WILLIAM DOUGHERTY
William Dougherty affectionately known as Billy Doc to all that know him. Billy is the proud owner of “Billy Doc’s Diner” located at 4056 H Street, here in Philadelphia.
During a time when civil unrest was high and Covid-19 was rampant, many businesses in the private industry were struggling; trying to navigate and figure out how to keep their doors open so their own employees could be paid. But Billy Doc……., he also thought about the men and women in blue.
Billy saw the officers on the street working long hours facing very dangerous and difficult issues, with little to no availability for breaks or refreshments. He then reached out to Lodge 5 volunteering his services to feed our officers at minimal cost. Sometimes at no cost at all.
Billy, on behalf of the men and women of the Philadelphia Police Department, we want to show our appreciation to you, your family and your employees for your generosity and support during a very difficult time.

STEVEN OSIECKI MARVIN VARGAS
During the summer of 2020 our city was plagued with civil unrest and lawlessness. Cops were working hours on end with no breaks or place to take a rest at times.
Steven Osiecki and Marvin Vargas, a broker and realtor of Remax Access located at 100 Spring Garden Street graciously opened their office despite being scheduled to closed to allow Officers to their facility. The officers were allowed to use the restroom, each lunch and simply rest if needed.
Steve and Marvin, we sincerely appreciate your hospitality and unwavering support you’ve extended to the men an women of the Philadelphia Police Department during their time of need!
RALPH TETI RICK POULSON
Ralph Teti and Rick Poulson are the attorneys who fight for our rights in collective bargaining and ensure the city adheres to our contract. At a time when there is open hostility in society toward police officers, they have stood up and defended our membership. In the toughest contract fight in years, Ralph and Rick were able to obtain 9.5 % raises and maintained our superior medical coverage and pensions. They also achieved parental leave for our members and their families. In addition to working hand-in-hand with John McNesby and the FOP Leadership, they are also litigating various lawsuits against the city, the District Attorney and City Council to protect our FOP Members. The working men and women of the FOP and their families are fortunate to have dedicated and experienced attorneys Ralph Teti and Rick Poulson and their legal team at Willig Williams & Davidson standing behind us. We thank them for always having our backs.


2nd DISTRICT P/O Sammy Sanchez #9671 P/O Joseph Basciano #1114
On August 15th, Officers Sanchez and Basciano responded to the 5300 block of Howland Street for a male armed with a gun. As the officers arrived on location, a witness stated the male was threatening to kill himself and is known to have several firearms inside. When the officers approached, the front door was open and the complainant was inside screaming obscenities. The complainant was yelling that he wanted the police to kill him and that he didn’t want to live anymore. The officers quickly took up tactical positions and attempted to establish a rapport with the complainant. The complainant continued to antagonize and curse at the officers, stating that he wasn’t going anywhere and that they were going to have to kill him. The complainant started to pace back and forth and was getting more and more agitated as time went on, begging the officers to just kill him already. Unsure if the male was armed with a gun or had firearms in close proximity, the officers continued to try to deescalate the situation as they continued to hold their tactical positions. Through patience and a calm demeanor, the officers were finally able to convince the male to exit the property and surrender to police unharmed. The complainant was transported to a medical facility for treatment. A subsequent search of the property yielded the seizure of four (4) handguns and a shotgun. Nicely done!
2ND DISTRICT P/O Arthur Jones #1756 P/O James Taylor #9916
On August 11th, a female operating a Mazda was carjacked point of gun in the area of 5700 Howard St. The description of the vehicle was broadcast citywide over police radio. A few hours later the complainant spotted the male who carjacked her operating her vehicle in the area of Colgate and Cheltenham Ave. The complainant notified 911 and Officers Jones and Taylor responded to the area. The officers vigorously search of the area and located the vehicle. The officers stopped the vehicle and conducted a felony vehicle investigation. The offender was positively identified and taken into custody. Recovered was a firearm, further investigation revealed the fire was stolen. These officers were able to take a dangerous individual and a stolen firearm off the streets of Philadelphia.


