed with breast cancer in the United States." Sadly, this year alone, approximately 43,550 women will lose their lives to this disease in the U.S. Dr. Elder emphasized the resilience of breast cancer patients, stating, "A lot of breast cancer patients are healthy women, and this is probably the worst thing that has ever happened to them in their lives.

Early Detection: Key to Success Early detection is crucial for improving breast cancer outcomes. Dr. Elder provided valuable guidance on what to look for when it comes to detection. She stressed that many patients have no symptoms, and their breast cancers are discovered through routine screening mammograms.

Primary care doctors often use risk assessment calculators to determine the appropriate screening plan. For patients with a greater than 20% lifetime risk of breast cancer, early screenings may be recommended, and insurance companies often cover these additional screenings.

Genetic testing is also becoming more accessible and affordable, further aiding in early detection.

This organization provides vital resources and services to breast cancer patients with limited financial means. Halloran described how beating breast cancer inspired her to do more to make a positive impact.

“I marched up to Joe Gross’s office, he was the CEO at the time, I had no appointment I just went right in there and I said, Mr. Gross. I'm Kathy Halloran, people call me Chick, I raised a lot of money for that new cancer center going in, and I'm telling you what, buddy, I need to be here. You need a breast cancer survivor on staff there to help these patients and I'm your chick,” she explained.

REVOLUTIONIZING OCULAR HEALTH David Miller on Groundbreaking Care Eyesight and Aging According to Dr. Miller, the realm of retinal surgery and diseases has increasingly shifted its focus toward macular degeneration over the past few decades. This heightened emphasis is largely tied to the aging demographic. The formal term for this condition is age-related macular degeneration (AMD), which has a genetic component prevalent in specific families and is notably linked to the aging process. This requires two key factors to align: a familial history of the condition, or the inheritance of specific genes from both parents, coupled with living long enough to encounter the ailment.

MDM Healthcare VP of Clinical Excellence Linda Robinson, MSN, RN, CPXP recently sat down with David G. Miller, MD, President and Medical Director of Cleveland Eye and Laser Surgery Center, to discuss ocular health, chronic eye diseases, and groundbreaking care to slow the progression of vision loss, prevent blindness, and even restore sight. The Centers for Disease Control and Prevention (CDC) reports that approximately six million Americans have vision loss and one million have blindness.

Diabetic Retinopathy One of the health conditions observed in November is Diabetic Eye Disease Awareness Month. In the United States, 37.3 million people have diabetes, and it is also increasing at an alarming rate 1-in-10 people have diabetes. Miller states that “diabetic retinopathy is the No. 1 cause of vision loss among the working age population. “It’s very important to manage your diabetes every day, and to get annual eye exams,” Dr. Miller said “This is crucial because if it is detected there are treatments that can slow it down or even reverse it.”

Prevention Dr. Miller recommends maintaining good health to prevent macular degeneration.

Research indicates that people who exercise regularly, uphold healthy blood pressure levels, refrain from smoking, and maintain a nutritious diet with ample dark green vegetables exhibit a reduced risk of age-related macular degeneration. Certain vitamins also appear to offer protective effects, and there's a potential role for them in prevention. Dr. Miller also recommends monitoring your vision regularly starting in your 40s.

“A simple test you can do is once a week when you're reading the Sunday paper, you close the left eye and you read the headlines and do the same thing with both eyes. And if something is wrong, or those eyes can't read, go see someone.”

Looking to the Future Recently, Dr. Miller was invited to join Akron Children's Hospital Medical Center in Ohio as a retinal surgeon in their pediatric ophthalmology department. His interest was piqued by the “groundbreaking genetic treatments for children born blind.”

These treatments involve injecting a virus under the retina to alter DNA, leading to

restored vision. This type of genetic treatment is already approved, and there is more new technology on the horizon. This technology is also being adapted for adults, offering a long-term cure with a single injection.

“It's a very exciting time in medicine,” Dr. Miller said. “Genetic alteration genetics supplementation is going to be really quite remarkable over the next couple of decades in terms of both pharmacotherapy and correcting genetic defects for people.”

UNLOCKING THE SECRETS TO LONGEVITY AND WELLNESS Insights From Mohsin Jaffer, M.D. A recent report from The Washington Post has unveiled a significant demographic shift taking place in the United States. As of February 2023, more than one in six Americans have crossed the threshold of 65 years or older.

