101 Frequently Asked Allergy Questions

What is an allergy?

An allergy is an abnormally exaggerated reaction to an otherwise harmless protein-based substance in the environment causing the immune system to go into attack mode. Common inhaled allergens include cat, dog, dust mite, plant pollen and mold.

How can I tell the difference between cold and allergies?

Runny nose and nasal congestion are symptoms common to both upper respiratory infections (colds) and allergies. Upper respiratory infections typically resolve after 7-10 days. Inhaled allergies can persist for weeks at a time as long as contact with allergen persists. Itchy eyes, ears and nose are more common with allergies while fever and body aches are more common with respiratory infection.

Are allergies hereditary?

Yes. Studies have shown that the chance of a child developing an allergy if one parent has allergies is about 30% and the risk increases to 70% if both parents have allergies.

My child has been diagnosed with asthma. Should I have my child checked for allergies?

Asthma in children is commonly triggered by allergies. Identifying and controlling the allergies, if present, frequently results in better asthma control with fewer daily symptoms and exacerbations. continued on page 8

Are You Experiencing Dizziness, Vertigo, or Balance Problems?

Vertigo, dizziness, and balance problems can be debilitating and life altering.

In fact, dizziness is the most common complaint in the primary care setting for patients over 70. The most common cause of vertigo is a condition called benign paroxysmal positional vertigo (BPPV).

BPPV will affect 50% of individuals at some point. This condition is characterized by room-spinning dizziness when lying down, rising, rolling, bending at the waist, or with head pitch. Individuals with BPPV also have a doubled fall risk. BPPV is a mechanical inner ear issue that can be easily treated with an accurate diagnosis.

The current author has personally treated nearly 5,000 patients with this condition, with a success rate approaching 99.6%.

In addition to BPPV, there are a multitude of conditions that may cause dizziness and balance problems. Fortunately, with a comprehensive evaluation and accurate diagnosis, these problems can often be effectively managed.

Hearing and Balance Specialists of Kansas City offers a comprehensive program to evaluate these complaints. This program is lead by Samuel N. Bittel, Au.D., vestibular audiologist. In addition to his clinical role, Dr. Bittel teaches the doctoral vestibular courses at both the KU Medical Center and A.T. Stills Medical School; lectures nationally on evaluation/treatment of dizziness; and publishes regularly.

What is “Tinnitus”? ”TIN-a-tus” or “tin-EYE-tus” is the medical term for perceiving sound when no external sound is present. It is most commonly described as “ringing in the ears”, but can be described as buzzing, hissing, cicadas, locusts, whistling, or roaring.

What causes ringing in the ears?

The most common reason people experience tinnitus is hearing loss, and even small changes in our hearing can trigger noise in our head or ears.

Furthermore, we know that the loss of certain sound frequencies, even if they are undetected by the tinnitus patient, leads to specific changes in how the brain processes sound. In short, as the brain receives less input from the ear around a specific frequency or pitch, it begins to adapt and change. The result is an increase in neurological noise, or tinnitus.

Tinnitus may be the brain’s way of filling in the missing sound frequencies it no longer receives from the auditory system.

Can something be done?

Many people are told that “nothing can be done” for tinnitus. This is not true! If tinnitus is negatively affecting your life, please contact us for a consultation.

Myself or my child is allergic to our cats and dogs. Should we get rid of them?

Not necessarily. Studies have shown that high levels of animal dander can be found in classrooms and the workplace (brought in by classmates or coworkers who have pets). Removing pets from the home does not completely eliminate exposure. Allergen immunotherapy (allergy shots) can be an effective treatment option.

Why should I see an allergist?

Board-certified allergists receive specialized training in diagnosing and treating allergic disease. Allergists treat more than symptoms. In addition to providing cost effective treatment, evaluation by an allergist can avoid misinterpretation of allergy tests thus avoiding over-diagnosis and over prescription of medication, as well as costly and unnecessary allergy avoidance measures.

How is allergy testing performed?

An allergist will obtain a history of your symptoms and perform a physical exam. If indicated, testing to identify a specific allergen can be performed. Skin testing is an accurate method providing immediate results. Skin is pricked with a tiny amount of allergen. If you are allergic, that area will become red and swollen like a mosquito bite. Results are available within 15 to 20 minutes.

If you take medication that could affect test results or have sensitive skin, allergy testing can be performed by a blood test. A blood sample will be sent to lab to measure IGE to potential allergens, and results will be available in several days. In addition to taking longer to receive results, allergy tests performed by blood work frequently cost more than skin testing.

How are allergies treated?

Allergies can be treated with medication that relieves symptoms. Once allergens are identified, avoidance measures can be initiated to help decrease exposure. Allergy immunotherapy desensitizes the patient to the specific allergen. Evaluation by an allergist can identify treatment appropriate for the individual. Appropriate treatment can lead to improved symptom control and improved quality of life.

Get comfortable. If you're tense on the exam table, tell the doctor or nurse what would help you relax. If you'd like a nurse, friend, or family member there, let the doctor know.