2ND DISTRICT Lt. Marc Rutizer Sgt. Nicholas Cione P/O Brian Williams P/O Anquistcar Topf
On August 14th, Lieutenantt Rutizer was flagged down in
the 6300 block of Revere St. by a woman who was holding an infant. The woman was screaming “My baby’s not breathing” . The Lieutenant quickly assessed the infant and observed that the child was not breathing turned blue. He immediately notified radio and began CPR. Officer Williams arrived on scene and took over the CPR. At this time Sergeant Cione arrived on location and the decision was made to have the infant immediately transported to St. Christopher’s Hospital. Officer Topf who was also on location, tried calming the mother down and put her in front seat of the patrol car, while Sergeant. Cione and Officer Williams were in the back continuing CPR on the infant. Eventually, Sergeant Cione and Officer Williams were able to remove an obstruction from the infant’s airway which allowed the infant to began breathing his own. The officers then arrived at the hospital where the infant was able to receive further treatment.

5TH DISTRICT Sgt Andre Simpson # 492 P/O John Mulqueeney # 9844
On Friday, August 21st at approximately 8:30pm, Officers Mulqueeney and Sgt Simpson responded to the Wissahickon section of Fairmont Park for an injured female hiker just of the yellow trail. While hiking in the afternoon the complainant fell hitting her head and was knocked unconscious for hours. Hours later when she regained consciousness, she was able to call 911 and relayed to them what trail she was on but could not provide much more information. Officers Mulqueeney and Sgt Simpson are very familiar with the trails in this section of Wissahickon Park. Due to this they were able to navigate in the dark. Using their flashlights and with their personal knowledge they mapped out a search area navigating uneven grounds, steep slopes and various pathways for miles. When they located the hiker, she was off the trail in the woods. Curled up in a ball and shaking uncontrollably. It appeared she was in shock. Her injuries had caused her to become immobile. The Officers successfully directed medics to the location. The complaint was then carried out to the medic unit where she was transported to Einstein Trauma Center. Without hesitation armed with their training and personal knowledge both Officers were able to successfully rescue the complainant. Both officers are considering moonlighting as Park Rangers! Just kidding! Excellent work Officers!

7TH DISTRICT P/O John Dougherty #9881 P/O Ismael Vasquez #6082
On May 7th Officers Dougherty and Vasquez were patrolling in the area of 11000 Roosevelt Blvd when they were flagged down by a female. The female directed to the officers to a gray vehicle occupied by an unresponsive male. The officers found the male in the driver’s seat. They gave the male verbal command to which he did not respond. Officer Vasquez opened the door and realized the make was having a medical emergency. With the help of Officer Dougherty, they got the male out of the car, notified rescue, and began CPR. After performing CPR for a few minutes, the male regained consciousness. Medics arrived on location and transported the male to the hospital. Due to their quick response and their training, these officers saved the man’s life. Excellent work officers!

9TH DISTRICT P/O Artur Bukowski #1659 P/O Steven Moffit #7022 P/O Omar Torres #4236
On August 21, Officers Bukowski, Moffit, and Torres responded to 1900 block of Mount Vernon Street for a person screaming. Upon arrival, the officers heard a female screaming from the second floor apartment. When the officers knocked and announced their arrival at the apartment; they heard a man scream through the door, “I didn’t call the police! Go away!” . The officers requested to see the woman. The man refused. The officers then heard a child crying inside the apartment. Believing the woman and her child were in danger, the officers breached the door. As the officers entered the apartment the man snatched the child from the woman and held the child in a bear hug refusing to let the child go; stating he was going to kill himself and the child if the officers didn’t leave. The officers attempted to deescalate the situation, but he refused to let the child go. Fearing for the baby, the officers quickly grabbed man and removed the child from his grasp. After a brief struggle, the officers were able to secure the male and return the baby her mother. A search of the male revealed, he had a knife in a sheath strapped to his forearm. It was later revealed the male suffered from mental illness and has become increasing combative during his episodes. The male was transported to a mental health facility for treatment. Great Job!
12TH DISTRICT P/O Shadel Sullivan P/O Timothy Kirby

