Southern New Hampshire Medical Center began as an eight-bed emergency hospital in 1893 and has grown to an acute-care facility that retains the personal touch of a traditional community hospital. As a clinical affiliate of Massachusetts General Hospital, SNHH provides its patients with access to collaborative programs in pediatric specialties, cancer care, trauma, and the management of stroke.

As well as a critical-access hospital, Valley Regional health care professionals are available to address cardiac care, childbirth, health care careers, hospital services, pediatric/child care tips, weight control and more. Don’t Just Sit There MOUNTING RESEARCH POINTS TO THE HAZARDS OF SITTING

f you had to take a guess, how many hours would you say that you spend seated?

Thanks to the prevalence of desk jobs and technology, most Americans aren’t moving much. A typical day often consists of a sedentary commute to and from a sedentary job, seated mealtimes, unwinding on the couch while watching TV at the end of the day and bed. All told, some of us spend upwards of 90 percent of our waking hours in a seated

position, says Amy Harrison, MPT, a physical therapist at Southern New Hampshire Rehabilitation Center in Nashua.

Have we in effect become sitting ducks?

Mounting research indicates that too much seat time can seriously sabotage health — and even ardent gym rats are not off the hook on this one.

“Numerous studies have shown that even if you have adequate exercise, leading a seden-

PREVENTION (continued) tary lifestyle — that is, sitting for more than six to eight hours [per day] — will lead to poor circulation and cardiovascular risk,” says Syed Peeran, MD, a vascular surgeon at Appledore Medical Group-Coastal Cardiothoracic and Vascular Surgery, based out of Portsmouth Regional Hospital.

Prolonged sitting causes blood to stagnate, and has been linked with a range of health problems that include varicose veins, leg swelling and pain, obesity, heart disease, diabetes, cancer and premature death.

While it isn’t always easy to avoid extended periods of sitting, there are ways that even 9-to-5 desk jockeys and cramped airline passengers can minimize the harmful effects of too much seat time. The key is to be as active as possible, and keep the calf muscles working, because they help propel blood upward toward the heart. Peeran recommends the following: • Take a break from sitting at least once every hour. “The more frequent, the better,” Peeran says. Take advantage of opportunities to get up and move. league who is in a different part of the office, for example, rather than sending an email or text message, get up and walk to the person.

• If you are preparing to embark on an airline trip that is three hours or more in duration, slip on compression stockings the day of the flight. Both medical-grade and over-thecounter compression stockings provide effective external support, Peeran says.

• If you can’t easily move around due to travel or other circumstances, do toe lifts, calf raises and leg extensions (straightening the

If you are on a plane and it’s OK to unbuckle, stand and walk up and down the aisle.

• If you can, use a treadmill desk. Despite the popularity of standing desks, they do not prevent pooling of the blood, Peeran says, and are not an effective way to avoid most of the health hazards that come from prolonged sitting. “You have to keep your legs churning,” Peeran says, “and keep your muscles contracting.”

• Consider investing in a Fitbit or other activity tracking device, or downloading an app (some of which are free) to raise your awareness of how many — or how few — steps you take each day, and to digitally poke you when it’s time to get up and move. “I think they’re wonderful,” Peeran says of such aids. Tracking devices and apps raise patients’ awareness, make them more likely to exercise, and provide valuable information to doctors about patients’ walking ability and exercise tolerance, Peeran says. Having a visible, running tally of activity can motivate even reluctant exercisers.

“It’s a constant reminder” on your wrist, belt or screen to walk more, Peeran says. Without it, he adds, “the day can just pass by and you won’t really notice how much you’ve walked or how much you’ve sat.”

Many people who do not have health restrictions aim for a minimum goal of taking 10,000 steps per day. Before beginning a walking program, however, talk to your primary care doctor, Peeran says. SHNH

Coping with Breast Cancer More hospitals today provide an array of support services and programs

As more women survive breast cancer, in part due to early detection, more hospitals are adding programs and services that address the full spectrum of difficulties breast cancer patients typically face, from the physical to the emotional and the practical.

Although every breast cancer patient’s experience is unique, there are common challenges, says Jessica Ryan, MD, FACS, a breast surgeon at Elliot Breast Health Center in Manchester. For starters, there is the emotional trauma of the breast cancer diagnosis itself. It can be a lonely time, Ryan says. Even a woman who has a robust support system with many involved family members and friends can feel alone and isolated as she struggles to emotionally process the diagnosis and its potential consequences.

