elcome to the Health Quest Medical Practice Service Directory, Hometown Health. Health Quest Medical Practice is the multi-specialty medical group of Health Quest, the region’s largest integrated provider of healthcare services. HQMP is comprised of more than 200+ providers including physicians and practitioners in primary, specialty, and urgent care, and partners with three award-winning Health Quest hospitals— Northern Dutchess Hospital, Putnam Hospital Center, and Vassar Brothers Medical Center. Offices are located throughout the region in Dutchess, Ulster, and Putnam counties, making it easier for you to access quality healthcare close to home. Our HQMP providers have one guiding mission—to serve your medical needs with knowledge, compassion, and collaboration. Key enhancements have been made over the year to give you the best experience possible: • The patient portal, MyHQ247, allows you to access your secure health information at your convenience. Features include the ability to view lab results and visit summaries, request referrals and appointments, and more. • HQMP Connect, a new scheduling service, offers patients the ability to set up appointments during off hours from 7am to 7pm. • The integration of an electronic medical record at both the office and hospital level allows our teams in primary, specialty and urgent care to offer a significantly enhanced continuity of care. There are also several initiatives in development to broaden the scope of our offerings and to focus our efforts on healthcare partnering for a lifetime. This directory introduces you to the many fine medical professionals who make up the Health Quest Medical Practice. Thank you for choosing us to serve your healthcare needs. Inpatient Services Many of the services provided by Health Quest Medical Practice are performed only in the hospital. The following information highlights some of these services. NEONATOLOGISTS

The Mid-Hudson Valley’s only Level III Neonatal Intensive Care Unit (NICU), located at Vassar Brothers Medical Center, offers a host of state-of-the-art technologies and trained personnel to ensure the best outcome for extremely premature newborns and other infants with special medical needs. When a baby is prematurely born, or is an at-term newborn with critical medical issues, a neonatologist joins the team of hospital caregivers. Neonatologists are pediatricians specializing in the care of newborns, and they are available around the clock to provide the latest in diagnosis and treatment. They work with other NICU staff to ensure the highest level of sophisticated care for the young patient’s lungs and airways, cardiovascular system, brain and nervous system, skin, nutritional needs, and overall health.

SURGICAL PHYSICIAN ASSISTANTS

Each of our hospitals is committed to providing the best care possible for inpatients. Among the services that allow us to do so is our staff of surgical physician assistants. Physician assistants (PAs) are trained in the same academic coursework as are physicians, and are medically licensed to examine, diagnose, and treat patients. Surgical PAs are specially trained in operative procedures and assist the surgeon in the operating room. In addition, surgical PAs in our hospitals are on duty day and night, every day of the year, to monitor patients after surgery and ensure continuity of high quality care. Every patient’s primary care physician, and other caregivers in and outside the hospital, work with the PAs as a patient-oriented team to deliver quality, cost-effective care for any issue that may arise.

HOSPITALISTS

Hospitalists, available at all three hospitals, are fully trained and board-certified medical doctors who specialize in the care of patients admitted to the hospital. With the focus of their practice on inpatient care, Hospitalists are attentive to the medical needs of each patient on a daily basis. If you are admitted for a stay in the hospital, your primary care physician will be in frequent contact with the Hospitalist, as will any other specialists you may be seeing. In addition to overseeing and evaluating your medical treatment, Hospitalists are available to answer questions, and will write prescriptions for any necessary medications upon discharge. Because Hospitalists do not see patients outside the hospital, your primary care provider once again is your care-managing doctor after discharge. If you do not have a primary care physician, the Hospitalists and other clinical staff can recommend local providers.

INTENSIVISTS

Vassar Brothers Medical Center is proud to bring an additional group of medical specialists—Intensivists—to its team of inpatient caregivers for hospitalized patients. Intensivists are physicians with advanced training in the care of critically ill or injured patients. Intensivists apply their knowledge and special skills to the needs of patients in the intensive care unit, as well as in other areas of the hospital as needed. Intensivists work closely with other doctors, nurses, pharmacists, case managers, nutritionists, and other caregivers to ensure the best critical care. With an around-the-clock presence at the hospital, Intensivists are always available to attend incoming emergency room patients, patients already in intensive care, or any other inpatient needing new or additional critical support.

pened in 2012, the Center for Healthy Aging is a comprehensive resource designed to meet the needs of older adults. A complete geriatric assessment including common elder care concerns such as falls, skin care, memory loss and management of multiple prescriptions is the first step in providing custom-tailored care in both inpatient and outpatient treatment. Among other services, the Center provides computer based neuro-psychological testing and social worker consultations for patients and caregivers facing bereavement, depression, life transitions, and other emotional concerns. Early in her career, Dr. Friedman realized she had a special affinity for mature individuals. Beyond simply finding their conversation delightful, she began to recognize gaps in the way their health care was being provided—needs that she was sure could be met in a better way. Dr. Friedman completed her BA in Psychology at Atlanta’s Emory University and earned her MD at the University of Pittsburgh Medical School. After completing her internship at the Department of Family Medicine at Thomas Jefferson University in Philadelphia, Dr. Friedman went on to become fellowship trained in geriatrics. She is a member of the American Board of Family Physicians and the American Geriatric Society. As medical director of the Center for Healthy Aging, located on the campus of Northern Dutchess Hospital, she helps patients and their caregivers to identify and cope with the concerns that can impact independence and quality of life by working with primary care providers and specialists as needed to create the safest, most positive experience possible at every step along the way.

Seven days a week, from 7 a.m. to 7 p.m., patients can speak with a locally based, real person, who can help set up appointments, coordinate referrals, and assist with insurance verification. HQMP CONNECT allows you the freedom to plan health care visits early morning, evening, and weekends—whenever it’s most convenient. HQMP CONNECT works around your schedule, not the other way around.

PATIENT PORTAL Health Quest’s patient portal, MyHQ247, allows you secure access to your health information at your convenience. It’s just a click away, 24 hours a day. With MyHQ247, patients can keep track of appointments, view lab results, view personal health records, receive educational materials, view and pay billing statements, update demographic information, and receive communication directly from HQMP Staff. You can also print out a registration form at Health-Quest.org/myHQ247 and bring it to any HQMP office.

MEET OUR PHYSICIANS Health Quest’s Meet the Provider videos provide patients an opportunity to learn about primary care physicians, OB/GYNs, and specialists from virtually anywhere via online. Simply visit Health-Quest.org/myHQMPdoc to hear HQMP’s practitioners speak in their own words about their background and expertise, as well as their philosophies of care. We know finding the right health care provider is important. These videos allow you to meet your neighborhood doctor before you choose your neighborhood doctor. Health-Quest.org/HQMP