On July 1st, at approximately 10:00pm, Officers Sullivan and Kirby were working an overtime detail, in the area of 7000 Dicks Ave. due to the increase of gun violence. While patrolling the area, the officers both observed a male riding a bicycle on the sidewalk. Because there are marked bike lanes in this area and the officers were aware of several shooting incidents committed by offenders on bicycles, the officers decided to talk to the male. To their surprise the male engaged them in conversation stating he had narcotics on him. When the officers attempted to pat him down, he attempted to flee but was quickly apprehended. As the officers were trying to secure him, he began to resist. This is when Officer Sullivan felt a firearm in is his waistband and verbally warned his partner. Officer Kirby deployed his taser incapacitating the male allowing the officers to gain control and take him into custody. Incident to arrest the officers recovered a loaded 9mm semi-automatic firearm and narcotics. The officers performed an intelligent and important act of police service and because of their alertness and quick action, they are a positive reflection on the entire police department. Job well done!

12TH DISTRICT P/O Miquel Hernandez P/O David Cartagena
On May 9th, Officers Hernandez and Cartagena were on patrol in the area of 66th St and Elmwood Ave. The officers heard gunshots in the area of 6500 Elmwood Ave. and could see a male laying in the middle of the street. As they approached, they saw a male walking away that was armed with a firearm. They quickly exited their vehicle, engaged the male and commanded him to stop and drop his weapon. The male, upon seeing the officers immediately surrendered without further incident and was quickly secured by the officers. The officers went to render first aid to the victim, when they noticed that he had suffered a gunshot wound to the head. Shortly after, the Officers discovered that there was an additional male shot in a restaurant on the same block. This male also suffered gunshot wounds to the head and body. Unfortunately, due to the severity of their injuries, medics pronounced both males dead at the scene. Subsequently, the defendant was arrested and transported to the homicide unit and all evidence was recovered by the investigators. Excellent Job
12TH DISTRICT P/O Michael Navedo P/O Hugo Lemos
On April 30th, Officer Lemos responded to a fight on the highway and a report of a stabbing and shooting in the 2000 block of Bonnaffon Street. Upon arrival the officer was approached by a complainant who informed them she was shot by a male on the block. The officer found a spent shell casing at the location and began to secure the front of the property. Officer Navedo, arrived and took a position in the rear of the property. While in the rear, a 2nd floor window opened, and a female called out “BABE” . At which time, a male that had been hiding in a yard next door popped his head up and began to move to the rear door of the property. The male ignored the police command to stop and proceeded to go into the property. The officers securing the property gave chase inside and were able to apprehend the male. The male was subsequently identified as the shooter and arrested. The firearm used recovered. Job well done Officers!

On May 30th Officers Diamond, Wagner and Little responded to a radio call for a strong arm robbery at Chew Avenue and Woodlawn Street. The officers received flash information from police radio which included a description of the offender, and that approximately $1000 had been taken from the complainant. While surveying the area the officers observed a male who fit the description given by radio in the area of Chew Avenue and Church Street. Officer Diamond attempted to stop the male for investigation however, he quickly fled on foot. Officer Diamon pursued the male with Officers Wagner and Little joining him in the chase. The three were able to quickly apprehend the suspect and place him into custody. The officers recovered a black backpack at the scene containing a large amount of cash and the complainant’s cell phone. Great Job!
15TH DISTRICT P/O Patrick Owens
On March 13th, Officer Owens responded to the 7200 block of Ditman St. for a suicidal male. The male was reportedly locked inside the bathroom with a knife threatening to kill himself. Upon arrival the information received was confirmed by a relative. The 17 yrs old male had previously attempted suicide was recently released from the hospital. Without hesitation, Officer Owens went inside the home and found a young man sitting on the bathroom floor with a 10inch long kitchen knife on his wrist. Officer Owens began talking to the young man, opening up a line of conversation, after a few minutes he was able to convince the young man to discard the knife into the tub. Officer Owens quickly secured him and him transported to Children’s Crisis Center for treatment. Officer Owens tactically descalated a deadly situation preserving the life of a young man. The diligence and professionalism shown by this officer reflects positively on all officers, and instills a sense of confidence in the department by the citizens of Philadelphia. Outstanding Job!
15TH DISTRICT P/O Andrew Martin P/O Jeremy Tees
On July 10th, Officers Martin and Tees responded to a radio call for a female who committed suicide. Upon arrival officers were met by a male who was screaming his wife was dead inside. The officers went into the home and found a female laying on the kitchen floor unresponsive. The officers checked for a pulse but could not feel one. However, they heard agonal gasp and observed vomit next to the female. The officers turned the female onto her side, cleared the airway of all obstructions and immediately started 2 person CPR in hopes that she would continue breathing. They continued this for approximately 3 to 5 minutes until medics arrived. Medics arrived and transported the female to the hospital where she received acute life saving measures by the attending doctor. The Doctor stated the Officer’s immediate actions of CPR, helped preserve the life of the patient until medics arrived. Outstanding Job!

