Type 2 Diabetes: An Overview Diabetes is a condition that makes it difficult for the body to us glucose which causes a buildup of glucose in the blood which means the body is not getting enough energy. Type 2 diabetes is often caused by a combination of factors. One factor is that your body begins to make less insulin. A second factor is that your body becomes resistant to insulin. This means there is more insulin in your body but your body cannot use it effectively.

Risk Factors Type 2 diabetes is more common in people who are 45 years or older. Other factors that increase your chance for type 2 diabetes include: • Prediabetes – impaired glucose tolerance and impaired fasting glucose • Metabolic syndrome – a condition marked by elevated cholesterol, blood glucose, blood pressure and central obesity • Excess weight or obesity • Lack of exercise • Poor diet – high intake of calories, processed meats, fats, sugar-sweetened foods and beverages continued on page 11

Symptoms You may have diabetes for years before you have symptoms. Symptoms caused by high blood sugar include: • Increased urination • Frequent or recurring infections • Extreme thirst • Poor wound healing • Hunger • Numbness or tingling in the hands • Fatigue or feet • Blurry vision • Problems with gums • Irritability • Itching Treatment aims to maintain blood levels as close to normal as possible, prevent or delay complications, and control other conditions, such as high blood pressure and high cholesterol.

Diet

• Follow a balanced meal plan, including carbohydrates, proteins and fats • Be aware of appropriate serving size. Measure your food to help understand ideal serving size • Do not skip meals. Plan your meals and snacks throughout the day to help avoid major changes in glucose levels • Eat plenty of vegetables and fiber • Limit the amount of fat (especially saturated and trans fats) in your foods • Eat moderate amounts of protein and low-fat dairy products • Carefully limit foods containing high concentrated sugar • Keep a record of your food intake Weight Loss If you are overweight, weight loss will help your body use insulin better. These options may help you lose weight: use a portion control plate; use a prepared meal plan; eat a Mediterranean-style diet.

Exercise Physical activity can make the body more sensitive to insulin; help you reach and maintain a healthy weight; and lower the levels of fat in your blood. Aerobic exercise is any activity that increases your heart rate. Resistance training helps build muscle. Medications Certain medications will help to manage blood glucose levels, including medication taken by mouth or by injection. Insulin may be needed if the body does not make enough of its own insulin or blood glucose levels cannot be controlled with lifestyle changes and medications.

Decreasing Risk of Complications Over a long period of time, high blood glucose levels can increase your risk of heart disease and damage vital organs. The kidneys, eyes, and nerves are most affected. Maintaining goal blood glucose levels is the first step to lowering your risk of these complications. Other steps include: taking good care of your feet; having your eyes checked once a year; no smoking; keeping track of your moods and being alert for persistent depressive symptoms; and regular medical visits.

Prevention To help reduce your chance of type 2 diabetes, participate in regular physical activity; maintain a healthy weight; drink alcohol in moderation; and eat a wellbalanced diet.

Diagnosing and Treating Depressive Disorders Depression is something most of us have experienced at some time or another in our lives. Thankfully, for most of us the symptoms are temporary and we can return to our typical functioning. However, some people are not able to bounce back or they may experience more significant symptoms of a depressive episode. The latter group of people could meet the criteria for a depressive disorder. This article outlines some of the criteria for depressive disorders, possible treatments for depression, self-help strategies and how to help people with depressive disorders.

Symptoms Many people seem to lack an understanding between feeling “down” or “blue” for a couple of days and meeting the criteria of a depressive disorder. This can lead some to believe it is just a matter of people “snapping out of” their depression. Clinical depression also lies on a continuum, ranging from mild, moderate or severe depressive disorders.

The severity and/or number of symptoms can help determine recommended strategies or treatments.

Self-help strategies Evidence suggests symptoms of depression can be alleviated by both self-help strategies and more traditional psychological/psychiatric treatments. Before these strategies are outlined, it should be noted that this article is intended to only provide information and anyone suffering from the symptoms described above should seek treatment from a qualified health/mental health professional and follow their recommendations. Self-help strategies which may alleviate some symptoms are very similar to those recommended for any healthy lifestyle and include regular exercise and sleep, a healthy diet, creating/having a routine, taking time to relax, getting support from others and giving support, among other strategies.

Treatments Traditional treatments for depressive disorders typically include some form of therapy and medication. It is also important that when seeking mental health/ psychiatric treatment that each individual finds a practitioner with whom they can develop a trusting working relationship.

Helping others People who meet the criteria can be challenging to support. However, frequently what they need is for their loved ones to continue to support them, to simply be present in their lives and to let them know they deserve to feel better.

Get comfortable. If you're tense on the exam table, tell the doctor or nurse what would help you relax. If you'd like a nurse, friend, or family member there, let the doctor know.

Be honest. Topics like sex or illegal drug use may be tough to discuss, but your doctor needs to know about them to give you the right care. Keep in mind, your doctor cannot discuss anything you confide without your permission. Having a friend or family member there may make it easier.

Take time. The better you're able to explain your concerns, the better your doctor can advise you about them. Stay focused, but don't rush through a symptom if giving more information could be more helpful for your doctor.

Ask follow-up questions. If you don't understand your diagnosis or treatment, speak up. If you don't feel that you can follow through with a treatment, find out if there are other options. If you need someone who speaks your preferred language, let your doctor know.

Take notes. Or bring along a friend or family member to take notes or ask questions.

•

Find out how to keep in touch. Can you email your doctor? Or is it better to leave a phone message? Before you leave, ask for the best way to reach the doctor if you have questions about what you've discussed.

As Summit GI's first staffed dietitian, Mallory Grassmuck's career goal is to educate, facilitate and motivate those whose overall health can be improved by making knowledgeable food choices. "The role of diet in gastrointestinal disorders is the key to a better quality of life," says Mallory. Teaching patients how to make small changes that fit into their lifestyle is essential to better health. Mallory is able to see a variety of gastrointestinal disorders, including gastroparesis, irritable bowel disease, diverticulitis, gastritis, food allergies and celiac disease. In May 2014, Mallory was recognized by the Kansas City Dietetic Association as Missouri's Young Dietitians of the Year. In April 2015, she received Young Dietitian of the Year award-Kansas City from the Missouri Academy of Nutrition and Dietetics.

Mallory graduated with her master’s degree in dietetics and nutrition from University of Kansas Medical Center.

She is a member of the Society of Gastroenterology Nurses and Associates, the American Nurses Association, and the American Academy of Nurse Practitioners.