From there, challenges quickly mount.

Patients can be overwhelmed by the cancer diagnosis and struggle to understand their treatment options. They can find it difficult to access care if they live in a rural area. Patients must contend with an altered body image and chemotherapy side effects such as nausea, fatigue, hair loss, nerve damage and impaired heart function. The quality of their sleep can deteriorate, leaving them more vulnerable to depression.

Additionally, the financial burden that comes with a cancer diagnosis can be a tremendous concern for patients, says Lisa

Delahanty, APRN, manager of the Concord Hospital Breast Care Center.

Even medication that is covered by insurance can cost upwards of $100 per month, and patients often worry about missing work and having inadequate healthcare coverage.

Difficulties related to sexual intimacy can also develop, says Sharon Gunsher, MD, FACS, medical director of Concord Hospital Breast Care Center and general surgeon at Concord Surgical Associates. Chemotherapy renders some women menopausal — an experience that is particularly difficult for younger women — forcing those patients to contend with hot flashes, weight gain and other menopause symptoms in addition to everything else they are going through.

And the anxiety that begins upon hearing the cancer diagnosis can stay for life. Some patients describe experiencing PTSD-like symptoms when the time comes to report for their first mammogram following cancer treatment. “I’ve had many patients tell me they worry that it’s going to start the whole thing all over again,” Gunsher says. “It’s

And the anxiety that begins upon hearing the cancer diagnosis can stay for life. something they really struggle with.” In fact, some patients never return for mammograms and follow-ups after treatment ends, Gunsher says, apparently choosing to risk that a problem will go undetected rather than subjecting themselves to the anxiety that the appointments will trigger.

The good news is, many hospitals recognize the significant emotional and physical upheaval that some diagnoses can bring, and offer a wealth of “survivorship” services and activities.

Survivorship focuses on life post-treatment, with a goal of helping patients not just survive but thrive. With the increasing attention and interest in this aspect of cancer care, “survivorship is becoming almost its own

>> specialty” within the medical field, Ryan says.

A sampling of survivorship programs at Granite State hospitals includes anti-cancer lifestyle courses that help patients make beneficial changes in their lives; talks given by prosthetics professionals who explain options after breast cancer surgery; and lectures, activities, and support groups that connect patients with each other as well as healthcare experts, and ease stress and anxiety.

Some hospitals also offer programs for care providers and family members.

Although the more social gatherings such as support groups are not everyone’s cup of tea, many women find them well worth the time.

“Patients get a lot of support from hearing each other’s story,” Ryan says. “It just makes them feel that they’re not alone. They realize, ‘Oh, I’m not the only one going through this,’” which can be a source of great relief and comfort.

And research shows that the mind-body connection is real, providing evidence that support groups alleviate stress, improve quality of life, and even reduce the risk of breast cancer recurrence, Delahanty notes.

— Jessica Ryan, MD, FACS While mind-body initiatives and other complementary therapies have clearly gained followers, Ryan cautions that complementary approaches should augment — rather than replace — conventional cancer care. But complementary approaches, she says, can provide women with an opportunity to focus on what should be their top priority: themselves and their well-being. Asked to name the most important person in their life, she says, most breast cancer patients name their husband or children. “They’ll list 12 people before it gets down to them,” Ryan says. “I tell them, ‘With this diagnosis, that needs to change. You have to be number one.’ And I think a big part of investing in survivorship programs and activities is really making that distinction.” SHNH

LOWER YOUR CANCER RISK THROUGH DIET AND EXERCISE To help keep cancer at bay, women should be sure to enlist two powerful allies: diet and exercise.

Sticking to a healthful diet and exercise plan “is hard,” acknowledges Sharon Gunsher, MD, FACS, medical director of Concord Hospital Breast Care Center and general surgeon at Concord Surgical Associates. “But it’s really important,” she says, “and I don’t think people realize how beneficial it is, not just for reducing the risk of breast cancer but for reducing their risk of heart disease and stroke and other types of cancer.”

Many people are also not aware, Gunsher says, that getting beneficial exercise needn’t involve arduous hours at the gym, but can be as simple as walking the dog for 30 minutes, five days a week.