17TH DISTRICT P/O Nordia Henry # 4390 P/O German LaBoy # 2153
On October 10th , Officers LaBoy and Henry were patrolling in the area of 3000 Dickinson Street, a high crime area with an extensive history of gun violence. They observed a 2000 Ford Explorer with heavy tint circling the block several times. Armed with information from the Criminal Intelligence Unit that a retaliatory shooting may take place in the 17th District this vehicle caught their attention. The Officers decided to stopp the vehicle to conduct an investigation. As they were approaching the vehicle they could see the vehicle was moving in a rocking motion left to right, an indicator that this was more than the ordinary movement of hands to retrieve paperwork. As they got closer, they could see was the rear driver-side passenger shoulders lower down, indicating to them something was afoot. When they approached the vehicle, they observed a total of 4 males inside, three of whom remained relatively calm. The Officers attention was drawn to the driver side rear passenger. He was wide eyed, staring straight ahead, breathing heavy and sweating. Alert to the demeanour of this male along with their prior observations as they were approaching the vehicle, the officers asked the male to exit the vehicle. After obtaining consent from the operator a visual safety frisk of the immediate area where the male was sitting resulted in the recovery of a firearm in the rear passenger side cup holder area. Due to the positioning of the gun and its proximity to the male it was apparent that only he could have placed it there. The male was arrested for VUFA violations. Further investigation revealed more than one of the occupants in the vehicle had been arrested for previous firearm violations. Due to their keen observation skills the officers more than likely prevented a shooting incident from occurring.
19TH DISTRICT P/O Bently Woods
On December 31st, Officer Woods was patrolling in the area of 1200 S. 53rd Street when he observed a silver Nissan Altima traveling northbound on the 1300 block of south 53rd with no head lights on. As the officer turned his vehicle around activating his lights and siren initiating a traffic stop, the operator of the vehicle immediately exited the vehicle and fled on foot. With Officer Woods in pursuit, the operator of the vehicle fell. Officer Woods then observed him pulling out a silver handgun as he was getting up off the ground. The male pointed the handgun at officer Woods and continued to flee. Approximately a half a block away, Officer Woods caught up with male, pushing him to the ground causing him to drop the firearm. The male started fighting Officer Woods and refusing all commands as they continued to struggle. Officer Woods was unable to use his radio to call for assistance as it had become disabled during the struggle. P/O Woods yelled to a passer-by to call 911 for assistance. Upon the arrival of back up the male was taken into custody. Recovered from the male who did not have a permit to carry was a 9mm Keltec loaded with six live rounds. This was dangerous situation which quickly escalated to a potentiality deadly situation. This Officer did not give up! He fought a good fight and was able to get a gun off the streets of Philadelphia! Great Job Officer Woods!