This demographic transformation mirrors the continuous aging of our population, and its consequences are both far-reaching and profound. According to a report published by the U.S. Department of Health and Human Services (HHS), 2020 Census data revealed a striking statistic emerging: approximately 17% of the U.S. population, or more than one in six individuals, were already in the 65 and older category in 2020, the highest rate in over a century.

The HHS report also sheds light on the present state of our aging population but also contains data that projects a substantial surge in the number of seniors in the years to come.

By the year 2040, it is anticipated that the population of residents aged 65 and older will reach a staggering 80.8 million, more than double the count recorded in 2000.

Furthermore, the report foresees a similar doubling of even more senior citizens, with the population of those aged 85 and older expected to grow from 6.7 million in 2020 to 14.4 million by 2040. Remarkably, in 2020, there were nearly 105,000 residents who had achieved the remarkable milestone of 100 years and older.

With our nation's population continuing to age, there is a growing demand for caregiving and support services. In a recent article published by the National Institute of Health (NIH), it was revealed that the incidence of incivility directed at nurses had exceeded 46%, with one-third of nurses reporting exposure to physical violence.

Despite nursing consistently ranking as the most trusted profession, the issue of incivility remains a persistent challenge.

Nursing leaders are taking proactive measures to address workplace incivility, and in a recent discussion, Linda Robinson, VP of Clinical Excellence at MDM Healthcare, spoke with nurse executive and incivility expert Connie Montalbo, DNP, MBA, RN, NEA-BC.

including toxic workplace behavior that ranges from eye-rolling to physical abuse, often stemming from impatience in stressful situations.

Incivility disrupts communication, a critical element in the healthcare profession.

She explained that the consequences of incivility could be devastating. "There's research out there that documented the impact, which could amount to patient harm and even documented patient death from uncivil behavior. There was high nurse turnover, PTSD, civil litigation for an organization for allowing it to go on. There were also financial costs and emotional costs."

Montalbo illustrated the intricate nature of incivility within the nursing profession, emphasizing that nurses grapple with a myriad of stressors, including the demands of patient care.

Furthermore, the workforce shortages resulting from the challenges posed by the COVID-19 pandemic have only served to exacerbate this already complex issue.

"I think there are just so many pieces to it. People come into the nursing profession from all walks of life; they bring their own personal stress into the workplace, then they face so much stress in the workplace. In addition to that, they have patients to take care of, the patients' families, the physicians, the leaders, their coworkers; there is just so much going on a nurse's plate all day every day that it is almost like a breeding ground for stress," she said.

Montalbo stated that the challenge in dealing with the root causes of incivility lay in finding healthy ways of coping with the everyday stresses nurses faced. "The challenge for us in leadership is how do we create the environment and set the expectations that will cope in a more healthy way that's respectful and that doesn't impede on the coworkers," she said.

Montalbo named stress, poor colleague communication, and passive management as the root causes of this issue. She emphasized the need to address this stress and the challenges the healthcare industry.

Robinson and Montalbo discussed an alarming cycle where management tends to avoid assigning tasks to toxic nurses, which places an excessive burden on more professional nurses, leading to burnout and impatience. This cycle could even be perpetuated by other colleagues and contributed to the stress faced by nurses.

Another issue highlighted was the phenomenon of nursing's habit of discouraging new graduates with a toxic work environment, referred to as "eating their own young" in nursing culture.

Montalbo emphasized the need to support those who were uncomfortable with confrontation.

In her doctoral project, Montalbo worked on improving nursing employees' communication by requiring disputes to be confronted within the employees' comfort zone.

One of the topics discussed was treating back pain, which, according to Brogden and Godfrey, is one of the primary concerns shared by those seeking treatment at their clinic.

“It is a very prevalent and common complaint reported by at least 65 million Americans. In fact, about 23% of the world's population suffers from back pain. Back pain can stem from something super mild to incredibly significant, and it can be very traumatic in our lives,” said Brogden.

As the evaluating therapist, Brogden's role is to determine the cause of pain. From there, she creates a treatment plan that Godfrey executes.

“We like to align the body first because when a patient is not in alignment, that can cause shooting pain into the lower or upper extremities.

After we get everything aligned, we work on strengthening the core, starting with gentle exercises before progressing,” said Godfrey.

Godfrey discussed how she builds on the progress patients make with each session.

As the treatment progresses, patients are gradually challenged to try some Pilatesinspired moves to build core strength.

Both Brogden and Godfrey agree that patients need to feel involved and in control of their treatment.