“People want to take a single pill [to prevent cancer], but what’s really important is diet and exercise — and all of those things that help people lead a better-quality, longer life,” Gunsher says.

Choosing when to buy organic food

ating healthy sounds like a relatively simple task.

We know that we should swap pizza for salad or lasagna for lean protein and vegetables, but not everything is so easy. Things can get complicated when a stop at the supermarket to grab chicken finds you standing before an array of packaged poultry bearing labels that trumpet products as antibiotic-free, hormone- and steroid-free, pasture-raised, GMOfree or organic. You hesitate, wondering whether the organic chicken will pretty much cover all the bases and therefore amount to the best choice — and whether the organic item is worth its higher price.

Food labeled as organic is not, in fact, always the best choice, but it can be the right choice for a variety of reasons. In deciding whether it makes sense to dig into your wallet to buy organic food, experts say, let the labels and your values be your guide.

First of all, consumers should know that they can trust the “USDA organic” seal. “If [food] is labeled ‘USDA organic,’ it follows a very certain, very rigorous set of standards that are related to how the food is grown and the practices of the farm,” says Miriam E. The guy down the street, for example, who raises chickens in his backyard and puts out a handmade sign advertising his “organic” eggs (or even a larger local farm that does the same) might indeed be selling organic eggs, but then again, might not be.

Seal or no seal, however, there is much to gain from buying locally produced food, whether the food is truly organic or not. Not only will local food be fresher and therefore likely nutritionally superior to similar products that have been shipped overseas or across the country, Nelson says, local food is usually much more sustainable and earth-friendly, and tastier than food that has traveled from afar.

MEET THE “DIRTY DOZEN” If you’re concerned about pesticides in your food, you can grow your own, or you can shell out the extra money that is typically charged for organic options at a store or farmstand.

But whatever you do, don’t let concerns about pesticides prevent you from getting good nutrition. When it comes to eating non-organic produce, “the health benefits outweigh the risks,” says Michelle R. Smith, MS, RDN, LD, nutrition counselor and wellness educator at Concord Hospital. Most Americans, Smith notes, do not consume the recommended amount of fruits and vegetables.

If you like the idea of buying organic food but can’t or aren’t always willing to pay a premium price, consider being strategic in your organic purchases. For example, become familiar with the Environmental Working Group’s (EWG) “Dirty Dozen.” Each year, the EWG, a nonprofit advocacy group, ranks popular fruits and vegetables that are most highly contaminated with pesticides. Last year’s “winners” are listed below. Keep them in mind if you want to stretch your organic dollar by choosing organic when it makes the most sense to do so.

Topping the EWG’s list for 2017: says. She cautions, though, that consumers should trust their intuition and their eyes when judging the overall quality of a farm; if a farm appears unkempt, shop elsewhere. But generally, if a product is sourced regionally or locally, “I don’t really care if it is organic or non-organic,” Nelson says. “I care that I’m supporting the open spaces in my region and the local farmers, and most farmers are doing their darnedest to do the best practices they can — especially in the state of New Hampshire.”

Consumers should also bear in mind that organic food is not necessarily healthful:

Deep-fried organic potato chips are not good for you, nor is organic soda. Avoid letting labels confuse or distract you from making smart choices, and instead look carefully at all the facts on labels, including the nutrition panel, when purchasing food.

Some individuals prefer to buy organic at every opportunity based on principles related to factors such as the environment and animal welfare. But since research has not proven that organic is always better for us, Nelson says, consumers who can’t or don’t want to always buy organic might want to decide on a case-bycase basis. They might, for example, consider buying organic strawberries, grapes and apples rather than their non-organic counterparts, because those foods routinely make the Environmental Working Group’s “Dirty Dozen” list of pesticide-laden fruits and vegetables.

Many local farmers use sound farming principles, she says, “regardless of whether they are organic or not.”

And Nelson urges consumers to remember that their buying habits have implications beyond nutrition. “I hope people when they’re thinking about this [know that] it’s not only about their own health, but it’s also about keeping open spaces, and thinking about the people that are growing this food,” Nelson says.

“The upstream and downstream piece of this is really important to be conscious about. It’s our rivers and everything else. It’s not just about us, but also about the environment.” SHNH