22nd District P/O Angel Ortiz P/O Joseph Marion
On April 11th, Officers Ortiz and Marion, responded to a robbery in progress on the 2600 block of Germantown Ave. The officers received flash information about the offender and observed a male fitting the exact flash in the area of 5th and Lehigh Ave. The officers took a tactical position on the offender then commanded him to get on the ground. With no resistance the offender complied. The officers were able to arrest him into custody without incident. Recovered from the offender was a 9mm firearm loaded with 13 live rounds and all the belongings taken from the complainant during the robbery. The offender was positively identified by the complaintant. The Officers quick response combined with their strategic tactical approach, allowed them to arrest a dangerous suspect without making the headline.

P/O ANDREW OLIVER #3201 24TH DISTRICT
On April 26th Officer Oliver was on patrol working in full uniform in a marked patrol car. At approximately 6:50 PM he stopped into Freddy and Tony’s restaurant located at Front Street and Indiana Avenue to conduct a security check on the establishment. The officer was met by staff members who informed him that one of the patrons had dropped a black 9mm Smith and Wesson handgun on the floor. Officer Oliver recovered the handgun and engaged the offender in a foot pursuit as he fled the restaurant. Officer Oliver quickly caught up to the suspect in the area of Ella Street and Indiana Avenue and was able to place him in custody. P/O Oliver’s initiative and attention to the businesses on his beat resulted in the recovery of an illegal firearm a dangerous offender being removed from the street. Good work!
P/O ROBERT HABERLE #1823 25TH DISTRICT
On August 1st, Officer Haberle responded to a report of an explosion at 3028 N. 8th Street. The radio call gave a description of a male in a black hooded sweatshirt who just lit multiple vehicles on fire. While surveying Officer Haberle observed a male fitting the description carrying a gas can and crouching behind a parked vehicle. The officer observed the suspect discard the gas can and drop a metal object to the ground. As the male walked away he was stopped and detained by Officer Haberle. When he returned to the area where he observed the offender drop the items the officer recovered a mask, gloves, the gas can, and a loaded Walther PPX handgun. The fire marshal responded to the scene and declared the car fire an arson. All evidence was taken for processing including the handgun which was later revealed to be stolen. Excellent work Officer!


35th District P/O Kevin Gallagher #9383 P/O Heng Dang #4781 P/O Gloria Toledo #7062 P/O John Burns #7630 P/O Kyra Carter #6718
On December 13th off duty officers Gallagher and Dang were in the area of Broad and Godfrey when they observed a house fire. While on location two people that escaped the fire informed them that someone was still inside. Without hesitation, the two officers along with arriving back up officers Toledo, Burns, and Carter rushed into the burning residence. Through the thick black smoke, the officers made their way to the 2nd floor where they could hear screaming. The officers kicked down a bedroom door and rescued a male victim. The officers were able to carry the male to safety. After exiting the burning house, the officers were all treated for smoke inhalation. Without hesitation, these officers put themselves into harm’s way and were able to save a man’s life.
P/O MICHELLE HORNER #5124 39TH DISTRICT P/O BRIAN KEENAN #2850 39TH DISTRICT
On July 3rd, while on routine patrol, Officers Horner and Keenan observed a red Hyundai Sonata traveling southbound at 2900 N. 24th Street with the passenger break light out. The officers conducted a BMV check which revealed that the vehicle was in stolen status. The officers activated their overhead lights and sirens and were able to pull the vehicle in question over. The occupants were placed into custody and recovered from one of them was a loaded Glock 34, 9mm handgun with laser site. Further investigation revealed that the handgun was in stolen status and that both offenders were vetted members of a violent street gang who had been the targets of a recent shooting incident. The suspects were charged with VUFA, theft and related offenses. Incredible job officers, keep up the good work!

On October 27th Officers Williams and Lacey in full uniform were working the 4pm x 12 and patrolling in the 19th District due to an increase in violent crime. The Officers turned their marked patrol vehicle on to 5500 Arch Street where they ob-

