All full-time employees are eligible for Tri-C’s flexible benefits. Your annual enrollment elections become effective Jan. 1, 2022.

Dependent Coverage

Your eligible dependents include the following, if applicable: • Your legal spouse • Employee spouses/domestic partners are not eligible for medical coverage under the College medical plan if they have access to medical coverage through their own employer. • Employees who have covered their spouse/domestic partner prior to Jan. 1, 2020, are exempt from this rule. • Your domestic partner • Your dependent children

Tri-C, as the plan sponsor, reserves the right to request documentation of eligibility for any eligible dependents. It is your responsibility to notify the Total Rewards office within 30 days of the date of ineligibility if your covered dependents no longer meet the definition of a dependent.

YOUR DEPENDENT CHILDREN INCLUDE:

• Your natural children, stepchildren and legally adopted children • Children placed and approved for adoption in your home • Adult children older than the usual eligibility age who have a physical or mental handicap that prevents self-support • Your domestic partner’s biological or legally adopted child • Children for whom you, your spouse or your domestic partner is the legal guardian or custodian • Children who, by court order, must be provided health care coverage by you, your spouse, or domestic partner

YOUR DEPENDENT CHILDREN’S ELIGIBILITY

Your dependent children are eligible for medical, dental, vision and dependent life insurance coverage until the age of 26, regardless of student or marital status.

DOMESTIC PARTNERSHIP

To be considered eligible for coverage, your domestic partner must meet the following requirements: • Be at least 18 years of age • Not legally married to another person • Not related by blood to a degree of closeness that would prohibit marriage • Be in an exclusive, committed relationship that is intended to be permanent • Share with you a mutual obligation of support and responsibility for each other’s common welfare • Has shared a principal residence with you for at least six months and intends to do so permanently

You must also submit an Affidavit of Domestic Partnership and Declaration of Tax Status form enrolling a domestic partner and his or her children. DOMESTIC PARTNER BENEFIT ENROLLMENT

If you wish to enroll your domestic partner in one of our benefit plans or the Remission of Fees benefit, you must first register them by completing the following forms located on my Tri-C space:

• Affidavit of Domestic Partnership • Declaration of Tax Status
