MAP USA Providing Hope Close to Home

The Problem More than 65 million Americans – nearly one in five – are uninsured or underinsured, leaving them vulnerable to one of the most pressing health inequity issues in the country: lack of access to essential medicine.

Patients living in poverty or working low-wage jobs often face impossible choices between food, rent, and life-sustaining prescriptions.

The consequences are staggering, 21% of adults report skipping or rationing medications due to cost (Kaiser Family Foundation study, 2023). Medication nonadherence contributes to an estimated 125,000 deaths and 10% of hospitalizations in the U.S. each year (Annals of Internal Medicine, 2018). Chronic diseases go unmanaged, leading to avoidable emergency room visits, worsening health outcomes, and preventable deaths.

At the same time, billions of dollars of safe, high-quality surplus medications are destroyed each year instead of reaching those who need them most.

Our Solution MAP USA is a bold solution to the nation’s medication access crisis. The program transforms surplus FDA approved medicine, health supplies and devices, along with purchased medicine into life-changing therapies for low-income, uninsured, and underinsured individuals.

MAP USA desires to operate at a national scale, delivering free medicine directly to communitybased clinics and pharmacies that serve the most medically underserved populations.

Building on nearly a decade of success through MAP ’s Domestic Medicine Program, MAP USA is positioned to become the largest charitable medication access network in the United States.

MAP is seeking a one-time philanthropic investment of $448,000 to expand MAP USA on a national scale. This is a moment of opportunity.

Impact Goals The startup investment requested in this proposal will fund the essential infrastructure, staffing, and systems required to expand MAP USA and position the program for national scale.

Over the 12-18-month startup phase, MAP USA will: 1. Serve at least 400,000 thirty-day prescriptions to uninsured and underinsured patients through partner clinics and pharmacies by the end of Year 1.

2. Engage and onboard 10-15 new beta clinic partners and seek to transition all 130+ existing partners to updated agreements, compliance protocols, and the new ordering platform by Month 18.

3. Establish a core formulary of 30+ essential medicines and secure a minimum of three new manufacturer or distributor sourcing agreements by Month 12.

4. Achieve a 4:1 return on philanthropic investment in medicine value delivered to partner sites by the end of the startup period.

5. By Year 5, maintain a 95% partner retention rate among clinics and pharmacies enrolled in MAP USA during the startup phase.

These startup deliverables will lay the groundwork for MAP USA to expand medicine access to hundreds of thousands more patients in the years ahead.

By Year 5, MAP USA anticipates: • Serving 230,000 patients annually through 380+ partner clinics and pharmacies.

• Distributing $200 million in medicine-value annually, with emphasis on chronic disease and maternal health.

• Improving medication adherence, lowering avoidable ER visits, and increasing clinic capacity through consistent formulary access.

Accountability From the startup phase onward, MAP USA will operate under a robust monitoring and evaluation framework: • Real-time tracking of platform performance, medicine movement, and patient impact.

• Quarterly partner and funder reports on utilization, geographic reach, and medicine value delivered.

• Baseline and ongoing data collection on chronic disease medication adherence and distribution patterns.

• Engagement of an independent evaluation partner in Year 3 to validate outcomes, assess scalability, and recommend improvements.

What We Have Done Since launching our Domestic Medicine Program, MAP has: • Built a trusted national network of 130+ partner clinics in 9 states, delivering no-cost medicine and health supplies to uninsured and underinsured patients.

• Expanded clinic reach to over 1,000,000 patient treatments, supporting chronic and acute care in medically underserved communities.

• Delivered over $4 million worth of medicines and healthcare supplies, ensuring each clinic receives resources tailored to its community’s needs.

• Strengthened the healthcare safety net by lowering clinic overhead costs, improving medication adherence, and enabling consistent care for conditions like type 2 diabetes and hypertension.

• Reached diverse and high-need populations, with more than 80% of supported clinics serving migrants, refugees, or people experiencing homelessness across rural, suburban, and urban areas.

Planned Investment To successfully launch MAP USA, MAP is seeking a one-time startup investment to build the core infrastructure required for national expansion. This includes establishing robust IT systems, ensuring legal and regulatory compliance, onboarding dedicated startup staff, and configuring logistics and distribution infrastructure.

MAP’s strong financial controls, experience managing complex in-kind logistics, and proven ability to steward foundation funding ensure that this investment will be deployed with efficiency, transparency, and accountability. MAP USA is ready to scale in alignment with foundation priorities and deliver measurable, lasting impact to communities most in need.