served a male victim, holding his head in pain and pointing toward a suspect who was running away. The officers gave chase becoming engaged in a lengthy foot pursuit of the suspect, which eventually resulted in the suspect being taken into custody. The victim informed the Officers he was walking on 5500 block of Arch st., when the suspect along with another male snuck up from behind, violently striking him on the head and took his phone. The victim positively identified the suspect as one of the two men that assaulted and robbed him. The victims phone was recovered and the suspect was arrested and charged with robbery, aggravated assault, theft and related offenses. Great job Officers and thank you for keeping us safe.
AIRPORT P/O Pablo Seda #2076 P/O William Hinderliter #5828
On July 2nd Officer Seda responded to a radio call in the Short-Term Parking Garage for a male down on the highway. Upon arrival P/O Seda observed an elderly male hanging partially out of the driver’s side of a vehicle. P/O Seda quickly determined the male had no pulse nor was he breathing. The officer requested assistance from fire rescue as he placed the male on the ground and began to administer CPR. P/O Hinderliter who was working in the area quickly responded with an AED unit and immediately attached the device. P/O Seda continued to perform chest compressions until he was nearly exhausted. The officers switched positions after the AED advised that a shock was necessary. While awaiting the arrival of medical personnel the AED administered approximately five shocks and both officers continued CPR protocol until the medics arrived on location and took command of the incident. Due to the efforts of both Officers, the male was able to developed a weak pulse and shallow breathing. The male was then transported to Methodist Hospital for critical care. Great team work Officers!
EAST DETECTIVE DIVISION Det Kenneth Rossiter #9054 Det Daphne Smith #660
On March 18th Detectives Smith and Rossiter were assigned to a robbery which took place in the area of 900 Orianna St. Taken in the robbery were a cell phone and a vehicle. The vehicle was entered into the system in stolen status. A day later 26th District Officers located the vehicle and 3 males that fit the description of the robbers. Recovered from one of the males was a key fob for a Jeep. Detectives Rossiter and Smith responded to the location and recovered the vehicle. They also surveyed the area for video footage and witnesses. The three males were also suspects in another robbery that occurred on the 1300 block of N. Marshall St. The detectives noticed the males were wearing the same clothes as the Marshall St. robbery. Det Rossiter contacted a complainant of robbery that took place on the 900 block of N. Marshall St. The complainant informed the detective that her 2018 Jeep had been taken in the robbery. The vehicle had been totaled and sent to a salvage yard in York Haven, PA. The detectives took the key fob they recovered to York Haven to see if the fob would start the vehicle, and it did! The detectives took video of the key fob working on the vehicle, which closed out another robbery. Through their thorough and meticulous investigation, these detectives closed out multiple robberies and put three criminals behind bars.




CENTRAL DETECTIVE DIVISION Det Edward Keppol #8065 Det Ronald Palumbo #8117
On June 21st Detectives Keppol and Palumbo responded to the area of 2900 Ogden Street and Hahnemann Hospital for a founded robbery and shooting of a cab driver. From information gathered by the detectives, they were looking for two males responsible for the robbery and shooting. Taken in the robbery was a wallet and cell phone. Detectives found a witness that corroborated the witness’ story. The witness described the males as fleeing in a gray vehicle with heavy front-end damage. The detectives learned a short time later that the complainant’s credit card was used at a grocery store. Video was retrieved from the store showing 2 males that matched the description from the robbery. After reviewing footage, one of the males was shown selling a cell phone to an employee as the other male used the complainant’s credit card at the ATM. This video was shown to the complainant and witness who positively identified both males. On June 27th Detectives Keppol and Palumbo went back to the store to speak with the employee who had bought the cell phone. While they were in the area, they observed a gray vehicle with front end damage. The vehicle was stopped. The vehicle was held for a warrant. Later, the cell phone that was stolen in the robbery was recovered from the store employee. All witnesses positively identified the males, and they were arrested for their crimes. One of the males gave a full confession. Through their dedication and hard work, these detectives were able to use their investigative skills to bring the suspects to justice. Great Job!