Be honest. Topics like sex or illegal drug use may be tough to discuss, but your doctor needs to know about them to give you the right care. Keep in mind, your doctor cannot discuss anything you confide without your permission. Having a friend or family member there may make it easier.

Take time. The better you're able to explain your concerns, the better your doctor can advise you about them. Stay focused, but don't rush through a symptom if giving more information could be more helpful for your doctor.

Ask follow-up questions. If you don't understand your diagnosis or treatment, speak up. If you don't feel that you can follow through with a treatment, find out if there are other options. If you need someone who speaks your preferred language, let your doctor know.

Transitioning From Childhood to Adult Services The transition from childhood to adult services can be a difficult time for young adults with special needs. Many families of young adults often say, “If I only had the resources back then, the transition would have been much smoother.” There are many factors to consider as young people with special needs approach this transition time. We have outlined a few tips to help you make this a time of discovery and adventure, rather than one of fear and trepidation.

1. Begin the Transition Process Early Starting early helps you see the journey ahead and makes the process less overwhelming.

Transition planning should start by age 16. The Individualized Education Program (IEP) team should come together and assist with goals and outlining the necessary skills for living, socializing, working and identifying adult agencies. Many schools have transition coordinators to help with this process; check with your school to see who that person may be and give them a call.

2. Support Coordination Support coordinators play a vital role in the lives of people with special needs and their families. They help to identify the support the person needs, plan for the life the person desires, and connect the person to services in order to achieve a quality life. If you are not already connected with a support coordinator, contact the Department of Mental Health

if you live in Missouri DMH.mo.gov or the Department for Aging and Disability Services if you live in Kansas KDADS.ks.gov.

3. Transition Fairs Many school districts offer transition fairs which give you the opportunity to talk with agencies who provide a variety of services to adults with special needs. It’s important to attend these fairs and learn as much as possible about your options. This can be both an exciting and anxious process as your child takes the next step toward independence, but the more information you have, the less stressful it will be. You will want to look into as many resources as possible to learn how to successfully set up transition services for your child.

4. Adult Agency Services Adult services are very different from childhood services. Unlike childhood services, a disability alone will not guarantee individuals will receive funding or be served continued on page 14

through adult services. Certain services, such as physical therapy, occupational therapy, speech therapy, and a full school day are often mandated by law for students.

These are not mandated in adult services. A person must qualify for services as an adult (see ‘Benefits’ section of this article), and it is the person’s responsibility to identify and secure these services. That’s where you as a parent become the best advocate for your child.

You will want to meet with agency representatives and tour programs before making a decision of where to send your adult child, and it’s important to start early as there are

often wait lists for these programs.

Your support coordinator should be able to give you a toolkit that includes a list of agencies to use as a guide in determining services.

5. Benefits Medicaid, Social Security Disability Insurance (SSDI), and Supplemental Security Income (SSI) are important gateways to accessing services.

Such services include Medicaid waiver slots, health care, assistive technology and therapeutic supports. Your state will determine if you qualify for these services once the application is submitted.

6. Guardianship At age 18, individuals become their own guardians, regardless of their mental or physical ability. If the disability interferes with the ability to make decisions, guardianship may need to be obtained. Parents are responsible for contacting a lawyer to obtain guardianship, which is another financial responsibility of the parent.

Although transitioning from child to adult services can be challenging for you and your family, having the information and starting early will be key in helping you navigate the

process. Be reassured there are wonderful adult agencies who will love and care for your adult child— and there are people to help you through the process. Check with your school’s transition coordinator, and don’t let road blocks stop you. Keep asking questions and checking resources until you find the appropriate services for your child.

Where Should I Go For Medical Care?

Whether it’s a bump on the head or a really bad cold, when your body suffers a blow, all you want to do is feel better. So where do you go to get the care you need?

Next time you experience a nasty fall or you think you’re suffering from a particularly serious case of the flu, use these guidelines to help you decide which healthcare provider makes the most sense for your medical needs.

Emergency Room (ER) The ER should be used for serious, life-threatening issues, such as: • Chest pain or pressure • Uncontrolled bleeding • Sudden or severe pain • Difficulty breathing • Sudden dizziness, weakness or changes in vision • Changes in mental status, such as confusion • Broken bones or complex fractures • A high fever that won’t go down Urgent Care If your physician is not available, an urgent care clinic can treat a more serious illness or non-life threatening injury that needs more immediate attention. 3. Get adequate sleep Ideally adults need 7 to 8 hours of good quality sleep. This means you should feel energized upon wakening. Poor sleep habits can interrupt a delicate cycle and make you more moody and can make it difficult to focus during the day. It can also make you more prone to illness and shorten your life expectancy.

4. Avoid toxins Smoking, drinking too much alcohol and consuming heavily processed foods full of chemicals puts a toxic load on the body and contributes to disease.

5. Exercise If you had to pick a place to start out of these five lifestyle factors, choose exercise. This lifestyle behavior has the greatest impact on the others as it can help you sleep, detoxify and manage stress.
