Aged Care Services

MLHD and My Aged Care work together to deliver a range of services to our residents. Some of these require an Aged Care Assessment through the Aged Care Assessment Team (ACAT). Visit the My Aged Care website on www.myagedcare.gov.au to learn about different types of care.

AGED CARE ASSESSMENT TEAM (ACAT)

ACAT are teams of medical, nursing and allied health professionals. Their role is to conduct a comprehensive assessment to assess the needs of the frail older person over 65, or over 50 for Aboriginal or Torres Strait Islander (over 50 for ATSI) and facilitate the services that best meet their needs.

• Help at Home

• Short Term Care

Residential Respite And Permanent Care

Entering an aged care facility for a short period of time (respite) or longer, permanent care.

Short Term Restorative Care

Short-term restorative care is delivered by a team of health professionals designed to help you reverse or slow the difficulties you are having with everyday tasks.

It could involve a program of activities and exercises to improve strength and balance. It may also mean arranging access to mobility aids or installing minor modifications around the house such as bathroom rails; or arranging personal care and assistance.

TRACS (TRANSITIONAL AGED CARE PROGRAM)

This service is a type of short term care (up to 12 weeks) that helps improve your health after a stay in hospital.

The referral needs to be done whilst in hospital. This service can include:

• Therapy Services

• Clinical Care eg. wound care, medication assistance

• Personal Assistance like bathing, dressing, etc.