“I feel like that brings a lot of comfort to a patient, knowing where they are in the process and how we’re going to heal it,” said Brogden.

New tools have recently emerged to help patients in the office. Godfrey is trained in both Augmented Soft Tissue Manipulation (ASTYM) and Graston. These techniques find restrictions in soft tissue and can regenerate them at an accelerated rate.

While stimulation was a popular technique in the past, Brogden noted that the team uses a range of techniques in their treatment plan, including dry needling.

Dry needling is a technique similar to acupuncture, where small needles are inserted into muscle “trigger points” to relieve pain and irritation. Brogden has recently been certified in dry needling.

Brogden and Godfrey also emphasize patient education, recognizing the importance of keeping clients well-informed about their progress.

Brogden stressed the importance of teaching patients exercises that can be completed in their own homes. This is made easier with an online platform that includes videos on how to exercise correctly and information on the therapy’s benefits.

This is an invaluable tool for patients because good form and proper body mechanics are some of the biggest defenses against back pain. Brogden urged back pain sufferers to consider that daily movements can cause worsening damage if patients are not careful.

According to Godfrey, education is a key part of their patients’ success.

To generate their healing images, Envision Kindness took a creative approach.“We had to build our content from scratch. The way we ended up building it was through creating an international kindness photography contest. ”We’ve done four iterations of it, and we are actually about to launch the fifth.

We get over 2,000 submissions from around the world each year,” he said. Dr. Fryburg further explained some of the science behind kindness. “Since I’m a scientist, I decided to do more than produce the images; I also studied their impact. We were very fortunate to have a team of psychologists help conduct our first study using still images of kindness.

Oncologists are tasked with verifying the presence of cancer through precise and thorough examinations, including biopsies, imaging studies, and other diagnostic tests. Only once the diagnosis is firmly established can healthcare providers embark on a well-informed journey to address the condition and provide the most suitable care.

2. What is the type of cancer, and what is the stage of the disease?

Understanding the specific type of cancer and its stage is paramount in tailoring a patient's treatment strategy. Dr. Jazieh emphasizes that not all cancers are the same; they vary significantly in their behavior, growth rates, and responsiveness to different therapies.

Knowing the type of cancer allows medical professionals to select the most appropriate and targeted treatments,

such as chemotherapy, immunotherapy, or surgery. Additionally, determining the stage of the disease, whether it's localized, regional, or metastatic, provides critical insight into how advanced the cancer is and guides decisions regarding the extent of medical interventions. By precisely characterizing the disease, oncologists can offer patients the best chance for successful treatment outcomes and improved quality of life.

Here, considerations encompass the patient's overall health, wishes, and available treatments, both cancer-directed and supportive therapies.

5. Is the patient a candidate for clinical trials?

Dr. Jazieh stressed the importance of not viewing clinical trials negatively, as they offer hope for patients, especially those with incurable diseases. He encouraged patients to seriously consider participating in trials, as they often lead to breakthroughs in treatment.

Dr. Jazieh emphasized the significance of continuously questioning the diagnosis, stage, and treatment options, as diagnostic errors can occur.

STRATEGIES TO EXPAND PHARMACY’S ROLE IN PATIENT EXPERIENCE with Angela Brunemann, PharmD Pharmacies have faced several challenges recently due to a variety of factors, including heightened competition, evolving consumer behavior, staffing challenges, and retail-related issues.

According to a recent Washington Post article, saw national pharmacy chains, including Rite Aid, Walgreens, and CVS, on a path to close numerous stores. These closures are primarily driven by economic considerations, changes in the retail landscape, and shifting consumer preferences.

The article reports that concerns arise among public health experts regarding the emergence of "pharmacy deserts" as a result of these closures. Pharmacy deserts refer to areas lacking accessible pharmacies, disproportionately affecting communities in need and exacerbating health disparities. The potential impact of these closures remains a significant concern for public health experts.

Many experts are now touting technologyenabled pharmacies.Incorporating technology into the pharmacy setting can empower pharmacists and technicians to provide better care to patients, enhancing job satisfaction and patient health outcomes.

Recently Angela Brunemann, PharmD, Director of Business Development for a prominent pharmacy care services company, discussed the role of technology in the patient experience on our podcast PX Space.

In the interview, Brunemann noted the challenges brought on by the COVID-19 pandemic.