On June 26th police were called to a residence for a male threatening suicide with a gun. Responding police officers declared a barricade and SWAT was notified. SWAT quickly assembled a negotiation arrest team. They took a position of cover near the open door of the property in an attempt to establish communication with the male. Swat officers observed the male inside the residence holding a gun to his head. Realizing the severity and mental state of the man, they did not want to escalate the situation. So they decided not rush into the residence. Officers Cassidy and Stephan began to negotiate with the male. After approximately 40 minutes, they convinced the male to put the gun down. Seeing an opportunity, rear containment officers were able to enter the home while the male was distracted by the negotiating officers. The Rear containment Officers Rechner and Rivera were able to stealthily position themselves between the man and where he laid the gun at. Once the rear team engaged the male, front containment officers entered the residence and helped detain the man. Due to the alertness and tactical training of these officers an armed suicidal male was disarmed and taken into custody without injury. He was transported to a mental health facility for treatment. The officers showed diligence, patience, and professionalism. Great job officers!

On December 3rd Officers James and Ortiz responded to a robbery in progress call at a beer distributor in the area of 3000 Franklin St. The officers were approached by two complainants who were robbed at gunpoint by three males. The males escaped with various items including a purse which contained a cell phone. Realizing a cell phone had been taken the officers immediately contacted the Real Time Crime Center who were able to track the cell phone. The phone was tracked to the area of 1300 Victoria St. The officers conducted a vehicle stop of the only car in the area occupied by two males. Inside the vehicle was a plastic bag containing assorted cigarettes and a large sum of money. The officers could hear the complainants phone pinging from the car. The males were taken into custody without incident and the vehicle was held for a search warrant. The complainants’ purse, cell phones, and money were recovered from the vehicle. Additionally, gloves and ski masks were recovered. Due to the officer’s quick response and diligence, the perpetrators were taken off the streets and all items were recovered for the complainants.

KEEP IN TOUCH
LEHB sends out numerous important mailings, notifications, and advertisements throughout the year by mail, phone call and email. LEHB must be notified directly of any address, email, or phone number changes. Even if you submit a change of address form to the Police Department, LEHB does not get notified. Please call LEHB to ensure we have your updated information on file. LEHB also posts advertisements and important notifications to our social media accounts. Please follow us! COORDINATION OF BENEFITS FORM
One of the many forms that LEHB sends to our members every year is the Coordination of Benefits (COB form). The COB form is required by the State of Pennsylvania, Independence Blue Cross, and the FOP/City of Philadelphia Contract. The forms are sent out every year at the end of October or beginning of November. If your form is not received by January 1st of the following year, your claims for that year will not be paid until your form is received. If you do not receive your form in the mail by December 1st each year, please call LEHB and we will have another copy sent to you or assist you in signing the form electronically.
STAYING IN THE NETWORK AND FINDING BLUECARD PROVIDERS.
Whenever you are choosing a new provider, it is important to make sure you are choosing an in-network provider. If you receive services at an out of network provider, you will incur out-of-pocket costs. For information on how to find an in-network medical provider please call LEHB at 215-7638290. To find an in-network dental or vision provider please call Vision and Dental at 215-364-3259. To find an in-network behavioral health provider please call the LEHB Health and Wellness Center at 267-350-5405. If you receive medical services out of the Independence Blue Cross Service area (Bucks, Chester, Delaware, Philadelphia and Montgomery counties) you may also incur out-of-pocket costs if the provider is not a BlueCard provider. To find a BlueCard provider please call 1-800-810-Blue.
MEDICARE DOCUMENTATION
When you enroll in Medicare it is important that LEHB receives a copy of your Medicare Card so that we can properly coordinate your benefits. If you enroll in Medicare when you are turning 65 and are still actively working, Medicare will be your secondary insurance and LEHB will be primary. If you have Medicare and are no longing actively working, Medicare is your primary insurance and LEHB is secondary. You should always show both cards when receiving services after you retire. Since Medicare primary only pays 80% of costs, you may receive bills in the mail. When you send your medical bills into LEHB, you must also submit a copy of the Medicare Explanation of Benefits (EOB). Independence Blue Cross Personal Choice will not pay secondary for any claims without a copy of the Medicare EOB. LEHB will be holding Medicare Seminars on November 14th and 16th at the FOP from 8:00 am to 4:00 pm. Please call to make an appointment. The Medicare Seminars are for members who no longer have LEHB coverage or will be need supplemental coverage in the near future.
