Requirements to qualify for rebate: Mail the following to: Centerforce Rebate 2266 Crosswind Drive Prescott, AZ 86301 Complete, Print, and Return this Rebate Coupon Include a copy of original sales receipt. Cut Out and include the UPC & Part number label from the product box for each part number.