“During the pandemic, many pharmacies faced closures or reduced hours, limiting their ability to reach patients. As a result, patients encountered difficulties accessing their necessary medications. The reduced availability of pharmacy services has created a pressing need to ensure ongoing accessibility to our patients. The question at hand is how we can harness technology to address this issue? Telepharmacy offers promising opportunities to broaden our reach and ensure patients have continued access to their essential medications," she said.

Brunemann emphasizes the importance of accessibility and education in patient care, particularly in hospital settings, where telepharmacy can help bridge gaps in access.

Patients face additional barriers to accessing medications as up to 45% of Americans do not fill prescriptions because of cost. This is also costly for hospitals as medication non-adherence annually leads to $100-$300B in excess morbidity and mortality. Not filling a medication prescription is considered primary non-adherence.

Brunemann noted that in the new healthcare landscape, care providers can now leverage telehealth and telepharmacy to bridge the gaps in communication and reach patients who may not have had access through traditional means. This necessitates a fresh approach to patient outreach, emphasizing the importance of education and fostering in-depth conversations to address patient inquiries on their own terms.

Linda Robinson, MSN, RN, CPXP, Vice President of Clinical Excellence at MDM Healthcare, highlighted the multifaceted reasons behind patients' medication non-adherence.

She also noted that telehealth allows patients to choose the most suitable time and part of the day for educational interactions, promising not only a more significant impact but also improved overall outcomes.

“It could be a lack of medication understanding, insufficient funds, poor communication, limited health literacy, and even something as simple as an inability to get to the pharmacy.”

Brunemann emphasized the significance of providing patients with this level of access and information within a hospital setting.

And a lot of people call those moments of truth because it's those touch points from the very beginning, that kind of set the stage for the patient, and then start to ask the question, okay, now how does pharmacy have an impact in that, because that's where we find new opportunities to impact patient experience,” she said.

Brunemann stressed the importance of developing programs centered around patients’ needs and using technology developed around that information. She also stressed that the design of patient engagement technology is important.

“We want technology to acclimate to the patient's environment. So it isn't generic, and it feels like it was almost like a concierge. This is your personal pharmacy technician,” she said. She also emphasized timing, personalization, and relevancy as key aspects of effective patient engagement technology.

Meds to Beds with Journey PX Journey PX has the ability to create a medsto-beds program that is customized to the unique needs of hospitals. Journey PX can integrate with the EMR and engage the patient at the bedside via prompts, medication education, and pharmacy information to drive medication health literacy.

There are several features that would frame the program: Auto assignment of medication education upon medication order, Meds to Beds participation prompt, notification via text, email, or EMR to the pharmacy that the patient would like to participate in the meds to beds program, pharmacy medication counseling visit in person or virtual via video Connect.

Filling patient prescriptions in-house also keeps revenue in-house that would otherwise go elsewhere. Upon admission when the patient TV is turned on a prompt will appear asking the patient if they would like their medication prescriptions filled prior to discharge. Choices of “yes” or “no, thank you” are offered. If the patient chooses yes a thank you response appears and notifies the patient of the program process.

Most often a pharmacy representative will be in to speak with them. Some hospitals personalize these messages with staff photos and a time frame to expect a visit. If the patient chooses a no response they receive a message thanking them and letting them know that if they change their mind to let their nurse know and the pharmacy can be notified or they can find the meds to beds program under the patient services tab on the Journey PX My Stay home screen and opt in to participate there at any time during their stay.

Once the prompt is received by the pharmacy a time would be scheduled to review the patient's discharge medications either in person or virtually. This scheduled time could appear on the patient’s My Day Today board (digital whiteboard). If virtual the patient could be alerted 15 minutes prior with a prompt via their TV to let them know they have a virtual visit scheduled and to be ready to take the call. Upon discharge, the medications are delivered to the patient’s room. Throughout the patient's stay as medications are ordered by their physician in the EMR Journey PX can auto-assign corresponding medication education content to be sent to the patients in room MY Stay and My Day Today solutions on their TV.

A red badge alert will be displayed in the My Education section alerting the patient that they have the education to view. In addition, when the medication is ordered the patient will also receive prompts to engage and encourage them to watch the education with the choices of “now” or” remind me later”. If they choose to remind me later the prompt will reappear again at predetermined intervals until the education is completed. The completion and understanding response can then be documented back into the EMR.

As the pharmacy industry faces challenges, opportunities lie with a focus on technology-enabled solutions and the importance of accessible and patient-centric care.
